OBJECTIVE: This study aimed to assess the effectiveness of our sound therapy with appropriate hearing aid fitting and periodic hearing aid adjustment in patients with chronic tinnitus. METHODS: We conducted a retrospective study. The study included 490 individuals who received treatment with hearing aids for chronic tinnitus at least for 3 months. To determine the effects of tinnitus on patients' quality of life, the participants completed a series of questionnaires, including the Tinnitus Handicap Inventory (THI), Visual Analogue Scale (VAS) for loudness and annoyance, and questionnaires of subjective symptom improvement. Data were collected at entry and 3 months and 1 year after treatment initiation. RESULTS: All 490 participants completed the questionnaires at 3 months; however, only 312 completed them at 1 year. The mean ± standard deviation THI score before treatment decreased significantly at 3 months (490 participants: 53 ± 25 to 11 ± 16 and 312 participants: 55 ± 24 to 12 ± 16) and 1 year (55 ± 24 to 9 ± 14) (P < 0.01). The mean VAS score for tinnitus loudness before treatment decreased significantly at 3 months (490 participants: 70 ± 22 to 25 ± 27 and 312 participants: 71 ± 22 to 27 ± 26) and 1 year (71 ± 22 to 21 ± 28) (P < 0.01). In addition, the mean VAS score for tinnitus annoyance before treatment decreased significantly at 3 months (490 participants: 75 ± 26 to 20 ± 26 and 312 participants: 75 ± 25 to 23 ± 27) and 1 year (75 ± 25 to 17 ± 26) (P < 0.01). Approximately 80% of patients noticed improvements in their tinnitus annoyance and loudness, as determined by their responses to the questionnaires of subjective symptom improvement. CONCLUSION: The results of this study suggest that treatment with sound therapy may ameliorate the symptoms of chronic tinnitus associated with hearing loss.

Bilateral acoustic hearing in cochlear implant (CI) recipients with hearing preservation may allow access to binaural cues. Sensitivity to acoustic binaural cues has been shown in some listeners combining electric and acoustic stimulation (EAS), yet remains poorly understood and may be subject to limitations imposed by the electrical stimulation and/or amplification asymmetries. The purpose of this study was to investigate the effect of stimulus level, frequency-dependent gain, and the addition of unilateral electrical stimulation on sensitivity to low-frequency binaural cues. Thresholds were measured for interaural time and level differences (ITD and ILD) carried by a low-frequency, bandpass noise (100-800 Hz). 16 adult CI EAS listeners (mean age = 50.2 years) each participated in three listening conditions: acoustic hearing only at 90 dB SPL, acoustic hearing only at 60 dB SPL with frequency-dependent gain, and acoustic hearing plus unilateral CI at 60 dB SPL with frequency-dependent gain applied to the acoustic channels only. Results revealed thresholds within the ecologically relevant ITD and/or ILD range for most EAS listeners. No significant effects of presentation level, frequency-dependent gain, or the addition of unilateral electrical stimulation on the resultant thresholds for ITDs or ILDs were observed at the group level. Correlational analyses related ITD and ILD thresholds to the degree of EAS benefit (i.e., advantage of acoustic hearing in the implanted ear) for speech recognition in diffuse noise. There was a significant relationship between EAS benefit and ITD thresholds, but no statistically significant relationship between EAS benefit and ILD thresholds. In summary, the results of this study are not consistent with our previous data obtained with simulated EAS in normal-hearing listeners, which showed significant binaural interference by a unilateral electrical "distractor" (Van Ginkel et al., 2019). The difference between studies suggests that chronic exposure to unilateral electrical stimulation combined with bilateral acoustic stimulation may reduce interference effects, perhaps because listeners adapt to the presence of the constant but binaurally incongruous CI stimulus. These results are consistent with past studies that demonstrated no interference in spatial hearing tasks due to the addition of a unilateral CI in adult EAS listeners.

