RESUMEN

PURPOSE: To evaluate the marginal and internal misfit of fixed partial denture zirconia frameworks developed from conventional impression and intraoral scanning, before and after being subjected to the thermal cycle of the covering ceramic. METHODS: A three-elements fixed partial denture was prepared, molded, and poured with polyurethane. Group CI (n= 7) was impressed by the conventional technique with polyvinyl siloxane material, and the plaster models scanned on the inEosX5 bench scanner. Group DI (n=07) was scanned using the CEREC Bluecam intraoral scanner. The models and images obtained were sent to the laboratory and the frameworks were made using zirconia blocks. After this, they were subjected to the ceramic thermal cycle, simulating the ceramic application. Marginal and internal misfits of the frameworks were measured before (T1) and after (T2) thermal cycle simulation using the replica technique in an optical microscope. Statistical analysis was performed using the mixed effects of linear model tests and comparisons. RESULTS: There were no statistical differences for axial misfit. Significant differences were found between the groups for occlusal, vertical, horizontal, and absolute misfit, where group CI had higher values than group DI (P< 0.001). At the time, there was a statistical difference only in the absolute misfit, where T1 had lower values than T2. The misfit in group CI was greater than in group DI; however, the average misfit values found are low and considered clinically acceptable. CLINICAL SIGNIFICANCE: Knowing marginal and internal misfit is an important step to consolidating digital impressions in fixed partial dentures, implying a secure use of this technique.

STATEMENT OF PROBLEM: The optimal disinfection protocol that controls adverse effects and promotes effective antimicrobial action on removable prostheses is unclear. PURPOSE: This in vitro study investigated the effect of disinfectant solutions on the biological, physical, mechanical, and chemical properties of removable prosthesis materials. MATERIAL AND METHODS: Specimens of polymethyl methacrylate (PMMA) and cobalt chromium (Co-Cr) alloy were immersed in distilled water (PMMA) or artificial saliva (Co-Cr) as the control and in 0.25% sodium hypochlorite (NaOCl0.25%), 0.5% chloramine T (CT0.5%), and 0.15% Triclosan (TR0.15%). The antibiofilm activity was evaluated by microbial load and cell metabolisms of the mixed biofilm. Physical (color change, sorption, solubility, and surface roughness), mechanical (hardness, flexural, and impact strength), and chemical (corrosion) properties were analyzed before and after simulating a 5-year immersion. Laser confocal microscopy, scanning electron microscopy (SEM), and dispersive energy spectroscopy (EDS) complemented the analyses. The data were analyzed by using the Mann-Whitney U test, Kruskal-Wallis with Dunn posttests, 1-way ANOVA, and repeated measures ANOVA (α=.05). RESULTS: All solutions were effective against bacteria, but only NaOCl0.25% eliminated Candida spp. TR0.15%, and CT0.5% increased cell metabolisms. For interaction (time and solution), there was a reduction in PMMA hardness in the control and TR0.15%. Color, sorption, solubility, and flexural strength did not change. CT0.5% and TR0.15% were similar for impact resistance. CT0.5% caused the lowest roughness. NaOCl0.25% showed the greatest corrosive potential. Dark spots were seen under SEM in Co-Cr stored with NaOCl0.25% and TR0.15%. EDS indicated different proportions of oxygen, cobalt, chromium, and molybdenum. CONCLUSIONS: NaOCl0.25% had the best antimicrobial action. CT0.5% and TR0.15% have potential. Hardness and roughness changes were clinically acceptable, and the other properties remained unchanged. All the solutions caused color changes. NaOCl0.25% was unsatisfactory for use with Co-Cr, CT0.5% was intermediate, and TR0.15% was suitable.

