RESUMO

Surgical treatment of head and neck cancer can cause loss of teeth, loss of hard and soft tissues and result in significantly altered anatomy. Prosthodontic rehabilitation for such patients can be challenging, requiring pre-surgical planning at a time-sensitive point in the patient's cancer pathway. Rehabilitative outcomes are optimised by early and collaborative planning at the multidisciplinary team discussion, involving surgeons, oncologists and consultants in restorative dentistry. Conventional and implant-based prosthodontics contribute to the armamentarium of rehabilitative approaches used in this patient cohort. In order to achieve the best possible outcomes for patients, collaborative planning and teamworking between head and neck surgeons and restorative dental consultants is required from the outset. Each plan is bespoke, considering the patient's needs and wishes within the context of their holistic and cancer-specific care and their general and dental health.

Objective: To provide a review of the literature about the photobiomodulation therapy (PBMT) dental treatment protocols in endodontic, restorative, and prosthetic dentistry based on validated clinical studies published so far. More specifically, this study was carried out to carefully review therapeutic protocol of PBMT in clinical studies and their conclusions. Background data: The importance of using low-power lasers and photobiomodulation (PBM) is increasing in dentistry mainly due to their painless and noninvasive function. However, lack of sufficient clinical studies has led to unclear results regarding PBMT in dentistry, and also lack of an available precise protocol for clinicians. Moreover, scarcity of clinical studies in this area has made conduction of a precise systematic review study difficult. Methods: In our study, published clinical studies up to April 2019 were reviewed from library sources, Google Scholar, PubMed and Medline, Elsevier, Embase, Cochrane, Scopus, and Web of science (ISI). Inclusion criteria included those presented in clinical trials and case report/case series, language (English), and studies available in full text. Exclusion criterion was in vitro studies. Results: In general, findings of clinical studies have shown that PBMT can have a significant role in reducing postoperative dental pain, increasing depth of anesthesia, improving tooth hypersensitivity, reducing inflammation of the tissue, and helping wound healing. Conclusions: A review of clinical studies showed that the use of alternative or adjunctive PBMT is of great importance in controlling postoperative pain after endodontic treatments. In addition, evidence suggests that different parameters of light can be efficient in the treatment of tooth hypersensitivity. Nevertheless, lack of sufficient clinical studies and reliable results do not allow introducing a precise treatment protocol.

PURPOSE: A survey was conducted in the Dental School, Aristotle University, Thessaloniki, Greece, (i) to assess the perceptions and experiences of senior dental students, Classes of 2014 and 2015, toward the old, specialty-specific (OC) and new, holistic-oriented (NC) undergraduate curriculum, respectively, in removable prosthodontics and (ii) to identify areas of improvement in prosthodontic training. MATERIALS AND METHODS: Structural components of both curricula were collected, including the total number of (i) laboratory/lecture/clinical training hours, (ii) written examinations, (iii) mandatory clinical cases, and (iv) European Credit Transfer and Accumulation System (ECTS) credits. A 20-item closed-type questionnaire using a five-point Likert scale was distributed in two consecutive groups of seniors, Classes of 2014 (n = 54) and 2015 (n = 57); 54 OC and 57 NC senior students filled out the questionnaire. RESULTS: A greater percentage of NC students stated that (i) they felt less confident to carry out jaw relation recording and removable partial denture metal framework design and (ii) the time allocated for clinical training was less sufficient. Students of both curricula reported that the laboratory courses and lectures contributed little to their clinical performance. Implant restorations and prosthetic management of patients with craniomandibular disorders were the most popular treatment options that the respondents felt that should be incorporated in the undergraduate curriculum. CONCLUSIONS: As a general trend, students completing the revised curriculum reported more difficulties during their training and felt more insecure about their clinical competences. To enhance students' self-confidence in prosthodontics, more effective preclinical training methods, introduction of additional treatment options, and alternative educational methods should be considered.

The aim of this investigation was to study the clinical prosthodontic decision-making process relating to dentitions with compromised molars among Swedish general dental practitioners (GDPs). Eleven Swedish GDPs were purposively selected, and all agreed to participate. Then, in-depth, semi-structured interviews were conducted and covered treatment considerations concerning two authentic patient cases, initially with complete dental arches, and later, a final treatment based on a shortened dental arch (SDA) was discussed. The cases involved patients with compromised teeth situated mainly in the molar regions. One patient suffered from extensive caries and the other from severe periodontal disease. Qualitative content analysis was used to analyse the data. In the systematic analysis, two main categories were identified: holistic and functional approach. Among the interviewed GDPs, focus was put on patients' needs, background history and motivation for treatment as well as the preservation of molar support. Within the limitations of this study, the following can be concluded: keeping a dental arch with molars seems to be important to Swedish general dental practitioners. The SDA concept does not seem to have a substantial impact on the prosthodontic decision-making relating to dentitions with compromised molars. The dentist's experiences, as well as colleagues' or consulting specialist advice together with aetiological factors and the patient's individual situation, influence the decision-making more than the SDA concept. The conflicting results in the prosthetic decision-making process concerning the relevance of age and the need for molar support need further investigation, for example based on decisions made in the dentist's own clinical practice.

