OBJECTIVES: To explore the acceptability of an optimised physiotherapy (OPTimisE) intervention for people with lateral elbow tendinopathy (LET) and feasibility of comparing it to usual care in a randomised controlled trial. DESIGN: Semistructured interviews, analysed using thematic analysis and mapped onto the COM-B model of behaviour change. SETTING: Conducted as part of the OPTimisE Pilot & Feasibility randomised controlled trial within physiotherapy departments in the United Kingdom National Health Service. PARTICIPANTS: 17 patients with LET (purposively sampled to provide representativeness based on age, sex, ethnicity, deprivation index and treatment allocation) and all 8 physiotherapists involved as treating clinicians or site principal investigators. RESULTS: Four themes were identified. First, participants reported the OPTimisE intervention as acceptable. Second, differences between the OPTimisE intervention and usual care were identified, including the use of an orthosis, holistic advice/education including modifiable risk factors, forearm stretches, general upper body strengthening and a more prescriptive exercise-dosing regimen. Third, participants provided feedback related to the trial resources, which were viewed positively, but identified language translation as a need. Fourth, feedback related to trial processes identified the need for changes to outcome collection and reduction of administrative burden. From the perspective of adopting the OPTimisE intervention, we found evidence that participants were able to change their behaviour. Considering the findings through the lens of the COM-B model, the intervention is likely to be deliverable in practice and the trial can be delivered at scale with some additional support for physiotherapists. CONCLUSIONS: Overall, the OPTimisE intervention was found to be different to usual care and acceptable to patients and physiotherapists. The study highlighted the need to refine trial processes and resources prior to a full-scale trial, to reduce administrative burden, increase support for physiotherapists, improve return rate of outcome questionnaires and provide language translation. OBJECTIVE: The aim of the study was to compare the effects of forearm counter force brace (FCB) and kinesio taping (KT) on pain severity, grip strength and functionality of patients with lateral elbow tendinopathy (LET). METHODS: The study was planned as a prospective, randomized and assessor-blinded study with 1-month follow-up period. Seventy-two patients, diagnosed as LET were randomly assigned to FCB (n = 41) or KT (n = 31) groups. In the FCB group, the patients were informed and instructed to wear the brace for three weeks continuously. In the KT group, tape was applied once a week for four weeks with muscle inhibition and fascia correction techniques. The outcome measures were pain pressure threshold (PPT), maximal pain-free hand grip strength measurement and patient-rated tennis elbow evaluation questionnaire (PRTEE). The assessments were performed at the baseline, immediately after treatment and one month later after treatment. RESULTS: PPT and grip strength were significantly increased over time in both groups. Pain, function and total scores of PRTEE were significantly decreased in both FCB and KT groups. The effect size of the improvement in PRTEE function score was within acceptable clinical significance in the KT group. However, there was no significant difference between groups. CONCLUSIONS: Pain severity, grip strength and functionality of patients with LET improved over time in both FCB and KT groups. However, neither was superior in the management of LET.

INTRODUCTION: Lateral elbow tendinopathy (LET) is one of the most prevalent work-related musculoskeletal conditions. Current treatments for LET focus mainly on the physiological symptoms of pain, grip strength, and function. Recently, a novel workplace-based hand therapy educational intervention, Working Hands-ED, was developed based on the Person-Environment-Occupation- Performance model, International Classification of Functioning, Disability and Health framework and the Australian Clinical Framework for the Delivery of Health Services. Combining a hand therapist's specialised knowledge and skills in upper limb rehabilitation with an approach that considers injured workers' occupations and work environments may provide a more holistic approach to managing work-related LET. To the best of our knowledge, no previous studies have investigated the experiences of hand therapists who perform workplace-based educational interventions for the management of LET. METHOD: An exploratory, descriptive qualitative design using semi-structured interviews was used with hand therapists who delivered the novel hand therapy intervention Working Hands-ED. FINDINGS: Ten occupational therapists working in hand therapy were interviewed. Three main themes and eight subthemes were identified from interview data: Person-centred approach, opportunity for therapists to provide enhanced service, and improved stakeholder engagement in the return-to-work process. Logistical challenges such as the costs and time spent away from the clinical setting were identified. CONCLUSION: All hand therapists reported delivering Working Hands-ED when managing work-related LET was a positive experience for them. They believed that the novel intervention could provide a more holistic approach to care that added value to their service delivery; however, there were some logistical factors to consider including the additional time and costs associated with the intervention.