PURPOSE: This study aimed to develop and evaluate a simple, non-destructive method for assessing the misfit and passivity of implant-retained prostheses frameworks. MATERIALS AND METHODS: To simulate the rehabilitation of a mandible posterior partially edentulous area using 3-unit screw-retained frameworks supported by two implants were fabricated and divided into the following five groups (n = 10 in each group): OP = one-piece framework cast in Co-Cr with the conventional method (control-group); Co-Cr frameworks sectioned and welded by laser (=LAS) or tungsten inert gas (=TIG); Co-Cr CAD-CAM = milled Co-Cr framework; Zir CAD-CAM = milled zirconia framework. The horizontal |X| and vertical |Y| misfits were measured using confocal laser scanning microscopy with one or both screws tightened. Data were analyzed by a two-way ANOVA with repeated measures and Bonferroni correction (α = 0.05). RESULTS: The greatest |X| misfit was observed in the OP group with both screws tightened (290 µm) and one screw tightened (388 and 340 µm). The conventional casting groups sectioned and welded by laser or TIG had lower mean values (235.35 µm, both screws tightened; and 275 µm, one screw tightened) than the OP framework. However, these values still exceeded those of the milled Co-Cr and zirconia frameworks (190 and 216 µm with both screws tightened). Across all reading conditions, every framework subjected to testing consistently maintained vertical |Y| misfit levels below the threshold of 53 µm; however, the milled frameworks exhibited higher vertical misfits than the frameworks obtained by the conventional cast method. CONCLUSIONS: The frameworks, whether cast and sectioned with laser welding or milled from Co-Cr, exhibit improved marginal misfit and enhanced passive fit when compared to other fabrication methods. Additionally, the use of confocal laser scanning microscopy is highly effective for passivity and misfit analysis.

STATEMENT OF PROBLEM: Denture-related stomatitis (DRS), an inflammation frequently present in human immunodeficiency virus-positive (HIV+) individuals, can be attributable to colonization by Candida spp., which is considered a main factor. The virulence factors of these species are often modulated by the systemic condition of their hosts. PURPOSE: The purpose of this clinical study was to evaluate the incidence, virulence, and morphology of Candida spp. isolated from biofilms of complete denture wearers with DRS, with and without an HIV diagnosis. In addition, the interaction of the systemic condition with the ability of Candida spp. to colonize was evaluated. MATERIAL AND METHODS: Fifty-five complete denture wearers diagnosed with DRS were divided into 2 groups: experimental (HIV+) and control (human immunodeficiency virus-noninfected participants [HIV-]). Biofilm was collected by a standardized method of ultrasonification of prostheses. The incidence was evaluated by a chromogenic method and polymerase chain reaction (PCR). The virulence factors were assessed by using the capacity for biofilm formation by counting colony-forming units (CFUs/mL), biofilm metabolism by tetrazolium salt metabolization, and proteinase and phospholipase production by using a fluorimetric kit. Morphology was verified by using the hyphae-inducing test, and participants' health data were collected with a form. Data were analyzed by using the Student t, Mann-Whitney U, Spearman, and Fisher tests (α=.05). RESULTS: The results of incidence were related to 55 participants (22 experimental and 33 control); in total, 63 Candida spp. samples were isolated, showing 28 Candida albicans and 36 nonalbicans strains. No significant difference was found between groups in baseline CFU/mL counts, biofilm formation capacity, cell metabolism, and phospholipase production. Proteinase production was higher for C. albicans in the control (P=.031) and for nonalbicans in the experimental (P=.016) groups. Relative to health data, the experimental group showed a moderate negative correlation between the CFU count/mL at baseline for nonalbicans and DRS classification (P=.020). CONCLUSIONS: C. albicans was the most prevalent species. No difference was found in the Candida spp. of complete denture wearers with DRS, with and without an HIV diagnosis, with regard to virulence factors (except for proteinase production) and morphology.