PURPOSE: To assess dietary and nutritional changes among the elderly following pros-thodontic rehabilitation. Another objective was to study the relationship, if any, between diet and nutrition, with extent of edentulism and different types of prosthodontic treatment. MATERIALS AND METHODS: One hundred and thirty-five patients who satisfied the inclu-sion and exclusion criteria and agreed to be a part of the study after informed consent were recruited to this longitudinal study. Following selection, they were investigated on four aspects: dental examination, dietary assessment, anthropometric assessment, and serum biochemical assessment. All measurements were collected twice, first at baseline and then 6 months following prosthodontic rehabilitation. Treatment modalities included were complete denture (CD), removable partial denture (RPD), and fixed partial denture (FPD). The RPD group was of two types: distal extension prosthesis (RPDD) and tooth-supported prosthesis (RPDT). Change (post-pre) in outcome measures was compared by one-way ANOVA, and significance of mean difference between the groups was done by Tukey's honestly significance difference post hoc test. RESULTS: The improvement in diet was found to be: CD > RPDD > RPDT > FPD. Significant improvement in weight (p < 0.001), BMI (p < 0.001), protein (p < 0.001), carbohydrate (p = 0.021), calorie (p < 0.001), iron (p = 0.002), and vitamin B (p < 0.001) in group CD as compared to partially edentulous patients (group RPDD + RPDT + FPD) was noted. The protein and calorie intake increased significantly in group RPD as compared to group FPD in partially edentulous patients. CONCLUSION: Prosthetic rehabilitation becomes increasingly important as the level of edentulism increases to improve dietary, anthropometric, and biochemical parameters.

Common practice of establishing occlusal vertical dimension by adopting traditional method for patients with long-term pathological conditions has left the results much less than desired, since the degree of pathological condition for each patient varies from one patient to another, and traditional methods to determine vertical dimension are mostly intended for normal patients. Ideally, the occlusal vertical dimension should be dictated by, and conformed to patient's comfort range for the physiological accommodation to each patient's condition. This article describes a method of a double checking system for occlusion determination by increasing vertical dimension of occlusion (VDO) by 1/2 mm each week to find the most comfort zone and beyond for patient, and then to gradually decrease ½ mm of the VDO each week to the original VDO to double check the comfort zone for the patient. During the process, Bi-Digital O-Ring Test was employed to corroborate, to determine, and to individualize the occlusal vertical dimension for patients with pathological abnormalities. Traditional methods of vertical dimension determination were merely used as a guiding reference in our treatment of those patients. Patient's comfort and discomfort were carefully recorded, compared and studied before the denture prosthesis was fabricated.

The intent of the redesign of the clinical component of the Harvard School of Dental Medicine (HSDM) curriculum from a traditional numerical procedures-based system to a patient-based comprehensive care system was to improve both patient care and student learning. The aim of this retrospective cohort study was to evaluate the outcomes of and students' perspectives on this patient-centered comprehensive care model introduced in 2009. Data were obtained from the school's Office of Dental Education for a study sample consisting of 205 fourth-year dental students in the graduating classes of 2009 through 2014 at HSDM. The results showed that students were completing more than the required number of comprehensive patient cases on average. A survey given to the Class of 2014 found that the respondents (35 of 36 students) were generally satisfied with the new curriculum and the clinical experience in relation to continuation of care and perceptions of comprehensive care. The results of this study suggest that the redesigned patient-centered assessment model of the clinical component of the curriculum helped improve patient care and student learning.

Overt dental disease is a distinguishing comorbidity associated with methamphetamine abuse, necessitating the need for special management to maximize treatment benefits. As this highly addictive stimulant increases in popularity, it has become imperative that clinicians are equipped to thoughtfully provide comprehensive care for these patients. This article reviews the impact of methamphetamine to systemic and oral health and proposes a comprehensive treatment plan and sequence for the methamphetamine-dependent patient. A multidisciplinary approach is recommended. Destructive oral and psychological changes must be identified and controlled. A thorough risk assessment, caries control, and preventative plan should be established before initiating prosthodontic treatment. Patient motivation, support, and a timely recall schedule are integral for dental longevity.

