RESUMO

General transcription factor IIIC subunit 5 (GTF3C5) encodes transcription factor IIIC63 (TFIIIC63). It binds to DNA to recruit another transcription factor, TFIIIB, and RNA polymerase III (Pol III) to mediate the transcription of small noncoding RNAs, such as tRNAs. Here, we report four individuals from three families presenting with a multisystem developmental disorder phenotype with biallelic variants in GTF3C5. The overlapping features include growth retardation, developmental delay, intellectual disability, dental anomalies, cerebellar malformations, delayed bone age, skeletal anomalies, and facial dysmorphism. Using lymphoblastoid cell lines (LCLs) from two affected individuals, we observed a reduction in TFIIIC63 protein levels compared to control LCLs. Genome binding of TFIIIC63 protein is also reduced in LCL from one of the affected individuals. Additionally, approximately 40% of Pol III binding regions exhibited reduction in the level of Pol III occupancy in the mutant genome relative to the control, while approximately 54% of target regions showed comparable levels of Pol III occupancy between the two, indicating partial impairment of Pol III occupancy in the mutant genome. Yeasts with subject-specific variants showed temperature sensitivity and impaired growth, supporting the notion that the identified variants have deleterious effects. gtf3c5 mutant zebrafish showed developmental defects, including a smaller body, head, and eyes. Taken together, our data show that GTF3C5 plays an important role in embryonic development, and that biallelic variants in this gene cause a multisystem developmental disorder. Our study adds GTF3C5-related disorder to the growing list of genetic disorders associated with Pol III transcription machinery.

OBJECTIVES: The aim of this study was to evaluate esthetic parameters in the anterior maxillary region by comparing single-piece zirconia versus titanium narrow-diameter implants. Additionally, clinical, radiological and patient-reported outcome measures (PROMs) were analyzed. MATERIALS AND METHODS: Thirty implants (tissue level implant) were placed in 30 patients in the maxillary esthetic sector. Depending on randomization, a zirconia (test) or titanium implant (control) was placed. Esthetic, clinical, and radiological parameters, including the implant crown esthetic index (ICAI), pink esthetic score (PES), probing pocket depth, bleeding on probing, plaque index, and marginal bone levels, were evaluated at 12, 36 and 60 months after loading. RESULTS: Sixty months after crown placement, no significant differences were found between groups. The ICAI values were 5.25 ± 4.21 and 4.50 ± 2.98 for the test and control groups, respectively. The corresponding PES values were 7.44 ± 1.93 and 7.43 ± 1.74 for the test and control groups, respectively. There were no significant intergroup differences for the rest of the parameters evaluated. CONCLUSION: It can be suggested that monotype zirconia implants may serve as a potential alternative to titanium implants in selected clinical scenarios. While the results demonstrated comparable esthetic, clinical, and radiological aspects for zirconia implants as compared to titanium implants after a 5-year follow-up period, further research with larger sample sizes and longer-term follow-up is recommended.

This in vitro study assessed peak temperature and temperature increase (ΔT) within the pulp chamber during different extended photoactivation techniques (EPT-applying similar radiant exposure values) to resin-based composites (RBCs) placed in a Class I cavity preparation in an extracted human lower third molar. A T-type thermocouple was placed in the pulp chamber and connected to a temperature analysis device (Thermes, Physitemp). The tooth was attached to an assembly simulating the in vivo environment (controlled baseline pulp chamber temperature and fluid flow). The real-time pulp chamber temperature was evaluated throughout the photoactivation (Bluephase N, Ivoclar Vivadent) of two bulk-fill RBCs: Tetric N Ceram Bulk Fill (TBF; shade: IVA; Ivoclar Vivadent); Surefill SDR flow + (SDR, shade: Universal; Dentsply Sirona), which were exposed to different curing techniques: 40 s-occlusal surface; 20 s-occlusal + 10 s-buccal + 10 s-lingual surfaces; 10 s-buccal + 10 s + lingual + 20 s-occlusal surfaces. Each EPT delivered 42.4 J/cm2. Vickers hardness (VHN) was measured on the removed, sectioned RBC restorations at the top and bottom middle areas after curing. ΔT and VHN data were analyzed using 2-way ANOVA followed by Bonferroni post-hoc test (α = 0.05). Peak temperature data were analyzed using one-way ANOVA and Dunnett's post-hoc test (α = 0.05). SDR showed higher ΔT values than TBF (p = 0.008) in some EPTs. Neither technique resulted in ΔT values greater than 5.5 °C. Both composites had acceptable bottom/top hardness ratios (greater than 80%), regardless of the photoactivation technique. The evaluated EPTs may be considered safe as a low-temperature increase was noticed within the pulp chamber.

