The aim of the study was to evaluate the changes in psoas major (PM), erector spinae (ES), multifidus (MF), and rectus abdominis (RA) morphometry in Lenke type V adolescent idiopathic scoliosis (AIS) and control. Forty-two Lenke V AIS patients and 30 control were enrolled. The cross-sectional area (CSA) and CSA index (CI) of the PM, ES, MF, and RA at the T9-L5 levels were measured and compared between Lenke type V patients and control. The CI of the RA was significantly higher in the control group than the Lenke V group in multiple levels. On the other hand, there was no significant difference between patients with Lenke V AIS and control regarding the CI of the RA, ES, and PM. There was no significant difference between concave and convex sides of the Lenke V group regarding the CI of the RA, PM, MF, and ES. RA muscle mass is significantly affected in Lenke type 5 AIS regardless of convex or concave sides. This difference may be related to postural differences and back pain development in Lenke type 5 patients. Level of evidence: Level III.

Background: Muscle and bone morphometry may be potent indicators for predicting femoral subtrochanteric shortening osteotomy (FSSO). We aimed to investigate muscular and bony factors that may be predictive of FSSO. Methods: Patients with unilateral Crowe type 4 developmental dysplasia of the hip (DDH) who underwent unilateral total hip arthroplasty (THA) without (Group 1, 31 patients) and with FSSO (Group 2, 39 patients) were included. Sixty healthy hips (Group 3) were selected as the control group. Several muscular and bony parameters were measured on the operative (O) and non-operative (NO) hips on radiographs and computed tomography (CT) images. Results: The mean NO gluteus medius cross-sectional area (CSA) index (NO-GMed CI), NO tensor fasia lata (TFL) CI, NO iliopsoas (IP) CI, and NO gluteus maximus (GMax) CI for the control group were lower than those for both Crowe type IV DDH with and without FSSO. The mean NO gluteus minimus CI (NO-GMin CI) for the control group were higher than those for both Crowe type IV DDH with and without FSSO. There was significant difference between Groups 1 and 2 regarding axial position of neo-acetabulum (APNA) (- 0.03 ± 8.5 vs. 5.27 ± 6.33, p = 0.004), coronal position of neo-acetabulum (CPNA) (33.39 ± 10.65 vs. 53.70 ± 12.27, p = 0.000), operative leg length discrepancy (O-LLD) (14.18 ± 15.14 vs. 24.44 ± 15.80, p = 0.001), O-gluteus minimus (GMin) length (155.34 ± 157.73 vs. 106.79 ± 20, p = < 0.01), and O-GMin CI (57.28 ± 58.59 vs. 29.95 ± 12.13, p = < 0.01). The cutoff values determined by the receiving operating curve analyses were as follows: 13.7 mm, 2.5 mm, and 41.4 mm for O-LLD, APNA, and CPNA, respectively. Conclusion: FSSO is associated with APNA, CPNA, O-LLD, O-GMin length, and O-GMin CI. By using cutoff values of APNA, CPNA, and O-LLD, predicting FSSO preoperatively is possible. Owing to these parameters, it may be possible to lower the technically demanding level of surgery.

BACKGROUND: Low back pain is a very common musculoskeletal complaint that impacts patients' quality of life in numerous ways. Facet joint injection is a widely used spinal intervention to relieve back pain. Effects of facet joint injection on spinopelvic parameters and the relationship between injection levels and spinopelvic parameter changes have not been evaluated before. OBJECTIVE: To compare spinopelvic parameters before and after injections at different levels, and to evaluate the correlation between these changes and functional outcome. METHODS: 144 patients were included in the study and retrospectively grouped by injection level: Group 1 (n= 72), L4-L5 and L5-S1, and group 2 (n= 72), L1-L2, L2-L3, L3-L4, L4-L5 and L5-S1. Pre- and post-injection Oswestry Disability Index (ODI), sacral slope, pelvic tilt, pelvic incidence, and intervertebral angles between T12 and S1 were compared. The correlation between ODI and radiographic parameter changes was evaluated. RESULTS: The pre- to post-injection ODI change was significantly lower in group 2 (p= 0.010). There was no significant difference between the groups in terms of pre- and post-injection spinopelvic parameters before and after injection (p> 0.05) except pelvic tilt (p= 0.001 and p= 0.007, respectively). There was a significant moderate positive correlation between the change in the ODI value and the change in pelvic tilt (P= 0.012, r= 0.581). CONCLUSIONS: Multilevel lumbar facet injections are clinically more effective than only two-level lower level lumbar injections. Pelvic tilt changes positively correlate with the ODI score changes.

