ABSTRACT Purpose To evaluate the efficacy of grafting a preseptal strip of orbicularis muscle to treat deep static glabellar wrinkles. Methods This was a prospective, interventional, case series study that included patients aged between 21 and 80, who underwent upper and lower blepharoplasty, and who presented varying degrees of deep static glabellar rhytids. After marking the glabellar rhytid, a subcision was made with 40 x 12mm needle along the entire length of the glabellar wrinkle. The orbicularis muscle graft was set into the recipient pocket already created in the glabella. The patients answered a self-assessment form about the results using the subject Global Aesthetic Improvement Scale after 6 months. Results Seventeen glabellar rhytid fillings were performed. As for the subjective evaluation of the result of the specific glabella filling procedure, the patients were distributed as follows: 12 rhytids (70.6%; 12 out of 17) were self-assessed as much better; 4 rhytids (23.6%; 3 out of 17) were self-assessed as better, and 1 rhytid (5.8%; 1 of 17) was self-assessed as unchanged. Conclusion This technique has excellent results, is easy to perform, has low morbidity and presents a high degree of satisfaction when patients evaluated themselves.


RESUMO Objetivo Avaliar a eficácia do enxerto de uma tira pré-septal do músculo orbicular no tratamento de rugas glabelares estáticas profundas. Métodos Trata-se de estudo prospectivo, intervencionista, do tipo série de casos, que incluiu pacientes com idade entre 21 e 80 anos, submetidos à blefaroplastia superior ou superior e inferior e que apresentavam graus variados de rítides glabelares estáticas profundas. Após marcação da rítide glabelar, foi feita subcisão com agulha 40x12mm em toda a extensão da ruga glabelar, criando uma porta de entrada para o enxerto e formando um leito receptor na hipoderme. O enxerto do músculo orbicular foi inserido no leito glabelar. Os pacientes foram acompanhados considerando pós-operatório mínimo de 6 meses. Eles responderam a um formulário de autoavaliação sobre os resultados por meio da Escala Global Aesthetic Improvement Scale. Resultados Foram realizados 17 enxertos de rítides glabelares (três pacientes submetidos ao procedimento bilateral). Quanto à avaliação subjetiva do resultado do procedimento, os pacientes foram distribuídos da seguinte forma: 12 rítides (70,6%; 12 de 17) autoavaliaram-se como muito melhores; 4 rítides (23,6%; 3 de 17) foram autoavaliadas como melhores e 1 rítide (5,8%; 1 de 17) foi autoavaliada como inalterada. Conclusão A presente técnica apresenta excelentes resultados, é de fácil execução, apresenta baixa morbidade e alto grau de satisfação na autoavaliação.

RESUMEN

Solitary fibrous tumor (SFT) is a rare mesenchymal spindle-cell neoplasm commonly found in the pleura; it is rare in the orbit and extremely rare in the lacrimal gland. We herein report a case of SFT of the lacrimal gland that mimicked a pleo morphic adenoma. We discuss the clinical, radiological, histopathological, and immunohistochemical findings that provided insight and rationale to accurately diagnose this case.

RESUMO Os autores relatam um caso de tumor fibroso solitário (TFS) de glândula lacrimal simulando um adenoma pleomórfico. O TFS é um raro tumor mesenquimal de células fusiformes, comumente encontrado na membrana pleural, raramente en contrado na órbita e extremamente raro na glândula lacrimal. Os autores discutem os achados clínicos, radiológicos, histológicos e imunohistoquímicos que são a chave para o correto diagnóstico desta rara entidade.

External dacryocystorhinostomy (DCR) is the gold standard surgical technique for the treatment of primary acquired nasolacrimal duct obstruction (PANDO). However, new techniques such as endoscopic DCR and transcanalicular dacryocystorhinostomy (T-DCR) are being studied in an attempt to reduce surgical time, avoid external scarring and preserve the lacrimal pump while achieving the same efficacy. The purpose of this study was to compare the efficacy between conventional T-DCR and modified transcanalicular dacryocystorhinostomy (MT-DCR) in patients with PANDO. MT-DCR is performed to remove nasal mucosa prior to laser osteotomy. This is a comparative, prospective, interventionist and randomized study. Patients with PANDO were selected to undergo MT-DCR or T-DCR by blocked randomization. PANDO was diagnosed based on clinical presentation, dye disappearance test and dacryocystography. All of the procedures were performed by the same surgery team members. Anatomical success outcome was defined as positive lacrimal syringing and functional success outcome was defined as the absence or improvement of epiphora. A total of 44 surgical procedures were performed (22 MT-DCR and 22 T-DCR). In the case of MT-DCR, the anatomical and functional success rates after 12 months were 90 and 86%, respectively. After T-DCR, these rates were 77 and 72%, respectively (p = 0.162). MT-DCR and T-DCR are both safe and fast procedures with low morbidity and well-tolerated.

