RESUMO

As Práticas Integrativas e Complementares (PICs) apresentam-se como métodos coadjuvantes, auxiliando na obtenção de sucesso em determinados tratamentos clínicos e/ou cirúrgicos, atuando na prevenção, auxiliando na cura e no equilíbrio psicossomático com a finalidade de reestabelecer concordância entre mente e corpo. O Conselho Federal de Odontologia (CFO) regulamentou práticas como acupuntura, hipnose, terapia floral e fitoterapia, entretanto, suas aplicabilidades dependem da emissão de uma habilitação e seu devido registro no Conselho (Resolução CFO 82/2008). Para a Odontologia, as PICs proporcionam novas ferramentas para exercer um atendimento integral do ser humano. Neste trabalho, propõe-se fazer uma revisão de literatura sobre as PICs e Odontologia. Esta revisão de literatura foi realizada por meio de busca de artigos na base de dados Scielo, PubMed e Scholar Google no período de 2000 a 2021. Foram encontrados artigos sobre as seguintes PICs atuando conjuntamente com a Odontologia: terapias florais, acupuntura, hipnodontia, ozonioterapia, laserterapia, fitoterapia, homeopatia e odontologia antroposófica. Essas práticas apresentam grande valia, pois promovem o bem-estar sem causar dano ao indivíduo, apresentando poucos ou nenhum efeito colateral possibilitando maior segurança aos pacientes, além de favorecer a relação entre paciente e cirurgião-dentista, otimizando o tempo de trabalho no consultório odontológico e reduzindo custos.


Integrative and Complementary Practices (PICs) are presented as supporting methods, helping to achieve success in certain clinical and/or surgical treatments, acting in prevention, assisting in healing and psychosomatic balance in order to reestablish agreement between mind and body. . The Federal Council of Dentistry (CFO) has regulated practices such as acupuncture, hypnosis, floral therapy and herbal medicine, however, their applicability depends on the issuance of a license and its proper registration with the Council (Resolution CFO 82/2008). For Dentistry, PICs provide new tools to provide comprehensive care to human beings. In this work, we propose to review the literature on PICs and Dentistry. This literature review was carried out by searching articles in the Scielo, PubMed and Scholar Google databases from 2000 to 2021. Articles were found on the following PICs working together with Dentistry: floral therapies, acupuncture, hypnodontics, ozone therapy , laser therapy, phytotherapy, homeopathy and anthroposophic dentistry. These practices are of great value, as they promote well-being without causing harm to the individual, presenting few or no side effects, allowing greater safety for patients, in addition to favoring the relationship between patient and dentist, optimizing working time in the dental office and reducing costs.


Las Prácticas Integrativas y Complementarias (PIC) se presentan como métodos coadyuvantes, ayudando a obtener el éxito en determinados tratamientos clínicos y/o quirúrgicos, actuando en la prevención, ayudando en la cura y en el equilibrio psicosomático con el fin de restablecer la concordancia entre mente y cuerpo. El Consejo Federal de Odontología (CFO) ha regulado prácticas como la acupuntura, la hipnosis, la terapia floral y la fitoterapia, sin embargo, su aplicabilidad depende de la emisión de un título y su correcto registro en el Consejo (Resolución CFO 82/2008). Para la odontología, los PIC aportan nuevas herramientas para la atención integral del ser humano. En este trabajo, nos proponemos hacer una revisión bibliográfica sobre los PIC y la Odontología. Esta revisión bibliográfica se realizó mediante la búsqueda de artículos en las bases de datos Scielo, PubMed y Scholar Google desde el año 2000 hasta el 2021. Se encontraron artículos sobre los siguientes CFP que actúan conjuntamente con la Odontología: terapias florales, acupuntura, hipnodoncia, ozonoterapia, terapia láser, fitoterapia, homeopatía y odontología antroposófica. Estas prácticas tienen un gran valor porque promueven el bienestar sin causar daño al individuo, presentando pocos o ningún efecto secundario, proporcionando mayor seguridad a los pacientes, y favoreciendo la relación entre el paciente y el dentista, optimizando el tiempo de trabajo en el consultorio y reduciendo los costos.

