RESUMEN

BACKGROUND: Concurrent chemoradiotherapy (CRT) is the preferred treatment strategy for inoperable esophageal cancer (EC). However, the effect of CRT needs to be improved. METHODS: This study comprehensively analyzed targeted agents combined with CRT for the treatment of EC by a network meta-analysis. The search was performed in public databases from incipient to 5 August 2021. Randomized controlled trials comparing the effect of targeted agents combined with CRT and CRT alone on EC patients were included. RESULTS: Ten studies were included. For progression-free survival (PFS), nivolumab (67.4%) and erlotinib (64.6%) had advantages based on Cox analysis. Regarding the frequency of PFS, cetuximab (OR: 1.39; 95% CI: 1.01, 1.91; p=0.042) and nivolumab (OR: 1.81; 95% CI: 1.34, 2.44; p<0.01) were significantly superior to the control. For overall survival (OS), nivolumab (71.6%) in Cox analysis and nimotuzumab (69.7%) in frequency analysis were found to have relative advantages. Nimotuzumab combined with CRT was significantly better than the control with regard to endoscopic and the pathologic complete response (epCR; OR: 2.81; 95% CI: 1.28, 6.14; p=0.011) and objective response rate (ORR; 4.71; 95% CI: 1.45, 15.29; p=0.008). The targeted drugs were not associated with significant SEA risk. CONCLUSION: In conclusion, compared to CRT alone, cetuximab and nivolumab combined with CRT were found to significantly improve the PFS rate only based on the frequency results. However, there was no benefit in terms of OS. For epCR and ORR, nimotuzumab was better than the blank control. Considering the limitations in this study, more well-designed RCTs are needed in the future to validate the results.

OBJECTIVE: To understand the infection status and the main risk factors of soil-transmitted nematodes in children in the poverty-stricken areas of Guizhou Province and Sichuan Province, so as to provide the evidences for making suitable control strategies in these areas. METHODS: A total of 95 villages and 6 primary schools in 6 poor counties in the two provinces were selected as investigation sites according to the stratified random sampling method. Eleven preschool children aged 3-5 years in each sample village and 11 school children aged 8-10 years in each sample school were chose as investigation objectives, their feces were collected and examined by Kato-Katz technique. In addition, the village doctor and cadres in each village were investigated by a standardized questionnaire to understand the deworming condition of children and the social economy and sanitary status in 2009. Then the correlation between average soil-transmitted nematode infection rate and its influencing factors was analyzed by the Tobit model. RESULTS: A total of 1 707 children from 95 villages in the two provinces were examined. In Guizhou Province, the total infection rate of soil-transmitted nematodes in children was 46.1%, and the infection rates of Ascaris lumbricoides, Hookworm, and Trichuris trichiura were 31.1%, 4.1% and 10.8% respectively, and the corresponding rates in Sichuan Provinces were 9.8%, 3.6%, 3.5%, 2.7%, respectively. In the dimension of school, a total of 890 school-aged kids from 46 schools in the two provinces were examined, the infection rates of soil-transmitted nematodes, Ascaris lumbricoides, Hookworm, and Trichuris trichiura in Guizhou Province were 53.8%, 32.7%, 6.6% and 14.4%, respectively, and the corresponding rates in Sichuan Province were 7.3%, 2.2%, 2.9% and 2.2%, respectively. The results from Tobit analysis indicated that the proportion of children accepted deworming treatment and the paddy field in the cultivated area were statistically correlated to the infections of soil-transmitted nematodes and Ascaris lumbricoides (all P < 0.05). CONCLUSIONS: The soil-transmitted nematode infection rates of children are still at a high level in poor areas of southwest China. In order to decrease the infection rates, besides the long-term deworming, water supply and sanitary improvement, and the health education should be strengthened, and the treatment of soil-transmitted nematodes should be covered by the New Rural Cooperative Medical System.

OBJECTIVE: To understand the infection status of soil-transmitted nematodes in southwest China and the correlation between soil-transmitted nematode infections and children's growth. METHODS: The prevalence of soil-transmitted nematode infections was determined by Kato-Katz technique, and in part of the children, the examination of Enterobius vermicularis eggs was performed by using the cellophane swab method. The influencing factors were surveyed by using a standardized questionnaire. The relationship between soil-transmitted nematode infections and children's growth was analyzed by the ordinary least square (OLS) method. RESULTS: A total of 1 707 children were examined, with a soil-transmitted nematode infection rate of 22.2%. The results of OLS analysis showed that there existed the negative correlation between soil-transmitted nematode infections and the indexes of children's growth including BMI, the weight-for-age Z score and height-for-age Z score. Furthermore, other correlated variables included the age, gender, educational level of mother and raising livestock and poultry, etc. CONCLUSIONS: Children' s retardation is still a serious issue in the southwest poor areas of China and correlated with the infections of soil-transmitted nematodes. For improving children's growth, it is greatly significant to enhance the deworming and health education about parasitic diseases in mothers.