PURPOSE: To establish the relationship between the performance on word recognition tasks, using words with and without sense and degree, and the configuration of hearing loss, by using Speech Inteligibility Index (SII) values as indicators, in children with hearing loss. METHODS: SII were established for 55 and 65 Decibel of Sound Pressure Level (dB SPL) input sounds of ten children presenting bilateral sensorineural hearing loss (SNHL), adapted with bilateral hearing aids, and who have oral language as the main mode of communication. The children were submitted to a word and nonsense-word repetition task of two or three intensity degrees. Their productions were analyzed according to the Word Association for Syllable Perception (WASP) Protocol. In the data analysis, the values of SII were compared with the results obtained in each analysis criterion. RESULTS: Pertaining to the words, there was statistically significant difference between the two types of stimuli in 55 dBSPL. As for the performance of consonants and point of articulation, there was a statistically significant difference between stimuli types in 65 and 55 dB SPL, and between intensities 65 and 55 dB SPL in nonsense words. CONCLUSION: Overall, there was no regularity in the relationship between hearing ability and performance in speech perception tasks. The results suggest that performance in the nonsense words recognition tasks was more related to intelligibility index than to words with meaning, possibly because it limits semantic closure strategies by the subject.


OBJETIVO: Estabelecer relações entre o desempenho em tarefa de reconhecimento de palavras com e sem sentido e grau e configuração da perda auditiva, utilizando valores de Índices de Inteligibilidade de Fala (SII) como indicadores, em crianças com deficiência auditiva. MÉTODO: Foram estabelecidos os SII para sons de entradas de 55 e 65 Decibéis Nível de Pressão Sonora (dBNPS) de dez crianças com perda auditiva neurossensorial usuárias de aparelho de amplificação sonora individual bilateralmente que têm a linguagem oral como principal modalidade de comunicação. As crianças foram submetidas à tarefa de repetição de palavras com e sem sentido em duas ou três diferentes intensidades. As emissões foram analisadas de acordo com o Protocolo Word Association for Syllable Perception (WASP). Na análise dos dados, o SII foi comparado com os resultados obtidos em cada critério de análise. RESULTADOS: Para o desempenho em palavras, houve diferença estatisticamente significante entre os dois tipos de estímulo em 55dBNPS. Para o desempenho em consoantes e ponto de articulação, houve diferença estatisticamente significante entre os tipos de estímulos em 65 e 55dBNPS e também entre as intensidades de 65 e 55 dBNPS nas palavras sem sentido. CONCLUSÃO: De modo geral, não houve regularidade na relação entre capacidade auditiva e desempenho em tarefas de percepção da fala. Os resultados sugerem que o desempenho nas tarefas de reconhecimento de palavras sem sentido tem maior relação com o índice de inteligibilidade do que as palavras com sentido, possivelmente por limitar as estratégias de fechamento semântico pelo sujeito.

Objective: Electrode impedance measures resistance encountered by electric current passing through wires, electrodes and biological tissue. This study was designed mainly to evaluate changes in electrode impedance values and psycho-electric parameters changes (i.e. threshold levels, comfortable levels, and dynamic range) in cochlear implant patients over time. Methods: It was a prospective study encompassing 20 patients implanted by MED-EL device programd using behavioral programs. Electrical stimulation levels and electrode impedance values were examined at 0, 1, 3 and 6 months after the first fitting session. Results: Electrode impedance values were reduced from the time of activation to the 6 months visit. Most comfortable levels increased and dynamic range widened until the 6 months visit. There was an inverse correlation between impedance values and most comfortable level as well as dynamic range, over time. Conclusion: Frequent monitoring of electrode impedance (for device and electrodes problems) and electric stimulation levels (for better performance, mapping and habituation) during the first 6 months of implant use is recommended.

OBJECTIVE: To estimate the prevalence of potential electric-acoustic stimulation (EAS) implant candidates in a hearing-impaired population through a review of auditory examinations. METHODS: In total, 7356 patients underwent audiometric examination in our department between 2011 and 2014. The prevalence of patients meeting the audiometric criteria for EAS and standard cochlear implant (CI) was assessed. RESULTS: The percentage of EAS implant candidates meeting the pure-tone audiometric criteria was 0.71% (n=34) among the hearing-impaired individuals (n=4758) examined in our department, whereas 2.52% (n=120) met the criteria for standard CI. Among the 34 EAS implant candidates, 2 individuals (5.83%) received EAS implant surgery after approval of the EAS device in Japan. CONCLUSIONS: There was a lower prevalence of EAS implant candidates than standard CI candidates. Nevertheless, healthcare professionals should carefully examine the audiograms of patients with high frequency hearing loss with regard to meeting the indication criteria for EAS implant. This will enable patients to gain access to adequate information relating to further examinations and treatment options.