RESUMO

The current health crisis has led to the emergence of new practices, new tools, and an increased use of digital health. Unfortunately, these are too often developed in a way that defies all logic or which has nothing to do with public health. Oral health professionals have been particularly affected during this crisis, and the use of digital health, and especially teledentistry, has been considered in many countries. However, the development of teledentistry is not free of bogus "good" ideas. It is time that teledentistry be adequately considered, while ensuring the quality of the medical procedure and with the aim of reducing inequalities in terms of access to oral healthcare and public health problems.

ETHNOPHARMACOLOGICAL RELEVANCE: The antimicrobial potential of propolis - a honey bee product - was correlated with its traditional use as a natural medicine, mainly known for antimicrobial and antioxidant properties. Moreover, research on natural products in dentistry has increased in recent years in the search for products with greater therapeutic activity, lower toxicity, better biocompatibility, and more affordable cost to the population. OBJECTIVE: Considering that the beneficial effect of propolis is acknowledged for several oral conditions, this study aimed to synthesize the research and technological forecasts of existing evidence on the use of propolis extract as a potential antimicrobial agent in dentistry. METHODS: Studies were identified through an investigation in the PubMed, Web of Science, Scopus, and Scielo electronic databases. Additionally, the following patent databases were screened: Google Patents, WIPO, INPI, Espacenet, and Questel Orbit. The data were tabulated and analyzed using Microsoft Office Excel 2013 and Questel Orbit. RESULTS: A total of 174 scientific articles and 276 patents fulfilled all the criteria and were included in the investigation. The highest number of patents (n = 144) was produced by China. Additionally, the most prevalent studies were performed on an experimental basis (72%), followed by clinical studies (n = 27) and review articles (n = 21). The effect of using propolis has been extensively observed in oral care products, periodontics, pathology, and cariology, among other dental specialties. CONCLUSION: It was possible to identify the current scientific and technological scenario of the application of propolis in dentistry, with the number of patents increasing in recent years. However, all studies related to the use of propolis in dentistry have shown a potentially safe antimicrobial agent in an extensive field of application.

Since the time that human population comprehended the importance of general health maintenance and the burden of disease, there has been a search for healing properties in the natural environment. Herbal medicine is the use of plants with medical properties for prevention and treatment of conditions that can affect general health. Recently, a growing interest has been observed toward the use of traditional herbal medicine alongside synthetic modern drugs. Around 80% of the population, especially in developing countries relies on it for healthcare. Oral healthcare is considered a major part of general health. According to the world health organization (WHO), oral health is considered an important part of general health and quality of life. The utilization of natural medications for the management of pathologic oro-dental conditions can be a logical alternative to pharmaceutical methods due to their availability, low costs, and lower side effects. The current literature review aimed at exploration of the variety and extent of herbal products application in oral health maintenance including different fields of oral healthcare such as dental caries, periodontal maintenance, microbial infections, oral cancers, and inflammatory conditions.

Topical anesthetics are widely applied in order to relieve the discomfort and anxiety caused by needle insertion and other painful superficial interventions at the oral cavity. So far, there are no commercially available effective topical anesthetic formulations for that purpose, and the most of developments are related to hydrophilic and low mucoadhesive forms. Therefore, we have prepared different hybrid nanofilms composed of biopolymer matrices (chitosan, pectin, and chitosan-pectin) blended with nanostructured lipid carriers (NLC) loading the eutectic mixture of 5% lidocaine-prilocaine (LDC-PLC), in order to fulfill this gap in the market. These dual systems were processed as hybrid nanofilms by the solvent/casting method, and its mucoadhesive, structural and mechanical properties were detailed. The most appropriate hybrid nanofilm combined the advantages of both pectin (PCT) and NLC components. The resultant material presented sustained LDC-PLC release profile for more than 8 h; permeation across porcine buccal mucosa almost twice higher than control and non-cytotoxicity against 3T3 and HACAT cell lines. Then, the in vivo efficacy of PCT/NLC formulation was compared to biopolymer film and commercial drug, exhibiting the longest-lasting anesthetic effect (> 7 h), assessed by tail flick test in mice. These pectin-based hybrid nanofilms open perspectives for clinical trials and applications beyond Dentistry.