OBJECTIVE: To evaluate the anti-biofilm action of chitosan, nanoparticulate chitosan, and denture cleanser Nitradine™ against biofilms comprising Candida albicans, Candida glabrata, Staphylococcus aureus, and Streptococcus mutans. BACKGROUND: Biofilm removal from removable partial dentures (RPD) is important for success in prosthetic rehabilitation. MATERIALS AND METHODS: The anti-biofilm action of the experimental chitosan-based solutions and Nitradine™ was evaluated on acrylic resin and cobalt-chromium alloy through assessing cell viability, cell metabolism, residual aggregated biofilm, and extracellular polymeric substance and biofilm morphology. RESULTS: Only chitosan reduced the viability of C. albicans on cobalt-chromium alloy surface, by 98% (a 1.7 log10 reduction in cfu). Chitosan-based solutions neither promoted substantial alteration of the metabolic activity of the four-species biofilm nor reduced the amount of the aggregated biofilm. After immersion in chitosan and nanoparticulate chitosan, viable microorganisms and extracellular polymeric substances distributed over the entire specimens' surfaces were observed. Nitradine™ reduced the viability and metabolic activity of biofilm grown on both surfaces, but it did not remove all aggregated biofilm and extracellular polymeric substances. After immersion in Nitradine™, approximately 35% of the specimens' surfaces remained covered by aggregated biofilm, mainly composed of dead cells. CONCLUSION: Although chitosan and Nitradine™ promoted changes in the viability of microorganisms, neither solution completely removed the four-species biofilm from the Co-Cr and acrylic resin surfaces. Thus, isolated use of hygiene solutions is not indicated for biofilm control on RPDs; this requires complementary mechanical removal.

PURPOSE: To compare the stress distribution through photoelasticity, microhardness and roughness of intact crowns of primary molars (CC) and the preformed crowns of stainless steel (SSC) and zirconia (ZC) used in dental restorations in pediatric dentistry. METHODS: Six healthy primary molars were selected. For the photoelastic models, the teeth were fixed in photoelastic resin. A load of 100 N was applied, and the models were analyzed by transmission polariscope. The Tardy method was used to quantify the fringe order which calculates the maximum stress (T) value in each selected point. The teeth were prepared for cementation of the crowns. The photoelastic test was repeated for each experimental crown. Knoop microhardness was assessed on the buccal surfaces of the CCs, SSCs, and ZCs using a microhardness tester. Parameters were 50 gf for 5 seconds. Roughness was evaluated using a confocal 3-D laser scanning microscope/software at 216x magnification. Roughness average (Ra) values from each model (expressed in µm) were collected and group means were calculated. The stress distribution, microhardness, and roughness data were compared by using one-way ANOVA and the Tukey's test (α= 0.05). RESULTS: There was no difference in the stress distribution for the CCs, SSCs and ZCs. For the microhardness analysis, the ZCs obtained the highest values compared to the CCs and the SSCs (P< 0.001). The CCs were significantly higher than the SSCs (P= 0.027). There was no difference in roughness for the three models (P= 0.615). The SSCs and ZCs showed satisfactory mechanical behavior. CLINICAL SIGNIFICANCE: The use of preformed crowns, especially those made of esthetic materials, is currently increasing in the field of pediatric dentistry. The knowledge of mechanical properties of stainless steel- and zirconia-prefabricated crowns provides scientific foundation for safe clinical application, especially in primary teeth.

STATEMENT OF PROBLEM: Frequent maintenance because of the limited lifetime of overdenture attachments with O-rings has led to the development of materials that might improve their functionality and longevity. However, testing of newly developed attachment materials is lacking. PURPOSE: The purpose of this in vitro study was to evaluate a newly developed attachment made of polyetheretherketone (PEEK) for an implant-retained overdenture. MATERIAL AND METHODS: Specimens of PEEK, polyacetal, and Teflon O-ring materials were prepared for analysis of roughness, surface hardness, and compressive strength. For the fatigue resistance test, new specimens based on acrylic resin were subjected to 2880 insertion and removal cycles. Compression and roughness data were compared with the Kruskal-Wallis and Dunn post hoc test; hardness data with ANOVA and t test; and fatigue and stereomicroscopy with ANOVA with repeated measures, t test, and Bonferroni adjustment (α=.05). RESULTS: Polyacetal had the lowest surface roughness (P=.038). There was a significant difference in hardness among the materials (P<.05). PEEK presented the highest compressive strength among the materials (P<.001). For the fatigue resistance, only polyacetal showed a difference between the initial time-point (P<.05) and subsequent time-points. Polyacetal had a higher fatigue resistance than Teflon (P<.001) and PEEK (P<.05). Regarding the internal deformation of the attachments, a significant difference was observed among the materials (P<.05). CONCLUSIONS: PEEK showed promising results regarding the physicomechanical properties necessary for use as an overdenture attachment when compared with other evaluated materials. In addition, the PEEK attachment showed results comparable to those in the control group (O-rings) in terms of retention.

