OBJECTIVE: To study the effects of Zhiweifangbian (ZWFB) capsule on lactic dehydrogenase (LDH), succinic dehydrogenase (SDH) and ATPase activities in gastric mucosa of chronic atrophic gastritis (CAG) rats with Qi deficiency and blood stasis syndrome. METHODS: Totally 90 rats were randomly divided into 2 groups: normal group (n = 10) and model group (n = 80). The CAG rat model of Qi deficiency and blood stasis type was induced by synthetic methods. After modeling for 12 weeks and the successful CAG model was determined, the CAG model rats were divided by random number table into model group (MG), ZWFB high-dose group (ZWFBH), ZWFB middle-dose group (ZWFBM), ZWFB low-dose group (ZWFBL) and Weimeisu group (WM), 9 rats in each group. The rats in the normal and model groups were intragastrically administrated with distilled water, 10 mL/kg every day; the ZWFB high-dose group with ZWFB, 0.6 g/ kg(-1) x d(-1); the ZWFB middle-dose group with ZWFB, 0.3 g/kg(-1) x d(-1); the ZWFB low-dose group with ZWFB, 0.15 g/kg(-1) x d(-1), and the WM group with suspension of WM, 0.25 g/kg(-1) x d(-1). The treatment was given for 90 consecutive days. Then general survival states were observed and the activities of LDH, SDH, Na(+)-K(+)-ATPase and Ca(2+)-Mg(2+)-ATPase in gastric mucosa tissue were detected. RESULTS: Compared with the normal group, activity of LDH in the gastric mucosa (P < 0.05) and activities of SDH, Na(+)-K(+)-ATPase and Ca(2+)-Mg(2+)-ATPase significantly decreased in the normal group (P < 0.05). Compared with the model group the activity of LDH decreased and activities of SDH, Na(+)-K(+)-ATPase and Ca(2+)-Mg(2+)-ATPase significantly increased in the high dose ZWFB group (P < 0.05). CONCLUSION: ZWFB capsule can promote energy metabolism and ATPase activity in the gastric mucosa cell, so as to protect the function of the gastric mucosa cell.

In the 1-year double-blind placebo-controlled intervention trial, it was shown that daily supplementation of patients with gastric premalignant lesions (intestinal metaplasia, IM) with a complex, containing Ester-C with antioxidantsand (2100 mg of Ca-ascorbate + 340 mg of bioflavonoids), produced a sharp decrease of abnormally high ornithine decarboxylase activity in IM gastric mucosa that was accom panied by practically total IM regression in 11 of 18 (61%) patients.

The repair enzyme O6-alkyltransferase will repair O6-methylguanine adducts in human DNA. In gastric mucosal DNA these adducts may be formed as a result of exposure to nitrosamines within the gastric lumen. The formation of these nitrosamines may be inhibited by vitamin C. We have examined the effect of oral vitamin C supplementation upon intragastric vitamin C levels and gastric mucosal O6-alkyltransferase levels in 48 patients. Intragastric vitamin C levels were significantly elevated in those patients with normal gastric mucosal histology after treatment, although a variable response in intragastric vitamin C to supplementation was seen in the presence of chronic atrophic gastritis. Gastric mucosal O6-alkyltransferase activities ranged from 100 to 950 fmol/mg protein before vitamin C administration. The range of enzyme activity was similar after the course of vitamin C (62-1137 fmol/mg) but O6-alkyltransferase activities were found to be higher in 33 of the 48 patients following treatment (P < 0.01). Once again this effect was more pronounced in patients with normal gastric mucosa than those displaying evidence of chronic atrophic gastritis. We speculate that inhibition of intragastric nitrosation by vitamin C results in decreased formation of O6-methylguanine-DNA. In consequence, less O6-alkyltransferase is consumed in repairing these adducts resulting in higher tissue levels of this enzyme.

Increase of ornithine decarboxylase (ODC) activity is known to be associated with cell proliferation and, very likely, with tumour promotion. This prompted us to study the activity of ODC in gastric mucosa of patients with chronic atrophic gastritis that has been considered as a precursor of stomach cancer. Examination of 124 patients with this disease revealed the considerable increase in ODC activity in atrophic mucosa (29.8 +/- 2.9 vs 7.9 +/- 1.8 units in normal mucosa, p = 0.001). Supplementation of the patient's diet with beta-carotene (20 mg daily during 3 weeks) results in a statistically significant decrease in ODC activity in gastric mucosa. The data obtained confirm the possibility of application of ODC determination to the detection of early premalignant lesions and suggest the antipromoter activity of beta-carotene in gastric carcinogenesis.
