RESUMEN

Background: The coexistence of heart failure and diabetes is prevalent, particularly in Intensive Care Units (ICU). However, the relationship between the triglyceride-glucose (TyG) index, heart failure, diabetes, and the length of hospital stay (LHS) in patients with cerebrovascular disease in the ICU remains uncertain. This study aims to investigate the association between the TyG index and LHS in patients with heart failure and diabetes. Methods: This retrospective study utilized the Medical Information Mart for Intensive Care (MIMIC)-IV database to analyze patients with diabetes and heart failure. Participants were categorized into quartiles based on the TyG index, and the primary outcome was LHS. The association between the TyG index at ICU admission and LHS was examined through multivariable logistic regression models, restricted cubic spline regression, and subgroup analysis. Results: The study included 635 patients with concurrent diabetes and heart failure. The fully adjusted model demonstrated a positive association between the TyG index and LHS. As a tertile variable (Q2 and Q3 vs Q1), the beta (ß) values were 0.88 and 2.04, with a 95% confidence interval (95%CI) of -0.68 to 2.44 and 0.33 to 3.74, respectively. As a continuous variable, per 1 unit increment, the ß (95% CI) was 1.13 (0.18 to 2.08). The TyG index's relationship with LHS showed linearity (non-linear p = 0.751). Stratified analyses further confirmed the robustness of this correlation. Conclusion: The TyG index exhibited a linearly positive association with the LHS in patients with both heart failure and diabetes. Nevertheless, prospective, randomized, controlled studies are imperative to substantiate and validate the findings presented in this investigation.

To understand the effects of different fertilizer applications on soil quality and arbuscular mycorrhizal colonization, we examined the changes in soil physical and chemical properties, mycorrhizal colonization and propagules, and their relationships in citrus under inorganic fertilization (IF), organic fertilization (OF), combined organic and inorganic fertilization (CF), and no fertilization (CK) treatments. Results showed that all fertilization treatments improved the content of rhizospheric soil organic carbon (SOC), nutrient contents, and electrical conductivity (EC). Both CF and OF significantly increased soil pH, soil aggregate stability, activities of urease, catalase, and sucrase, and the colonization and reproduction of arbuscular mycorrhizae fungi (AMF) in citrus rhizosphere. However, IF treatment significantly decreased soil pH and the colonization and reproduction of AMF in citrus rhizosphere. The number of mycorrhizal colonization and propagation was positively correlated with soil aggregate stability, SOC content, total nitrogen content, total phosphorus content, urease activity, and soil pH. Combined with the principal component analysis, we concluded that application of inorganic fertilizer alone could cause soil acidification and inhibit AMF colonization in citrus orchards. In contrast, organic fertilizer combined with inorganic fertilizer in citrus could improve the soil quality and AMF colonization.

To improve and perfect the quality standards and propose recommendations for the revision of quality standards for Andrographis Herba and its processed slices in Chinese Pharmacopoeia(ChP)(2020 edition) based on the problems and limitations in ChP(2015 edition). TLC identification method with andrographolide and control herbs as references was established using silica gel G thin layer plate, with chloroform-methylbenzene-methanol(8∶1∶1) as developing solvent, and 10% sulfuric acid ethanol solution as colour-developing agent. This method has good reproducibility, strong specificity and high sensitivity. As compared with the original method in ChP 2015, this method has better development effect and clearer spots. Based on the previous research, a quantitative analysis of multi-components by single-marker(QAMS) method with andrographolide as the internal reference substance was developed to simultaneously determine the contents of 4 diterpene lactones: andrographolide(S), neoandrographolide(A), 14-deoxyandrographolide(B), and dehydroandrographolide(C). The relative correction factors of f_(A/S), f_(B/S), and f_(C/S) were determined as 1.12, 0.79, and 0.63, respectively. The relative retention time of t_(A/S), t_(B/S), and t_(C/S) was 1.95, 2.18, and 2.25, respectively. According to the content determination results in 46 batches of crude drugs and 38 batches of processed slices, it was stipulated that the total contents of 4 diterpene lactones should not be less than 1.5% and 1.2% in crude drugs and processed slices, respectively. As compared with the original method in ChP 2015, the present QAMS method could not only reduce the detection cost and improve the efficiency, but also can be used to evaluate the quality of Andrographis Herba and its processed slices more comprehensively and objectively. Diterpene lactones are generally recognized as the effective components in Andrographis Herba, and their contents in leaves were much higher than those in stems. However, almost all of the current commercial processed slices are processed from stems, so their quality is gene-rally poor and the efficacy is hard to be guaranteed. Therefore, the weight percentage of leaves should be added into the inspection items of the processed slices and it should not be less than 25%.