This study aimed to test and compare 2 novel dental mini-implant designs to support overdentures with a commercial model, regarding the stress distribution, by photoelastic analysis. Three different mini-implant designs (Ø 2.0 mm × 10 mm) were tested: G1-experimental threaded (design with threads and 3 longitudinal and equidistant self-cutting chamfers), G2-experimental helical (design with 2 long self-cutting chamfers in the helical arrangement), and G3-Intra-Lock System. After including the mini-implants in a photoelastic resin, they were subjected to a static load of 100 N under two situations: axial and inclined model (30°). The fringe orders (n), that represents the intensity of stresses were analyzed around the mini-implants body and quantified using Tardy's method that calculates the maximum shear stress (τ) value in each point selected. In axial models, less stress was observed in the cervical third mini-implants, mainly in G1 and G2. In inclined models (30°), higher stresses were generated on the opposite side of the load application, mainly in the cervical third of G2 and G3. All mini-implant models presented lower tensions in the cervical third compared with the middle and apical third. The new mini-implants tested (G1 and G2) showed lower stresses than the G3 in the cervical third under axial load, while loading in the inclined model generated greater stresses in the cervical of G2.

PURPOSE: This randomized controlled trial compared four denture hygiene protocols in terms of patient satisfaction, oral health-related quality of life (OHRQoL), and salivary parameters in complete denture wearers with denture stomatitis. MATERIAL AND METHODS: For this randomized, double-blind, controlled clinical trial, 108 participants were assigned to soak their dentures in one of the following solutions: (1) 0.25% sodium hypochlorite (positive control), (2) 0.15% Triclosan, (3) denture disinfecting tablets, or (4) denture disinfecting tablets plus palatine mucosa brushing solution. The outcomes of patient satisfaction, OHRQoL, and salivary parameters (salivary flow rate and pH) were measured at baseline and after 10 days. The Kruskal-Wallis and Dunn tests (between groups), and Wilcoxon test (between times) were used to compare the results. (α = 0.05). RESULTS: After the hygiene protocols, and when compared with baseline, the overall patient satisfaction, maxillary denture satisfaction, maxillary denture comfort, and maxillary denture retention were ameliorated. A significant improvement was noted in OHRQoL in 3 of 4 domains evaluated (orofacial pain and discomfort, masticatory discomfort and disability, and psychological disability and discomfort). The salivary flow rate (unstimulated and stimulated) and salivary pH were not significantly affected at the times evaluated. CONCLUSIONS: Complete denture wearers may feel more satisfied with their complete dentures when treated for denture stomatitis. The tested treatments lead to similar improvement in terms of patient satisfaction and OHRQoL.

PURPOSE: To evaluate the color stability (ΔE), surface roughness (ΔRa), and flexural strength of denture base acrylic resin after short (20 minutes) and long-term (8 hours) immersions simulating a period of 5 years of use. METHODS: 120 disk-shaped (16 mm x 4 mm) and 120 rectangular (65 mm x 10 mm x 3.3 mm) specimens were prepared (Lucitone 550) and distributed into three groups (n=20): distilled water (Control); Corega Tabs (CT) and Polident (Po), for immersions of 20 minutes and 8 hours. ΔE were determined by a colorimeter, ΔRa by a profilometer and the flexural strength by a universal testing machine. Data were evaluated by two-way ANOVA followed by Tukey's test and Bonferroni correction (α= 0.05). RESULTS: Short-term immersions for 20 minutes resulted in significant differences in ΔE, as CT values (1.46 ± 1.41) were higher than Po (0.55 ± 0.25) and Control (0.47 ± 0.21). Specimens immersed for 8 hours showed more extensive alterations overall than the control (ΔE [Po: 7.27 ± 0.53; CT: 5.58 ± 0.49; Control: 1.26 ± 0.23]; 916;Ra [Po: 0.16 ± 0.08; CT: 0.12 ± 0.11; Control: 0.07 ± 0.06]; flexural strength [Po: 42.99 ± 9.34; CT: 63.96 ± 12.98; Control: 64.59 ± 12.87]). The tested alkaline peroxide-based solutions promoted significant alteration on acrylic resin properties after overnight immersion. Therefore, short-term immersions seem to be more suitable for daily denture hygiene. CLINICAL SIGNIFICANCE: Alkaline peroxides were able to alter color stability, surface roughness and flexural strength of acrylic resin after overnight immersions, in a 5-year-simulated period of use.