OBJECTIVES: There are a wide range of physiotherapy treatment options for people with lateral elbow tendinopathy (LET); however, previous studies have reported inconsistent approaches to treatment and a lack of evidence demonstrating clinical effectiveness. This study aimed to combine the best available research evidence with stakeholder perspectives to develop key components of an optimised physiotherapist-led treatment protocol for testing in a future randomised controlled trial (RCT). DESIGN: Online consensus groups using nominal group technique (NGT), a systematic approach to building consensus using structured multistage meetings. SETTING: UK National Health Service (NHS). PARTICIPANTS: 10 physiotherapists with special interest in LET, 2 physiotherapy service managers and 3 patients who had experienced LET. INTERVENTIONS: Two consensus groups were conducted; the first meeting focused on agreeing the types of interventions to be included in the optimised treatment protocol; the second meeting focused on specific details of intervention delivery. Participants were sent an evidence summary of available treatments for LET prior to the first meeting. All treatment options were discussed before anonymous voting and ranking of priority. Consensus for inclusion of each treatment option was set at ≥70% based on OMERACT guidelines. Options with 30%-69% agreement were discussed again, and a second vote was held, allowing for a change of opinion. RESULTS: The optimised physiotherapist-led treatment package included: advice and education, exercise therapy and orthotics. Specific components for each of these interventions were also agreed such as: condition-specific advice, health-promotion advice, exercise types, exercise into 'acceptable' levels of pain, exercise dosage and type of orthoses. Other treatment options including electrotherapy, acupuncture and manual therapy were excluded. CONCLUSION: An optimised physiotherapist-led treatment protocol for people with LET was successfully developed using an online NGT consensus approach. This intervention is now ready for testing in a future pilot/feasibility RCT to contribute much needed evidence about the treatment of LET. Introducción: La epicondilitis constituye uno de los motivos de consulta más frecuentes tanto en la asistencia primaria como especializada y sin duda alguna, es uno de los problemas que tiene mayor repercusión en la persona que la padece. El tratamiento de las epicondilitis constituye un reto para la medicina debido a enormes implicaciones sanitarias, sociolaborales y el dolor e impotencia funcional que provoca. Objetivo: Evaluar la efectividad del lisado plaquetario autólogo como alternativa de tratamiento en pacientes enfermos con epicondilitis. Método: Se realizó un estudio cuasi experimental analítico longitudinal prospectivo en el que se evaluó el uso de lisado plaquetario autólogo como alternativa de tratamiento en pacientes con epicondilitis. El universo estuvo constituido por los pacientes que acudieron a consulta de Ortopedia y traumatología con el diagnóstico de epicondilitis, durante el periodo comprendido entre octubre de 2014 y julio de 2018. La muestra quedo constituida por 80 pacientes que cumplieron con los criterios de inclusión y exclusión. Resultados: El grupo de edad entre 36-56 años y del sexo femenino son los de mayor representación en padecer esta enfermedad. Las infiltraciones de lisado plaquetario autólogo aportan mejores resultados al convencional y se observa la mayor representación de pacientes que tuvieron una remisión total. Las complicaciones fueron mucho más evidentes en el tratamiento convencional. También es relevante el costo-beneficio del tratamiento con lisado plaquetario autólogo. Conclusiones: El tratamiento con lisado plaquetario autólogo puede ser una alternativa para mejorar la calidad de vida de los pacientes con epicondilitis(AU)


Introduction: Epicondylitis is one of the most frequent reasons for attending consultation in both primary and specialized care; while it is undoubtedly one of the problems with the greatest impact on the person who suffers from it. The managment epicondylitis is a challenge for medicine, due to the enormous health-related and social implications, as well as the pain and functional impotence that it causes. Objective: To assess the effectiveness of autologous platelet lysate as a treatment alternative in patients with epicondylitis. Method: A prospective, longitudinal, analytical and quasiexperimental study was carried out, in which the use of autologous platelet lysate as an alternative treatment in patients with epicondylitis was assessed. The universe consisted of patients who attended the orthopedics and traumatology consultation, during the period between October 2014 and July 2018, with a diagnosis of epicondylitis. The sample was made up of eighty patients who met the inclusion criteria; exclusion criteria were also considered. Results: The age group between 36 and 56 years, together with the female sex, are the most represented with respect to suffering from this disease. Infiltrations of autologous platelet lysate provide better outcomes than the conventional one, while greater representation of remitted patients is observed. Complications were much more evident in conventional treatment. The cost-benefit relationship of treatment with autologous platelet lysate is also relevant. Conclusions: Treatment with autologous platelet lysate can be an alternative to improve the quality of life of patients with epicondylitis(AU)