This study aims to establish an UPLC-DAD method for the simultaneous determination of nine active components in phenolic acids, anthocyanins, monoterpene glucosides and flavonoids in the petals of Paeonia lactiflora flowers from different cultivars and processing methods. UPLC analysis was performed on a Waters ACQUITY UPLC HSS T3 C_(18) column(2.1 mm × 100 mm, 1.8 µm)for gradient elution with a mobile phase consisting of 0.5% formic acid solution(A)-acetonitrile(B)at a flow rate of 0.3 mL·min~(-1).The detection wavelength was determined with a switched wavelength method and the column temperature maintained at 20 ℃, with an injection volume of 2 µL.The contents of the above components in the samples with different sources and treatment methods were obtained. The principal component analysis(PCA)of P. lactiflora flowers was conducted by using SIMCA 14.0 software, and the results showed that the P. lactiflora flowers could be classified into two categories according to cultivars, including P. lactiflora 'Bozhou-shaoyao' as one category with high contents of the above components, P. lactiflora 'Baihuachuanshaoyao' and P. lactiflora 'Honghuachuanshaoyao' as the other category. Dried P. lactiflora flowers could be classified into three categories, including baking-drying, sun-drying and shade-drying, with baking-drying method as the optimal one.The P. lactiflora flowers were not greatly affected by origins, growing altitudes, growing years, or blooming stages.The results of contents determination can be used to guide the cultivar selection, harvest and processing of P. lactiflora flowers.

The study was designed to observe the effects and relationship of exercise on chronotropic response (CR) and exercise capacity in patients with type 2 diabetes mellitus (T2DM). A total of 30 patients with T2DM underwent symptom-limited cardiopulmonary exercise testing (CPET) after excluding contraindication. For each subject individualized exercise prescription was formulated, and they received 12 weeks of exercise training after CPET retest to complete the comparison of CR indicators, including the ratio of maximum exercise heart rate to predicted maximum heart rate value (rHR), heart rate reserve rate (HRRes), heart rate recovery (HRR) of 1-6 min after exercise termination (HRR1-6), exercise capacity (peak VO2/kg) and other indicators. The results showed that after 12 weeks of exercise treatment, rHR, HRRes, HRR1-6, and peak VO2/kg were significantly higher than before (P<0.05), with peak VO2/kg being positively correlated to rHR and HRRes (P<0.01). In conclusion, exercise training can improve cardiac dysfunction, abnormal HRR, enhance exercise capacity and adaptability of the cardiovascular system to exercise stress in T2DM patients.

OBJECTIVE: To study the application of Geographic Information System (GIS) electronic fence technique in Oncomelania hupensis snail monitoring. METHODS: The electronic fence was set around the history and existing snail environments in the electronic map, the information about snail monitoring and controlling was linked to the electronic fence, and the snail monitoring information system was established on these bases. The monitoring information was input through the computer and smart phone. RESULTS: The electronic fence around the history and existing snail environments was set in the electronic map (Baidu map), and the snail monitoring information system and smart phone APP were established. The monitoring information was input and upload real-time, and the snail monitoring information was demonstrated in real time on Baidu map. CONCLUSIONS: By using the electronic fence technology based on GIS, the unique "environment electronic archives" for each snail monitoring environment can be established in the electronic map, and real-time, dynamic monitoring and visual management can be realized.