PURPOSE: To compare the critical shoulder angle (CSA), acromion index (AI), acromion angulation (AA) and glenoid version angle (GVA) between patients with full-thickness rotator cuff tears (RCTs) and patients with intact rotator cuffs. METHODS: Between 2014 and 2018, the CSA, AI, AA and GVA were measured in consecutively included patients aged > 40 years who underwent shoulder arthroscopy for full-thickness RCTs. A total of 437 patients with RCTs and a mean age of 51.2 years (± 5.8) were included, 35.7% of whom were male. In the control group, there were n = 433 patients (36.3% male) with an intact rotator cuff, and the mean age was 50.7 years (± 5.3). RESULTS: The mean AI for the RCT group was 0.7 ± 0.1, which was significantly higher than the mean AI for the control group (0.6 ± 0.1, p < 0.001). The mean CSA for the RCT group was 33.6° ± 3.9°, which was significantly higher than the mean CSA for the control group (31.5° ± 4°, p < 0.001). The mean AA for the RCT group was 13.9° ± 9°, which was significantly higher than the mean AA for the control group (12.4 ± 8.6, p = 0.012). The mean GVA for the RCT group was - 3.5° ± 4.6° and significantly retroverted compared with the mean GVA for the control group (- 2.2° ± 4.6°, p < 0.001). The cutoff values determined by the ROC curve analyses were as follows: 0.6 for AI, 31.4° for CSA, 9.6° for AA and - 2.6° for GVA. CONCLUSION: The CSA, AI, GVA and AA values measured by MRI were determined to be significantly related to full-thickness rotator cuff ruptures. The AI, CSA, AA and GVA may be considered risk factors for degenerative rotator cuff tears. Assessing the CSA, AI, GVA and AA can be helpful for diagnostic evaluation of patients with full-thickness RCTs. LEVEL OF EVIDENCE: III.

AIM: To compare the postoperative changes of both sagittal spinal and spinopelvic parameters in patients with Lenke Type 1 adolescent idiopathic scoliosis (AIS) who underwent selective and non-selective fusion surgery. MATERIAL AND METHODS: We conducted a retrospective study among 53 Lenke Type 1 AIS patients who underwent corrective surgery at our centre between 2006 and 2012. Patients were classified as group 1 if they underwent selective surgery and as group 2 if they underwent non-selective surgery. Surgical results of preoperative and postoperative sagittal and spinopelvic measurements, pelvic tilt (PT), pelvic incidence (PI), sacral slope (SS), lumbar lordosis (LL) and thoracic kyphosis (TK) values were analysed using the SURGIMAP© Software (Nemaris Inc. USA) measurement system. RESULTS: In both groups, a comparison of pre- and postoperative sagittal spinal parameters did not show a statistically significant difference. In both groups, pre- and postoperative measurements of LL and TK did not show a statistically significant difference. CONCLUSION: After selective and non-selective surgery, sagittal spinal and spinopelvic parameters are not affected in the middle term. We think that the long-term studies to be done in this regard will increasingly require the necessity of keeping the pelvis in mind while evaluating the sagittal plan in AIS surgery.

BACKGROUND: Informing the individuals on family planning including emergency contraception is a significant step for preventing unintended pregnancies. Although there is a number of studies on family planning and emergency contraception globally and in Turkey, no such data are available in the Turkish Republic of Northern Cyprus. The objective of this study was to evaluate the knowledge and perceptions on family planning and emergency contraception of the first year students of an international medical school in Nicosia, Northern Cyprus and to increase awareness for developing new policies on the issue. METHODS: The data of this cross-sectional study were collected in February 2016 by a questionnaire of 36 questions. Of the 229 students, 189 (82.5%) completed the questionnaire. The data were evaluated by SPSS 18.0 statistical program. The differences of variables were evaluated by Chi square test, p < 0.05 being accepted as significant. RESULTS: The distribution of participants from 23 countries according to nationality revealed three leading countries: Nigeria, Turkey and Syria. Of the students, 53.6% knew the definition of family planning. The sources of information were mainly school, the internet and media, with a total of 60.9% of the participants who stated having prior information on the subject. Awareness of contraceptive methods was indicated by more than 90% and emergency contraception by 66.1% of the participants. However, the students were unable to differentiate between modern and traditional family planning methods; 85.6% did not have knowledge of the most effective period for emergency contraception and 63.1%, of the definition of emergency contraceptive pills. CONCLUSIONS: In conclusion, the knowledge and awareness level of the first year medical students on family planning and emergency contraception was insufficient. Family planning and emergency contraception education should be provided for the students at the first year of all faculties as well as medical schools and relevant programs should be included in the curricula of medical education.