Evaluate, through a randomized clinical trial, the efficacy of brushing associated with oral irrigation in maintaining implant and overdenture hygiene. Thirty-eight participants, who had a clinically acceptable conventional maxillary complete denture and mandibular overdenture retained by either implants or mini-implants using an O-ring-retained system, were enrolled to participate in the study. They were instructed to use two different hygiene methods, in a random sequence for a period of 14 days, with a 7-day wash-out interposed period: (I) mechanical brushing (MB); (II) association of mechanical brushing with oral irrigation (WP). Biofilms from both subgingival sulci and overdentures were collected and processed by Checkerboard DNA-DNA hybridization method at baseline and after using the proposed hygiene protocols. Comparisons were performed using Wilcoxon test and Friedman test with Benjamini-Hochberg false discovery rate, followed by Conover post-hoc test (α = 0.05). In the subgingival sulci-related biofilm, a lower number of microbial cells were detected, after WP compared to the MB method (P < 0.001). The findings of overdenture-related biofilm suggest that both methods were similar (P = 0.607) being the identified microbiota qualitatively coincident after each method. Despite the number of microbial counts, it was concluded that the association of mechanical brushing with oral irrigation was more effective in reducing microorganisms in the subgingival sulci biofilm; however, the same outcome was not observed in the overdentures.

STATEMENT OF PROBLEM: Oxidation heat treatment has been studied to increase the metal-ceramic bond strength. However, information about its use with cobalt-chromium (Co-Cr) alloys is lacking. PURPOSE: The purpose of this study was to evaluate the effect of oxidation heat treatment and oxidation heat treatment with alumina airborne-particle abrasion on the metal-ceramic bond strength of Co-Cr alloys compared with that of nickel-chromium (Ni-Cr) alloys. MATERIAL AND METHODS: In total, 165 metal cylinders (∅5×8 mm) made of 5 base metal alloys were obtained by casting: 2 Ni-Cr (Fit Cast-SB and Fit Cast-V) and 3 Co-Cr alloys (Keragen, StarLoy C, and Remanium 2001). The specimens were divided into groups (n=11): no treatment, oxidation heat treatment, and oxidation heat treatment with airborne-particle abrasion. Oxidation heat treatment was performed starting at 650 °C and rising to 980 °C. The airborne-particle abrasion was performed with 100-µm alumina (0.2-MPa pressure, 5 seconds). One specimen had the surface topography and composition evaluated by scanning electron microscopy and energy dispersive X-ray spectrometry. The feldspathic ceramic was applied to the base metal alloy specimens (n=10). Shear tests were performed to obtain the metal-ceramic bond strength (MPa). The failure modes were evaluated. Data were evaluated by 2-way ANOVA and the Tukey post hoc test, Pearson correlation, and Fisher exact tests (α=.05). RESULTS: The group without treatment showed the highest roughness. The treatments increased oxygen and chromium levels and decreased nickel, molybdenum, and tungsten levels. Oxidation heat treatment provided an increase in metal-ceramic bond strength (P<.05) for base metal alloys with over 7% molybdenum (Fit Cast-SB, Fit Cast-V, and Remanium 2001). With oxidation heat treatment with airborne-particle abrasion, there was improvement only in Fit Cast-SB. No treatment was better for StarLoy C. A weak correlation was found between metal-ceramic bond strength and failure mode (ρ=.166; P=.043). The mixed failures were prevalent in Co-Cr alloys (P<.001) and oxidation heat treatment with airborne-particle abrasion (P=.008). CONCLUSIONS: The oxidation heat treatment was only beneficial for base metal alloy with a molybdenum content of over 7%. Although the oxidation heat treatment with alumina airborne-particle abrasion was a better treatment for Fit Cast-SB, its use is not justified because it showed no difference for oxidation heat treatment and requires another step in the surface treatment.