Abstract Objective: To quantitatively and qualitatively evaluate postoperative outcomes and patient satisfaction after upper blepharoplasty and to correlate the findings with changes between preoperative and postoperative eyelid measurements using a digital imaging system. Methods: A total of 60 eyelids in 30 patients with dermatochalasis who were treated in the ambulatory center of the Department of Oculoplastic Surgery at the Anápolis Ophthalmology Hospital were evaluated. Patients ranged from 40 to 80 years of age. Photographs were taken before the upper blepharoplasty procedure and 90 days after as well. The images were transferred to the ImageJ 1.34n program. The parameters analyzed were palpebral fissure height in primary position and margin-crease distance. The correlations between these measurements and patient satisfaction 90 days after surgery were evaluated. Results: This study revealed an increase in the margin-crease distance after upper blepharoplasty and a high positive correlation (0.64) between the increase in this height and the level of satisfaction that the patients attributed to the surgery. There was no statistically significant difference between preoperative and postoperative palpebral fissure heights. Conclusion: The margin-crease distance may serve as a quantitative measurement of a good cosmetic and functional outcome, since it has been found to be strong correlated with patient satisfaction.


Resumo Objetivo: Avaliar de maneira quantitativa e qualitativa o resultado pós-operatório e a satisfação de pacientes submetidos à blefaroplastia superior e correlacionar com as medidas palpebrais antes e após a cirurgia utilizando o sistema de imagem digital. Métodos: Foram avaliadas 60 pálpebras de 30 pacientes com dermatocálase atendidos no ambulatório de Plástica Ocular do Hospital Oftalmológico de Anápolis, com idade entre 40 e 80 anos. Foram realizadas fotografias antes e 90 dias após blefaroplastia superior. Essas imagens foram transferidas para o programa Image J 1.34n e analisados os parâmetros de altura da fenda palpebral em posição primária do olhar e distância margem-sulco palpebral. Foram avaliadas as correlações dessas medidas com a satisfação do paciente após 90 dias de pós-operatório. Resultados: O estudo mostrou um aumento da distância margem-sulco palpebral após blefaroplastia superior e uma correlação fortemente positiva (0,64) entre o aumento dessa medida e a nota de avaliação atribuída pelo paciente à cirurgia. Não houve diferença estatisticamente significante na altura da fenda palpebral antes e após a cirurgia. Conclusão: A utilização da medida da distância margem-sulco pode servir como parâmetro quantitativo de um bom resultado estético e funcional, apresentando uma forte correlação com a satisfação dos pacientes no pós-operatório.

To our knowledge, we report the first case of an extraconal orbital cystic schwannoma in Brazilian literature. The tumor grew slowly and progressively and was associated with minimal eccentric proptosis and diplopia. The radiologic study (orbital computed tomography) revealed a rounded, encapsulated, and extraconal cystic mass in the superior medial region of the right orbit in the supraorbital and supratrochlear nerve topography. An anterior orbitotomy with full excision of the tumor was performed, and the histopathology examination revealed that the tumor was a schwannoma. After the surgery, the patient experienced total remission of his symptoms.

O tumor apresentou aparecimento lento e progressivo, associado a proptose excêntrica e diplopia leves. O exame de imagem (tomografia computadorizada de órbita) revelou uma lesão nodular, arredondada, capsulada, extracônica, cística, na região súpero-medial de órbita direita, na topografia do nervo supraorbitário e supra-troclear. O paciente foi submetido à orbitotomia por via anterior, com exérese integral da tumoração. O exame histopatológico revelou o diagnóstico de schwannoma. Após a cirurgia, o paciente teve regressão total dos sintomas.

ABSTRACT The authors report the first case in Brazilian literature of orbital apocrine hidrocystoma with immunohistochemistry confirmation. The tumor had been growing slowly and progressively, there was no history of impaired vision, diplopia, watering, discharge, or prior trauma. There was no proptosis and extraocular mobility was normal. The radiologic study by orbital computerized tomography revealed an extraconal nodule, with partially defined limits, with cystic and solid areas in the superomedial right orbit. An anterior orbitotomy, with full excision of the tumor was performed. A histopathology examination revealed apocrine hidrocystoma and immunohistochemistry confirmed the diagnosis. After surgery, the patient had total remission of symptoms.