BACKGROUND: Clonorchiasis caused by ingesting improperly prepared fish ranks among the most important but still neglected food-borne parasitic diseases, especially in the People's Republic of China (P.R. China). To promote the implementation of interventions efficiently, the demonstration of an epidemiological profile of Clonorchis sinensis infection is essential in hyper-epidemic areas. METHODS: In one community with higher levels of economic development in Guangdong province, P.R. China, villagers were motivated to provide stool samples for examining helminth eggs. Then, those infected with C. sinensis completed the structured questionnaire including demographical characteristics, knowledge and behavior. RESULTS: A total of 293 villagers infected with C. sinensis participated in questionnaire investigation. Among them, 94.54% were adult and 93.17% were indigenous. The geometric mean of C. sinensis eggs per gram of feces in the children, adult females and adult males was 58, 291 and 443, respectively. The divergence between knowledge and behavior in the adults, especially the adult males, was shown. Out of 228 persons eating raw fish, 160 did it more frequently at restaurants, the proportion of which varied in different populations, showing 25.00%, 54.88% and 80.28% in the children, adult females and adult males, respectively. CONCLUSIONS: Different interventions need to be adopted in different populations. Chemotherapy should be prioritized in the adults, especially the adult males. In addition, health education targeting the children, is essential and may play a crucial role in controlling clonorchiasis in the long term. In order to successfully control clonorchiasis, intervention in the restaurant should not be overlooked in some endemic areas.

BACKGROUND: This study aimed to explore the relationship between cancer-related fatigue (CRF) and personality in patients with breast cancer after chemotherapy. METHODS: A cross-sectional study was conducted to study the relationship between CRF and personality in breast cancer patients after chemotherapy. CRF and personality were measured by the cancer fatigue score and the Eysenck Personality Questionnaire, respectively. RESULTS: A total of 300 breast cancer patients who had received chemotherapy were recruited to this study. Eysenck Personality Questionnaire scores of psychoticism, introversion, and extroversion in the patients were lower than the norm level (p < 0.01), but those of neuroticism and lie were higher than the norm level (p < 0.01). Multivariate analyses showed positive correlation between psychoticism and affective fatigue, neuroticism and total fatigue, and physical fatigue and cognitive fatigue. Multivariate analyses also showed negative correlation between introversion or extroversion and total fatigue, physical fatigue or affective fatigue, and lie and total fatigue or cognitive fatigue. CONCLUSIONS: There was CRF in patients with breast cancer after chemotherapy. Psychoticism, extroversion/introversion, neuroticism, and lie are correlated with CRF in breast cancer patients after chemotherapy.

OBJECTIVE: To investigate the infection status of Enterobius vermicularis among children in 9 Provinces/Autonomous regions/Municipalities (P/A/M) of China, and analyze its risk factors. METHOD: From April to December 2011, one provincial capital (prefecture-level city) and one county (city, district) were chosen as investigation spots from Guangdong, Guangxi, Hainan, Chongqing, Sichuan, Zhejiang, Fujian, Anhui and Guizhou, respectively. Children aged 2 to 12 were examined by using adhesive cellophane anal swab with round-bottom tube. Information of children's family condition, health behavior and school environment were collected by questionnairing. RESULTS: 14 964 children were examined, and 14 582 qualified questionnaires were collected. The total prevalence was 17.8% (2 659/14 964). Of the 9 P/A/M, the prevalence was highest in Hainan Province (51.1%, 869/1 701) and lowest in Anhui Province (0.8%, 13/1 589). The prevalence in urban areas (7.3%, 552/7 581) was lower than that of rural areas (28.5%, 2 107/7 383) (chi2 = 1156.73, P < 0.01). The highest prevalence in urban and rural areas was found in Haikou City(38.0%, 322/847) and Wanning City (64.1%, 547/854) of Hainan Province. The prevalence rate in males and females was 17.4% (1 410/8 128) and 18.3% (1249/6 834), respectively (chi2 = 2.192, P > 0.05). The highest prevalence in males (61.2%, 300/490) and females (67.9%, 247/364) was found in children of Wanning City. Multivariate logistic regression analysis showed that residence, education level of parents, occupation of parents, nail biting, types of classroom ground and type of boarding were the risk factors on E. vermicularis infection. CONCLUSION: The prevalence of enterobiasis in children is still high in many areas of China, and the prevention and control measures should be taken according to the risk factors.

