RESUMO

The review is devoted to analysis of the use of carbon sorbents for medicinal purposes in China and description of some innovative technologies in this field in Ukraine. The review underlines the presence of common roots of sorption therapy development in these two countries determined by pioneer works of Prof. T.M.S. Chang, created to the concept of artificial cells. High level of works of Chinese scientists on sorption purification of blood and combined extracorporeal methods has been mentioned. At the same time, by author's opinion, two other methods of sorption therapy, namely enterosorption and sorption therapy of wounds and burns, has not been properly developed in China. In the review, there are also described the essential results of Ukrainian scientists in the field of blood purification from protein-bound toxins and other harmful compounds what is important for treatment of many serious human pathologies, and also the important data on the use of oral sorbents and dressings from activated carbon materials, which could be considered as a useful addition to achievements of Chinese scientists in the field of the development and use of sorbents for medicinal purposes.

Experimental studies on 26 rats showed a stimulating effect of beet pectin at small intestine with paresis, mucosal repair, and bactericidal action on intestinal microflora. In comparison of activity intensity of small intestine solder at 1 day was 2.5-1.75 times less than 3 days and it is not reached. In the experimental group at 1 day solder activity was lower than the norm for the divisions in the 1.7-1.64--1.53-fold, and by 3 days, respectively, exceeded the norm of 0.8-2.4--2.25 times. We examined 150 patients aged 19 to 86 years with poured peritonitis primarily toxic (85.3%). III. The main causes of peritonitis were acute appendicitis (24.0%) and perforation of gastroduodenal ulcers (21.3%). For a comparative analysis of results of treatment were formed 3 patient groups using paired sample. Control group patients received conventional treatment. Enterosorption conducted through nazointestinalny probe known carbon sorbent UAO-A and pektincontents products (SAPs) the dining beet sublimate materials. The results are compared. Reducing the time used to resolve paresis of intestine and nazointestinal intubation in 1,3 times, reducing the severity of the patient on a scale of SAPS is already at 2 hours improving immunity more rapid decline in toxicity of blood plasma and leukocyte index of intoxication, reduction of postoperative pneumonia in 12% and postoperative mortality from 14% to 6.6% showed the advantages enterosorption modified pectin contents drug.

Surgical endotoxicosis entails dysfunction of all organs and systems including immune system that considerably aggravates severity of current disease and slows down a recovery in the postoperative period. Searching of various ways of immunocorrection in peritonitis of various etiology is the actual commitment of the contemporary surgery. The complex method of simultaneous enterosorbtion and immunocorrection with the modified pectin contained specimen in patients with a peritonitis is offered. In study the role natural enteral sorbents in optimization of the immune answer due to restoration of microbiocenosis sorbtion and immunocorrection modified with the modified pectin contained intestines is proved. Entero specimen provide decreasing of medium molecular weight substances' level to 89.0% in reactive stage of a peritonitis and to 69.0%--in toxic stage during 3 days. By 5th day of treatment the level of medium molecular weight substances and leucocyte intoxication index were already reached to the normal parameters. It allows us to recommend this introducted method in a complex of medical treatment in postoperation period in peritonitis.

Geophagy or deliberate ingestion of soils was observed among Asian elephants (Elephas maximus) in the Udawalwe National Park, Sri Lanka, for several years. The geochemical and mineralogical composition of the clayey soil layers which are purposefully selected and eaten by elephants in the park were studied, in order to identify the possible reasons for elephant geophagy. The concentrations of major and trace elements were determined by means of X-ray fluorescence spectrometry in 21 soil samples from eight geophagic sites and six soil samples collected from four non-geophagic sites. The mineralogical composition of selected soil samples was investigated using X-ray diffractometry (XRD). These geochemical analyses revealed that geophagic soils in the study areas are deeply weathered and that most of the elements are leached from the soil layers under extreme weathering conditions. The XRD data showed that the soils of the area consisted mainly quartz, feldspar, and the clay minerals kaolinite, Fe-rich illite, and smectite. Although no significant geochemical differences were identified between geophagic and non-geophagic soils, a clear difference was observed in their clay mineralogical content. Soils eaten by elephants are richer in kaolinite and illite than non-geophagic soils, which contain a higher amount of smectite. It is suggested that elephants in Udawalawe National Park ingest soils mainly not to supplement the mineral contents of their forage but to detoxify unpalatable compounds in their diet.

RESUMO

We examined 110 patients treated conservatively for cicatricial esophageal stenosis including expansion on the string. The patients were divided into three groups: controls (n = 35), receiving adjuvant SUMS-1 (n = 38) and given adjuvant enterosgel (n = 37). According to electron microscopy, enterosorbents make esophageal mucosa denser by decreasing interstitial spaces as a result of microcirculatory improvement and reduction of edema. Enterosorbents elevate total protein and sugar in the blood.