Background: The aim of the study was to examine the effect of Tai Chi on balance and functional mobility in children with congenital sensorineural hearing loss.Methods: The study included 39 children, aged 10-14 years, with congenital sensorineural hearing loss. The participants were divided into three groups as the Tai Chi group, conventional exercise group, and control group. The Tai Chi group and the conventional exercise group received a 1-h exercise program twice a week for 10 weeks. The balance function of the children was assessed using the Pediatric Balance Scale, the balance subtest of Bruininks-Oseretsky Test 2-Short Form, and the Functional Reach Test. The Timed Up and Go Test and the Timed Up and Down Stairs Test were used to assess functional mobility. The Wilcoxon rank, Kruskal-Wallis. and Mann-Whitney U-tests were used for statistical analyses.Results: When the pre-training values of the groups were compared, with the exception of the Timed Up and Go test, there was no statistically significant difference with respect to demographic data, balance, and functional mobility parameters (p > 0.05). After training, the overall balance and functional mobility tests improved compared to pre-training values in both the Tai Chi and conventional exercise groups (p < 0.05). When the post-training values were compared between the groups, with the exception of the Functional Reach Test and the Timed Up and Down Stairs Test, the results of both exercise groups were superior to those of the control group (p < 0.05).Conclusions: The results of this study indicate that Tai Chi and conventional exercise programs have positive effects on balance and functional mobility in children with congenital sensorineural hearing loss. However, no superiority of Tai Chi or the conventional exercise programs was determined over the other. Both Tai Chi and conventional exercise programs could be used to improve balance and functional mobility in children with congenital sensorineural hearing loss.Implications for rehabilitationTai Chi and conventional exercises are effective on balance in children with congenital sensorineural hearing loss.Tai Chi and conventional exercises are effective on functional mobility in children with congenital sensorineural hearing loss.Tai Chi may be added to the rehabilitation program for children with congenital sensorineural hearing loss.

OBJECTIVE: To evaluate the clinical feasibility and auditory benefits of hearing rehabilitation using electroacoustic stimulation (EAS) after cochlear implantation (CI) and to identify the predictive factors for successful EAS rehabilitation in children with limited low-frequency hearing. STUDY DESIGN: Retrospective case review. SETTING: Tertiary referral hospital. PATIENTS: Seventeen children (21 ears) under the age of 15 years with residual low-frequency hearing who underwent CI using hearing preservation techniques. INTERVENTION: Patients underwent CI using hearing preservation techniques, and the postoperative audiograms were obtained to evaluate the hearing preservation rate. EAS rehabilitation was applied in patients with successful low-frequency hearing preservation. OUTCOME MEASURES: Improvements in speech perception in both quiet and noise conditions were compared between the EAS mode and the CI-only mode. The predictive factors for successful EAS rehabilitation in children were analyzed. RESULTS: Functional low-frequency residual hearing less than or equal to 85 dB at 250 and 500 Hz was achieved postoperatively in six of 21 ears, and successful EAS rehabilitation was possible in nine of 21 ears. Better speech perception scores were observed in quiet conditions using the EAS mode compared with the CI-only mode, although the difference did not reach statistical significance. Significantly, better scores were observed in noise conditions with the EAS mode compared with the CI-only mode. Postoperative low-frequency pure-tone average was the only significant predictive factor of successful EAS rehabilitation. CONCLUSION: CI surgery using hearing preservation techniques with EAS rehabilitation should be performed in children, even in patients with limited residual hearing, to improve auditory outcomes.