OBJECTIVE: To construct the Oncomelania hupensis snail monitoring system based on the Baidu Map. METHODS: The environmental basic information about historical snail environment and existing snail environment, etc. was collected with the monitoring data about different kinds of O. hupensis snails, and then the O. hupensis snail monitoring system was built. Geographic Information System (GIS) and the electronic fence technology and Application Program Interface (API) were applied to set up the electronic fence of the snail surveillance environments, and the electronic fence was connected to the database of the snail surveillance. RESULTS: The O. hupensis snail monitoring system based on the Baidu Map were built up, including three modules of O. hupensis Snail Monitoring Environmental Database, Dynamic Monitoring Platform and Electronic Map. The information about monitoring O. hupensis snails could be obtained through the computer and smartphone simultaneously. CONCLUSIONS: The O. hupensis snail monitoring system, which is based on Baidu Map, is a visible platform to follow the process of snailsearching and molluscaciding.

A series of processes have been developed to facilitate the rapid discovery of new promising thermoelectric alloys. A novel combinatorial facility where elements are wire-fed and laser-melted was designed and constructed. Different sample compositions can be achieved by feeding different element wires at specific rates. The composition of all the samples prepared was tested by energy dispersive X-ray spectroscopy (EDS). Then, their thermoelectric properties (power factor) at room temperature were screened in a specially designed new high-throughput setup. After the screening, the thermoelectric properties can be mapped with the possibility of identifying compositional trends. As a proof-of-concept, a promising thermoelectric ternary system, Al-Fe-Ti, has been identified, demonstrating the capability of this accelerated approach.

Previous studies have drawn attention to dendritic cell (DC) vaccines; particularly the application of the tumor-associated antigen-targeted DC vaccine. The present study analyzed DCs derived from a normal individual and pulsed the cells with heat shock protein 70 peptide (Hsp70) and/or hepatitis B virus x antigen (HBxAg), a hepatocellular carcinoma (HCC)-associated antigen. It was then investigated whether this method of vaccination induced strong therapeutic antitumor immunity. The results revealed that the Hsp70/HBxAg complex-activated phenotype improves the functional maturation of DCs compared with using Hsp70 or HBxAg alone. Compared with either Hsp70 or HBxAg alone, matured DCs pulsed with the Hsp70/HBxAg complex stimulated a high level of autologous T-cell proliferation and induced HCC-specific cytotoxic T lymphocytes, which specifically killed HCC cells through a major histocompatibility complex class I mechanism. These results indicated that a vaccination therapy using DCs co-pulsed with the Hsp70/HBxAg complex is an effective strategy for immunotherapy and may offer a useful approach to protect against HCC.

