RESUMEN

OBJECTIVE: To develop and validate a rapid administration questionnaire to assess patient satisfaction with the pharmaceutical care (PhC) service in Spanish community pharmacies. METHODS: A semi-structured and self-administered questionnaire was designed focused on the cumulative experience of patients with the PhC service. This was reviewed by a panel of 8 experts in 2 rounds. The validation was carried out in community pharmacies that performed the PhC service and agreed to participate. All patients or their caregivers who attended a pharmacy during the study period, and who could read and write, were included. An exploratory factor analysis was performed to assess the validity as evidence of internal structure and reliability was estimated using Cronbach's alpha coefficient. RESULTS: A total of 8 community pharmacies belonging to 7 provinces of Spain took part and 103 questionnaires were obtained. Factor analysis revealed a single component explaining 33.6% of the total variance. We obtained a Cronbach's alpha coefficient of 0.75. Patients additional comments expressed in 9.7% of the questionnaires were related to emphasising, and thanking for, the care provided by the PhC service. CONCLUSIONS: The questionnaire developed has provided evidence of content validity, internal structure validity, and reliability to assess the patient satisfaction with the PhC service in Spanish community pharmacies.

RESUMEN

The acute distention of stomach in the newborn is conditional by metabolic alterations like hypocalcemia or by septic states. We present the case of a newborn with antecedents of membrane rupture for 12 hours obtained by cesarean procedure, of 38 weeks by Capurro, score without perinatal asphyxia and 3320 g of birth weight, which evolutioned with respiratory distress and its first exams showed leukopenia, low platelet count and the cerebrospinal fluid showed 390 cells with polymorphonuclear predominium considering the diagnosis of neonatal sepsis and meningitis. At the second day he presented abdominal distention secondary to gastric camera dilatation, were made a laparotomy to descart congenital obstruction and only found gastric distention. In all newborn with acute gastric distention its important for first instance descart congenital intestinal obstruction and is on sidered this entity by exclusion. We recommend in this patients sepsis worshop. The treatment is drainage by orogastric tube and antibiotics. The prognosis is good once upon treating the basic problem.

RESUMEN

Cardiac nursing is nearly 30 years old. Changes in the role of cardiac nursing have directly corresponded to the three distinct medical phases of care for the acute myocardial infarction patient--arrhythmia detection, pump failure, and limiting infarct size. Nursing care delivery in the cardiac setting, the cardiac patient, and the role of the cardiac nurse each have undergone changes over the years, which together has provided a challenge to health care delivery. Future effects of technological and pharmacologic advances, social changes, and nursing professional practice issues on cardiac care delivery are imperative to consider as health care moves into the 1990s.