RESUMEN

PURPOSE: To investigate the impact of peroxide-based solutions in reducing viability and metabolic activity of multispecies biofilms on denture base acrylic resin surfaces and for removing them from these surfaces. BACKGROUND: Denture cleansers are effective in reducing monospecies biofilm; however, studies evaluating their action on multispecies biofilms are scarce. MATERIALS AND METHODS: Sixty-nine denture base acrylic resin specimens (Ø 15 × 3 mm) were sterilised then contaminated with Candida albicans, Staphylococcus aureus and Pseudomonas aeruginosa to form multispecies biofilms. Biofilms were grown for 24 hours; subsequently, specimens were immersed in three different cleansing solutions (n = 9): nitradine (NI), fixodent (FX) and phosphate-buffered saline (Control), according to the respective manufacturer's instructions. After applying the hygiene protocols, viability of microorganisms was evaluated by counting colony-forming units and assessing metabolic activity. Moreover, biofilm removal capacity was estimated based on extension of cell-covered areas visualised in fluorescent microscopy micrographics. RESULTS: Microbial counts were solution-dependent; NI was effective against all microorganisms (P < .05). FX exhibited moderate antimicrobial action, reducing P aeruginosa (P < .05) and S aureus (P < .05) viability by approximately 2 logs. Both peroxide-based solutions reduced metabolic activity (P < .001) and biofilm-covered areas on specimen surfaces (P < .001). CONCLUSION: Under the experimental conditions tested, these results demonstrated that peroxide-based solutions had favourable antimicrobial activity but promoted no broad elimination of aggregated multispecies biofilm. NI might be more suitable as complementary chemical agent for controlling multispecies denture biofilm.

Hygiene of implant-supported prostheses and the health of peri-implant tissues are considered to be predictive factors of successful prosthetic rehabilitation. Therefore, the purpose of this study was to evaluate the effectiveness of brushing associated with oral irrigation measured as biofilm-removing capacity, maintenance of healthy oral tissues, and patient satisfaction. A randomized, crossover clinical trial was conducted with 38 patients who wore conventional maxillary complete dentures and mandibular overdentures retained by the O-ring system. The patients were instructed to use the following hygiene methods for 14 days: mechanical brushing (MB; brush and dentifrice, control) and MB with oral irrigation (Waterpik [WP], experimental). Biofilm-removing capacity and maintenance of healthy oral tissues were evaluated by the modified plaque index (PI), gingival index (GI), probing depth (PD), and bleeding on probing index (BP) recorded at baseline and after each method. In parallel, patients answered a specific questionnaire using a visual analog scale after each hygiene method. Data were analyzed by Friedman and Wilcoxon tests (α = .05). The results showed significantly lower PI, GI, PD, and BP after application of the hygiene methods (P < .001) than those observed at baseline. However, no significant difference was found between MB and WP. The responses to the satisfaction questionnaire for both methods showed high mean values for all questions, with no statistically significant difference found between the answers given after the use of MB and WP (P > .05). The findings suggest that WP was effective in reducing PI, GI, PD, and BP indices and provided a high level of patient satisfaction.

