BACKGROUND: ALK-negative anaplastic large cell lymphoma associated with breast implant (i-ALCL) has been recently recognized as a distinct entity. Among 43 830 lymphomas registered in the French Lymphopath network since 2010, 300 breast lymphomas comprising 25 peripheral T-cell lymphomas (PTCL) were reviewed. Among PTCL, ALK-negative ALCL was the most frequent and all of them were associated with breast implants. PATIENTS AND METHODS: Since 2010, all i-ALCL cases were collected from different institutions through Lymphopath. Immuno-morphologic features, molecular data and clinical outcome of 19 i-ALCLs have been retrospectively analyzed. RESULTS: The median age of the patients was 61 years and the median length between breast implant and i-ALCL was 9 years. Most implants were silicone-filled and textured. Implant removal was performed in 17 out of 19 patients with additional treatment based on mostly CHOP or CHOP-like chemotherapy regimens (n = 10/19) or irradiation (n = 1/19). CHOP alone or ABVD following radiation without implant removal have been given in two patients. The two clinical presentations, i.e. effusion and less frequently tumor mass correlated with distinct histopathologic features: in situ i-ALCL (anaplastic cell proliferation confined to the fibrous capsule) and infiltrative i-ALCL (pleomorphic cells massively infiltrating adjacent tissue with eosinophils and sometimes Reed-Sternberg-like cells mimicking Hodgkin lymphoma). Malignant cells were CD30-positive, showed a variable staining for EMA and were ALK negative. Most cases had a cytotoxic T-cell immunophenotype with variable T-cell antigen loss and pSTAT3 nuclear expression. T-cell receptor genes were clonally rearranged in 13 out of 13 tested cases. After 18 months of median follow-up, the 2-year overall survival for in situ and infiltrative i-ALCL was 100% and 52.5%, respectively. CONCLUSIONS: In situ i-ALCLs have an indolent clinical course and generally remain free of disease after implant removal. However, infiltrative i-ALCLs could have a more aggressive clinical course that might require additional therapy to implant removal.

Sensing carbohydrates via the oral cavity benefits performance outcomes during brief high intensity bouts of exercise. However, the extent to which carbohydrates need to be present in the oral cavity to influence sprint performance is less understood. The purpose of this study was to determine if serial increases in carbohydrate rinse time across sprint sets attenuates increases in sprint time compared to no serial increases in carbohydrate rinse time across sprint sets. Fifteen sprint trained participants completed three repeated anaerobic sprint tests (RAST), 3 sets of 6 x 35-m sprints, under two different carbohydrate mouth rinsing (CMR) conditions; (1) rinsing for only 5 seconds (s), and (2) rinsing for 5 s, 10 s and 15 s (serial rinse). Prior to a RAST, participants provided perceived recovery status (PRS) and perceived feeling of arousal (FAS). Upon completion of each individual sprint, participants gave a rating of perceived exertion (RPE). A lactate sample was taken upon completion of each individual sprint set and after all 3 RASTs a session rating of perceived exertion (S-RPE) was measured. There were no significant differences in peak (p = 0.18) and average sprint time (p = 0.41). There were no significant differences in perceptual measures: RPE, PRS, FAS, S-RPE or for blood lactate concentration between CMR conditions. Overall, serial rinsing resulted in changes that were most likely trivial, but showed a 50% chance in perceiving a sprint session as less difficult. Rinsing carbohydrates in a serial manner between repeated sprint sets produces trivial changes of sprint speed and perceptual measures from sprint performance.

OBJECTIVE: To determine the clinical effects of injecting DiHA (a mixture of dextranomer molecules and 1% hyaluronan solution in equal proportions) as a "space filler" into the vocal folds of patients experiencing insufficient closure of the vocal folds resulting from either unilateral vocal fold paralysis or bowed vocal folds. STUDY DESIGN: A consecutive series of patients with insufficient closure of the vocal folds treated with DiHA injections and prospectively followed up with videostroboscopy and voice recordings that were evaluated by expert panels. METHODS: During a 21/2-year period, 14 patients with insufficient closure of the vocal folds were included. Six had a unilateral vocal fold palsy and 8 patients had "bowed" vocal folds. DiHA was injected into one vocal fold. Laryngoscopic examination and voice recordings were performed pre- and postoperatively. Expert panels did the evaluation of the stroboscopy examination and the perceptual voice analysis. RESULTS: Three patients were operated on with local and 11 under general anesthesia. All patients with unilateral palsy improved their stroboscopic status regarding both the wave and the closure after injection. Also, their voice parameters were significantly improved. Among the patients with bowed vocal folds, all except 2 showed an improvement in the glottic wave, 5 of 8 improved in glottic closure, and 3 improved their voice parameters significantly. CONCLUSION: This early clinical experience indicates that DiHA fulfills most of the requirements of an ideal "space-filling" substance for voice restoration in patients experiencing insufficient vocal fold closure.

