RESUMEN

The authors propose a method of endoural ultrasound therapy in parallel with local introduction of ozonized physiological solution for patients with otitis media purulenta chronica. Locally used was the solution containing ozone in concentration 10 mg/l. Low-frequency ultrasound therapy was conducted using Tonsillor-2 unit with special intraauricular wave guides. Treatment outcomes in 132 patients are analysed for 5 years. The response was registered in the majority of cases.

RESUMEN

The article presents classification of mixed forms of hypoacusis based on certain combination of audiological criteria reflecting the impact of a hypoacusis neurosensory component. A differential approach to rehabilitation of patients with mixed hypoacusis caused by chronic pathological processes in the middle ear is proposed. The approach allows for severity of neurosensory hearing disorders.

Hearing disorders are characterized for citizens of the East Siberia belonging to Europeoid or Mongoloid race. Interpopulation features of hypoacusis of various genesis and effects of natural-geographic and ethnic factors on hypoacusis manifestations in native Mongoloid and migrant Europeoid Siberian population are shown.

RESUMEN

227 young men and boys aged 9-22 years underwent treatment for juvenile angiofibroma of the base of the skull (JAFBS) in the ENT clinic of the Russian Academy of the Advanced Medical Education between 1977 and 1998. JAFBS was most frequently diagnosed at the age 15-18 years. An original clinicotopographic JAFBS classification is proposed basing on the disease spread. A complex of tests has established that JAFBS patients develop changes in hormonal and immunological statuses. Indications to ligation of the external carotid arteries and a modified nasomaxillary approach to angiofibroma providing radical removal of the tumor are described. After surgical treatment only 11.5% of the patients exhibited recurrent tumors.

RESUMEN

A novel radiosurgical method of cutting tissues and coagulation of ENT operations is presented. Indications, technique, device operation regimes are provided. Radiosurgery has evident advantages: saving cut with minimal damage to the adjacent tissues, good coagulating effect warranting "dry field" for the surgeon, absence of necrosis of the wound walls, fast healing, simple operative procedure. Radiosurgery holds great promise for otorhinolaryngological practice.

RESUMEN

Experimental and clinical trials have shown that anesthesia and intensive therapy of shock incorporating tranexamic acid enhance analgesia, hyporeflexia, thus ensuring stable, adequate anesthesia which is safe under reduced doses of standard anesthesiological modalities. Application of tranexamic acid provided more favourable course of postanesthesiological and postoperative periods, decreased the number of complications and lethal outcomes compared to standard anesthesia.

RESUMEN

Optimal methods of endotracheal anesthesia using non-opiate drugs of four classes (clofelin, transamine, contrykal, ketanes) have been fundamentally grounded developed and tried in extensive and traumatic operations for ENT malignant and vascular tumors as well as inflammation in patients of high anesthesiological operative risk. The drugs were used in 174 patients including 13 children. 87.9% of the patients had concomitant diseases: blood hypertension, coronary heart disease, chronic nonspecific pulmonary diseases, bronchial asthma, asthmatic bronchitis, diabetes mellitus, anemia. Adequate stable anesthesia was achieved in reduced dosage of conventional anesthesiological agents.

RESUMEN

Ninety six patients with chronic epitympanitis treated surgically were examined immunologically and allocated in 4 groups. Suppressed T- and B-cell immunities were found. Patients of group 1 received antibiotics and antihistamine drugs (controls), patients of group 2, 3 and 4 were given in addition timontin, myelopid and timontin + myelopid, respectively. Monotherapy with timontin and myelopid improved immunological status of the patients who exhibited better outcomes of conventional treatment than controls. Combined use of the above immunomodulators corrected the immunological defects and resulted in good clinical response.

Patients of young age predominated in the group of 113 patients (69 males, 44 females) with orbital rhinogenic inflammation observed by the authors. 57.5% of the cases arose due to acute sinusitis, 42.5%-due to exacerbation of chronic sinusitis. The inflammation focus developed more frequently in the frontal sinuses and ethmoid labyrinth. Periostitis was diagnosed in 17 patients, subperiosteal orbital abscess in 9 patients. Inflammation of the orbital soft tissues presented as eyelid and orbital fat edema in 66 patients, as retrobulbar abscess in 11 and phlegmon in 10 patients. The treatment policy combined intensive antiinflammatory therapy and surgery if indicated. The treatment aimed at both elimination of the inflammatory focus in the sinus and drainage of the inflammatory focus in the sinus and drainage of the secondary suppuration focus in the orbit. A 100% cure was achieved, though residual phenomena such as limited movements of the eyeball, partial ptosis, reduced visual acuity, loss of vision (3, 3, 2 and 1 patients, respectively) remained.

RESUMEN

The paper concerns conduction of the internal carotid artery and internal jugular vein reconstruction in 3 and 2 patients, respectively, operated for metastases of laryngopharyngeal carcinoma. 4 patients are still alive, 1 died on postoperative day 6. Bilateral one-stage Crile operation with application of autograft venous reconstruction of the internal jugular vein is detailed. Further research in this field is necessary.

RESUMEN

The results of the treatment of 340 patients with malignant tumors in the laryngopharynx show the advantages of combined and surgical methods over radiation therapy: five-year survival after application of the above methods made up 43% and 12%, respectively. A new method of autovenous plastics of carotid artery is proposed to treat advanced metastatic tumors. Different variants of restoring pharyngeal integrity after the destructive operations are evaluated.