The safety of the new enterosorbent noolit has been evaluated within the framework of its preclinical characterization. Single introduction of noolit to rats and mice (females and males) in maximum doses for intragastric administration (5.0 - 20.0 g/kg) does not lead to the loss of experimental animals. Single administration of large doses (5 - 40 times the effective dose) can reduce the growth of the total body weight and lead to the development of nonlethal pathological changes of hemopoietic organs, which is manifested by a weak regenerative anemia (5% of cases); neutrophile, eosinophile, and basophile leukocytosis (3% of cases); and a decrease in the glucose level in blood serum. In chronic experiments on rabbits, the administration of noolit for 3 months in a dose of 0.5 and 2.0 g/kg (2 and 8 times that recommended for humans) did not reveal any toxic action on the functional and morphological state of the main systems and organs (blood, liver, kidneys, lungs, heart, gastrointestinal tract, sex organs, etc.).

A total of 47 patients with chronic pyelonephritis in remission were divided into two groups. Group 1 received balneotherapy with attenuated in both groups. In group 1 total cholesterol in blood lowered to normal values, LDLP cholesterol fell by 35.2%, VLDLP cholesterol rose by 17.7%. In group 2, urine Na decreased by 23.8% (p < 0.01), blood Na - by 52.9% (p < 0.01), HDLP cholesterol rose by cholesterol, VLDLP cholesterol, LDLP cholesterol normalized. Thus, it is recommended to include enterosorbents on the basis of alginic acid into therapy of pyelonephritis.

Fifty-eight patients aged from 18 to 65 years with peritonitis of different etiology in toxic and terminal phases were treated. Nasoenteral intubation, enterosorption and early nutritious support with pectin-containing preparation (PCP) were carried out in all the patients just after surgery with standard course of 5 days. Baseline data was compared with that after PCP-supported treatment. Powder products of red beet were used as PCP. Immunodeficiency in peritonitis is characterized by imbalance of stress-realizing and stress-limiting mechanisms of immunocompetent cells. Enterosorption and early nutritious support with PCP in peritonitis decrease the level of plasmic cortisol and enhance stress-limiting reception that reduces a harmful effect of cortisol and reestablishes immunity.

UNLABELLED: One hundred and fifty patients aged 19 to 86 years with general peritonitis mainly in a toxic (85.3%) stage were examined. The leading causes of peritonitis were acute appendicitis (24.0%) and perforation of gastroduodenal ulcers (21.3%). Three groups of patients were formed with a pair samples method for a comparative analysis of treatment RESULTS: The control group of patients was treated conventionally. Entero-sorption was performed through naso-intestinal tube with well-known carbonic sorbent UAO-A and pectin-containing medication made of red beet. The results were compared. A 1.3-fold decrease in entero-paresis naso-intestinal intubation duration, a relief in severity of the patients' condition by SAPS scale as early as on day 2, improvement of immunity indices, faster reduction of toxicity of blood plasma and leukocytic index of intoxication, a 12% decrease in postoperative pneumonia morbidity and postoperative lethality from 14 to 6.6% demonstrated advantages of entero-sorption with pectin-containing medication.

RESUMO

In 45 rats and 120 fetuses the protective effect of food, containing the additives of natural ceolites, on the outcome of pregnancy complicated by an acute experimental endotoxicosis, caused by hyperthermic challenge of the animals, was studied. Using gravimetric, histological, histochemical, electron microscopical methods and monitoring the markers of endogenous intoxication, it was demonstrated that the consumption of ceolite additives contributed to the increase of resistance of the organism to the extremal challenge. Ceolite enteroprotection of acute endotoxicosis, caused by hyperthermic challenge of the pregnant rats, resulted in the increased survival of animals, lower embryonic losses, adaptive changes in placenta and maternal liver.

As a complement of standard radioprotective measures, apple-pectin preparations are given, especially in the Ukraine, to reduce the 137Cs uptake in the organism of children. The question has been raised: is oral pectin also useful when children receive radiologically clean food, or does this polysaccharide only act in binding 137Cs in the gut, blocking its intestinal absorption? In this case, pectin would be useless if radiologically clean food could be given. The study was a randomised, double blind placebo-controlled trial comparing the efficacy of a dry and milled apple-extract containing 15-16% pectin with a similar placebo-powder, in 64 children originating from the same group of contaminated villages of the Gomel oblast. The average 137Cs load was of about 30 Bq/kg bodyweight (BW). The trial was conducted during the simultaneous one-month stay in the sanatorium Silver Spring. In this clean radiological environment only radiologically "clean" food is given to the children. The average reduction of the 137Cs levels in children receiving oral pectin powder was 62.6%, the reduction with "clean" food and placebo was 13.9%, the difference being statistically significant (p <0.01). The reduction of the 137Cs load is medically relevant, as no child in the placebo group reached values below 20 Bq/kg BW (which is considered by Bandazhevsky as potentially associated with specific pathological tissue damages), with an average value of 25.8 +/- 0.8 Bq/kg. The highest value in the apple-pectin group was 15.4 Bq/kg, the average value being 11.3 +/- 0.6 Bq/kg BW.