OBJECTIVE: To discuss a best predictive score index in predicting death in patients with severe trauma, death warning score, and to provide a simple score for clinical use. METHODS: The clinical data of 394 traumatic patients admitted to Department of Critical Care Medicine of Xi'nan Hospital of the Third Military Medical University, Daping Hospital of the Third Military Medical University, and Affiliated Hospital of Zunyi Medical College from January 2014 to December 2014 were retrospectively analyzed. The patients were divided into survival group (n = 330) and non-survival group (n = 64). The clinical data in two groups were recorded as following: gender, age; respiratory rate, heart rate, and systolic blood pressure at admission; the lowest values of serum creatinine (SCr), white blood cell count (WBC), platelet count (PLT), hematocrit (Hct), respectively, within 24 hours after admission; acute physiology and chronic health evaluation II (APACHE II) score, Glasgow coma scale (GCS) score, sequential organ failure assessment (SOFA), systemic inflammatory response syndrome (SIRS) score, injury severity score (ISS) within 24 hours of final diagnosis; the performance of emergency operation or intubation within 24 hours; incidence of sepsis, and clinical outcomes. Each observed indicator was analyzed by univariate analysis, and factors leading to death were further analyzed by logistic regression. Risk factors of severe trauma patients were sorted, from which the meaningful indicators were included to calculate the warning score of death. Receiver operating characteristic curve (ROC) was plotted to evaluate the predictive value of the warning score of death in severe trauma patients. RESULTS: Compared with the survival group, the age in non-survival group was older (years old: 51.50 ± 18.31 vs. 45.54 ± 14.70, t = -2.456, P = 0.016); SCr was increased (µmol/L: 94.18 ± 65.51 vs. 72.42 ± 28.22, t = -2.611, P = 0.011); APACHE II score (24.30 ± 6.23 vs. 16.38 ± 6.19, t = -9.353, P < 0.001) and SOFA score were higher (7.84 ± 3.68 vs. 4.43 ± 2.75, t = -7.049, P < 0.001); and the incidence of emergency intubation [79.7% (51/64) vs. 42.7% (141/330), χ² = 29.309, P < 0.001] and sepsis was also higher [48.4% (31/64) vs. 30.3% (100/330), χ² = 18.512, P < 0.001], but PLT count (X 109/L: 112.75 ± 59.85 vs. 144.12 ± 68.28, t = 3.428, P = 0.001) and GCS score (6.44 ± 4.20 vs. 11.02 ± 3.93, t = 8.449, P < 0.001) were significantly lower. There was no significant difference in gender, respiratory rate, heart rate, systolic blood pressure, WBC, Hct, SIRS score, ISS, or emergency operation between two groups. The indicators with statistically significant difference from the univariate analysis were further analyzed by multivariate logistic regression, and the indices included in the regression model were age ≥ 65 years [95% confidence interval (95%CI) = 0.176-1.974, P = 0.019], APACHE II score ≥ 21 (95% CI = 0.121-2.725, P = 0.032), GCS < 6 (95% CI = 0.201-3.221, P = 0.026), severe sepsis (95%CI = 0.421-2.735, P = 0.008) or septic shock (95%CI = 0.430-3.636, P = 0.013), with assigning scores of 1.0, 1.5, 1.5, 1.5, 2.0, respectively. Finally these five indicators were included into the warning score of death. It was shown by ROC curve analysis that the area under ROC curve (AUC) of warning score of death in predicting mortality in critically ill trauma patients was 0.867, which was significantly higher than that of the APACHE II score (AUC = 0.812, P = 0.022) and GCS score (AUC = 0.783, P = 0.001). When the cut-off value of warning score of death was 1.5, the sensitivity, specificity, positive predict value (+PV), negative predict value (-PV), positive likelihood ratio (+LR), negative likelihood ratio (-LR), and Youden index was 75.00%, 85.40%, 50.0%, 94.6%, 5.16, 0.29, and 0.605, respectively. CONCLUSIONS: Age ≥ 65 years, APACHE II score ≥ 21, GCS < 6, severe sepsis or septic shock were the risk factors of death in patients with severe trauma, and they can be considered as warning score of death in patients with severe trauma. With the score mortality can be better predicted than any other kind of score for patients with severe trauma.

Silicon is a promising alternative to current thermoelectric materials (Bi(2)Te(3)). Silicon nanoparticle based materials show especially low thermal conductivities due to their high number of interfaces, which increases the observed phonon scattering. The major obstacle with these materials is maintaining high electrical conductivity. Surface functionalization with phenylacetylene shows an electrical conductivity of 18.1 S m(-1) and Seebeck coefficient of 3228.8 µV K(-1) as well as maintaining a thermal conductivity of 0.1 W K(-1) m(-1). This gives a ZT of 0.6 at 300 K which is significant for a bulk silicon based material and is similar to that of other thermoelectric materials such as Mg(2)Si, PbTe and SiGe alloys.