BACKGROUND: Cochlear implant (CI) outcomes can be assessed using objective measures that reflect the integrity of the auditory pathway. One such measure is the middle latency response (MLR), which can provide valuable information for clinicians. PURPOSE: Traditional stimuli for evoking MLRs, that is, clicks or tone bursts, do not stimulate all parts of the cochlea simultaneously, whereas chirp stimuli compensate for the cochlear neural delay and, therefore, produce more synchronous responses from the different neural elements of the cochlea. The purpose of the present study was to determine whether chirp stimuli can elicit reliable MLRs in CI users and whether those responses correlate with clinical outcomes and with deprivation-related factors. RESEARCH DESIGN: We presented 2,000 free-field optimized chirp stimuli to CI and control participants while their electroencephalography (EEG) was being recorded. STUDY SAMPLE: Twenty-four adult CI users and 24 matched normal-hearing (NH) individuals (age range from 18 to 63 years) participated in this study. DATA COLLECTIONS AND ANALYSIS: The EEG was recorded from 64 active electrodes placed on the scalp. EEG signals were processed using EEGLAB and ERPLAB toolboxes. We characterized the latencies and amplitudes of the different MLR components in both groups. RESULTS: Chirp stimuli reliably evoked qualitatively similar MLRs across all NH and CI participants with a couple of differences observed between the NH and CI group. Among the different MLR components, the Na latency was significantly shorter for the CI group. A significant amplitude difference was also found between groups for the Pa-Nb complex, with higher amplitudes observed in the NH group. Finally, there were no significant correlations between MLR latencies (or amplitudes) and clinical outcomes or deprivation-related measures. CONCLUSIONS: Free-field-presented optimized chirp stimuli were shown to evoke measurable and reliable MLRs in CI users. In this experiment, the MLR morphology in CI users was similar to those observed in NH participants. Even though we did not replicate here a significant relationship between MLR and speech perception measures, we were able to successfully collect acoustically evoked MLRs, which could constitute an important supplemental measure to the standard behavioral tests presently being used in postoperative clinical evaluation settings.

OBJECTIVES: The goal of this study was to identify the effects of auditory deprivation (age-related hearing loss) and auditory stimulation (history of hearing aid use) on the neural registration of sound across two stimulus presentation conditions: (1) equal sound pressure level and (2) equal sensation level. DESIGN: We used a between-groups design, involving three groups of 14 older adults (n = 42; 62 to 84 years): (1) clinically defined normal hearing (≤25 dB from 250 to 8000 Hz, bilaterally), (2) bilateral mild-moderate/moderately severe sensorineural hearing loss who have never used hearing aids, and (3) bilateral mild-moderate/moderately severe sensorineural hearing loss who have worn bilateral hearing aids for at least the past 2 years. RESULTS: There were significant delays in the auditory P1-N1-P2 complex in older adults with hearing loss compared with their normal hearing peers when using equal sound pressure levels for all participants. However, when the degree and configuration of hearing loss were accounted for through the presentation of equal sensation level stimuli, no latency delays were observed. These results suggest that stimulus audibility modulates P1-N1-P2 morphology and should be controlled for when defining deprivation and stimulus-related neuroplasticity in people with hearing loss. Moreover, a history of auditory stimulation, in the form of hearing aid use, does not appreciably alter the neural registration of unaided auditory evoked brain activity when quantified by the P1-N1-P2. CONCLUSIONS: When comparing auditory cortical responses in older adults with and without hearing loss, stimulus audibility, and not hearing loss-related neurophysiological changes, results in delayed response latency for those with age-related hearing loss. Future studies should carefully consider stimulus presentation levels when drawing conclusions about deprivation- and stimulation-related neuroplasticity. Additionally, auditory stimulation, in the form of a history of hearing aid use, does not significantly affect the neural registration of sound when quantified using the P1-N1-P2-evoked response.

