RESUMO

This report describes hemochromatosis associated with chronic parenteral iron dextran administration in 2 female olive baboons (Papio anubis). These baboons were enrolled on an experimental protocol that induced and maintained anemia by periodic phlebotomy for use in studying potential treatments for sickle cell anemia. The 2 baboons both presented with clinical signs consistent with iron overload, including decreased appetite, weight loss, elevated liver enzymes, and hepatosplenomegaly. Histopathologic findings supported a morphologic diagnosis of systemic hemosiderosis, as evidenced by the overwhelming presence of iron in the reticuloendothelial system and liver after the application of Prussian blue stain. This finding, combined with the clinical presentation, lead to a final diagnosis of hemochromatosis. This case report suggests that providing anemic patients with chronic parenteral iron supplementation in the absence of iron deficiency can result in iatrogenic iron overload and subsequent systemic toxicity. Furthermore, these subjects may present with hemochromatosis and its associated clinical signs many years after cessation of iron supplementation.

BACKGROUND: In a previous study, it was demonstrated that bone marrow analysis using the Sysmex XT-2000iV hematology analyzer produced differential counts in untreated rats that were comparable to microscopic differential counts. OBJECTIVE: The aim of this study was to modulate hematopoiesis in rats in vivo either through pharmacologic treatment or serial phlebotomy, and to determine whether the Sysmex XT-2000iV could accurately analyze bone marrow quantitative changes when compared with results obtained by microscopy. METHODS: Rats were treated once with 0, 5, 20, and 40 mg/kg cyclophosphamide (CP), 0, 50, 100 IU/kg erythropoietin (EPO) on 4 consecutive days, or serial phlebotomy of 1-2 mL of blood for 4 days. Modulation of hematopoietic populations in bone marrow was evaluated using the Sysmex XT-2000iV hematology analyzer, and compared with microscopic differential counts. RESULTS: Correlation coefficients between M:E ratios determined by Sysmex and the microscopic method were 0.94, 0.96, and 0.98 for CP, EPO, or serial phlebotomy treatments, respectively. Mean concordance correlation coefficients for M:E demonstrated method agreement of 0.63, 0.92, and 0.85 for the 3 treatments. Quantitative automated and microscopic bone marrow differential counts were within the expected 95% confidence intervals for CP, EPO or serial phlebotomy. CONCLUSIONS: The Sysmex XT-2000iV provides quantitative bone marrow differential counts of bone marrow cell series in rats with treatment-induced changes which are comparable to microscopic differential counts. Reliable automatic bone marrow differential counting allows increased throughput, sensitivity, reproducibility, and enhanced interpretation of bone marrow evaluation in rodent preclinical studies.

Blood samples were obtained from the dorsal coccygeal vein and the brachial vein of five adult (four females and one male) and two subadult males of marginated tortoise (Testudo marginata) and hematologic and biochemical parameters were compared. Significant differences were found for red blood cell count, hematocrit, hemoglobin concentration, total proteins, uric acid, aspartate aminotransferase, alanine aminotransferase, lactate dehydrogenase, alkaline phosphatase, calcium, and phosphorus, which were greater in the brachial vein samples. Hemodilution due to lymph was observed when collecting blood from the dorsal coccygeal vein, and it is thought to be the cause of the differences found. This research documented that the brachial vein is a more reliable and consistent venipuncture site than dorsal coccygeal vein in marginated tortoise.

Polycythaemia vera was diagnosed in a three-year-old domestic shorthaired cat referred because of seizures and a high packed cell volume (PCV). Laboratory examination revealed severe erythrocytosis (PCV 79 per cent). Diagnosis was reached by excluding causes for relative and secondary absolute polycythaemia. As phlebotomy proved impossible for initial treatment due to hyperviscosity, four leeches were used to suck blood and the PCV was consequently reduced to 64 per cent. A further 24 hours later, when bleeding at the sites of sucking had stopped, the PCV was 56 per cent. Long-term management of the condition was achieved with hydroxyurea (100 mg/cat once daily) and intermittent phlebotomy. Initial treatment using leeches in cases of polycythaemia vera is a simple, non-invasive, well tolerated and effective method where phlebotomy is not possible.

Because the haemoglobin concentration Hb[ during exercise may be at the upper limit of blood viscosity for effective oxygen delivery, we hypothesised that administration of blood would not further enhance oxygen delivery or exercise capacity. Six Thoroughbred geldings were used in 5 incremental treadmill exercise tests over a period of 4 weeks. The first test was performed 6 days prior to phlebotomy, which involved the removal of 20 ml/kg bwt of venous blood. Exercise tests were performed at 1, 8 and 15 days after blood removal. Six days after the 15 day post phlebotomy exercise test, blood from each horse was reinfused and the final test performed 24 h after blood reinfusion. During the 3 weeks following blood collection, the blood, collected into acid citrate dextrose, was stored at 3 degrees C. Each exercise test involved measurements of arterial and mixed venous blood gases, plasma lactate concentrations, heart rate and VO2 using an open flow system. Cardiac output was measured by direct Fick. The removal and reinfusion of blood had significant effects on packed cell volume (PCV) with the lowest PCV value during exercise of 0.57 +/- 0.04 l/l being recorded in the second post phlebotomy test and the highest value of 0.67 +/- 0.04 l/l found after blood reinfusion. There were no significant effects of phlebotomy or blood reinfusion on arterial blood gas values. However, arteriovenous oxygen content difference was significantly altered by phlebotomy and reinfusion. The maximal values during the 3 tests following phlebotomy were significantly lower (P < 0.05) by about 7% than those before phlebotomy and after blood reinfusion, mirroring the alterations in Hb[. There was no significant effect of phlebotomy or blood reinfusion on heart rate but stroke volume was significantly higher (P < 0.01) during the 3 post phlebotomy tests than for the control or blood reinfusion tests. There was no significant effect of phlebotomy or blood reinfusion on either the submaximal or maximal values for VO2. No significant effect of blood removal or reinfusion was found on the treadmill run time to fatigue. We concluded that haemodynamic adjustments following phlebotomy or blood reinfusion maintained oxygen delivery during exercise. No detrimental effects on exercise capacity were found from phlebotomy nor beneficial effects from autologous blood transfusion. However, it should be noted that the extent of change induced in Hb[ was relatively small and with the number of horses in the study, the experimental power was not great.