Some dental patients use the orofacial region to play wind instruments; however, musical performance has not been objectively evaluated following prosthodontic treatment in such patients. The purpose of this report was to describe prosthodontic treatment for a clarinet player using sound analysis. The patient required a removable partial denture for his maxillary anterior teeth. Sound analysis was performed before and after denture adjustment, and the patient completed a questionnaire regarding his perceptions while playing his clarinet. After adjustment, the denture showed better performance, and patient satisfaction increased compared with that before adjustment.

This study assessed the impact of a course on communication skills for third-year undergraduate dental students at a dental institute in India. A randomized pretest, posttest controlled trial was conducted with all the students from four cohorts of third-year dental undergraduate students, divided into an intervention group (n=30) and a control group (n=30). The course was developed using Kern's six-step approach to curriculum development. Needs assessment was ascertained, and readings, lectures, and role-plays with real and simulated patients were implemented. Encounters of students during two patient interviews (simulated and real) were rated by two raters using a twenty-seven-item dental consultation communication checklist with a rating scale 0 to 3. Students completed a questionnaire regarding their acceptance of the course. A 2×2 (group × time) ANOVA with group as a between-subjects factor (control vs. experimental) and time as a within-subjects factor (pre vs. post) was performed. The two groups did not differ at pretest but differed significantly at posttest. This study showed that simply attending to patients during a clinical course did not improve professional communication skills. In contrast, the implementation of a course on communication skills did improve the students' dentist-patient interactions. Integrating the teaching and development of a relevant, outcome-based course on communication skills provided clear evidence of communication skills acquisition among these dental students. The course could be introduced in other Indian dental schools.

The curriculum of the dental faculty at the University of Bergen, Norway, was revised and a new curriculum implemented in 1998 based on the principles of holistic teaching and patient-centered treatment. The first candidates graduated in 2003. The change of curricula, experience gained, and lack of an evidence base for holistic teaching justify a general discussion of all relevant aspects associated with this approach. The purpose of this article was to make a contribution towards such a discussion. A PubMed search regarding holistic teaching in dentistry was performed. Of the 211 entries on holistic teaching, few discussed holism in depth; none reported outcome measures comparing old and new curricula. Data collected from students graduating in 2003 (new curriculum) and 2000 (old curriculum) on their satisfaction with the teaching comprise a possible outcome measure. In most respects, using prosthodontics as an example, no differences between the two groups of students were found. Students studying under the new holistic curriculum were less satisfied than those studying under the old one regarding the number of available teachers and teachers' feedback on student performance. Both holistic teaching/patient-centered treatment and a more traditional subject-specific approach have advantages and disadvantages, and neither can be practiced in its pure form for ethical and practical reasons. The quantitative results of this study did not support the hypothesis that holism improved students' satisfaction with the teaching. A wide discussion of holism in dental education is needed, along with outcome measures when curricula are changed.

The purpose of this study was to evaluate the effects of a new clinical curriculum on dental student productivity as measured by number of procedures performed in the student teaching practice. Harvard School of Dental Medicine adopted a new clinical education model for the predoctoral program in summer 2009 based upon a Case Completion Curriculum (CCC) rather than a discipline-based numeric threshold system. The two study groups (threshold group and case completion group) consisted of students who graduated in 2009 and 2010. Clinical performance was assessed by clinical productivity across five major discipline areas: periodontics, operative dentistry, removable prosthodontics, fixed prosthodontics, and endodontics. The relationships between the two study groups with regard to number of procedures performed by category revealed that the case completion group performed a significantly higher number of operative and removable prosthodontic procedures, but fewer periodontal and endodontic procedures (p≤0.03). No statistically significant difference in number of procedures was observed with fixed prosthodontic procedures between the two groups. Clinical productivity as a result of redesigning the clinical component of the curriculum varied in selected disciplines. The CCC, in which the comprehensive management of the patient was the priority, contributed to achieving a patient-based comprehensive care practice.

RESUMO

The University of the Pacific, Arthur A. Dugoni School of Dentistry in San Francisco established a comprehensive dental care program at Laguna Honda Hospital, a public, skilled nursing facility. The program had three goals: (1) to provide dental students and residents an opportunity to provide oral health care for adults who were frail and medically compromised who could not come into the clinics, (2) to increase students' access to patients who needed removable prosthodontics, and (3) to fulfill Pacific's commitment to public service. Laguna Honda and Pacific pooled their resources to bring comprehensive dental care to patients who were not able to access the dental school clinics. The long-term goals are to restore and maintain the oral health of those who reside in the facility, and to educate future dentists to provide oral health care for similar populations.

