RESUMO

Procedural sedation has become popular for describing a semiconscious state that allows patients to be comfortable during certain surgical or diagnostic procedures. Sedation may be enhanced by locoregional anesthetic techniques to produce sufficient analgesia and muscle relaxation for surgery to occur. Sedation and local anesthesia for standing diagnostic and surgical procedures on the horse's head circumvents the potential complications of general anesthesia (particularly, complications related to recovery). However, the implementation of a locoregional anesthetic technique requires a thorough understanding of the anatomy to maximize success and minimize possible complications.

RESUMO

We describe a combination of 3 surgical procedures for implant placement in the posterior maxilla with insufficient bone quantity and quality. In these situations, we have performed 3 minimally invasive, safe, and reliable surgical procedures: buccal bone graft with bone harvested from the mandibular molar region, submucous vestibuloplasty using artificial dermis, and bone-added osteotome sinus floor elevation under endoscopic control. These modified procedures can be performed in the office setting under outpatient local anesthesia and may minimize patient discomfort and reduce postoperative complications.

OBJECTIVES: With natural bovine bone mineral (BioOss) as carrier, the study aimed at investigating the effect of autogenous osteoblast-like cells on bone regeneration in an orthotopic (maxillary sinus) and an ectopic (muscle) site. MATERIALS AND METHODS: Autogenous osteoblast-like cells were isolated from iliac cancellous bone of six minipigs and expanded in an autogenous serum-supplemented osteogenic medium. After confirmation of osteogenicity, the expanded cells were precultivated on BioOss granules for 1 week. Four milliliter of cell-seeded BioOss were used for sinus augmentation of right sinus and a Straumann solid screw (dental) implant was simultaneously installed. The contralateral (control) side was filled with cell-free BioOss. Besides, 2 ml of the corresponding granules were placed in a pouch in the latissimus dorsi muscles bilaterally. Polychrome sequential labeling was performed postoperatively. Specimens were harvested at week 6. Undecalcified sections were evaluated with microradiography, fluorescence microscopy, histology and histomorphometry. RESULTS: In the test side, the coronal part of dental implant demonstrated higher bone-implant contact (BIC) than the apical part (34.88+/-28.86% vs. 16.68+/-13.80%, P=0.039), as well as higher bone density (BD) in the corresponding zone (14.88+/-6.37% vs. 11.10+/-4.54%, P=0.021). However, the test side demonstrated no advantage over control side in either BD (12.25+/-4.22% vs. 8.45+/-11.04%, P=0.473) or BIC (24.15+/-21.97% vs. 22.05+/-19.00%, P=0.270). Rare bone was formed in the muscles in both sides. CONCLUSIONS: The expanded autogenous osteoblast-like cells failed to enhance bone formation in the minipig model of sinus augmentation.

RESUMO

The uses of cytokine growth factors in soft tissue applications has been reported on. The scientific background and various processing techniques to prepare the autologous materials are reviewed here. An ongoing research project to study grafting of the maxillary sinus for bone regeneration and implant site preparation with the growth factor Platelet Rich Plasma is outlined.

RESUMO

Porous Al2O3 ceramic material with pore size of 100-750 micrometers was used for implants placed subperiosteally as alveolar ridge augmentation onlays in six Cercopithecus monkeys. Clinical and radiographic examination during the postoperative period revealed rapid healing and anchorage of the implant in five of the animals. One implant was lost. The implants were retrieved after 4, 10, 11 and 12 months' implantation time. Histologic and microradiographic examination demonstrated fibrous and bone tissue invasion into the pore system at the base of the subperiosteal implant, with good anchorage of the ceramic material to the jaw. Only fibrous tissue was found to project into the ceramic openings on the implant sides facing the periosteum. Labeling of the mineralizing tissue was attempted using intravenously administered tetracycline (Reverin) and calcein (Calcein) at varying intervals prior to sacrifice, but the labeling results were not impressive.