Polychlorodibenzo-para-dioxins (PCDDs) and Polychlorodibenzofurans (PCDFs) are ubiquitous in the environment. They are mainly formed as unwanted byproducts during various chemical, industrial, and combustion processes. Thus, these pollutants can be found in the food chain. The aim of this experiment was to study the transfer of PCDD/Fs from spiked milk to arterial blood in pigs, which are considered as valid models for humans. Pigs were fed with 900 mL of milk spiked with a mixture of 17 dioxins. The levels of PCDD/Fs in the serum extracts were determined using HRGC/HRMS prior to consumption of the milk, and at 3, 5, and 7 h after milk ingestion. Concentrations of PCDD/Fs in arterial plasma increased from 3 h to 5 h and decreased at 7 h. At time point 5 h, concentrations were found between 500 and 10000 pg x g(-1) fat. The transfer ratio <> was usually found between 0.7 and 3%. Related to the different milk concentrations, results of this study indicate a similar behavior of the studied molecules.

Plastic pollution is ubiquitous throughout the marine environment, yet estimates of the global abundance and weight of floating plastics have lacked data, particularly from the Southern Hemisphere and remote regions. Here we report an estimate of the total number of plastic particles and their weight floating in the world's oceans from 24 expeditions (2007-2013) across all five sub-tropical gyres, costal Australia, Bay of Bengal and the Mediterranean Sea conducting surface net tows (N = 680) and visual survey transects of large plastic debris (N = 891). Using an oceanographic model of floating debris dispersal calibrated by our data, and correcting for wind-driven vertical mixing, we estimate a minimum of 5.25 trillion particles weighing 268,940 tons. When comparing between four size classes, two microplastic <4.75 mm and meso- and macroplastic >4.75 mm, a tremendous loss of microplastics is observed from the sea surface compared to expected rates of fragmentation, suggesting there are mechanisms at play that remove <4.75 mm plastic particles from the ocean surface.

RESUMEN

A global ocean circulation model is coupled to a particle-tracking model to simulate the transport of floating debris washed into the North Pacific Ocean by the Tohoku tsunami. A release scenario for the tsunami debris is based on coastal population and measured tsunami runup. Archived 2011/2012 hindcast current data is used to model the transport of debris since the tsunami, while data from 2008 to 2012 is used to investigate the distribution of debris on timescales up to 4years. The vast amount of debris pushed into ocean likely represents thousands of years worth of 'normal' litter flux from Japan's urbanized coastline. This is important since a significant fraction of the debris will be comprised of plastics, some of which will degrade into tiny particles and be consumed by marine organisms, thereby allowing adsorbed organic pollutants to enter our food supply in quantities much higher than present.

Five patients with a trisomy of the main part of the short arm of a chromosome 5 represented with the clinical picture of the reversetype of the Cri-du-chat syndrome. Its main features are the spurt of gonions, prominent chin, prominent nose root, subnormal skull perimeter, low I.Q. and muscular hypotonia.

BACKGROUND: Serious complications, such as emboli and mycotic aneurysms, are still frequent in documented cases of infective endocarditis. Infecting organisms other than Streptococcus viridans and Staphylococcus are becoming more common. CASE REPORT: A 8 year-old girl was admitted because of a sudden pain in the right calf followed by complete disability. She had low-grade fever and presented with a moderate heart murmur with no sign of congestive heart failure, a severe pain at palpation of her calf with no Homans sign; she had many dental caries. Laboratory data indicated leukocytosis with increased percentage of polymorphonuclear cells and increased sedimentation rate. Ultrasonography of the calf showed laceration of the muscle with blood suffusion. Echocardiography showed vegetations involving the mitral valve. Intravenous antibiotic therapy with penicillin G and netilmicin was instituted, but mitral insufficiency appeared 7 days later while the fever persisted. At that time, the brain CT scan showed ischemic lesions, while angiography showed several mycotic aneurysms. Neisseria mucosa was recovered from the 5 initial blood cultures 16 days after the onset, and penicillin G was replaced by ampicillin. A second vegetation involving the aortic valve was seen a few days later, and a recent arterial embolism to the right leg was suspected because fever and pain reappeared. The brain ischemic lesions gradually disappeared and a second angiography performed 3 months after the first showed that all but one large mycotic aneurysm had disappeared; this last aneurysm was excised. Four years later, the child is in good health without any neurological sequelae but having mitral insufficiency. CONCLUSION: This girl presented with classical complication of infective endocarditis due to Neisseria mucosa, a saprophytic organism of the oral cavity. This is the second report of such an infection in children.