OBJECTIVE: To evaluate the effect of different modeling agents on color changes, surface roughness, and translucency parameters over time in Bulkfill resin composites. MATERIAL AND METHODS: Sixty specimens were prepared using three Bulkfill resin composites (Tetric N-Ceram Bulkfill, Filtek One Bulkfill and Opus Bulkfill) and three modeling agents (Wetting Resin, Optibond FL and Ambar APS). All specimens were subjected to a simulated tooth-brushing (Baseline, 50.000 cycles, 100.000 cycles), and surface roughness was measured at each interval. Surface topography was evaluated. The translucency parameter and color changes were assessed after 24-h, 7-, 14- and 30-day immersion in water, wine, and coffee. Data was analyzed using ANOVA and Bonferroni test. RESULTS: All specimens increased roughness after simulated tooth-brushing (p < 0.05). Wetting Resin decreased roughness on Tetric N-Ceram Bulkfill (p < 0.05). The translucency parameter remained unaltered for all groups. All groups changed color after 30-days of immersion in wine and coffee (p < 0.05). Ambar APS and Wetting Resin on Tetric N-Ceram Bulkfill showed higher color changes when immersed in coffee (p < 0.05). CONCLUSION: The use of modeling agents does not jeopardize translucency properties but may affect surface roughness and color properties of Bulkfill resin composites depending on their composition. CLINICAL SIGNIFICANCE: The findings of this study suggest that surface roughness and color properties are affected by the differences in the composition between modeling agents and Bulkfill resins composites.

OBJECTIVE: In this study, we investigated the modulatory effects of PI3Kγ on IL-17A expression and the progression of experimental periodontitis in vivo. METHODS: Ligature-induced periodontitis was developed around the first molar of mice. Animals were treated with anti-mouse IL-17A or IPI-549 (PI3Kγ inhibitor). In addition, PI3Kγ-deficient mice (PI3Kγ-/-) were used in the study. Alveolar bone loss was measured and real-time PCR of Il17a and Rankl genes was performed. A bioinformatics analysis was carried out using the Gene Set Enrichment Analysis computational tool. RESULTS: Nine days after ligature placement, alveolar bone loss scores were significantly increased, with upregulation of Il17a and Rankl genes in the gingival tissues. Treatment with anti-mouse IL-17A (100 µg/mice) significantly attenuated alveolar bone loss. Mice with ligature-induced periodontitis treated with IPI-549 (3 mg/kg) or PI3Kγ-/- mice showed reduced alveolar bone loss and downregulation of Il17a and Rankl gene expression in the gingival tissues. Consistent with this, the bioinformatics analysis showed upregulation of IL17F, IL17A, IL17D, and STAT3 genes, as well as greater activation of IL-17 and PI3KCI pathways (upregulation of PIK3CG gene) in the gingival tissue of patients with periodontitis. CONCLUSION: PI3Kγ plays an important role in modulating IL-17A expression and alveolar bone loss in vivo and can be considered a promising pathway for the management of periodontal disease and the development of new therapies.

