BACKGROUND: Acupuncture is widely used worldwide; however, studies on its effectiveness have been impeded by limitations regarding the design of appropriate control groups. In clinical research, noninvasive sham acupuncture techniques can only be applied through validation studies. Therefore, this systematic review aimed to evaluate the scope of existing literature on this topic to identify trends. METHODS: We queried Pubmed, EMBASE, and the Cochrane Central Register of Controlled Trials databases from inception to July 2022 for relevant articles. Author names were used to identify additional relevant articles. Two independent reviewers assessed the identified articles based on the inclusion and exclusion criteria. The following data were extracted: study design, information regarding acupuncturists and participants, general and treatment-related characteristics of the intervention and control groups, participants' experience of acupuncture, and research findings. RESULTS: The database query yielded 673 articles, of which 29 articles were included in the final review. Among these, 18 involved the use of one of three devices: Streitberger (n = 5), Park (n = 7), and Takakura (n = 6) devices. The remaining 11 studies used other devices, including self-developed needles. All the included studies were randomized controlled trials. The methodological details of the included studies were heterogeneous with respect to outcomes assessed, blinding, and results. CONCLUSIONS: Sham acupuncture validation studies have been conducted using healthy volunteers, with a focus on blind review and technological developments in sham acupuncture devices. However, theren may be language bias in our findings since we could not query Chinese and Japanese databases due to language barriers. There is a need for more efforts toward establishing control groups suitable for various acupuncture therapy interventions. Moreover, there is a need for more rigorous sham acupuncture validation studies, which could lead to higher-quality clinical studies.

Background: Several studies have assessed the safety and efficacy of acupuncture, but none have reviewed patent trends related to acupuncture as an intervention. This study analyzed trends in patents for the development of acupuncture-related technologies in Korea. Methods: The Korea Intellectual Property Rights Information Service (KIPRIS) was searched for information on acupuncture-related patents registered until August 11, 2021, without any limit on the search period. Only acupuncture and electro-acupuncture were included in this review. The current status, contents, and technological characteristics of the relevant patents were analyzed systematically to identify the overall status of acupuncture-related patents. Results: Overall, 126 patents were identified from the KIPRIS database, of which 93 were unrelated to the research topic and were excluded. Finally, 33 patents were assessed. Interest in the development of acupuncture technology has increased steadily over the past decade. Patent applications reached their peak in 2018 with six instances, while patent registration peaked in 2019 with seven instances. The interval between the application and registration of an individual patent was 14.3 months (449 days). Twenty-two acupuncture-related patents were for acupuncture (66.6%) and 11 for electro-acupuncture (33.3%). The technical specifications for the patents included acupuncture devices (30.3%), acupuncture manufacturing methods and equipment (36.4%), and electro-acupuncture devices (33.3%). Conclusion: Acupuncture-related technology currently focuses on technological advancements for the safe and effective delivery of stimulation. The findings demonstrate trends toward new diverse and complex technological advancements for acupuncture devices, manufacturing methods and equipment, and electro-acupuncture devices.

Background: : Post-stroke upper limb spasticity (PS-ULS) causes a decline in the quality of life of patients by reducing their ability to perform normal daily activities. Objectives: : We performed pre- and post-treatment analyses of the changes in stroke patients with ULS following constant electroacupuncture therapy (EAT). Methods: : Thirty patients with PS-ULS underwent 12 sessions of EAT over 4 weeks. After performing acupuncture therapy at four acupoints on the affected arm (LI11, LI10, LI4, and TE5), electrostimulation (60 Hz) was performed for 20 min. The modified Ashworth scale (MAS) and the Fugl-Meyer assessment of the upper extremity (FMA-UE) were measured as the outcome variables. Results: : Following EAT, there was a significant decrease in the elbow MAS score (p < 0.001), a significant decrease in the wrist MAS score (p < 0.01), and a significant increase in the FMA-UE score (p < 0.001). Adverse events related to EAT were not reported. Conclusion: : EAT decreased upper limb spasticity and improved functional recovery at the elbow and wrist. Large-scale and rigorous clinical trials are needed to verify the efficacy of EAT.