CONTEXT: There is a lack of consensus on the best management approach for lateral elbow tendinopathy (LET). Recently, scapular stabilizer strength impairments have been found in individuals with LET. OBJECTIVE: The purpose of this study was to compare the effectiveness of local therapy (LT) treatment to LT treatment plus a scapular muscle-strengthening (LT + SMS) program in patients diagnosed with LET. DESIGN: Prospective randomized clinical trial. SETTING: Multisite outpatient physical therapy. PATIENTS: Thirty-two individuals with LET who met the criteria were randomized to LT or LT + SMS. INTERVENTIONS: Both groups received education, a nonarticulating forearm orthosis, therapeutic exercise, manual therapy, and thermal modalities as needed. Additionally, the LT + SMS group received SMS exercises. MAIN OUTCOME MEASURE: The primary outcome measure was the patient-rated tennis elbow evaluation; secondary outcomes included global rating of change (GROC), grip strength, and periscapular muscle strength. Outcomes were reassessed at discharge, 6, and 12 months from discharge. Linear mixed-effect models were used to analyze the differences between groups over time for each outcome measure. RESULTS: The average duration of symptoms was 10.2 (16.1) months, and the average total number of visits was 8.0 (2.2) for both groups. There were no significant differences in gender, age, average visits, weight, or height between groups at baseline (P > .05). No statistical between-group differences were found for any of the outcome measures. There were significant within-group improvements in all outcome measures from baseline to all follow-up points (P < .05). CONCLUSION: The results of this pilot study suggest that both treatment approaches were equally effective in reducing pain, improving function, and increasing grip strength at discharge as well as the 6- and 12-month follow-ups. Our multimodal treatment programs were effective at reducing pain and improving function up to 1 year after treatment in a general population of individuals with LET.

BACKGROUND: Although image-guided interventional procedures of the elbow and wrist are routinely performed, there is poor evidence in the literature concerning such treatments. Our aim was to perform a Delphi-based consensus on published evidence on image-guided interventional procedures around the elbow and wrist and provide clinical indications on this topic. METHODS: A board of 45 experts in image-guided interventional musculoskeletal procedures from the European Society of Musculoskeletal Radiology were involved in this Delphi-based consensus study. All panelists reviewed and scored published papers on image-guided interventional procedures around the elbow and wrist updated to September 2018 according to the Oxford Centre for Evidence-based Medicine levels of evidence. Consensus on statements drafted by the panelists about clinical indications was considered as "strong" when more than 95% of panelists agreed and as "broad" if more than 80% agreed. RESULTS: Eighteen statements were drafted, 12 about tendon procedures and 6 about intra-articular procedures. Only statement #15 reached the highest level of evidence (ultrasound-guided steroid wrist injections result in greater pain reduction and greater likelihood of attaining clinically important improvement). Seventeen statements received strong consensus (94%), while one received broad consensus (6%). CONCLUSIONS: There is still poor evidence in published papers on image-guided interventional procedures around the elbow and wrist. A strong consensus has been achieved in 17/18 (94%) statements provided by the panel on clinical indications. Large prospective randomized trials are needed to better define the role of these procedures in clinical practice. KEY POINTS: • The panel provided 18 evidence-based statements on clinical indications of image-guided interventional procedures around the elbow and wrist. • Only statement #15 reached the highest level of evidence: ultrasound-guided steroid wrist injections result in greater pain reduction and greater likelihood of attaining clinically important improvement. • Seventeen statements received strong consensus (94%), while broad consensus was obtained by 1 statement (6%).