AIM: This bibliometric study analyzed the characteristics of the 100 most cited articles on bone grafts in dentistry. MATERIALS AND METHODS: A database search was performed on the Web of Science Core Collection using a specific search strategy. Scopus and Google Scholar were also consulted for citation comparisons. Data extracted included: title, citation metrics, publication year, journal, study design, graft material, surgical technique, authors, institution, and country. Bibliometric networks were generated using VOSviewer. RESULTS: The identified articles were published between 1991 and 2019. Citation counts ranged from 120 to 1161 (mean: 240, 30). Clinical Oral Implants Research was the most cited journal (5175 citations; 25/100). Xenogeneic bone graft material was the most frequently used (5130 citations; 22/100). Europe had 62 articles (14,604 citations), and the United States was the most prominent country (5209 citations; 22/100). The University of Bern had the highest number of citations (2565 citations; 13/100), with Buser D as the author with the largest number of articles (2648 citations; 12/100). CONCLUSION: This study shows the scientific progress on bone grafts in dentistry. The use of xenogeneic grafts for horizontal and/or vertical ridge augmentation was the most prominent trend.

RESUMO

Apert syndrome is characterized by a wide spectrum of craniofacial clinical features that have been successfully addressed via a variety of midface advancement techniques. Although surgeons have individual preferences as to which specific procedures should be performed to best treat Apert patients, craniofacial plastic surgeons, working in tandem with pediatric neurosurgeons, can identify and evaluate functional limitations and facial morphologic disproportions, and establish appropriate criteria for effective midface advancement technique indication and selection. The purpose of this review article is to present and discuss our rationale for midface advancement technique selection based upon the most common craniofacial characteristics presented by Apert syndrome patients. The present article also provides a grading system that stratifies as major, moderate, and mild, the effect of each midface advancement technique on the different types of Apert syndrome facial features. Surgeons should take into consideration the maximum effect and benefit of each craniofacial osteotomy and how these procedures will alter the craniofacial skeleton. By understanding the long-term effect of each osteotomy on the most common craniofacial characteristics of Apert syndrome patients, craniofacial plastic surgeons and neurosurgeons will be able to customize the surgical procedures they perform in order to achieve the best possible outcomes.

The temperature and Vickers Hardness (VH) at the top and bottom surfaces of three resin-based composites (RBCs) were measured when light-cured using five light-curing units (LCUs). The spectrum, power, and energy delivered to the top of the RBCs and transmitted through the RBCs were measured. Starting at 32℃, the temperature rise produced by the Monet Laser (ML-1 s and 3 s), Valo Grand (VG-3 s and 10 s), DeepCure (DC-10 s), PowerCure, (PC-3 s and 10 s) and PinkWave (PW-10 s) were measured at the bottom of specimens 2 mm deep × 6 mm wide made of Filtek Universal A2, Tetric Evoceram A2 and an experimental RBC codenamed Transcend UB. The VH values measured at the top and bottom of these RBCs were analyzed using ANOVA and Scheffe's post hoc test (p < 0.05) to determine the effects of the LCUs on the RBCs. The transmitted power from the ML was reduced by 77.4% through 2 mm of Filtek Universal, whereas light from PW decreased by only 36.8% through Transcend. The highest temperature increases from the LCU combined with the exothermic reaction occurred for Transcend, and overall, no significant differences were detected between Filtek Universal and Tetric Evoceram (p = 0.9756). Transcend achieved the highest VH values at the top and bottom surfaces. The PinkWave used for 10 s produced the largest temperature increase (20.2℃) in Transcend. The Monet used for 1 s produced the smallest increase (7.8℃) and the lowest bottom:top VH ratios.

