RESUMEN

OBJECTIVE: SARS-CoV-2 causes acute respiratory disease, interstitial and alveolar pneumonia, and involves numerous organs and systems such as the kidney, heart, digestive tract, blood, and nervous system. We aimed to evaluate the incidence of renal manifestations in patients diagnosed with COVID-19 infection. PATIENTS AND METHODS: We performed a monocentric, cross-sectional, observational study, conducted on 114 patients with SARS-CoV-2. Clinical and laboratory parameters [renal function, serum electrolytes, inflammatory state, blood gas analysis, Interleukin 6 (IL-6) and urinalysis] were evaluated. The same values were checked out after two months (T1), however after negativization. RESULTS: We enrolled 114 patients (59 males) with a mean age of 63.8 ± 13.9 years. We found hematuria in 48 patients (55.8%), proteinuria in 33 patients (38.4%), leukocyturia in 61 patients (70.9%), acute kidney injury (AKI) in 28 patients (24.6%), AKI in chronic kidney disease (CKD) in 24 patients (21.1%). Moreover, we found a significant increase of inflammatory indexes as C Reactive Protein (CRP), lactic dehydrogenase (LDH), alpha 1 and alpha 2 globulins with a subsequent reduction at T1 (p = 0.016, p < 0.001, p = 0.005, p = 0.007; respectively). Hemoglobin and erythrocyte values significantly decreased (p < 0.001, p = 0.003, respectively), and we found lymphopenia (p < 0.001). Also, we found elevated levels of the D-Dimer (p < 0.001) and a significant increase in the International Normalized Ratio (INR) (p = 0.038). We also showed a significant improvement after negativization in oxygen partial pressure (p = 0.001) and oxygen saturation (p < 0.001) and a significant increase in pH (p = 0.018) and bicarbonate concentration (p = 0.042). Moreover, we found a significant increase in IL-6 (p = 0.004). Also, we reported mild hyponatremia and hypokalemia with subsequent significant recovery (p < 0.001, p < 0.001, respectively) and mild hypochloremia with a recovery to the limits of statistical significance (p = 0.053). At the entrance, we found an increase in serum glucose with a significant reduction during recovery (p < 0.001). CONCLUSIONS: The prevalence of AKI and/or CKD and/or abnormal urinalysis in patients diagnosed with COVID-19 on admission seems to be high and appears as a negative prognostic factor. Urinalysis appears to be very useful in unveiling the potential kidney impairment of COVID-19 patients; therefore, urinalysis could be used to reflect and predict the disease severity. We also recommend a careful evaluation of metabolic alterations, inflammatory states, and electrolytic disorders in COVID-19 patients.

RESUMEN

We report an application of Kalman filtering to the inverted pendulum (IP) of the Virgo gravitational wave interferometer. Using subspace method system identification techniques, we calculated a linear mechanical model of Virgo IP from experimental transfer functions. We then developed a Kalman filter, based on the obtained state space representation, that estimates from open loop time domain data, the state variables of the system. This allows the observation (and eventually control) of every resonance mode of the IP mechanical structure independently.

In-vacuum Faraday isolators (FIs) are used in gravitational wave interferometers to prevent the disturbance caused by light reflected back to the input port from the interferometer itself. The efficiency of the optical isolation is becoming more critical with the increase of laser input power. An in-vacuum FI, used in a gravitational wave experiment (Virgo), has a 20 mm clear aperture and is illuminated by an almost 20 W incoming beam, having a diameter of about 5 mm. When going in vacuum at 10(-6) mbar, a degradation of the isolation exceeding 10 dB was observed. A remotely controlled system using a motorized lambda=2 waveplate inserted between the first polarizer and the Faraday rotator has proven its capability to restore the optical isolation to a value close to the one set up in air.

The monitoring of the residence time difference in bistable sensors has been recently proposed as a valid scheme for improving the detection capabilities of sensors as diverse as fluxgate magnetometers, ferroelectric sensors and mechanical sensors. In this paper we propose an approach to the residence time based detection strategy based on the measurement of the slope m of the sensor output integral. We demonstrate that such a method, far from degrading the detection performances can provide an easier way to realize fast and reliable sensors without the computationally demanding task related with the computation of the residence time difference. We introduce the receiver operating characteristic curve as a quantitative estimator for the comparison of the two methods and show that the detector performances increase with increasing the periodic bias amplitude A up to a maximum value. This condition has potentially relevant consequences in the future detectors design.

OBJECTIVE: To characterize the interaction between midintrared radiation of cutting lasers used or proposed for vitreoretinal surgery and fluid vitreous substitutes commonly used in vitreoretinal surgery. METHODS: Optical transmittance of vitreous substitutes was measured with a double-beam spectrophotometer. Measurements were performed in a wide spectral range of infrared radiation, including the 2120-nm wavelength of the holmium-YAG laser and the water absorption peaks at 1440, 1930, and 2940 nm. RESULTS: The wavelengths considered have a penetration depth varying from 410 to 1 microns in Ringer's solution, balanced salt citrate-buffered solution, balanced salt bicarbonated-buffered solution, hyaluronate sodium, and hydroxypropyl methylcellulose ophthalmic solution, from 2000 to 13 mm in perfluorocarbon liquid, and from 52 to 2.5 mm in silicone and fluorosilicone oils. CONCLUSIONS: Midinfrared optical radiation exhibits dramatic differences of penetration depth in different vitreous substitutes. High-absorbing liquids should be used mainly with contact laser procedures and could provide a shield for remote structures. Low-absorption vitreous substitutes allow noncontact laser surgical procedures, but they also may cause direct optical damage to remote tissues. The knowledge of wavelength transmittance of vitreous substitutes is necessary to evaluate and optimize the efficacy and safety of cutting laser sources.