Ozone is a highly reactive compound composed of three oxygen atoms that acts as an oxidant and oxidizer. It exists at the ground level as an air pollutant and a constituent of urban smog, as well as in the Earth's upper atmosphere as a protective layer from ultraviolet rays. Healthy cells contain antioxidants such as vitamins C and E to protect against ozone oxidization. However, pathogens such as bacteria contain very trace amounts of antioxidants in their membranes, which make them susceptible to ozone and destroy the cell membrane. This review explores the history, composition, and use of ozone worldwide in dentistry. Ozone therapy has thus far been utilized with wound healing, dental caries, oral lichen planus, gingivitis and periodontitis, halitosis, osteonecrosis of the jaw, post-surgical pain, plaque and biofilms, root canals, dentin hypersensitivity, temporomandibular joint disorders, and teeth whitening. The utility of ozone will undoubtedly grow if studies continue to show positive outcomes in an increasing number of dental conditions.

OBJECTIVES: Traditionally, the acquisition of manual skills in most dental schools worldwide is based on exercises on plastic teeth placed in a "phantom head simulator". No manual trainings are done at home. Studies revealed that preliminary training of one motoric task leads to significant improvement in performance of the required motoric task that has similar components. Performing tasks indirectly via a dental mirror are complicated for the young dental students. We hypothesized that instructed training of basic skills required in dentistry at home on a tool simulating the phantom laboratory will improve the capabilities of the students and will be reflected by their clinical grades. METHODS: We developed a portable tool PhantHome which is composed of jaws, gingival tissue, rubber cover and a compatible stand. Specific teeth produced by a 3D printer with drills in different directions were placed in both jaws. Students were requested to insert pins by using tweezers and dental mirror according to instructions initiating with easy tasks and continue to ones that are more complicated. 106 first clinical year dental students participated in the study; 65 trained only in the traditional phantom lab (control). 41 trained at home by the PhantHome tool two weeks before and 2 months during the initial stage of phantom lab. The students grades routinely provided in the phantom laboratory at different stages were compared. RESULTS: Students who trained with the portable tool performed better than the control group in the first direct and second indirect preparations (p<0.05). These exams were taken when the PhantHome was available to the students. Then, the tool was returned and the phantom course continued regularly. We believe that this is why no differences between the grades of the groups were observed further on. CONCLUSIONS: Training by the PhantHome improves motor skills and consequently the clinical performances.

Wish fulfilling medicine (human enhancement (therapy)) concerns medical treatment without a direct medical need. In traditional medicine, a classical triad applies: 1. after investigation symptoms of illness lead to 2. a diagnosis, followed by 3. a proposal for treatment by the health care provider. In wish fulfilling medicine, the emphasis lies on patient's wishes. Wish fulfilling medicine concerns medical treatment, often at the request of the patient, and should be distinguished from shared decision-making,a form of communication seeking to meet the preferences of the patient and to actively involve the patient in his treatment. In the Netherlands, in accordance with the Healthcare Quality, Complaints and Disputes Act, health care providers have to offer good care at a good level, which is safe, effective, efficient and client-orientated, offered in a timely fashion and geared to the real needs of the client. Good care has to meet professional standards. In this way, fulfilling patients' wishes for treatment without a medical need can be restricted by law.

Objetivo: El objetivo del presente trabajo es demostrar la eficacia de los juegos de rol como método de aprendizaje para el desarrollo de habilidades comunicativas en alumnos de Odontología. Métodos: Se diseñó una dinámica de juego de roles con cinco casos generados mediante el uso de escenarios de baja fidelidad en la simulación clínica de la asignatura de Odontopediatría II, orientados a la satisfacción de dos objetivos del programa de estudio (conocer cómo realizar la historia clínica a un paciente infantil y desarrollar la competencia de comunicación). Resultados: un 88,68% de los estudiantes valoraron positivamente la utilización de escenarios para el desarrollo de la competencia de comunicación. El manejo de situaciones de crisis fue el aprendizaje mejor valorado seguido de la toma de decisiones. Conclusiones: Consideramos de suma eficacia la introducción de metodologías activas, como los juegos de rol, utilizando diversos escenarios para mejorar las habilidades comunicativas en los alumnos de Odontología con el fin de conseguir una adecuada relación paciente-odontólogo (AU)


