OBJETIVOS: Conocer el imaginario de las mujeres embarazadas sobre la maternidad. MATERIAL Y MÉTODO: Estudio llevado a cabo con metodología cualitativa. La información se generó mediante entrevistas semiestructuradas y se realizó un análisis sociológico de los discursos. Los sujetos de estudio fueron 10 mujeres embarazadas de 36-37 semanas de gestación. Se atendió a los aspectos éticos y legales. RESULTADOS: La decisión de ser madre es, en ocasiones, poco explícita: la maternidad aparece como destino femenino, enaltecida por ser fuente de nuevas experiencias y satisfacciones, pero también relacionada con un aumento de las obligaciones y las responsabilidades, con las consiguientes repercusiones en la vida personal y laboral. Las mujeres expresan inseguridad e incertidumbre ante el nuevo papel requerido: el de madre. El instinto maternal aparece en el imaginario de las mujeres, pero no el paternal. CONCLUSIONES: La fuerte asociación entre maternidad e identidad femenina en el orden cultural y simbólico hegemónico actúa como motor en la decisión de ser madre. El instinto maternal aparece en el imaginario de las mujeres para dar una explicación naturalista al rol de madre. Surge la preocupación acerca de las estrategias necesarias para conciliar la vida laboral con la familiar: las renuncias laborales están relacionadas con la naturalización del rol de madre. La maternidad es una experiencia individual contextualizada en un entorno social, cultural, histórico, económico y laboral. Estudiar las creencias y los valores de una sociedad androcéntrica nos permitirá conocer las interacciones que se dan en ella, y reconocer a las mujeres como agentes capaces de construir nuevos significados. OBJECTIVES: To know how pregnancy, delivery and postpartum period are affected by myasthenia gravis and to analyze the management of the disease during this period. METHODS: A bibliographic search has been done in CINAHL, PubMed, Science Direct, Scopus, Proquest, Lilacs, Scielo, Cochrane Plus, Cuiden Plus and IME databases. RESULTS: We have chosen 40 articles. Myasthenia in pregnancy is associated to an increased risk of preterm delivery, premature ruptured membranes and neonatal mortality. Instrumental labor is recommended to avoid maternal weakness. Breastfeeding is not contraindicated if maternal disease is well-controlled. CONCLUSIONS: Myasthenia gravis increases the risk of complications during this period. Pregnancy has to be planned in advance. Fetal monitoring is essential

Normal autoimmune function is dependent on adequate levels of activated vitamin D, 25 hydroxy vitamin D [25(OH)D]. A recent study presented deficiency of 25(OH)D levels in Swedish MG patients. We aimed to study 25(OH)D levels in patients with MG and autoimmune polyneuropathies (PNP) at a southern latitude in Greece. Plasma levels of 25(OH)D were analyzed in Greek patients with MG (n = 19), immune-mediated PNP (N = 11) and in 30 Greek healthy age- and sex-matched controls. Ten MG patients received supplementation with vitamin D3. The MG Composite Score (MGC) and MG quality of life assessed disease severity in MG patients, whereas the INCAT Disability Scale assessed clinical features in the PNP patients. MG patients with and without vitamin D3 supplementation had higher 25(OH)D levels (mean 58.8 ± 16.3 and 62.0 ± 22.4 nmol/L, respectively) than PNP patients (mean 42.1 ± 11.5 nmol/L, p = 0.01) and healthy controls (mean 45.7 ± 13.8 nmol/L, p = 0.01). Plasma 25(OH)D levels was lower with age in all groups. There were no correlations between 25(OH)D and disease duration, MGC score, or INCAT score. Vitamin D deficiency was found in all Greek patient groups and healthy controls. Levels of 25(OH)D were higher in MG patients with as well as without vitamin D supplementation compared to healthy controls, whereas CIDP/GBS patients had levels similar to controls.

Myasthenia gravis (MG) is an autoimmune disorder that affects the neuromuscular junction. The initiating factors of MG remain unclear. However, allergy has been regarded as a potential risk factor. We included 410 children with MG diagnosed between 2000 and 2008, as well as 1640 randomly selected controls. The odds ratios of MG were calculated to determine the association between MG and preexisting allergic diseases. The children with allergic diseases were at increased subsequent risk of MG, which was associated with the cumulative effect of the concurrent allergic diseases and the frequency of seeking medical care.