STATEMENT OF PROBLEM: Recently, as patients'expectation and interest for esthetics are increasing, concerns of esthetic restoration for removable dentures as well as fixed prosthodontics are also increasing. And the color stability of artificial teeth will affect a long term success rate of the denture. But the stain or discoloration of these artificial teeth as well as denture resin has caused esthetic problem. PURPOSE: This study was designed to evaluate the influence on color stability of artificial teeth when soy sauce, red pepper paste and coffee which many Koreans have eaten were applied. MATERIAL AND METHODS: For artificial teeth type (Endura Anterio(R), Physio Duracross(R), Trubyte Biotone(R)) selected for the study, 10 specimens each were soaked into individual beakers of soy sauce, red pepper paste, coffee and distilled water. And L*, a*, b* value were measured for evaluation of the color difference (delta E*) with spectrophotometer on the 1 day, 1 week, 2 weeks, 4 weeks and 8 weeks after immersion. RESULTS: 1. delta E* value of artificial teeth which were soaked in soy sauce and coffee was various according to soaking periods. However there was significant difference between Trubyte Biotone(R) and Physio Duracross(R) in red pepper paste regardless of soaking period (P < .05). 2. Except for 8 weeks of Endura Anterio(R), 4 weeks and 8 weeks of Physio Duracross(R), artificial teeth soaked in red pepper paste regardless of the type had significant difference of _E* value compared with other groups (P < .05). 3. a*, b* value of Endura Anterio(R) and Trubyte Biotone(R) which were soaked in red pepper paste had significant difference compared with the value of other group (P < .05). CONCLUSION: Red pepper paste had the greatest effect on color difference of artificial tooth. Physio Duracross(R) showed relatively less color difference than Trubyte Biotone(R) and Endura Anterio(R). But as delta E* difference were all less than 3.3, these artificial resin teeth seemed appropriate for clinical use.

PATIENT: The patient was a 41-year-old woman. She had already been using removable partial dentures but more teeth needed to be extracted. She was offered comprehensive treatment in accordance with a curative program for patients expected to lose a number of teeth. DISCUSSION: Prosthetic treatment for a patient with severe periodontal disease succeeded in maintaining her dental mastication and esthetics. CONCLUSION: When deciding treatment order for patients in whom positive treatment is necessary for heavy periodontitis, treatment result satisfying the patient was obtained by planning prosthetic treatment to include maintenance of function and esthetic, as well as consideration of the patient's age, desires, and background.

STATEMENT OF PROBLEM: Over the past decade, increased demand for esthetically pleasing restorations has led to the development of all-ceramic systems. Recent reports suggest that the all-ceramic crowns have excellent physical properties, wear resistance, and color stability. In addition, numerous ceramics have excellent biocompatibility, a natural appearance, and improved physical bonding with resin composite luting agents. However, the brittle nature of ceramics has been a major factor in their restriction for universal usage. Functional occlusal loading can generate stress in the luting agent, and the stress distribution may be affected by the marginal geometry at the finish line. Tooth preparation for fixed prosthodontics requires a decision regarding the marginal configuration. The design dictates the shape and bulk of the all ceramic crowns and influences the fit at the margin. PURPOSE: The purpose of this study was to evaluate the stress distribution within marginal configurations of all- ceramic crowns (90-degree shoulder, 110-degree shoulder, 135-degree shoulder). MATERIAL AND METHODS: The force is applied from a direction of 45 degrees to the vertical tooth axis. Three-dimensional finite element analysis was selected to determine stress levels and distributions. RESULTS AND CONCLUSION: The result of stress level for the shoulder marginal configuration was more effective on stress distribution at 135-degree shoulder margin. But the stresses concentrated around at 135-degree shoulder margin. The stress decreased apically at the surface between cements and alumina core, and increased apically at the surface between alumina core and veneering porcelain.

AIM: To gain an overview of knowledge of local analgesia, pulpal therapy and restorative procedures in the primary dentition amongst dental undergraduate students (DS), dental therapy students (DTS), recently-qualified dentists (QD) and dental therapists (DT), working within a Dental Hospital setting. STUDY DESIGN: A survey undertaken at Dundee Dental Hospital and School, NHS Tayside, United Kingdom to determine current knowledge regarding the use of local analgesia, pulp anatomy and pulp therapy techniques in addition to restoration of primary teeth. RESULTS: Data were available for 24 individuals (DS: 5; QD: 6; DTS: 8; DT: 5). Deficiencies in knowledge regarding the maximum safe dose for local analgesia, pulp canal anatomy, pulp therapy medicaments and the preparation required prior to placement of a pre-formed metal crown were noted in both student and staff groups. CONCLUSIONS: The knowledge of basic dental procedures for children amongst a group of dental students, student dental therapists and recently qualified dentists and dental therapists, was found to be imperfect. These findings indicate that more research is needed on the educational procedures used in the transfer of such knowledge and skills. Deficiencies in knowledge were identified in all areas assessed. Courses should be designed at both the pre- and postgraduate level to address and avoid such gaps in knowledge.