PURPOSE: This prospective study evaluated and correlated the impact of ocular prostheses on quality of life and stress with socioeconomic level and clinical characteristics. The clinical difficulties and perceptions of patients after ocular rehabilitation were evaluated. MATERIALS AND METHODS: Anophthalmic patients (at least 18 years of age) who were not users of ocular prostheses were recruited. The Medical Outcomes Short-Form Health Survey (SF-36) and Perceived Stress Scale (PSS-10) questionnaires were administered before and after 3 and 6 months of prosthesis installation. Clinical characteristics, difficulties, and perceptions were evaluated by quantitative and descriptive analysis (7 days, 3 months, 6 months). Data were analyzed by the Friedman test and Pearson Correlation test (α = 0.05). RESULTS: The final sample consisted of 26 patients. Quality of life showed improvement in the "Bodily Pain" and "General Health" domains. Wearing the prosthesis did not influence perceived stress. The clinical evaluation showed clinical discharge over 6 months and presence of pain only at 7 days. A weak correlation occurred between sociodemographic characteristics and the categories "Role-Physical" (r = 0.423) and "General Health" (r = 0.494); cause of anophthalmia and "Role-Physical" (r = -0.471); and type of surgery and "General Health" (r = -0.432). CONCLUSIONS: According to the results of this study, the provision of ocular prostheses showed positive influence in 2 domains of quality of life and weak correlations with socioeconomic level, type of surgery, and cause of loss. Ocular rehabilitation did not influence stress. The use and care of the prostheses did not affect tissue inflammation, but the discharge was continuous.

OBJECTIVE: To evaluate a pilocarpine spray as a treatment for xerostomia in patients treated with radiotherapy (RT) for head and neck cancer (HNC). METHODS: This was a placebo-controlled, double-blind, crossover clinical trial of patients complaining of dry mouth after RT for HNC. Forty patients were randomly assigned to either placebo or pilocarpine (1.54%) spray and instructed to use three times a day for 3 months. After 1-month washout period, patients were crossed over to receive placebo or pilocarpine. The assessments were salivary flow (Stimulated Whole Saliva Flow - SWSF), xerostomia (Xerostomia Inventory - XI), and quality of life (QoL/Oral Health Impact Profile - OHIP-14), assessed at baseline, 1 hr (only SWSF), and at 1, 2, and 3 months of treatment. RESULTS: Posttreatment SWFS was not statistically different between pilocarpine and placebo regardless of the treatment sequence (paired T test; p > .05), except for the SWFS rates at 2 months after therapy. When comparing pilocarpine with placebo in the time points, there was no significant difference (p > .05) for QoL or XI. Significant differences in improvement in QoL and xerostomia experience appeared along time for pilocarpine group. CONCLUSION: The topical application of pilocarpine spray tested was similar to placebo on SWSF assessments in patients treated with RT for HNC.

STATEMENT OF PROBLEM: Antifungals are used to treat Candida infections. However, because of increased antifungal resistance and the length of antifungal therapy, Candida spp. infections can be prevented using the prosthesis hygiene method. Therefore, establishing efficient, safe, and low-cost hygiene protocols for complete denture wearers is necessary. PURPOSE: The purpose of this clinical trial was to compare 10% Ricinus communis (RC10%) and 0.5% chloramine-T (CT0.5%) with negative (water) and positive (0.25% sodium hypochlorite [SH0.25%]) controls to establish a protocol to treat denture stomatitis (DS), remove denture biofilm, reduce overall microbiota, and decrease Candida spp. on the palate and denture bases. MATERIAL AND METHODS: This randomized, double blind, controlled clinical trial allocated 60 DS-positive participants in parallel groups: RC10%, CT0.5%, negative control, and SH0.25%. All participants brushed their palate and dentures and applied 1 of the solutions only to the denture. The following outcomes were assessed at baseline and after 7 and 37 days: Candida spp. counts, frequency of species by presumptive identification, DS severity, and photographic quantification of biofilm. The Kruskal-Wallis and Friedman tests with stepwise step-down post hoc test compared the anticandidal effect and the DS score (between groups and time). ANOVA and the Tukey post hoc test were used for biofilm removal comparison (α=.05). RESULTS: Microbial counts were solution- and time-dependent for dentures, with C. albicans, C. tropicalis, and C. glabrata being the most prevalent species. RC10% presented similar results to baseline and control after 7 and 37 days. CT0.5% reduced the CFU/mL compared with the baseline. SH0.25% was the most effective. DS reduced in all groups, independent of the solution. SH0.25% reduced biofilm the most, followed by RC10%. CT0.5% was similar to the control. CONCLUSIONS: SH0.25% demonstrated potential for Candida spp. control in denture wearers with DS. The other protocols showed intermediate activity and might be more suitable for longer immersion periods.