Objective: The objective of the present work is to demonstrate the effectiveness of role plays as a learning method for the development of communicative skills in dentistry students. Methods: A role play dynamics was designed with five cases generated through the use of low fidelity scenarios in the clinical simulation of the Pediatric Dentistry II subject, oriented to the satisfaction of two objectives of the curriculum (knowing how to do the Clinic history to a pediatric patient and develop communication skills). Results: 88.68% of the students positively assessed the use of scenarios for the development of communication competence. The management of crisis situations was the best valued learning followed by decision making. Conclusions: We consider very important the introduction of active methodologies, such as role plays, using different scenarios to improve communicative skills in dental students in order to achieve an adequate patient-dentist relationship (AU)

CONTEXT: Herbal therapies are used worldwide to treat health conditions. In Mexico, generations have used them to treat gingivitis, periodontitis, mouth infections, and discoloured teeth. However, few studies have collected scientific evidence on their effects. OBJECTIVE: This study aimed at searching and compiling scientific evidence of alternative oral and dental treatments using medicinal herbs from Mexico. METHODS: We collected various Mexican medicinal plants used in the dental treatment from the database of the Institute of Biology at the National Autonomous University of Mexico. To correlate with existing scientific evidence, we used the PubMed database with the key term '(scientific name) and (oral or dental)'. RESULTS: Mexico has various medical herbs with antibacterial and antimicrobial properties, according to ancestral medicinal books and healers. Despite a paucity of experimental research demonstrating the antibacterial, antimicrobial, and antiplaque effects of these Mexican plants, they could still be useful as an alternative treatment of several periodontal diseases or as anticariogenic agents. However, the number of studies supporting their uses and effects remains insufficient. DISCUSSION AND CONCLUSION: It is important for the health of consumers to scientifically demonstrate the real effects of natural medicine, as well as clarify and establish their possible therapeutic applications. Through this bibliographical revision, we found papers that testify or refute their ancestral uses, and conclude that the use of plants to treat oral conditions or to add to the dental pharmacological arsenal should be based on experimental studies verifying their suitability for dental treatments.

Background and Objective. Anxiety/pain are experiences that make dental treatment difficult for children, especially during the time of anesthesia. Hypnosis is used in pediatric clinical situations to modify thinking, behavior, and perception as well as, recently, in dentistry; therefore the aim of this study was to evaluate the effectiveness of hypnosis combined with conventional behavior management techniques during infiltration anesthetic. Methods. Anxiety/pain were assessed with the FLACC scale during the anesthetic moment, as well as heart rate variability and skin conductance before and during the anesthetic moment, between the control and experimental group. Results. A marginal statistical difference (p = 0.05) was found in the heart rate between baseline and anesthetic moment, being lower in the hypnosis group. No statistically significant differences were found with the FLACC scale or in the skin conductance (p > 0.05). Conclusion. Hypnosis combined with conventional behavior management techniques decreases heart rate during anesthetic infiltration showing that there may be an improvement in anxiety/pain control through hypnotic therapy.

An estimated 1 in 1,000 local anaesthetic injections in the maxilla or the mandible lead to unwanted effects on the ipsilateral eye. We have seen a case with diplopia and lateral rectus palsy following bimaxillary anaesthesia. A systematic literature review of reports on this type of complication resulted in a total of 144 documented cases published between 1936 and 2016. The most frequently reported symptoms included diplopia (72%), partial or full ophthalmoplegia (26%), ptosis (22%), mydriasis (18%), amaurosis (13%) and orbital pain (12%). This type of complication is best understood by pathophysiological hypotheses that include intra-arterial injection, intravenous injection, autonomic dysregulation, or deep injection and diffusion. If ocular symptoms appear after local intraoral anaesthesia, the patient should be reassured. In the case of diplopia, the eye should be covered with a gauze dressing, and the patient should be instructed about associated safety risks. If symptoms persist or when vision deteriorates, referral to an ophthalmologist is advisable.

