BACKGROUND: Tooth loss represents a known marker of health inequality. The association between tooth loss and unfavorable socioeconomic conditions is evident when analyzed at an individual level. However, the effects of contextual characteristics on tooth loss need to be better investigated and understood. The objective of this study was to analyze tooth loss among Brazilian adults (35-44 years of age), in accordance with individual and contextual social characteristics. METHODS: This was a multilevel cross-sectional study with data from 9564 adult participants from the Brazilian Oral Health Survey - SBBrasil 2010. The dependent variable was the number of lost teeth and the independent variables were grouped into structural (socioeconomic & political context) and intermediary (socioeconomic position, behavioral & biological factors, and health services) determinants. Multilevel Hierarchical Negative Binomial Regression was conducted and the Mean Ratio (MR) was estimated. RESULTS: Brazilian adults lost a mean of 7.57 (95% CI 7.1-8.1) teeth. Among the contextual variables, the number of teeth lost was higher among residents of municipalities with high and medium/low Municipal Human Development Index (MHDI) and in municipalities that did not have public water fluoridation. Among the individual variables, dental loss was higher among those who declared themselves yellow/black/brown/indigenous, were older, who had lower income, who had never visited a dentist, who had used dental services for more than a year and those whose most recent visit to the dentist was due to oral health problems. On the other hand, dental loss was lower among adults with higher education levels and males. CONCLUSIONS: The number of missing teeth was associated with unfavorable contextual and individual conditions, which reinforces the need to reduce social inequality and guarantee regular, lifetime access to dental services.

OBJECTIVE: To investigate the association of socio-demographic and systemic health factors according to the normative conditions of oral health care (dental caries, edentulism, periodontal disease and oral mucosal lesion) in elderly individuals. MATERIAL AND METHODS: A cross-sectional study was carried out in a group of elderly with access to community health care (n = 200). The normative conditions of oral health were then investigated according to the WHO and the SB Brazil criteria. Bivariate analyses were evaluated by the chi-square test and Fisher's exact test. An estimation of prevalence for the covariates was performed using Poisson's regression models. RESULTS: The prevalence of edentulism and oral mucosal lesions was detected in 58% and 21.5% of elderly patients, respectively. In the dentate subjects, the prevalence of dental caries and periodontal disease was 51.2% and 20.8%, respectively. Older men and individuals from lower-income groups exhibited a higher prevalence of dental caries. Elderly women, illiterate individuals, and individuals over the age of 65 years exhibited a higher prevalence of edentulism. Elderly 60-64 years old and those who are employed had a significant association with periodontal disease. CONCLUSION: Socio-demographic factors were associated with some notable oral diseases in the elderly.

This study evaluated whether the use of dental services of the Unified Health System (SUS) by elderly Brazilians is equitable. Individual data collected by qualified professionals during the national oral health survey (2010) and contextual data from DATASUS and IBGE were used. The dependent variable was the type of service used: SUS and others. Multilevel analyses were conducted using logistic regression (α=5%) (OR/CI 95%) using SPSS 24.0. A total of 6,303 elderly people were included and the prevalence of dental service use in SUS was 30%. Associated contextual factors were: access to bathroom and running water (1.54/1.19-2.00) and the dental care index (1.41/1.10-1.81); the individual factors were: age (0.77/0.66-0.90), years of schooling (1.83/1.53-2.20), family income (2.57/2.20-3.01), reason for last dental visit (0.75/0.60-0.93), decayed teeth total (1.26/1.08-1.48), plugged teeth total (0.63/0.54-0.74), dental prosthesis use (2.23/1.91-2.59), dental pain (1.36/1.11-1.67), self-perception of the need for dental treatment (1.20/1.12-1.51) and need for dental prosthesis (1.38/1.20-1.59). Dental service from SUS has been equitable, however it is necessary to organize the working process, enabling its use in a regular and preventive manner, thereby seeking universality and comprehensiveness.


Avaliou-se, entre idosos brasileiros, se o uso dos serviços odontológicos provenientes do Sistema Único de Saúde (SUS) é equânime. Utilizaram-se dados individuais de exames conduzidos por profissionais calibrados do levantamento nacional de saúde bucal (2010) e dados contextuais originários do DATASUS e do IBGE. A variável dependente foi o tipo de serviço utilizado: SUS e Outros. Foram conduzidas análises multiníveis através de regressão logística (α = 5%) (OR/IC 95%), através do SPSS 24.0. Participaram 6.303 idosos, a prevalência do uso no SUS foi de 30%, os fatores contextuais associados foram o acesso a banheiro e a água encanada (1,54/1,19-2,00) e o índice de cuidados odontológicos (1,41/1,10-1,81); já os individuais: idade (0,77/0,66-0,90), anos de estudo (1,83/1,53-2,20), renda familiar (2,57/2,20-3,01), motivo da última consulta (0,75/0,60-0,93), no de dentes cariados (1,26/1,08-1,48), no de dentes obturados (0,63/0,54-0,74), uso de próteses (2,23/1,91-2,59), dor de dente (1,36/1.11-1,67), autopercepção da necessidade de tratamento odontológico (1,20/1,12-1,51) e da necessidade de próteses (1,38/1,20-159). O uso no SUS tem sido equânime, porém é preciso organizar o processo de trabalho, viabilizando tal uso de forma regular e preventiva buscando a universalidade e a integralidade.