OBJECTIVES: Work in normal-hearing (NH) adults suggests that spoken language processing involves coping with ambiguity. Even a clearly spoken word contains brief periods of ambiguity as it unfolds over time, and early portions will not be sufficient to uniquely identify the word. However, beyond this temporary ambiguity, NH listeners must also cope with the loss of information due to reduced forms, dialect, and other factors. A recent study suggests that NH listeners may adapt to increased ambiguity by changing the dynamics of how they commit to candidates at a lexical level. Cochlear implant (CI) users must also frequently deal with highly degraded input, in which there is less information available in the input to recover a target word. The authors asked here whether their frequent experience with this leads to lexical dynamics that are better suited for coping with uncertainty. DESIGN: Listeners heard words either correctly pronounced (dog) or mispronounced at onset (gog) or offset (dob). Listeners selected the corresponding picture from a screen containing pictures of the target and three unrelated items. While they did this, fixations to each object were tracked as a measure of the time course of identifying the target. The authors tested 44 postlingually deafened adult CI users in 2 groups (23 used standard electric only configurations, and 21 supplemented the CI with a hearing aid), along with 28 age-matched age-typical hearing (ATH) controls. RESULTS: All three groups recognized the target word accurately, though each showed a small decrement for mispronounced forms (larger in both types of CI users). Analysis of fixations showed a close time locking to the timing of the mispronunciation. Onset mispronunciations delayed initial fixations to the target, but fixations to the target showed partial recovery by the end of the trial. Offset mispronunciations showed no effect early, but suppressed looking later. This pattern was attested in all three groups, though both types of CI users were slower and did not commit fully to the target. When the authors quantified the degree of disruption (by the mispronounced forms), they found that both groups of CI users showed less disruption than ATH listeners during the first 900 msec of processing. Finally, an individual differences analysis showed that within the CI users, the dynamics of fixations predicted speech perception outcomes over and above accuracy in this task and that CI users with the more rapid fixation patterns of ATH listeners showed better outcomes. CONCLUSIONS: Postlingually deafened CI users process speech incrementally (as do ATH listeners), though they commit more slowly and less strongly to a single item than do ATH listeners. This may allow them to cope more flexible with mispronunciations.

OBJECTIVES: This study investigates outcomes in children fit with electric-acoustic stimulation (EAS) and addresses three main questions: (1) Are outcomes with EAS superior to outcomes with conventional electric-only stimulation in children? (2) Do children with residual hearing benefit from EAS and conventional electric-only stimulation when compared with the preoperative hearing aid (HA) condition? (3) Can children with residual hearing derive benefit from EAS after several years of listening with conventional electric-only stimulation? DESIGN: Sixteen pediatric cochlear implant (CI) recipients between 4 and 16 years of age with an unaided low-frequency pure tone average of 75 dB HL in the implanted ear were included in two study arms. Arm 1 included new recipients, and Arm 2 included children with at least 1 year of CI experience. Using a within-subject design, participants were evaluated unilaterally with the Consonant-Nucleus-Consonant (CNC) word list in quiet and the Baby Bio at a +5 dB SNR using an EAS program and a conventional full electric (FE) program. Arm 1 participants' scores were also compared with preoperative scores. RESULTS: Speech perception outcomes were statistically higher with the EAS program than the FE program. For new recipients, scores were significantly higher with EAS than preoperative HA scores for both the CNC and Baby Bio in noise; however, after 6 months of device use, results in the FE condition were not significantly better than preoperative scores. Long-term FE users benefited from EAS over their FE programs based on CNC word scores. CONCLUSIONS: Whether newly implanted or long-term CI users, children with residual hearing after CI surgery can benefit from EAS. Cochlear implantation with EAS fitting is a viable option for children with HAs who have residual hearing but have insufficient access to high-frequency sounds and poor speech perception.

PURPOSE: Verify the performance of musical perception and temporal auditory resolution and ordering skills in pre- and post-music therapy patients with cochlear implants (CI). METHODS: Study participants were 11 postlingual CI users with mean age of 47.64 years. All individuals underwent 10 weekly music therapy sessions. Auditory assessment was conducted using the Montreal Battery of Evaluation of Amusia (MBEA) and the Frequency Pattern Test (FPT). All participants were submitted to a placebo condition prior to music therapy and were evaluated at three different times. RESULTS: Significant improvement was observed in the subtests of contour and melody memory after music therapy. No placebo effect or difference in the FPT was observed after music therapy. CONCLUSION: Music therapy is a useful tool to improve musical skills in adult postlingual users of CI.


OBJETIVO: Verificar o desempenho das habilidades de percepção musical e das habilidades auditivas temporais de resolução e ordenação pré e pós-musicoterapia em pacientes pós-linguais usuários de implante coclear. MÉTODO: Participaram do estudo 11 indivíduos (média de idade: 47,64 anos) pós-linguais implantados, que foram submetidos a 10 sessões de musicoterapia, sendo uma por semana. Para a avaliação auditiva, foram utilizados o teste Montreal Battery Evaluation of Amusia e o teste de padrão de frequência (TPF). Todos os participantes passaram por um momento placebo antes da musicoterapia e foram avaliados em três momentos distintos. RESULTADOS: Foi observada melhora significativa nos subtestes das habilidades musicais de contorno e melodia após musicoterapia. Não foi observado efeito placebo e nem diferença do TPF após a musicoterapia. CONCLUSÃO: A musicoterapia foi uma ferramenta útil para melhorar as habilidades auditivas musicais em indivíduos adultos pós-linguais usuários de IC.