The study evaluated the pulp temperature (PT) increase in Class I and V preparations when exposed to the Monet Laser (for 1 and 3 s), the PinkWave (for 3 and 10 s), the Valo Grand (for 3 and 10 s), the PowerCure, (for 3 and 10 s) and the SmartLite Pro (for 10 s). Non-retentive Class I and Class V cavities were prepared in one molar fixed in an acrylic plate and positioned in a warm water bath. The PT baseline was kept at 32 °C to simulate physiological conditions. Two T-type thermocouples were inserted through the roots into the pulp chamber in two positions: close to the pulp horn and the buccal wall close to the Class V cavity. The water flow was adjusted to 0.026 mL/min, and real-time temperature data were collected every 0.5 s. PT measurements were made with the tip of the LCU 0 and 6 mm away from the tooth surface. The radiant exitance (mW/cm2) and radiant exposure (J/cm2) were calculated. One-way ANOVA compared the effect of the pulpal flow, and ΔT values were subjected to two-way ANOVA, followed by Scheffe's post hoc tests. The Monet Laser used for 3 s and the PinkWave used for 10 s produced the greatest PT rise in the Class I cavity. The simulated pulpal flow did not influence the PT rise. Overall, cavities exposed at the 0 mm distance had higher ΔT values than groups at 6 mm distance. The placement of a rubber dam for Class V restorations may prevent positioning LCUs directly over the cavity, which may affect the rise in PT.

OBJECTIVE: To evaluate the accuracy of five brands of radiometers in reporting the irradiance (mW/cm2 ) from twelve brands of LCUs compared to a 'Gold Standard' (GS) reference obtained from a hand-held laboratory-grade radiometer. MATERIALS AND METHODS: The irradiance was measured from two examples of twelve brands of previously used LCUs on two examples of five brands of dental radiometers. The emission spectrum was also obtained. Irradiance data from each brand of LCU against each meter was analyzed using the Shapiro-Wilk test for normality. The irradiance values were subjected to a two-way ANOVA followed by Bonferroni tests for each LCU brand. Finally, a descriptive analysis was made using a 95% confidence interval around the mean irradiance. RESULTS: The power output from the LCUs ranged from 271 mW to 1005 mW. Among the tested radiometers, only the Bluephase Meter II could accurately report the irradiance from 11 out of the 12 brands of LCU evaluated in this study. When measured using the "GS" system, the mean irradiance values from the two examples of nine brands of previously used LCU were not always within ±10% of the irradiance values stated by the manufacturer. CONCLUSIONS: The mean irradiance values from 9 of the 12 brands of used LCUs were beyond ±10% of the irradiance values stated by the manufacturer. Only the Bluephase Meter II could accurately report the irradiance from 11 out of the 12 brands of LCU evaluated in this study. CLINICAL SIGNIFICANCE: There was a wide range in the power output from the LCUs tested. It was impossible to accurately measure the irradiance from all the LCUs using the dental radiometers examined. However, dental radiometers should still be used in dental offices to monitor the light output from LCUs and verify that they are working correctly before they are used on patients.

OBJECTIVE: To evaluate the in vitro pulpal temperature rise (ΔT) within the pulp chamber when low- and high-viscosity bulk-fill resin composites are photo-cured using laser or contemporary light curing units (LCUs). MATERIALS AND METHODS: The light output from five LCUs was measured. Non-retentive Class I and V cavities were prepared in one upper molar. Two T-type thermocouples were inserted into the pulp chamber. After the PT values reached 32°C under simulated pulp flow (0.026 mL/min), both cavities were restored with: Filtek One Bulk Fill (3 M), Filtek Bulk Fill Flow (3 M), Tetric PowerFill (Ivoclar Vivadent), or Tetric PowerFlow (Ivoclar Vivadent). The tooth was exposed as follows: Monet Laser (1 and 3 s), PowerCure (3 and 20 s), PinkWave (3 and 20 s), Valo X (5 and 20 s) and SmartLite Pro (20 s). The ΔT data were subjected to one-way ANOVA followed by Scheffe's post hoc test. RESULTS: Monet 1 s (1.9 J) and PinkWave 20 s (30.1 J) delivered the least and the highest amount of energy, respectively. Valo X and PinkWave used for 20 s produced the highest ΔT values (3.4-4.1°C). Monet 1 s, PinkWave 3 s, PowerCure 3 s (except FB-Flow) and Monet 3 s for FB-One and TP-Fill produced the lowest ΔT values (0.9-1.7°C). No significant differences were found among composites. CONCLUSIONS: Short 1- to 3-s exposures produced acceptable temperature rises, regardless of the composite. CLINICAL SIGNIFICANCE: The energy delivered to the tooth by the LCUs affects the temperature rise inside the pulp. The short 1-3 s exposure times used in this study delivered the least amount of energy and produced a lower temperature rise. However, the RBC may not have received sufficient energy to be adequately photo-cured.