OBJECTIVE: To understand the infection status and main risk factors of soil-transmitted nematodes in southwest China so as to provide the evidence for making the control programs for soil-transmitted nematodiasis. METHODS: The prevalence of soil-transmitted nematode infections was determined by Kato-Katz technique and influencing factors were surveyed by using a standardized questionnaire, and in part of the children, the examination of Enterobius vermicularis eggs was performed by using the cellophane swab method. The relationship between soil-transmitted nematode infections and influencing factors was analyzed by the multiple probit estimated method. RESULTS: A total of 1 707 children were examined, with a soil-transmitted nematode infection rate of 22.2%, the crowd infection rates ofAscaris lumbricoides, hookworm, and Trichuris trichiura were 16.0%, 3.8% and 6.6% respectively and 495 children were examined on Enterobius vermicularis eggs, with the infection rate of 5.1%. The results of probit estimated analysis suggested that the effects of 4 factors on soil-transmitted nematode infections were significant (all P values were less than 0.05), namely the number of sib, educational level of mother, drinking unboiled water and raising livestock and poultry. Among the factors above, the educational level of mother could reduce the probability of infection (ME = -0.074), while the number of sib, drinking unboiled water and raising livestock and poultry could increase the probability of the infections (with ME of 0.028, -0.112 and 0.080, respectively). CONCLUSIONS: Soil-transmitted nematode infection rates are still in a high level for children in southwest poor areas of China, with Ascaris lumbricoides as a priority. The changes of children's bad health habits, raising livestock and poultry habits, and implementing the health education about parasitic diseases in mothers would be of great significance for the prevention and control of soil-transmitted nematodiasis.

BACKGROUND: Clonorchiasis is among the most neglected tropical diseases. It is caused by ingesting raw or undercooked fish or shrimp containing the larval of Clonorchis sinensis and mainly endemic in Southeast Asia including China, Korea and Vietnam. The global estimations for population at risk and infected are 601 million and 35 million, respectively. However, it is still not listed among the Global Burden of Disease (GBD) and no disability weight is available for it. Disability weight reflects the average degree of loss of life value due to certain chronic disease condition and ranges between 0 (complete health) and 1 (death). It is crucial parameter for calculating the morbidity part of any disease burden in terms of disability-adjusted life years (DALYs). METHODOLOGY/PRINCIPAL FINDINGS: According to the probability and disability weight of single sequelae caused by C. sinensis infection, the overall disability weight could be captured through Monte Carlo simulation. The probability of single sequelae was gained from one community investigation, while the corresponding disability weight was searched from the literatures in evidence-based approach. The overall disability weights of the male and female were 0.101 and 0.050, respectively. The overall disability weights of the age group of 5-14, 15-29, 30-44, 45-59 and 60+ were 0.022, 0.052, 0.072, 0.094 and 0.118, respectively. There was some evidence showing that the disability weight and geometric mean of eggs per gram of feces (GMEPG) fitted a logarithmic equation. CONCLUSION/SIGNIFICANCE: The overall disability weights of C. sinensis infection are differential in different sex and age groups. The disability weight captured here may be referred for estimating the disease burden of C. sinensis infection.

RESUMEN

Tanghinigenin, a cardiac glycoside, is isolated from the seeds of Cerbera manghas L. In this study, we demonstrated that tanghinigenin reduced the viability of human promyelocytic leukemia HL-60 cells in a time- and dose-dependent manner, and efficiently induced apoptosis in HL-60 cells as evidenced by the Annexin V/PI binding assay, DNA fragmentation and AO/EB staining studies. In addition, stimulation of HL-60 cells with tanghinigenin induced a series of intracellular events including the activation of caspase-3, -8, and -9, as well as up-regulation of Fas and FasL protein level. Taken together, caspase activation and Fas/FasL interaction was found to be involved in tanghinigenin-induced HL-60 cell apoptosis.

The purpose of this study was to evaluate the inhibitory effect of renierol, extracted from marine sponge Halicdona.SP., on xanthine oxidase (XO) and its hypouricemic effect in vivo. Renierol and a positive control, allopurinol, were tested for their effects on XO activity by measuring the formation of uric acid and superoxide radical from xanthine. Renierol inhibited XO in a concentration-dependent and competitive manner. IC(50) value was 1.85 microg.ml(-1) through the measuring of uric acid and was 1.36 microg.ml(- 1) through the measuring of superoxide radical. Renierol was found to have an in vivo hypouricemic activity against potassium oxonate-induced hyperuricaemia in mice. After oral administration of renierol at doses of 10, 20 and 30 mg.kg(- 1), there was a significant decrease in the serum urate level (4.08 +/- 0.09 mg.dl(- 1), P < 0.01), (3.47 +/- 0.11 mg.dl(- 1), P < 0.01) and (3.12 +/- 0.08 mg.dl(- 1), P < 0.01), when compared to the hyperuricaemic control (6.74 +/- 0.23 mg.dl(- 1)). Renierol was a potent XO inhibitor with hypouricemic activity in mice.