One objective way to evaluate the effect of noise reduction algorithms in hearing aids is to measure the increase in signal-to-noise-ratio (SNR). To this end, Hagerman and Olofsson presented a method where multiple recordings take place and the phase of one signal is inverted between the measurements. This phase inversion method allows one to separate signal and noise at the output of the hearing aid so that the increase in SNR can be evaluated. However, only two signals can be distinguished, for example, speech and noise. As many realistic situations include more than two signals, we extend the method to an arbitrary number of signals. Two different approaches are discussed. For the first one, groups of the signals are created and presented in such a way that the basic phase inversion method can be used. The second, more efficient approach defines a linear system of equations considering all signals. As the robustness of this approach depends on the structure of the system matrix, the design of this matrix is described in detail. To prove the concept, the proposed efficient method was applied to a setup in which nine different signals were presented by eight loudspeakers, and an analysis of errors was performed. With this setup, a state-of-the-art hearing aid was analyzed for four different settings, that is, with the digital noise reduction or the directional microphones turned on or off. As a result, the SNRs for all directions can be investigated individually.

There is conflicting evidence about the relative benefit of slow- and fast-acting compression for speech intelligibility. It has been hypothesized that fast-acting compression improves audibility at low signal-to-noise ratios (SNRs) but may distort the speech envelope at higher SNRs. The present study investigated the effects of compression with a nearly instantaneous attack time but either fast (10 ms) or slow (500 ms) release times on consonant identification in hearing-impaired listeners. Consonant-vowel speech tokens were presented at a range of presentation levels in two conditions: in the presence of interrupted noise and in quiet (with the compressor "shadow-controlled" by the corresponding mixture of speech and noise). These conditions were chosen to disentangle the effects of consonant audibility and noise-induced forward masking on speech intelligibility. A small but systematic intelligibility benefit of fast-acting compression was found in both the quiet and the noisy conditions for the lower speech levels. No detrimental effects of fast-acting compression were observed when the speech level exceeded the level of the noise. These findings suggest that fast-acting compression provides an audibility benefit in fluctuating interferers when compared with slow-acting compression while not substantially affecting the perception of consonants at higher SNRs.

Clinicians have long used self-report methods to assess hearing aid benefit. However, there are fewer data as to whether self-report instruments can be used to compare differences between signal processing settings. This study examined how self-perceived performance varied as a function of modifications in signal processing using two self-report measures. Data were collected as part of a double-blind randomised crossover clinical trial. Participants were fit with two fittings: mild processing (slow time constants, disabled frequency lowering) and strong processing (fast time constants, frequency lowering enabled). The speech, spatial, and qualities of hearing (SSQ) questionnaire and the Effectiveness of Auditory Rehabilitation (EAR) questionnaire were collected at multiple time points. Older adults with sensorineural hearing loss who had not used hearing aids within the previous year participated (49 older adults were consented; 40 were included in the final data analyses). Findings show that listeners report a difference in perceived performance when hearing aid features are modified. Both self-report measures were able to capture this change in perceived performance. Self-report measures provide a tool for capturing changes in perceived performance when hearing aid processing features are modified and may enhance provision of an evidence-based hearing aid fitting.

Recent studies have shown that activating the noise reduction scheme in hearing aids results in a smaller peak pupil dilation (PPD), indicating reduced listening effort, at 50% and 95% correct sentence recognition with a 4-talker masker. The objective of this study was to measure the effect of the noise reduction scheme (on or off) on PPD and sentence recognition across a wide range of signal-to-noise ratios (SNRs) from +16 dB to -12 dB and two masker types (4-talker and stationary noise). Relatively low PPDs were observed at very low (-12 dB) and very high (+16 dB to +8 dB) SNRs presumably due to 'giving up' and 'easy listening', respectively. The maximum PPD was observed with SNRs at approximately 50% correct sentence recognition. Sentence recognition with both masker types was significantly improved by the noise reduction scheme, which corresponds to the shift in performance from SNR function at approximately 5 dB toward a lower SNR. This intelligibility effect was accompanied by a corresponding effect on the PPD, shifting the peak by approximately 4 dB toward a lower SNR. In addition, with the 4-talker masker, when the noise reduction scheme was active, the PPD was smaller overall than that when the scheme was inactive. We conclude that with the 4-talker masker, noise reduction scheme processing provides a listening effort benefit in addition to any effect associated with improved intelligibility. Thus, the effect of the noise reduction scheme on listening effort incorporates more than can be explained by intelligibility alone, emphasizing the potential importance of measuring listening effort in addition to traditional speech reception measures.