Clinical classification and age distribution in myasthenia gravis (MG) cases seem different between Oriental and Caucasian populations, but there have rarely been any clinical studies on MG patients from mainland China. The goal of the current study was to perform a comprehensive survey of myasthenia gravis in a hospital in China, establishing contemporary cohort data and clinical features. 1,108 unselected patients with MG attending the 309th Hospital of PLA, Beijing, China were studied during a 36-month period from July 2008 to June 2011. The sex ratio was 1:1 (F:M). 62.5 % of patients presented as adolescents and adults. Ocular MG cases accounted for 65.6 % childhood MG patients. A positive response was observed in 96.8 % of the patients for neostigmine tests, whereas a positive decremental response to low frequency repetitive nerve stimulation (RNS) was observed in 77.4 % of the patients. The highest stimulating positive rate was 65.3 % in stimulated facial nerve. Thymoma was significantly increased in those patients with severe MG, especially in the cohort involving the respiratory muscles (p < 0.001). The study revealed higher frequency of ocular and childhood MG compared to other studies in USA and European countries, which can be a result of optimum case ascertainment, increased disease duration, or application of complex diagnostic tests. The relative increase in the prevalence of ocular myasthenia can be attributed to the impact of an aging population.

BACKGROUND AND PURPOSE: The relationship between myasthenia gravis (MG) and extrathymic malignancies has not been determined. This study aimed to explore the risk of extrathymic malignancy in patients with MG based on a nationwide population-based dataset. METHODS: We identified 2614 patients with MG from the Taiwan National Health Insurance database between 1997 and 2005 and compared the incidence rates of extrathymic malignancies with 15, 684 randomly selected age-, sex-, and comorbidity-matched subjects without MG. Both cohorts were followed until the end of 2009. Cox proportional hazard model was used to evaluate the predictors of extrathymic malignancy in the MG cohort, including age, sex, comorbidities, and prescription drugs. RESULTS: After an average follow-up of 8 years, the MG cohort had a higher risk of extrathymic cancers with an incidence rate ratio (IRR) of 1.38 (95% CI 1.12-1.68, P = 0.002) than the control cohort. Although breast cancer was the most common cancer found, no statistically significant relationship between MG and any specific malignancy was observed. Cox multivariate proportional hazards analysis showed that only age (HR = 1.05, 95% CI 1.04-1.06, P < 0.001) and liver cirrhosis (IRR = 3.85, 95% CI 1.22-12.14, P = 0.021) were predictors of extrathymic cancers in the MG cohort. CONCLUSIONS: Our study showed that patients with MG had an increased risk of extrathymic malignancy in a follow-up of 8 years, but no specific susceptibility to certain malignancies was found.

Objetivo: evidenciar el significativo incremento de la incidencia de miastenia gravis (MG) en la población anciana, así como conocer las particularidades del manejo de esta enfermedad en esos pacientes. Metodología: análisis retrospectivo de los casos de MG en pacientes mayores de 65 años atendidos en el hospital comarcal de la Merced de Osuna, Sevilla, durante los años 1995 a 2003.Resultados: obtuvimos una serie de 9 pacientes (5 varones y 4 mujeres) con una edad media de 77,6 (ñ 8,6) años y edad media de inicio de 68,1 (ñ 13,8) años. El tiempo medio de evolución era de 9,3 (ñ 11) años. Ocho pacientes (88,8 por ciento) presentaron MG de inicio tardío. Clínicamente 7 pacientes (77,7 por ciento) presentaron la forma ocular (grupo I) y 2 la generalizada (22,3 por ciento), uno leve (grupo IIa) y otro moderada (grupo IIb). Los síntomas oculares fueron las manifestaciones clínicas iniciales preponderantes (88,8 por ciento). Sólo uno de los pacientes presentó timoma. Los anticuerpos antirreceptores de acetilcolina (Ac-AchR) fueron positivos en todos los casos. Todos los pacientes recibieron tratamiento con piridostigmina, y en 5 pacientes (55,5 por ciento) se usaron además esteroides. No se produjo ninguna muerte, aunque en 3 enfermos (33,3 por ciento) los resultados del tratamiento fueron peores. Conclusiones: es necesario cambiar el concepto clásico de MG como patología del adulto joven y considerarla, como refleja nuestro estudio, como una patología emergente en la población anciana. Creemos que la MG es una enfermedad que debemos conocer, dado que la detección y el tratamiento precoces consiguen generalmente una mejoría física y funcional del paciente mayor (AU)