RESUMEN

An apparatus capable of rapid measurement of the Seebeck coefficient and electrical resistivity at room temperature is reported. The novel aspect of this apparatus is the use of 4 multifunctional probes that comprise a junction of two conductors at the tip and serve as both thermocouples and electrical contacts. In addition, one of the probes has a built-in heater that can establish a temperature gradient in the sample for the Seebeck measurement. The technique does not require special sample geometries or preparation of contacts and is suitable for bulk and thin film materials. Together with automated sample stage and data acquisition, the equipment is able to measure both the Seebeck coefficient and electrical resistivity in less than 20 s with good accuracy. Less than 5% and 4% relative errors were found for the measurement of the Seebeck coefficient and electrical resistivity, respectively. This makes the apparatus especially useful for high throughput evaluation of thermoelectric materials.

An apparatus for measuring the Seebeck coefficient (α) and electrical resistivity (ρ) was designed to operate under an infrared microscope. A unique feature of this apparatus is its capability of measuring α and ρ of small-dimension (sub-millimeter) samples without the need for microfabrication. An essential part of this apparatus is a four-probe assembly that has one heated probe, which combines the hot probe technique with the Van der Pauw method for "simultaneous" measurements of the Seebeck coefficient and electrical resistivity. The repeatability of the apparatus was investigated over a temperature range of 40 °C-100 °C using a nickel plate as a standard reference. The results show that the apparatus has an uncertainty of ±4.9% for Seebeck coefficient and ±5.0% for electrical resistivity. The standard deviation of the apparatus against a nickel reference sample is -2.43 µVK(-1) (-12.5%) for the Seebeck coefficient and -0.4 µΩ cm (-4.6%) for the electrical resistivity, respectively.

AIM: S-1 is an oral anticancer fluoropyrimidine formulation consisting of tegafur, 5-chloro-2,4-dihydroxypyridine and potassium oxonate. The aim of this study was to evaluate the pharmacokinetics and bioequivalence of a newly developed generic formulation of S-1 in Chinese cancer patients in comparison with the branded reference formulation of S-1. METHODS: A single-dose, randomized-sequence, open-label, two-way self-crossover study was conducted in 30 Chinese cancer patients. The subjects alternatively received the two formulations (40 mg/m(2), po) with a 7-d interval. Plasma concentrations of FT, CDHP, Oxo, and 5-Fu were determined using LC-MS/MS. Pharmacokinetic parameters, including Cmax, Tmax, t1/2, AUC0-t, and AUC0-∞ were determined using non-compartmental models with DAS2.0 software. Bioequivalence of the two formulations were to be evaluated according to 90% CIs for the log-transformed ratios of AUC and Cmax of S-1. Adverse events were evaluated through monitoring the symptom, physical and laboratory examinations, ECGs and subject interviews. RESULTS: The mean values of Cmax, AUC0-t, and AUC0-∞ of FT, 5-Fu, CDHP, and Oxo for the two formulations had no significant differences. The 90% CIs for natural log-transformed ratios of Cmax, AUC0-t, and AUC0-∞ were within the predetermined bioequivalence acceptance limits. A total of 11 mild adverse events, including fatigue, nausea and vomiting, anorexia, diarrhea and myelosuppression, were observed, and no serious and special adverse events were found. CONCLUSION: The newly developed generic formulation and reference formulation of S-1 have similar pharmacokinetics with one dose (40 mg/m(2)) in Chinese cancer patients. Both the formulations of S-1 are well tolerated.

RESUMEN

The genus Oniella Matsumura 1912 is reviewed and seven species from China are recognized, including a new species, O. shaanxiana sp. nov., a new synonym, O. flavomarginata, syn. nov. and a new combination, O. albula (Cai & Shen, 1998) n. comb. transferred from Sophonia. O. excelsa (Melichar) is removed from this genus. The seven species are described and illustrated in detail, and a key to all species of the genus is provided.