Inflammation is associated with glenohumeral arthritis and rotator cuff tendon tears. Epigenetically, miRNAs tightly regulate various genes involved in the inflammatory response. Alterations in the expression profile of miRNAs and the elucidation of their target genes with respect to the pathophysiology could improve the understanding of their regulatory role and therapeutic potential. Here, we screened key miRNAs that mediate inflammation and linked with JAK2/STAT3 pathway with respect to the coincidence of glenohumeral arthritis in patients suffering from rotator cuff injury (RCI). Human resected long head of the biceps tendons were examined for miRNA profile from two groups of patients: Group 1 included the patients with glenohumeral arthritis and massive rotator cuff tears and the Group 2 patients did not have arthritis or rotator cuff tears. The miRNA profiling revealed that 235 miRNAs were highly altered (fold change less than -3 and greater than +2 were considered). Data from the NetworkAnalyst program revealed the involvement and interaction between 3,430 different genes associated with inflammation out of which 284 genes were associated with JAK2/STAT3 pathway and interconnect 120 different pathways of inflammation. Around 1,500 miRNAs were found to play regulatory role associated with these genes of inflammatory responses and 77 miRNAs were found to regulate more than 10 genes. Among them, 25 genes with less than tenfold change were taken to consideration which altogether constitute for the regulation of 102 genes. Targeting these miRNAs and the underlying regulatory mechanisms may advance our knowledge to develop promising therapies in the management of shoulder tendon pathology.

ABSTRACT OBJECTIVES: To evaluate the results of percutaneous golfer's elbow release under local anesthesia. METHODS: From December 2010 to December 2013, 34 elbows in 34 patients (10 males and 24 females) that presented golfer's elbow for over one year were recruited from the outpatient department. All patients were operated under local anesthesia and were followed-up for 12 months. The functional outcome was evaluated through the Mayo Elbow Performance Index (MEPI). RESULTS: Pain relief was achieved on average eight weeks after surgery. The results were excellent in 88.23% (30/34) cases and good in 11.76% (4/34) cases. Neither wound-related complications nor ulnar nerve complications were observed. On subjective evaluations, 88.23% (30/34) patients reported full satisfaction and 11.76% (4/34) patients reported partial satisfaction with the results of treatment. CONCLUSION: Percutaneous golfer's elbow release under local anesthesia is a minimally invasive procedure that can be performed in an outpatient setting. This procedure is easy, quick, and economical, presenting a low complication rate with good results.


RESUMO OBJETIVO: Avaliar os resultados da liberação percutânea do cotovelo de golfista sob anestesia local. MÉTODOS: Entre dezembro de 2010 e dezembro de 2013, 34 cotovelos em 34 pacientes (10 homens e 24 mulheres) que apresentavam cotovelo de golfista havia mais de um ano foram recrutados do ambulatório. Todos os pacientes foram operados sob anestesia local e foram acompanhados por 12 meses. O resultado funcional foi avaliado pelo Mayo Elbow Performance Index (MEPI). RESULTADOS: O alívio da dor foi alcançado em média oito semanas após a cirurgia. Os resultados foram excelentes em 88,23% (30/34) dos casos e bons em 11,76% (4/34) dos casos. Não se observaram complicações relacionadas à ferida nem complicações do nervo ulnar. Em avaliações subjetivas, 88,23% (30/34) dos pacientes relataram satisfação total e 11,76% (4/34) dos pacientes relataram satisfação parcial com os resultados do tratamento. CONCLUSÃO: A liberação percutânea do cotovelo de golfista sob anestesia local é um procedimento minimamente invasivo que pode ser feito em ambulatório. Esse procedimento é fácil, rápido e econômico, apresenta um baixo índice de complicações e bons resultados.

BACKGROUND: Traditional Chinese Medicine (TCM) uses a sophisticated diagnostic system called pattern differentiation, which allows for the tailor-made treatment of individuals. OBJECTIVE: This study aimed to identify the commonly used TCM patterns for lateral elbow pain (LEP), as well as the clinical features associated with these patterns. METHOD: Journal literature in 15 major Chinese and English databases and textbooks from five medical libraries as well as Chinamaxx were reviewed. RESULTS: Five hundred and fifty-five full-text journal articles, as well as 465 textbooks, were retrieved for further assessment, but only eight studies and seven books that mentioned LEP patterns could be identified and included in this study. The top four patterns that were identified were the wind-cold-dampness pattern, the qi stagnation and blood stasis pattern, the dual deficiency of qi and blood pattern, and the retained dampness-heat pattern, each of which was associated with unique features that were made up of distinct signs and symptoms. CONCLUSIONS: The unique features identified for each of the major patterns might be the key to understanding TCM individualized treatment based on pattern differentiation. Furthermore, the approach taken in studying TCM pattern of LEP may also be useful for studying the TCM patterns of other musculoskeletal conditions.