Mulberry fruits are rich sources of anthocyanins that exhibit beneficial biological activity. These anthocyanins become instable in an aqueous media, leading to their low bioavailability. In this study, a colloidal dispersion was produced by processing mulberry samples with hot-melt extrusion. In this process, hydrophilic polymer matrices were used to disperse the compound in an aqueous media. Mulberry samples were processed with hot-melt extrusion and in the presence of an ionization agent and sodium alginate to form mulberry-extrudate solid formulations. The particle size of mulberry-extrudate solid formulations decreased, while the total phenol content, the total anthocyanin content, and solubility increased. Fourier transform infrared spectroscopy (FT-IR) revealed that mulberry-extrudate solid formulations now contained new functional groups, such as -COOH group. We investigated whether mulberry-extrudate solid formulations had a positive impact on the stability of anthocyanins. The non-extrudate mulberry sample and mulberry-extrudate solid formulations were incubated with a simulated gastric fluid system and an intestinal fluid system. The number of released anthocyanins was determined with HPLC. We found that anthocyanins were released rapidly from non-extrudate mulberry extract. Mulberry-extrudate solid formulations contained a large number of available anthocyanins even after being incubated for 180 min in the intestinal fluid system. Thus, hot-melt extrusion enhanced water solubility and stability of anthocyanins with the prolonged release.

This study was carried out to develop a high-performance liquid chromatography method for short-time analysis of the main cannabinoids in the inflorescence of hemp (Cannabis sativa L.). We also performed decarboxylation of the raw material using our advanced analysis technique. In this study, the UV spectrum was considered to analyze each of the four common cannabinoids, solvents, and samples, where the uniform elution of acidic cannabinoids without peak tailing and acids was tested. Optimal results were obtained when readings were taken at a wavelength of 220 nm using water and methanol containing trifluoroacetic acid as mobile phases in a solvent gradient system. The established conditions were further validated by system suitability, linearity, precision, detection limit, and quantitation limit tests. The decarboxylation index (DT50) confirmed that Δ9-THCA decarboxylated faster than CBDA, and both maintained a linear relationship with time and temperature. In addition, the loss of cannabidiol was better prevented during the decarboxylation process in the natural state than in the extracted state.

[Purpose] The purpose of this study was to investigate interface pressure redistribution in healthy volunteers when applying different cushions and anterior wedge heights. [Subjects and Methods] This study included 36 healthy individuals in their 20s. The peak and mean pressures were measured by applying different cushions and anterior wedge heights. The results were analyzed by using a one-way analysis of variance and post-hoc analysis. [Results] The peak and mean pressures were statistically significant based on the cushion types and anterior wedge height. The peak pressure was at its highest and lowest when sitting on a 6-cm anterior wedge and a foam cushion, respectively. The mean pressure was greatest when sitting on a 6-cm anterior wedge of a firm surface and smallest when sitting on a 5 cm foam cushion. [Conclusion] This study shows that the most effective method for pressure redistribution was sitting on a 5 cm foam cushion without an anterior wedge.

[Purpose] The aim of this study was to investigate the effect of an intensive sensorimotor stimulation program on the motor function of chronic hemiparetic patients. [Subjects and Methods] The subjects were three chronic stroke patients whose sensory function was intact, who had Mini-Mental State Examination - Korean version scores of more than 26, and manual muscle test scores of more than fair for affected shoulder and elbow. The research design was an A-B single subject experimental design. The intervention consisted of 4 baselines phase sessions, and 12 sensorimotor stimulation phase sessions. The sensory and motor stimulation was performed for 30 minutes per session. The efficacy of the program was evaluated by the Box and Block test, and the 10-second test. [Results] Box and Block test and 10-second test scores of each subject improved after the 8 weeks intervention. [Conclusion] The intensive sensorimotor stimulation program for the upper extremity may be an efficacious method for improving the function of the affected limb of chronic stroke patients.

BACKGROUND: In 2009, there was an influenza pandemic in South Korea. The aim of this study was to evaluate the epidemiological, clinical and laboratory characteristics of this infection in children and adults. METHODS: We evaluated the epidemiologic characteristics of patients infected with the 2009 H1N1 influenza A virus (4,463 patients, age range from 2 mo to 86 y), and the clinical and laboratory findings of 373 inpatients (80/217 children, ≤ 15 y, had pneumonia and 36/156 adults, > 16 y, had pneumonia) in a single hospital during the epidemic. RESULTS: The majority of infected patients (94%) were less than 40 y, and greater than 90% of cases occurred during a two-month period. The rates of admission and pneumonia were 8.4% (373/4,463) and 2.5% (116/4,463), respectively. The rates of admission and pneumonia, total duration of fever, the frequency of underlying diseases, and the values of C-reactive protein and erythrocyte sedimentation rate tended to increase as age increased; highest rates were found in the ≥ 65 y group. Pneumonia was founded more boys than girls in children, but more female than male in adults. The adult patients with pneumonia had higher leukocyte counts with lower lymphocyte differentials than the group without pneumonia, as shown in children group. CONCLUSION: Our results suggest that the immunologic reaction to viral insults may be associated with age, sex and underlying diseases, and that unknown herd immunity may affect populations. The patients with underlying diseases, especially in older patients may have immunologic insufficiency that is associated with immunologic consumption by the underlying diseases.