RESUMO Os autores relatam o primeiro caso de hidrocistoma apócrino orbitário com confirmação imunohistoquímica. O tumor apresentou crescimento lento e progressivo, sem relato de baixa de acuidade visual, diplopia, epífora, secreção ou trauma prévio. Não houve proptose ou alteração da motilidade extraocular. O exame de imagem por tomografia computadorizada da órbita revelou uma lesão nodular, extraconal, de limites imprecisos, com áreas císticas e sólidas no seu interior, na região súpero-medial de órbita direita. O paciente foi submetido à orbitotomia por via anterior, com exérese integral da tumoração. O exame histopatológico revelou o diagnóstico de hidrocistoma apócrino e a imunohistoquímica confirmou o diagnóstico. Após a cirurgia, o paciente teve regressão total dos sintomas.

ABSTRACT Purpose: The aim of this study is to describe a variation in technique of the frontalis sling surgery with silicone rods and its results, using an alternative needle with similar effectiveness to the Wright needle at a reduced cost. Methods: This was a prospective, interventional, noncomparative study of patients with severe ptosis who underwent surgical correction using a simple and modified frontalis sling surgery technique. Patients were included in this study from January 2012 to January 2014. 23 surgeries were performed on 15 patients. The minimum "follow-up" was 12 months. Results: Most patients had congenital ptosis (86%) and the average preoperative margin reflex distance 1 (MRD1) was -1.1 mm (range -3 to 0 mm). 1 week post-operation, this was 2.7 mm ( 1.8 to 3.8 mm), 1.8 mm after 1 month and 1.7 mm (1 to 2.5 mm) after one year. The satisfaction rate was 80% (12 patients). Among the dissatisfied patients, one had extrusion and infection with subsequent explantation of the wire, one had asymmetry greater than 2 mm and one had persistent lagophthalmos and punctate keratitis, with subsequent explantation of silicone. Conclusion: The use of silicone rods with tarsal fixation using an alternative needle was effective in the treatment of severe ptosis with few complication rates, a low rate of dissatisfaction and good stability of the results in the follow-up period.


RESUMO Objetivos: O objetivo deste estudo é descrever uma variação da técnica cirúrgica da suspensão ao músculo frontal com fio de silicone e seus resultados, utilizando uma agulha alternativa à agulha de Wright, com a mesma eficácia e melhor razão custo-benefício. Métodos: Foram analisados prospectivamente todos os casos de blefaroptose severa submetidos à correção cirúrgica, utilizando a técnica de suspensão ao músculo frontal modificada, no período de janeiro de 2012 à janeiro de 2014. Foram realizadas 23 cirurgias em 15 pacientes. O "follow-up" mínimo foi de 12 meses. Resultados: A grande maioria dos pacientes apresentavam blefaroptose congênita (86%), a média da distância margem reflexo pré-operatória foi de -1,1mm (-3 a 0 mm), no pós-operatório de 1 semana foi de 2,7 mm (1,8 a 3,8 mm), após 1 mês foi de 1,8 mm e após 1 ano, de 1,7mm (1 a 2,5 mm). O índice de satisfação foi de 80% (12 pacientes). Dos 3 pacientes insatisfeitos, 1 teve extrusão e infecção com posterior explante do fio, 1 apresentou assimetria maior que 2mm e 1 evoluiu com lagoftalmo persistente e ceratite puntacta, com posterior explante do silicone. Conclusão: O uso do fio de silicone com fixação tarsal usando uma agulha alternativa, se mostrou eficaz no tratamento da blefaroptose severa, com poucas complicações, baixo índice de insatisfação e boa estabilidade dos resultados no período de seguimento.

PURPOSE: To verify the quality of life in a Brazilian glaucoma population and the influence of possible modifiers (e.g., visual acuity, visual field impairment). METHODS: Forty-five consecutive patients from CEROF - Federal University of Goias, Brazil were included prospectively in the study. The quality of life was assessed using the Portuguese version of the VFQ. Possible modifiers were evaluated, initially separately with the Spearman's Correlation and then together in a regression model. RESULTS: The mean age was 59.6 +/- 12.4 years. The mean quality of life score was 79 +/- 15 (range 45-100). Age (r = -0.402, p = 0.006), visual acuity in the better eye (r = -0.497, p = 0.001) and in the worse eye (r = -0.608, p < 0.001), Hodapp-Parrish-Anderson visual field grading scale in the better (r = -0.353, p = 0.01) and worse eye (r = -0.387, p = 0.009), visual field Mean Deviation (MD) in the better (r = 0.355, p = 0.01) and worse eye (r = 0.320, p = 0.04) and ability to perform a visual field test in both eyes (r = -0.397, p = 0.007) were significantly correlated with the quality of life scores. However, only age (younger, better quality of life, p = 0.008) and visual acuity in the better eye (direct relation, p = 0.04) were significant in the regression model (r = 0.633, r(2) = 0.401). CONCLUSIONS: The VFQ may be a useful tool to assess the quality of life in glaucoma patients. The preliminary results indicate that age and visual acuity in the better eye are the main factors related to the quality of life in these patients.