OBJECTIVE: Binaural cues such as interaural level differences (ILDs) are used to organise auditory perception and to segregate sound sources in complex acoustical environments. In bilaterally fitted hearing aids, dynamic-range compression operating independently at each ear potentially alters these ILDs, thus distorting binaural perception and sound source segregation. DESIGN: A binaurally-linked model-based fast-acting dynamic compression algorithm designed to approximate the normal-hearing basilar membrane (BM) input-output function in hearing-impaired listeners is suggested. A multi-center evaluation in comparison with an alternative binaural and two bilateral fittings was performed to assess the effect of binaural synchronisation on (a) speech intelligibility and (b) perceived quality in realistic conditions. STUDY SAMPLE: 30 and 12 hearing impaired (HI) listeners were aided individually with the algorithms for both experimental parts, respectively. RESULTS: A small preference towards the proposed model-based algorithm in the direct quality comparison was found. However, no benefit of binaural-synchronisation regarding speech intelligibility was found, suggesting a dominant role of the better ear in all experimental conditions. CONCLUSION: The suggested binaural synchronisation of compression algorithms showed a limited effect on the tested outcome measures, however, linking could be situationally beneficial to preserve a natural binaural perception of the acoustical environment.

OBJECTIVE: Single-channel noise reduction (SCNR) and dynamic range compression (DRC) are important elements in hearing aids. Only relatively few studies have addressed interaction effects and typically used real hearing aids with limited knowledge about the integrated algorithms. Here the potential benefit of different combinations and integration of SCNR and DRC was systematically assessed. DESIGN: Ten different systems combining SCNR and DRC were implemented, including five serial arrangements, a parallel and two multiplicative approaches. In an instrumental evaluation, signal-to-noise ratio (SNR) improvement and spectral contrast enhancement (SCE) were assessed. Quality ratings at 0 and +6 dB SNR, and speech reception thresholds (SRTs) in noise were measured using stationary and babble noise. STUDY SAMPLE: Thirteen young normal-hearing (NH) listeners and 12 hearing-impaired (HI) listeners participated. RESULTS: In line with an increased segmental SNR and spectral contrast compared to a serial concatenation, the parallel approach significantly reduced the perceived noise annoyance for both subject groups. The proposed multiplicative approaches could partly counteract increased speech distortions introduced by DRC and achieved the best overall quality for the HI listeners. CONCLUSIONS: For high SNRs well above the individual SRT, the specific combination of SCNR and DRC is perceptually relevant and the integrative approaches were preferred.

OBJECTIVES: Model-based hearing aid development considers the assessment of speech recognition using a master hearing aid (MHA). It is known that aided speech recognition in noise is related to cognitive factors such as working memory capacity (WMC). This relationship might be mediated by hearing aid experience (HAE). The aim of this study was to examine the relationship of WMC and speech recognition with a MHA for listeners with different HAE. DESIGN: Using the MHA, unaided and aided 80% speech recognition thresholds in noise were determined. Individual WMC capacity was assed using the Verbal Learning and Memory Test (VLMT) and the Reading Span Test (RST). STUDY SAMPLE: Forty-nine hearing aid users with mild to moderate sensorineural hearing loss divided into three groups differing in HAE. RESULTS: Whereas unaided speech recognition did not show a significant relationship with WMC, a significant correlation could be observed between WMC and aided speech recognition. However, this only applied to listeners with HAE of up to approximately three years, and a consistent weakening of the correlation could be observed with more experience. CONCLUSIONS: Speech recognition scores obtained in acute experiments with an MHA are less influenced by individual cognitive capacity when experienced HA users are taken into account.