The scope of this study was to identify factors associated with lack of access to dental services. It involved a cross-sectional study and a probabilistic complex sample by conglomerates in two stages with 857 participants over 18 years of age in a large city. Multiple analyses by means of logistic and multivariate regression in decision trees were made. The lack of access to dental services was considered a dependent variable. It was identified that 10.3% did not have access. In the multiple and multivariate analyses an association with age was verified and in the logistic regression a greater possibility of lack of access was found for each year of increased age, among those with the lowest per capita income and those who ranked appearance of teeth and gums as "fair/poor/very poor." The lack of access to dental services was greater among the most socially vulnerable. There is a pressing need to increase the allocation of public resources to promote health education and provide knowledge about how to access services when they are needed, focusing on dental care as a human right and ensuring that lack of access does not occur as users get older or among those with low income and also those dissatisfied with their oral appearance.


Objetivou-se identificar fatores associados à falta de acesso aos serviços odontológicos. Estudo transversal, amostra complexa probabilística por conglomerados em dois estágios de 857 participantes, com mais de 18 anos em um município de grande porte populacional. Foram feitas análises múltiplas através da regressão logística e multivariada em árvores de decisão. Considerou-se como variável dependente o acesso aos serviços odontológicos. Identificou-se que 10,3% não obtiveram acesso. Nas análises múltipla e multivariada constataram-se associação com a idade, na regressão logística constatou-se maior chance de falta de acesso a cada ano de idade incrementado, entre aqueles com menor renda per capita e entre os que classificaram a aparência dos dentes e gengivas como "regular/ruim/péssima". A falta de acesso aos serviços odontológicos é maior entre os mais vulneráveis socialmente. Há necessidade de incremento na alocação de recursos públicos que promovam educação em saúde e gere conhecimento sobre como acessar os serviços quando necessitar, tendo em foco os cuidados odontológicos como um direito humano e que possibilitem que não ocorra a falta de acesso à medida que os usuários envelhecem ou entre os com baixa renda e também os insatisfeitos com a aparência bucal.

The routine use of dental services by elderly participants in a representative survey of Brazilians (the SB Brazil Project) was investigated. 2,305 were dentate and 2,704 edentulous. Prevalence of use was 20% and 17%, respectively. Poisson regression showed that for dentate individuals, use was lower among residents who: lived in rural areas; had not received preventive oral health information; had lower incomes; needed a dental prosthesis; had periodontal problems; perceived their chewing as fair, bad, or terrible; felt that oral health affected their social interaction; and reported tooth pain. Among edentulous individuals, use was higher among those who paid out-of-pocket for dental services and was lower among those who: lived in rural areas; had used dental services more than a year previously; self-identified as non-white; had not received preventive oral health information; had less schooling; needed a dental prosthesis; and felt that oral health affected their social interaction. Inequalities, economic barriers, and lack of information jeopardized the routine use of dental services. Targeted public policies are required to correct these problems.

The scope of this paper was to evaluate the satisfaction regarding dental care services and to identify the association between dissatisfaction and contextual/individual variables. It involved a cross-sectional study of a representative sample of 8,943 adults from 177 municipalities, in which 14.9% of adults were dissatisfied. In the multiple analysis there was a greater chance of dissatisfaction with dental services among adults residing in cities with greater social inequality (OR: 1.53, 95% CI: 1.31-1.81) and with a lower proportion of dentists per inhabitant (OR: 1.17; 95% CI: 1.00-1.37); yellow/black/brown/indigenous (OR: 1.12; 95% CI: 0.99-1.27); lower schooling (OR: 1.14; 95% CI: 0.98-1.33); consultation due to oral problems (OR: 1.23; 95% CI: 1.04-1.44); (OR: 2.60; 95% CI: 2.53-3.02) and impact of oral disorders on daily performance (OR: 1.48; 95% CI: 1.30-1.69). The implementation or adequacy of public policies with the aim of improving satisfaction with dental services should prioritize those municipalities with greater social inequality and with fewer dentists and socially disadvantaged users, who self-perceive oral problems, are dissatisfied with their oral health and suffer impacts resulting from oral problems.