The fundamental pathophysiologic response for the survival of all organisms is the process of wound healing. Inadequate or lack of healing constitutes the etiopathologic basis of many oral and systemic diseases. Among the numerous efforts to promote wound healing, biophotonics therapies have shown much promise. Advances in photonic technologies and a better understanding of light-tissue interactions, from parallel biophotonics fields such as in vivo optical imaging and optogenetics, are spearheading their popularity in biology and medicine. Use of high-dose lasers and light devices in dermatology, ophthalmology, oncology, and dentistry are now popular for specific clinical applications, such as surgery, skin rejuvenation, ocular and soft tissue recontouring, and antitumor and antimicrobial photodynamic therapy. However, a less well-known clinical application is the therapeutic use of low-dose biophotonics termed photobiomodulation (PBM) therapy, which is aimed at alleviating pain and inflammation, modulating immune responses, and promoting wound healing and tissue regeneration. Despite significant volumes of scientific literature from clinical and laboratory studies noting the phenomenological evidence for this innovative therapy, limited mechanistic insights have prevented rigorous and reproducible PBM clinical protocols. This article briefly reviews current evidence and focuses on gaps in knowledge to identify potential paths forward for clinical translation with PBM therapy with an emphasis on craniofacial wound healing. PBM offers a novel opportunity to examine fundamental nonvisual photobiological processes as well as develop innovative clinical therapies, thereby presenting an opportunity for a paradigm shift from conventional restorative/prosthetic approaches to regenerative modalities in clinical dentistry.

Studies have shown the use of laser therapy at points of acupuncture as an alternative to metal needles. The scientific literature in the area of laser acupuncture is rather large; however, the actual mechanisms and effects have not yet been proven in detail. Therefore, the current manuscript reviews the existing literature regarding the effects of laser acupuncture in Dentistry, seeking treatment modalities in which this technique is used and which are able to generate positive clinical results. Thus, the literature survey was conducted in electronic databases--Medline/Pubmed, VHL and Science Direct--using the uniterms "alternative medicine", "low-power laser and acupuncture", "laser acupuncture and dentistry" and "laser therapy and acupuncture". Retrospective and prospective clinical studies were considered. According to the findings of the literature, laser therapy at points of acupuncture was effective for the treatment of various orofacial problems encountered in dentistry, but there are still many differences among the parameters used for irradiation and there is a lack of important information reported by the studies, such as the wavelength, dose, power density, irradiation time and frequency, points of acupuncture selected for irradiation and therapy outcomes. Although these results indicate the potential benefit of the use of laser therapy at points of acupuncture on Dentistry, further double-blinded, controlled clinical trials should be carried out in order to standardize protocols for clinical application.

RESUMO

The 21(st) century dental practice is quite dynamic. New treatment protocols and new materials are being developed at a rapid pace. Ozone dental therapy falls into the category of new treatment protocols in dentistry, yet ozone is not new at all. Ozone therapy is already a major treatment modality in Europe, South America and a number of other countries. What is provided here will not be an exhaustive scientific treatise so much as a brief general introduction into what dentists are now doing with ozone therapies and the numerous oral/systemic links that make this subject so important for physicians so that, ultimately, they may serve their patients more effectively and productively.

INTRODUCTION: Pemphigus vulgaris is a relatively rare, chronic, autoimmune vesiculobullous disorder characterized by formation of intraepithelial vesiculae and/or bullae in the skin and mucous membrane. Systemic steroids are considered to be the standard first-line therapy for pemphigus vulgaris. However, for patients unresponsive to standard therapy, the new treatment modalities are being sought. Low-level laser therapy has been accepted as an alternative or adjunctive treatment modality for many conditions in medicine and dentistry. Therefore, this study was aimed at presenting the effects of low-level laser therapy in the treatment of pemphigus vulgaris and to emphasize the crucial role of dentists in early recognition and diagnosis of pemphigus vulgaris. MATERIAL AND METHODS: The articles published until May 2013 were obtained from the Medline/PubMed online database, using following search terms and key words: "laser therapy" and "pemphigus vulgaris", "low-level laser irradiation" and "pemphigus vulgaris", "lasers" and "pemphigus vulgaris" and "pemphigus vulgaris". RESULTS: Low-level laser therapy could result in immediate and significant analgesia and improved wound healing within the observation period and follow-up. Furthermore, a decrease in patients' discomfort as well as the absence of recurrence of the pemphigus vulgaris lesions has been claimed. CONCLUSION: Even though available literature suggests that low-level laser therapy can be efficiently used in treatment of oral pemphigus vulgaris, either independently or as a part of combined therapy approach, these results should be interpreted with caution since there are no solid evidence-based proofs to provide the guidelines for the treatment of pemphigus vulgaris with low-level laser therapy. Therefore, further long-term randomized controlled clinical studies are necessary in order to give any solid recommendations on the use of low-level laser therapy in the treatment of pemphigus vulgaris.

