RESUMEN

Blood thyroid hormones were measured before and after the rationed exercise test in 79 patients with angiographically verified diagnosis of coronary heart disease. A relationship was demonstrated between postexercise blood T3 variation and functional coronary circulation reserve. Hormonal baselines, and exercise-induced blood T4 changes were basically similar in patients with coronary disease of different severity. The results are suggestive of pathogenetic importance of thyroid function in these patients.

RESUMEN

A difference between baseline and final plasma hormonal (insulin, cortisol, somatotrophic hormone, triiodothyronine (T3) and thyronine (T4) levels, related to the pattern of response to physical stress, was demonstrated in 67 coronary patients with angiographically verified diagnosis. Post-exercise increase in these hormones was associated with their lowered baseline and elevated final blood levels, as compared to those patients who showed a decrease in hormonal levels in response to exercise, with the baselines exceeding final values.

During surgical correction of Fallot's tetrad the authors discovered the decrease of macroergic phosphate concentration and moderately marked signs of cardiomyocytic ultrastructural changes, such as mitochondrial injuries, decrease of glycogen levels, insignificant intracellular edema, no increase of membrane penetration. Changes of endothelial cells were absent. In children not over 1 year old under myocardial protection with hypothermic perfusion (at low volume rates) some decrease of macroergic phosphates occurred, as well as a moderate decrease of the mitochondrial index, glycogen content decrease, there was no intracellular edema. The discovered structural changes are minimal and reversible. A close correlation was found between the electron microscopic data and the character of myocardial energetic metabolism, the biochemical changes preceding the ultrastructural ones. Comparison of morpho- and biochemical changes in the myocardium of the majority of patients with impaired myocardial function confirmed the existence of a complex relationship between them.

RESUMEN

Plasma insulin, cortisol and somatotropic hormone (STH) levels of coronary patients are related to the extent of atherosclerotic lesion of coronary arteries. The relationship between coronary atherosclerosis and hormonal activity becomes particularly evident through the pattern of hormonal response (a rise or a fall) to rationed exercise. An elevated insulin baseline and reduced postexercise levels are typical for coronary patients with only slightly narrowed (25% or less) coronary arteries. A tendency to higher baseline and postexercise plasma cortisol levels was noted in more severe coronary cases. Increased postexercise plasma STH levels were significantly more frequent in patients with less marked atherosclerotic lesions of coronary arteries.

Sixty-seven patients with angiographically-confirmed coronary heart disease (CHD) were investigated. Three patterns of thyroid hormone (T3 and T4) response to exercise were identified: rising, falling and unchanged levels. Coronary patients with rising T3 levels in response to exercise typically showed smaller left-ventricular ejection values as compared to those patients whose T3 levels declined. Coronary patients with low baseline T3 tended to have elevated left-ventricular end diastolic pressure after exercise. The demonstrated changes in plasma thyroid hormone levels are related to the severity of coronary disease and can be "protective" or, on the contrary, aggravating. Because of smaller biologic activity, T4 does not basically affect clinical manifestations of CHD.

Fifteen patients with idiopathic hyperaldosteronism were examined. Ten patients were subjected to surgery with 9 of them having an operation of the "portalization" of the renal and adrenal venous blood flow in combination with contralateral adrenalectomy aimed at inactivating the hormone of the adrenal cortex in the liver. One patient had unilateral adrenalectomy in combination with the incision of the celiac ganglions. There were no lethal outcomes following surgery. Postoperationally, all patients were followed up from 3 months to 2.5 years. In 78% of the cases the results of surgery were assessed as good, in 22% the arterial pressure decreased insignificantly although it became more sensitive to hypotensive therapy. The results obtained indicate the efficacy of surgery for the "portalization" of the renal and adrenal venous blood flow in conjunction with contralateral adrenalectomy in patients with idiopathic hyperaldosteronism.

RESUMEN

Twenty-one patients with severe arterial hypertension were operated on using an original procedure, which consisted in creating a left-side renoportal venous anastomosis and right-side adrenalectomy. A considerable hypotensive effect was noted at both early and late follow-up. A drop in vascular peripheral resistance as a result of decreased blood aldosterone and cortisol levels as well as sodium elimination was found to be the principal mechanism of the hypotensive effect.

RESUMEN

The important role of toxemia in the pathogenesis of cadiogenic shock was demonstrated in 38 experiments on dogs. It was found that the blood becomes toxic later than the lymph. Lymphosorption arrests the progress of toxemia but does not relieve it if it has already developed. The hemosorption method produced a good effect. Studies showed, however, that it does nor prevent the arrival of lymph with a high toxin content. The best effect was produced when lympho- and hemosorption were combined under conditions of stimulation of lymph production and lymph drainage.

RESUMEN

Analysis of data obtained in the study of the blood enzymatic and isoenzymatic spectra (creatine phosphokinase and lactate dehydrogenase with simulatenous study of aspartate and alanine transaminases) in patients with various forms of ischemic heart disease showed the close correlation of the studied tests with the clinical picture of the disease, the degree and depth of myocardial involvement. The enzymatic tests studied are of diagnostic and prognostic importance and may be used in the differential diagnosis of various forms of ischemic heart disease.

RESUMEN

Action of nicotinic acid (NA) on components of fibrinolysis in vitro and in vivo and on fibrinolytic activity in vivo was studied. It is shown that NA represses action of inhibitors as well as of fibrinolysis activators. Interaction of NA with plasma in vitro is associated with enhanced fibrinolytic activity on account of inhibition of antiplasmins. Action of NA of fibrinolytic activity of blood during intravenous administration depends on dose. Small doses of NA induce slight activation of fibrinolysis due to binding of antiplasmins. Administration of high doses of NA induces activation of fibrinolysis through thrombogenesis involving central nervous system.