CONTEXT: Overdoses with enteric-coated preparation are common. The optimal means by which to limit drug absorption in such cases is controversial. OBJECTIVE: To describe the recommendations for gastrointestinal decontamination issued by North American poison control centers for a hypothetical patient, (an adult male with normal vital signs), presenting 1 hour after ingesting 500 mg/kg of enteric-coated aspirin. DESIGN: Telephone survey of 76 poison control centers in North America. Seven toxicologists who contributed to the American Academy of Clinical Toxicology/European Association of Poison Centres and Clinical Toxicologists position statements on gastrointestinal decontamination were also surveyed for informal comparison. RESULTS: Most poison control centers (99%) and all of the toxicologists (100%) participated in the survey. Four centers (5 %) recommended syrup of ipecac and 38 (51%) recommended gastric lavage, compared with 0% and 0% of toxicologists, respectively. Seventy-three centers (97%) recommended at least one dose of activated charcoal, compared with 6 toxicologists (86%). Twenty-one poison centers (28%) recommended whole-bowel irrigation, compared with 3 toxicologists (43%). A total of 36 different courses of action were suggested by respondents at the poison centers. Some of these recommendations were potentially harmful. CONCLUSIONS: Considerable variability exists in the recommendations of North American poison control centers for the gastrointestinal decontamination of patients with large, acute overdoses of enteric-coated aspirin.

The local immune homeostasis changed of the colon play a significant role in the pathogenesis of toxic colitis, induced by lead acetate. Chronic toxic colitis is accompanied by a considerable redistribution of plasmocytes-producers of the main immunoglobulins classes in the colon walls towards a primary IgA producers increase, a correlation between them and the local immune reactions tension and destabilization as well. Enterosorbent SCNP-2 influence on a pathological process in the colon under the chronic lead intoxication results in the local immune reactions improvement in the studied organ, its functional and structural elements impairment rate reduction as well as their adaptive properties increase.

The paper is dedicated to the matter of topical interest--that of diagnosing allergic reactions during the preclinical stage and of potentialities of clearing away adverse reactions to drugs without employment of medicinal agents. A characteristic feature of the paper is an original approach based on the authors' method. Reflected in the table are observations of the authors in respect of six entities versus analogous diagnosis without allergy and essentially healthy subjects--blood donors. The development of allergy is accompanied by a striking augmentation of CIC, which fact is to be considered in routine medical practice as a diagnostic criterion to be used together with the tests suggested by the authors.

A total of 143 patients with schizophrenia, who ranged from 28 to 42 years old, were studied. Of these, 68 patients were running a continuously progredient course, 75 were in the phase of exacerbation of the attack-like progredient course of schizophrenia. Group I (n = 76) of patients received the enterosorbent polysorb and a complex of antioxidants (tocoferolum acetatum, ascorbic acid, quercetin) as a supplement to the conventional therapy, group II (n = 67) was placed on the conventional therapy only. A complex of the antioxidants as well as the enterosorbent had a positive effect on the clinical course of the condition in 63.2% of group I patients who managed, among other therapeutic benefits, to achieve a stable remission. They have also demonstrated a concomitant improvement or normalization of indices for lipid peroxidation.

RESUMO

Intensity was studied of sorption of cholesterol, bile acids, and phospholipids by cereals food fibre in samples of vesicular and hepatic bile. Intensive absorption has been shown of these fractions by food fibres. Clinical observation over 92 patients with chronic noncalculous cholecystitis confirmed the beneficial effect of cereals food fibre.

The article lists, characterizes, and discusses the mechanism of the action of endosorbents possessing hypocholesteremic and hypolipidemic effects. Among them are natural endosorbents--food fibers (cellulose, hemicellulose, pectins, gum, mucus, lignin and chitin compounds, etc.); artificial specific affinin and nonspecific carbonic enterosorbents (carbonitrate family, granulated, fibrous), as well as silica (aerosil, polysorb). The effectiveness and pathogenetic expediency of correcting disorders of lipid metabolism in atherosclerosis with enterosorbents is substantiated.