RESUMO

Integrating oral appliance therapy into the delivery of care for sleeprelated breathing disorders has been a challenge for dental and medical professionals alike. We review the difficulties that have been faced and propose a multidisciplinary care delivery model that integrates dental sleep medicine and sleep medicine under the same roof with educational and research components. The model promises to offer distinct advantages to improved patient care, continuity of treatment, and the central coordination of clinical and insurance-related benefits.

Os cachorros-do-mato (Cerdocyon thous Linnaeus, 1766) são animais amplamente encontrados em países da América do Sul. Apesar de não ser uma espécie ameaçada de extinção, é possível que muitas populações sofram impactos decorrentes de atropelamentos de indivíduos nas rodovias do país, pois, trata-se de uma das espécies de carnívoros com elevada ocorrência de mortes deste tipo. Foram avaliados 32 sincrânios de C. thous, oriundos de vida livre, armazenados na coleção osteológica do Laboratório de Anatomia Veterinária da Universidade Vila Velha (Vila Velha/ES), a fim de diagnosticar doenças que acometeram estes indivíduos enquanto vivos. As lesões macroscópicas identificadas foram: apinhamento dentário, ausência dentária, cálculo dentário, dentina terciária, desgaste, escurecimento dentário, exposição de polpa, fenestração óssea, fratura dentária, fratura de esmalte, giroversão, pigmentação e reabsorção da crista alveolar. Os achados mais comumente observados foram: ausência dentária, desgaste dentário e fratura dentária. Ausências dentárias anteriores à morte, alterações ósseas, cálculos dentários, apinhamento e giroversão aparentemente não causaram quaisquer prejuízos aos animais enquanto vivos.


The crab-eating foxes (Cerdocyon thous Linnaeus, 1766) are animals widely found in South American countries. Although it is not an endangered species, it is possible that many of them suffer impacts from being hit by car on the highways of the country, and this is the reason why it is one of the carnivores' species with high frequency of deaths. 32 free living sincranium stored in the osteological collection in the Laboratory of Veterinary Anatomy of Vila Velha University (Vila Velha/ES) were studied in order to diagnose diseases suffered by these individuals while alive. The main lesions identified were: crowding, no dental, dental calculus, tertiary dentin, detrition, darkening tooth, exposure of pulp, bone fenestration, fractured tooth, enamel fracture, giroversion, pigmentation and resorption of the alveolar crest. The most common findings were: no dental, dental detrition and fractured tooth. No dental before death, bone changes, dental calculus, crowding and giroversion apparently did not cause any harm to the animals while alive.

The aim of this study was to examine the effect of ozone gas on the remaining bacteria after chemomechanical instrumentation of tooth root canal. The study was carried out at the Department of Endodontics and Restorative dentistry, School of Dental Medicine, University of Zagreb. A total of 37 tooth root canals from 23 teeth (10 incisors, 2 canines, 8 premolars and 3 molars) with a diagnosis of chronic apical periodontitis (17 untreated teeth and 6 retreatments) from 20 adult patients (11 females and 9 male) were selected. Endodontic samples consisted of 74 swabs from 37 canals. The first root canal swab was taken following a completed chemomechanical instrumentation by a sterile paper point after rinsing the root canal with a sterile saline solution. The canal was dried and treated with ozone gas for 40 seconds (HealOzone, Kavo, Germany). After the ozone treatment the canal was rinsed with a sterile saline solution a second swab was taken. The swabs were stored in transport media until cultivation. Microbiological identification was performed by macromorphological, micromorphological, commercial biochemical test microbiological analysis and bacteria count. A significant decrease in the number of bacteria (p < 0.001) was found after the ozone treatment: the total number of bacteria was 82%, 67% of aerobic and 93% of anaerobic bacteria. When analysing individually, a significant decrease was found for Streptococcus mitis and Propionibacterium acnes (p < 0.05). The results of this study shows the efficacy of ozone on the bacterial count reduction in the root canal treatment.