Se realizó una intervención terapéutica en 100 pacientes de 20-59 años, con epicondilitis humeral externa, atendidos en la Sala de Rehabilitación del Policlínico Comunitario Docente “Eduardo Mesa Llull”, del municipio de II Frente, de la provincia Santiago de Cuba, desde septiembre de 2013 hasta marzo de 2014, para evaluar la eficacia del tratamiento acupuntural en ellos. La muestra fue dividida en 2 grupos de 50 integrantes cada uno, seleccionados de forma aleatoria. Los del grupo de estudio fueron tratados con acupuntura más corriente Trabert y los del control con corriente Trabert solamente. Como principales resultados predominaron: el grupo etario de 40-49 años (64,0 %), el sexo femenino (66,0 %) y los obreros de servicio (56,0 %). Se obtuvo que los tratados con acupuntura tuvieron una mejor evolución, de donde se infiere que la terapia empleada resultó efectiva para aliviar el dolor en quienes se aplicó.


A therapeutic intervention in 100 patients with 20-59 years, presenting external humeral epicondylitis, assisted in the Rehabilitation Department of "Eduardo Mesa Llull" Community Polyclinic, in II Frente municipality, Santiago de Cuba province was carried out from September, 2013 to March, 2014, to evaluate the effectiveness of the acupunctural treatment in them. The sample was divided into 2 groups of 50 members each, randomly selected. Those of the study group were treated with acupuncture and Trabert current and those of the control group received Trabert current only. The age group 40-49 years (64.0%), the female sex (66.0%) and the service workers (56.0%) prevailed as main results. It was obtained that those treated with acupuncture had a better clinical course, from where it is inferred that the used therapy was effective to decrease pain in those who received it.

Se realizó un estudio experimental, de tipo intervención terapéutica, en 60 pacientes de ambos sexos, con epicondilitis humeral externa, atendidos en la Sala de Rehabilitación del Policlínico Comunitario Docente "Eduardo Mesa Llull", del municipio de II Frente, de la provincia Santiago de Cuba, desde noviembre de 2013 hasta igual mes de 2014, con vistas a evaluar la eficacia del tratamiento segmentario con corriente Trabert según variables de interés para la investigación. La muestra fue dividida en 2 grupos de 30 integrantes cada uno. Los del primero recibieron tratamiento segmentario; los del segundo, tratamiento a puntos de gatillo doloroso. Con la terapia aplicada se logró una recuperación total de 27 pacientes del grupo de estudio y 26 del control, de manera que la eficacia del tratamiento segmentario en la epicondilitis humeral externa es similar a la de gatillo doloroso.


An experimental study, of therapeutic intervention type was carried out in 60 patients of both sexes, with humeral, external epicondylitis assisted in the Rehabilitation Room of "Eduardo Mesa Llull" Teaching Community Polyclinic, from II Front municipality, of Santiago de Cuba province, from November, 2013 to same month of 2014, with the objective of evaluating the effectiveness of the segmental treatment with Trabert current according to variables of interest for the investigation. The sample was divided into 2 groups of 30 members each. Those of the first group received segmental treatment; those of the second group, were treated with painful trigger points. With the applied therapy a total recovery of 27 patients of the study group and 26 of the control was achieved, so that the effectiveness of the segmental treatment in the humeral external epicondylitis is similar to that of painful trigger.

Se realizó un estudio de intervención terapéutica con 32 pacientes seleccionados aleatoriamente de 36 que asistieron a consultas de ortopedia y traumatología en el Hospital Provincial Docente Clinicoquirúrgico "Saturnino Lora Torres" de Santiago de Cuba desde octubre de 2010 hasta junio de 2011 por presentar epicondilitis humeral, a fin de evaluar la efectividad de la terapia acupuntural en ellos. Se asignaron números pares para el grupo control e impares para el de estudio. La evaluación final de los pacientes tratados permitió afirmar que estos respondieron mejor al tratamiento acupuntural (en 14 de 16, para 88,0 %) y que aunque se presentaron algunas complicaciones, su eficacia supera con creces los procedimientos occidentales para alivio del dolor.


A therapeutical intervention study was carried out on 32 patients randomly selected of 36 who attended the orthopedics and traumatology departments in "Saturnino Lora Torres" Teaching Clinical Surgical Provincial Hospital in Santiago de Cuba from October, 2010 to June, 2011 for presenting humeral epicondylitis, in order to evaluate the effectiveness of the acupunture therapy in them. Pair numbers were assigned for the control group and odd numbers for the study. The final evaluation of patients treated allowed to affirm that they responded better to the acupuntural treatment (in 14 of 16, for 88.0%) and that although some complications were presented, their effectiveness overcomes the western procedures for the relief of pain.