PURPOSE: Teeth with poor prognosis are generally recommended to be extracted prior to head and neck radiotherapy (RT) to reduce the risk of developing osteoradionecrosis (ORN), although controversies have been reported. The present systematic review aimed to determine whether tooth extraction prior to head and neck RT may be associated with a reduced risk of developing ORN compared to dental extraction during or after RT. METHODS: The review protocol was registered in PROSPERO (CRD42021241631). The review was reported according to the PRISMA checklist and involved a comprehensive search of PubMed, Scopus, Embase, Cochrane Library, LILACS, and Web of Science, in addition to the gray literature. The selection of studies was performed in two phases by two reviewers independently. The risk of bias of individual studies was analyzed using the Joanna Briggs Institute checklist for cross-sectional studies, and the certainty of evidence was assessed using the GRADE tool. RESULTS: Twenty-eight observational studies were included in the qualitative synthesis, which showed substantial heterogeneity regarding the association between the timing of tooth extraction and ORN development. Twenty-seven of 28 studies were pooled in a meta-analysis that demonstrated a significant association between an increased risk of ORN and post-RT tooth extraction (odds ratio: 1.98; 95% CI: 1.17-3.35; p = 0.01). CONCLUSION: It was confirmed with moderate certainty that dental extractions should be performed prior to the start of head and neck RT to reduce the risk of ORN.

OBJECTIVES: To evaluate the influence of the composite resin translucency used in direct anatomic fiber posts on the bond strength (BS) and microhardness (VHN) of a luting agent into flared roots. MATERIALS AND METHODS: The root canals of 70 single-rooted premolars were endodontically treated and prepared to simulate an oversized root canal. Prior to post cementation, composite resins with varying translucency (high translucent, HT; medium translucent, MT; high opacity, HO) were placed around the fiber posts to create anatomic fiber posts. The attenuation profile (%) of light passing either through the post or through the anatomic posts (n = 8) was obtained prior to the cementing procedures. A positive control group (PC) in which a prefabricated fiber post (PFP) with the diameter compatible with the root canal was cemented and a poorly adapted fiber post (negative control group, NC) were also evaluated. For both tests, the data were subjected to 2-way ANOVA and Bonferroni tests (α = 0.05). RESULTS: A more severe light attenuation through the post at the cervical (P < .001) and medium (P < 0.001) thirds was noted when less translucent composite resin surrounded the anatomic post. HO groups showed lower BS (P = .009) and VHN (P < .001) values than the other groups, regardless of root third. No significant difference in BS values was noted between PC and HT groups. CONCLUSION: The use of a more translucent composite resin in anatomic fiber posts increased the microhardness and bond strength of a dual polymerization self-adhesive RLA compared to the use of MT and HO composite. A well-adapted PFP showed the highest adhesive and mechanical behavior. CLINICAL RELEVANCE: Clinicians should choose more translucent composite resins to create direct anatomic fiber posts to be cemented in flared root canals. That choice may allow improved mechanical properties of self-adhesive RLA and higher bond strength to the root canal as a consequence.