OBJECTIVE: To understand the status of newly-developed advanced schistosomiasis patients in schistosome transmission-interrupted areas. METHODS: The newly diagnosed advanced schistosomiasis patients confirmed by professionals at province and county levels according to the available standards were investigated. Their detailed epidemiology history and present signs/ symptoms were surveyed, and the relevant medical examinations were performed. RESULTS: There were 75 patients including 22 males and 53 females. The average age was (70.0 +/- 7.0) years and they were all infected with schistosome previously. They received anti-schistosome therapy (mean 1.85 times) and stool examinations (mean 26.24 times) continuously from 1976 to 1985, and the results of stool examination were all negative. In majority of the patients' resident areas, the last time that Oncomelania hupensis snails were found was 1976, and the latest was 1983. During this period, no patients were up to the diagnostic criteria of advanced schistosomiasis after multiple examinations, and they were diagnosed as advanced cases between 2008 and 2011. The survey indicated that liver fibrosis was found in all the patients with 82.7% showing grad III pathological change, 90.7% of them had splenomegaly, 20.0% had ascites, and 22.7% had portal vein broadening. The mean value of enzyme indexes of liver function was 31.81 U/L (ALT), 53.19 U/L (AST) and 89.28 U/L (gamma-GT), and the positive rate was 21.3%, 49.3% and 57.3%, respectively. As for the fibrosis indexes, the positive rate of HA, LN, C VI and P III P was 73.3%, 13.3%, 17.3% and 9.3%, respectively, and the mean value of HA was 3 times higher than that of the upper limit of normal standard. Conclusions Newly-developed advanced schistosomiasis patients are those who were dignosed and cured parasitologically but progressed pathologically without any chance of re-infection. Anti-fibrosis therapy is necessary as the patients show considerable progressive liver fibrosis.

Bile acid-induced lung injury has become an important topic for neonatologists after the discovery of a high incidence of infant respiratory distress syndrome complicated from maternal intrahepatic cholestasis. To explore the molecular pathway of bile acid-induced lung injury, we investigated the cytotoxicity of the glycochenodeoxycholate (GCDC) to alveolar epithelial type II cells (AECII), as the main component of bile acid. The results demonstrated that glycochenodeoxycholate induced oxidative stress, mitochondrial damage, and increased caspase activity in the primary cultured AECII. Moreover, ROS scavengers and caspase inhibitors could rescue cell death induced by GCDC in rat AECII. Our results also indicated that GCDC inhibited AECII surfactant secretion. In conclusion, this study suggested that cell death prevention and cell therapy should be considered as therapeutic strategies for infant respiratory distress syndrome complicated from maternal intrahepatic cholestasis.

OBJECTIVE: To assess and analyze the risk and community treatment of hypertension in rural population of Changshu city, Jiangsu province. METHODS: A stratified cluster sampling technique was conducted to select a sample of 800 people, aged 35 - 74 years old, in a rural village according to the proportion of the national population in Changshu. Weight, height, blood pressure, serum lipid, blood glucose and other related factors were examined and analyzed. RESULTS: The prevalence rate of hypertension in the rural area was 32.01%, and significantly higher with the increase of age (P < 0.01). The average systolic blood pressure of hypertension in females was significantly positively correlated while, the average diastolic blood pressure of male and female hypertensives was significantly negatively correlated to age. The proportions of risk factors of cardiovascular diseases (CVD) as senility, hyperlipemia, premature familial history of CVD and overweight of hypertension were significantly high than those without hypertension (P < 0.01). The percentage of medium-low risk on hypertension was 85.72%, but of high risk was 14.28%. The ratio of using medication, exclusive non-medication, comprehensive treatment or total cure were 42.05%, 2.84%, 14.20% and 59.09%, accordingly. The rates of blood pressure control under the above approaches were 35.14%, 40.00%, 36.00% and 39.77%. CONCLUSION: Assessment on the risk of hypertension in the rural areas suggested that the priority should be given to medium-low risk groups, while the rate of medication and non-medicine intervention was at low level. It is necessary to strengthen a comprehensive intervention program for hypertension control.