In this study, we investigated the affect and the role of growth factors on liver damage. 110 Sprague-Dawley rats were divided into 11 groups: a sham group, a control group, HGF, EGF, IGF, TGF groups of irreversible jaundiced rats and a control group and HGF, EGF, IGF, and TGF groups of reversible jaundiced rats (n = 10). In the irreversible jaundiced groups, the common bile duct was explorated, double ligated, and cut. 150 µg/kg/day HGF, 5 µg/kg/day EGF, 5 µg/kg/day IGF, and 5 µg/kg/day TGF ß-1 were injected intraperitoneally after the seventh post-operative day. In the reversible jaundiced group, the common bile duct was ligated and the ligation was resolved on the seventh post-operative day. For 5 days, growth factors were injected at the same dose. Ductal proliferation scores significantly decreased after growth factor administration in the EGF-A and TGF-A groups. Furthermore, ductal proliferation was decreased in the TGF-B group. As a result of this study, HGF was effective in the irreversible jaundiced groups and ineffective in the reversible jaundice groups. EGF was effective in the reversible jaundiced groups and ineffective in the irreversible jaundiced groups. In both the irreversible jaundiced and reversible jaundiced groups, IGF was ineffective, although TGF ß-1 was effective. We believe that these results arise from the positive effects of effective doses of growth factor on liver damage.

BACKGROUND: Intestinal ischemia is a serious and common clinical status. It develops as result of superior mesenteric artery (SMA) obstruction caused by many etiologic factors. Sepsis and multiple organ failure could develop following intestinal ischemia. The present study aimed to investigate the effects of ligustrazin, which has a vasodilator impact on intestinal ischemia. METHODS: Forty male Wistar rats were divided into three groups randomly. Sham operation was performed on Group S (n=7); mesenteric ischemia and then 60 minutes reperfusion of the intestine process was performed on Group MI (n=7); mesenteric ischemia and then 60 minutes reperfusion of the intestine process was performed and 80 mg/kg ligustrazin was administrated intraperitoneally on Group MI+L (n=7). Intestinal tissue samples were taken for tissue MDA, SDO and nitric oxide (NO) levels, and ileum and jejunum samples were taken for histopathologic examination. RESULTS: Tissue MDA levels and tissue NO levels of Group MI-L was determined to have significantly decreased. Tissue SOD levels were found similar to Group S. Chiu classification score of the jejunum and ileum was determined to have decreased in Group MI-L compared to Group MI. DISCUSSION: As a result of this study, Ligustrazin was found to adjust lipid peroxidation in biochemical parameters during mesenteric I-R and decrease the severity of damage of I-R on the histopathological scores of the jejunum and ileum.

Malignant mesothelioma is an uncommon neoplasm arising in body cavities lined by the mesothelium. Immunohistochemical stains are useful for making a diagnosis, but the correct combination of antibodies as should be selected in a comprehensive assessment. A peritoneal origin combined with desmoplastic histology is an extremely rare disease entity. Here, we report a case of the primary peritoneal malignant mesothelioma. A 53-year-old man admitted to the hospital with abdominal distension and pelvic pain. In laparotomy, peritonitis carcinomatosa situation was exposed. Multiple biopsies were taken from omentum, peritoneum and fascia. Calretinin, WT-1, D2-40, keratin 5/6, mesothelin, keratin 7, keratin 20, CD99, CEA, smooth muscle actin, desmin, CD34 and S-100 were negative. With these findings tumor was evaluated as desmoplastic malignant mesothelioma of the peritoneum. Currently, no established standard treatments for malignant peritoneal mesothelioma, but early diagnosis by exploratory laparotomy followed by chemotherapy may have contributed to longer survival for patients.