OBJECTIVE: The aims of this study were to evaluate whether the use of a dentifrice containing 5% potassium nitrate (KNO3 ) prior to and during in-office dental bleaching reduces bleaching-induced tooth sensitivity and affects bleaching efficiency. MATERIALS AND METHODS: Thirty-eight individuals were randomly distributed into two groups (n = 19). The experimental group performed toothbrushing using a dentifrice containing 5% KNO3 , one week before treatment and before the first and second bleaching sessions. In the control group, a placebo dentifrice without KNO3 was applied as described for the first group. Tooth sensitivity was recorded on visual analog scales (VAS) and numeric rating scales (NRS) immediately and up to 48 h after bleaching sessions. Color change at different time intervals, was evaluated with shade guide units (∆SGU) and a digital spectrophotometer (∆E CIELab 1976 and CIEDE2000) at baseline and 7, 15, and 30 days post-bleaching. Mann-Whitney test and t-test were used to evaluate TS intensity for NRS and VAS scales, respectively, and T-test was used for color difference evaluation. RESULTS: No significant difference in tooth sensitivity's absolute risk and intensity were observed between tested groups in any evaluated treatment time for NRS (p = 0.91) or VAS scales (p = 0.48). T-test showed no significant difference in both ∆E and ∆SGU tooth color among the experimental and control groups during the different evaluation times (p = 0.27). CONCLUSION: The use of a dentifrice containing 5% KNO3 does not prevent post-operatory tooth sensitivity but allows the same whitening efficiency as a regular dentifrice. CLINICAL SIGNIFICANCE: The use of a dentifrice-containing KNO3 did not prevent bleaching-induced tooth sensitivity when high-concentrated hydrogen peroxide was used for in-office bleaching.

RESUMO

This clinical report describes the replacement of a traumatized maxillary canine that was extracted after unsuccessful orthodontic extrusion with an implant-supported restoration. Guided bone regeneration surgery was performed, followed by a second surgery with implant placement plus simultaneous bone and tissue regeneration. Esthetics and function were achieved with the placement of a veneered zirconia implant-supported restoration.

Chia mucilage (CM) is an emerging resource in food applications. However, the mechanism of this biopolymer as a stabilizer/emulsifier ingredient has not yet been well defined. A non-uniform viscoelastic tridimensional network was observed on emulsions with CM, while the surface activity of the CM ingredient has been associated with its protein content. To understand its functionality in food, this review focused on discussing and summarizing the rheological properties of dispersions and emulsions composed of CM under different conditions, such as pH, temperature, salt content, and mucilage content. For example, emulsions and dispersions with CM showed pseudoplastic behavior. An increase in the CM concentration increased the viscosity and the consistency index and decreased the behavior index. The consistency index of dispersions with CM increased with pH. The future evaluation of emulsions and dispersions properties, such as viscoelastic properties and microstructure, is particularly important for the successful use of CM in the food industry. The principal studies have evaluated the use of CM in dairy and meat systems as an emulsifier, stabilizer, or lipid replacer. The nutritional quality of the products with CM was maintained or improved, but sometimes an undesirable darkening was observed. Future evaluation of the cold extraction method of CM might improve the color and overall sensory acceptability of food products with CM. Integrated chia seed processing, including mucilage, oil, and protein extraction could be carried out to make chia seed industrial processing viable. © 2022 Society of Chemical Industry.

AIM: The objective of this in vitro study was to evaluate the viability and morphology of human fibroblasts and keratinocytes cells, both grown on stainless steel (steel) (18Cr14Ni2.5Mo), and polyether-ether-ketone (PEEK) surfaces, hypothesizing the use of these surfaces as novel materials for prosthetic components. MATERIALS AND METHODS: Gingival human keratinocytes and gingival human fibroblasts lines were grown on discs made by steel (n = 36), PEEK (n = 36), and titanium (Ti) (Ti6A14V) (n = 36)-control. For viability assay, cultures were grown at 24 hours (TV1), 48 hours (TV2), and 72 hours (TV3) times and evaluated by the colorimetric tetrazolium assay (MTT). For morphology and cell adhesion assays, after 24 hours (TM1), 48 hours (TM2), and 96 hours (TM3) of cell culture, cells were examined by scanning electron microscopy (SEM) and analyzed at magnifications with 500×, 1,000×, and 2,500×. RESULTS: Regarding the viability, the keratinocytes did not present statistical difference on the different materials, in TV1 and TV3 times of culture. Their growth rate increased on all materials, being more expressive in steel; the fibroblasts did not present statistical difference on the different materials, in TV2 and TV3 times of culture. The growth rate of these decreased on all materials, being more expressive in PEEK. The morphology analyses show increase in cell numbers, adequate spreading, and adhesion at all cultivation times (TM1, TM2, and TM3) in both cell lines, on all materials. CONCLUSION: All materials tested are suitable for use in the manufacture of prosthetic components for implant-supported rehabilitations, considering the limitations of this study. CLINICAL SIGNIFICANCE: This work analyzes the cellular response of cells present in the human gingiva, as a way to simulate the peri-implant tissue response around novel angular prosthetic components made of stainless steel and PEEK.