This study analyzed the biomechanical behavior of rigid and nonrigid tooth-implant supported fixed partial dentures. Different implants were used to observe the load distribution over teeth, implants, and adjacent bone using three-dimensional finite element analysis. A simulation of tooth loss of the first and second right molars was created with an implant placed in the second right molar and a prepared tooth with simulated periodontal ligament (PDL) in the second right premolar. Configurations of two types of implants and their respective abutments-external hexagon (EX) and Morse taper (MT)-were transformed into a 3D format. Metal-ceramic fixed partial dentures were constructed with rigid and nonrigid connections. Mesh generation and data processing were performed on the 3D finite element analysis (FEA) results. Static loading of 50 N (premolar) and 100 N (implant) were applied. When an EX implant was used, with a rigid or nonrigid connection, there was intrusion of the tooth in the distal direction with flexion of the periodontal ligament. Tooth intrusion did not occur when the MT implant was used independent of a rigid or nonrigid connection. The rigid or nonrigid connection resulted in a higher incidence of compressive forces at the cortical bone as well as stress in the abutment/pontic area, regardless of whether EX or MT implants were used. MT implants have a superior biomechanical performance in tooth-implant supported fixed partial dentures. This prevents intrusion of the tooth independent of the connection. Both types of implants studied caused a greater tendency of compressive forces at the crestal area.

Rehabilitation with implant prostheses in posterior areas requires the maximum number of possible implants due to the greater masticatory load of the region. However, the necessary minimum requirements are not always present in full. This project analyzed the minimum principal stresses (TMiP, representative of the compressive stress) to the friable structures, specifically the vestibular face of the cortical bone and the vestibular and internal/lingual face of the medullary bone. The experimental groups were as follows: the regular splinted group (GR), with a conventional infrastructure on 3 regular-length Morse taper implants (4 × 11 mm); and the regular pontic group (GP), with a pontic infrastructure on 2 regular-length Morse taper implants (4 × 11 mm). The results showed that the TMiP of the cortical and medullary bones were greater for the GP in regions surrounding the implants (especially in the cervical and apical areas of the same region) but they did not reach bone damage levels, at least under the loads applied in this study. It was concluded that greater stress observed in the GP demonstrates greater fragility with this modality of rehabilitation; this should draw the professional's attention to possible biomechanical implications. Whenever possible, professionals should give preference to use of a greater number of implants in the rehabilitation system, with a focus on preserving the supporting tissue with the generation of less intense stresses.

PURPOSE: To compare the effect of solutions of effervescent tablets (ET), cetylpyridinium chloride (CPC), and experimental solutions of Ricinus communis on the surface of cobalt-chromium (Co-Cr) alloys. MATERIALS AND METHODS: Fifty-five specimens of Co-Cr were prepared by the lost-wax casting method using circular patterns (∅12 × 3 mm). The specimens were randomly divided into 5 groups: deionized water (control); 2% R. communis; 10% R. communis; ET, and CPC. The surface roughness of specimens (n = 10) was evaluated before immersion (baseline), and at simulated times of ½, 1, 2, 3, 4, and 5 years, by laser confocal microscope (Sa, µm) and profilometer (Ra, µm). The surface topography and chemical composition (n = 1) was qualitatively analyzed with scanning electron microscopy (SEM), and energy dispersive X-ray spectrometry (EDS). Data were subjected to Kruskal-Wallis followed by Dunn tests, and Friedman followed by Wilcoxon tests (α = 0.05). RESULTS: For Sa, there was no difference for the solution factor. For the time factor a significant difference was found with 2% R. communis solution among baseline and ½, 2, 3, and 5 years (p < 0.001) and with 10% R. communis solution between 1 and 2 years (p = 0.007), with decreasing roughness over time. For Ra, cetylpyridinium chloride exhibited less roughness than 10% R. communis solution in ½ (p = 0.048) and 5 years (p = 0.013). In the SEM and EDS analysis the solutions did not present deleterious effects or changes in the chemical composition on the surfaces. CONCLUSIONS: Although a significant difference was found for the roughness, the results, below 0.2 µm, are clinically acceptable. Thus, all solutions can be used safely in removable partial denture cleaning for a period of 5 years.
