RESUMEN

210 sick dontogenous maxillary sinusitis were studied. To 150 patients surgical treatment with use of a method of a osteo-plastic sinusotomy of a genyantrum is lead. Patients have been taken on a dispensary observation. According to the lead postoperative computer tomography of additional sinuses of a nose in two projections, and also ortopantomography, rentgenographies of sinuses in semiaxial projection, the pneumatization of the operated genyantrums has not been disturbed, the osteal forward wall has been kept on all extent, was absent retraction soft tissues of infraorbital area, that as a whole speaks about efficiency and not so traumaticaly the lead surgical treatment. The offered way of surgical treatment promotes to the greatest degree to conservation of anatomic structures of maxillofacial area, possessing the least traumatism in comparison with other ways. This technique is simple, effective and does not demand application of the expensive equipment.

RESUMEN

Results of rapid laser-assisted identification of microorganisms for diagnostics of microbial processes based on auto-fluorescence effect in bacteria-containing materials are summarized. It is proposed to use the auto-fluorescence technique for express diagnostics of pyoinflammatory diseases, evaluation of microflora conditions (eubiosis, dysbiosis) and sensitivity to antibiotics, monitoring and prognostication, assessment of the quality of antibiotic therapy. Priority in the development of this medical technology for laserofluorescent diagnostics and its practical application is protected by 15 patents.

RESUMEN

The problem of medical rehabilitation of patients with unilateral upper and lower macrognathia is discussed. A method of osteo-reconstructive intervention on the facial skull is described, which restores the proper relationships between the jaw bones with respect to the supraorbital plane, which serves as the reference when planning surgical treatment. A comprehensive approach to planning and realization of surgical treatment brings about good esthetic and functional results.

RESUMEN

The problem of medical rehabilitation of patients with asymmetrical deformation of the facial skull, complicated by cross occlusion, is discussed. The authors suggest osteotomy of the maxillary complex with translocation of the osteotomied fragment in the sagittal, transverse, and vertical planes with osteotomy to a distance needed for elimination of deep cross occlusion. This method in combination with orthodontic treatment repairs the anatomic and functional abnormalities and brings about good esthetic results.

RESUMEN

Observations of 31 patients aged 16-31 years (11 men and 20 women) with asymmetrical maxillary deformations showed that medical rehabilitation of this patient population requires a comprehensive orthodontic and surgical approach. Such an approach leads to good esthetic and functional results without relapses of the deformation. A modified method of maxillary osteotomy was used, taking into consideration the clinical manifestations of asymmetrical deformation. Effective surgical treatment implies thorough planning of the volume of intervention and reliable fixation of osteotomied maxillary complex, which helps rule out secondary deformations of facial skull. Purposeful timely orthodontic treatment essentially accelerates medical rehabilitation.

RESUMEN

Unilateral subcondylar vertical osteotomy of the mandibular ramus was performed in 26 patients with asymmetric mandibular prognathism (AMP). 16 of 26 patients had temporomandibular joint (TMJ) symptoms before surgery. Asymmetric mandibular prognathism was combined with maxillary micro-retrognathism in 15 patients. Condyle position was evaluated by pre- and postoperative computed tomography (CT) data. Unlike mandibular sagittal split osteotomy with rigid fixation, the vertical subcondylar osteotomy with wire osteosyntheses allows to keep preoperative condylar head position on the side of hypertrophy and thus to prevent and eliminate TMJ symptoms postoperatively. Wire osteosynthesis promotes the most complete mechanism of adaptation and self-regulation of TMJ elements in surgical treatment of patients with asymmetric mandibular prognathism. All 26 patients had no TMJ symptoms postoperatively.

RESUMEN

Prevention of postoperative deformation of the nose after bone repair surgery (transposition of the maxillary complex forward) is an important problem. The proposed method for osteotomy of the maxillary complex prevents or minimizes the postoperative deformation of the nasal cartilage after transposition of the maxillary complex. The method was used with good results in 35 patients with upper microretrognathia.

RESUMEN

The temporomandibular joint has been examined in patients with combined deformations of the jaws (upper micro- or retrognathia and lower macro- or prognathism) before and after surgical treatment of the maxilla and mandible. Analysis of changes in the temporomandibular joint at all stages of medical rehabilitation showed that structural changes develop only in 8% patients only after planar osteotomy of the mandible through external or oral approach.

RESUMEN

The results of treatment of patients with congenital combined deformations of the jaws are assessed after 6 months to 8 years. The results are analyzed for 96 patients with maxillary micro- or retrognathia and mandibular macro- or prognathia, maxillary macro- or prognathia, and mandibular micro- or retrognathia. Remote results were assessed by clinical, x-ray, and functional methods of examination. Stable esthetic and functional results were attained in all patients, which were observed during both immediate and remote periods of follow-up. No signs of deformation relapses were observed in any case.

RESUMEN

Forty-six patients with combined deformations of the jaws were subjected to surgical treatment. Formalin-treated bone allotransplants together with wire splints were found to play the retention and fixing role for a year after surgery. During this period complete consolidation of osteotomied fragment of the maxillary complex takes place. No inflammations at the site of intervention were observed during the immediate and late periods after the operation. Formalin-treated bone allograft formed from tibial or femoral bone is resistant to infection.

RESUMEN

The anthropometric and anatomical features of the pterygopalatine fossa were studied in 63 skulls of subjects belonging to 3 age groups, 60 roentgenograms of subjects without deformations of the facial skull were analyzed for the same purpose, and 50 patients with various deformations of the facial and cerebral skull were examined. The authors have detected a relationship between the length and width of the pterygopalatine fossa and the size of the posterior facial height, which fact should be born in mind to prevent the complications of bone reconstructive surgery in the middle zone of the facial skull.

The suggested modified technique for assessing the tongue position in the oral cavity is based on teleroentgenography in the lateral projection and estimation of cephalometric parameters. Theoretical rationale is given for the use of skeletal check points in the mandibular area and cervical portion of the spine and of the mandibular and spinal reference lines. Indications for the use of this method are defined and its advantages described.

A new method for x-ray diagnosis of nasopharyngeal size is based on the use of long-distance roentgenography in the lateral projection and of skeletal markers to estimate the cephalometric parameters. The authors draw attention to the usefulness of measuring the size of bone base, soft-tissue structures, and nasopharyngeal air column when assessing the function of the nasopharynx and throat.