During the current pandemic scenario, maxillofacial rehabilitation specialists involved with supportive care in cancer must transform its practice to cope with COVID-19 and improve protocols that could quickly return the oral function of complex cancer patients who cannot wait for surgical complex rehabilitation. This includes the role of the maxillofacial prosthodontist for the rehabilitation of surgically treated patients with maxillary cancers by the means of filling obturator prostheses that are considered an optimal scientific-based strategy to reduce hospital stay with excellent pain control, oral function (speech, swallowing, mastication, and facial esthetics), psychologic and quality of life outcomes for the patients following intraoral cancer resection. Therefore, the aim of this commentary was to bring new lights to the strategic use of obturator prostheses for the rehabilitation of oral cancer patients during the COVID-19 pandemic as well as to present a protocol for managing such cases.

Photobiomodulation therapy (PBMT) has demonstrated efficacy in the prevention and treatment of oral mucositis (OM) in hematopoietic cell transplantation (HCT). However, based on the cell stimulation properties, its long-term safety has been questioned, mainly in relation to risk for secondary malignancies in the oral cavity. The aim of this study was to investigate if different PBMT protocols for OM control have association with immediate and late adverse effects in HCT patients. Data on autologous and allogeneic transplantation, conditioning regimen, PBMT protocols, and OM severity were retrospectively collected from medical and dental records. Presence of secondary malignancies in the oral cavity was surveyed during a 15-year follow-up. Impact of OM on overall survival was also analyzed. Different PBMT protocols for prevention and treatment of OM were recorded over the years. Severe OM (grades 3 and 4) was infrequently observed. When present, we observed a significant decrease of the overall survival. No immediate adverse effect and secondary malignancy was associated to PBMT. In conclusion, the PBMT protocols used in the study were considered safe. The low frequency of severe OM observed encourages the implementation of this technique, with a special emphasis on the dosimetry adjustments focused on the HCT context.

AIMS: Evaluate the abundance of the selected targets, alpha-1-antitrypsin (A1AT) and macrophage migration inhibitory factor (MIF), and correlate these findings with the risk of developing severe oral mucositis (OM). MATERIALS AND METHODS: Head and neck squamous cell carcinoma (HNSCC) patients submitted to radiotherapy (RT) or chemoradiotherapy (CRT) were assessed. OM grade and pain were evaluated daily during treatment. Two protein targets, A1AT and MIF, were evaluated, using selected reaction monitoring-mass spectrometry (SRM-MS), in whole saliva, collected prior to oncologic treatment. The results obtained from the targeted proteomic analysis were correlated with OM clinical outcomes. RESULTS: A total of 27 patients were included, of whom 21 (77.8%) had locally advanced disease (clinical stage III or IV). Most patients (70.4%) received CRT. OM grades 2 (40.8%) and 3 (33.3%) were the most prevalent during RT with a mean highest reported OM-related pain of 3.22 through the visual analogue scale (VAS). The abundance of A1AT and MIF correlated significantly with severe (grades 3 or 4, p < 0.02) compared with moderate-low (grades 1 or 2, p < 0.04) OM grade. CONCLUSIONS: There is a correlation between the abundance of salivary A1AT and MIF and oncologic treatment-induced OM. The correlation of MIF expression with severe OM appears to be compatible with its physiological pro-inflammatory role. These results open up great possibilities for the use of salivary MIF and A1AT levels as prognostic markers for effective therapeutic interventions, such as photobiomodulation therapy, patient-controlled analgesia, or personalized medicaments.