Objetivou-se avaliar a satisfação quanto aos serviços de assistência odontológica e identificar associação entre a insatisfação e as variáveis contextuais/individuais. Estudo transversal multinível de dados secundários de uma amostra representativa de 8.943 adultos de 177 municípios. Encontravam-se insatisfeitos 14,9% dos adultos. Registrou-se maior chance de insatisfação com os serviços odontológicos entre adultos que residiam em municípios com maior desigualdade social (OR:1,53;IC95%:1,31-1,81) e com menor proporção de dentistas por habitante (OR:1,17;IC95%: 1,00-1,37); amarelos/negros/pardos/indígenas (OR:1,12; IC95%: 0,99-1,27); menor escolaridade (OR:1,14; IC95%: 0,98-1,33); consulta por motivo de problemas bucais (OR:1,23; IC95%: 1,04-1,44); insatisfeitos com os dentes e boca (OR:2,60;IC95%:2,53-3,02) e com impacto das desordens bucais no desempenho diário (OR:1,48;IC95%:1,30-1,69). A implementação ou adequação de políticas públicas com o intuito de melhorar a satisfação com os serviços odontológicos deve priorizar os municípios com maior desigualdade social e com menos dentistas e usuários socialmente desfavorecidos, que autopercebem problemas bucais, insatisfeitos com sua saúde bucal e com impactos decorrentes dos problemas bucais.

Aim: To verify changes in the frequency of oral hygiene among basic education teachers in Minas Gerais during the COVID-19 pandemic, according to gender. Methods: This is a web survey, epidemiological study. Data collection took place from August to September 2020 through digital form. The dependent variable was the frequency of oral hygiene during the pandemic, categorized as: remained the same, increased, and decreased. Multinomial Logistic Regression was performed. Results: In this study, 15,641 teachers participated, 81.9% of whom were women. Regarding the frequency of oral hygiene, 73.4% reported that it remained the same, 20.1% increased, and 6.5% decreased, with no significant difference between genders. The chances of a decrease in the frequency of oral hygiene were greater in women under 60 years of age, those without children, those who tested positive for COVID-19, those with a worsening health during the pandemic, those with an increased body weight during the pandemic, and those who were sad or depressed during the pandemic. Among men, the chances of reduction were greater among those who did not live with a spouse, those with a worsening health during the pandemic, those who were sad or depressed during the pandemic, and among smokers or ex-smokers. Conclusion: Although the prevalence of changes in the frequency of oral hygiene in the pandemic did not differ between men and women, the factors related to the increase and decrease in frequency were different for each sex.


Introdução: Cuidar da higiene bucal é fundamental para a manutenção da saúde. Objetivo: Verificar as mudanças na frequência de higienização bucal entre professores durante a pandemia da COVID-19, segundo sexo. Métodos: Trata-se de um estudo transversal, do tipo websurvey, realizado com professores da educação básica pública de Minas Gerais. A coleta de dados ocorreu em 2020. A variável dependente foi a frequência de higiene bucal durante a pandemia. Foi realizada Regressão Logística Multinomial. Resultados: Participaram do estudo 15.641 professores, sendo 81,9% mulheres. Em relação à frequência de higiene bucal, 73,4% relataram que permaneceu a mesma, 20,1% aumentou e 6,5% diminuiu, sem diferença significativa entre os sexos. As chances de diminuição da frequência de higienização bucal foram maiores em mulheres com menos de 60 anos, sem filhos, que testaram positivo para a COVID-19, com piora da saúde durante a pandemia, com aumento de peso corporal durante a pandemia e aquelas que estavam tristes ou deprimidas durante a pandemia. Entre os homens, as chances de redução foram maiores entre aqueles que não moravam com cônjuge, com piora da saúde durante a pandemia, aqueles que estavam tristes ou deprimidos durante a pandemia e entre os fumantes ou ex-fumantes. Conclusão: A frequência de higienização bucal na pandemia não apresentou diferenças significativas entre os sexos, no entanto, observou-se, em ambos os sexos, que variáveis referentes às características sociodemográficas, hábitos de vida e condições de saúde, foram associados ao aumento e diminuição da frequência de higienização bucal na pandemia.

This study sought to investigate factors associated with the lack of access to information on oral health among adults. It is a cross-sectional study, carried out among 831 adults (35-44 years of age). The dependent variable was access to information on how to avoid oral problems, and the independent variables were gathered into subgroups according to the theoretical model for literacy in health. Binary logistic regression was carried out, and results were corrected by the design effect. It was observed that 37.5% had no access to information about dental problems. The lack of access was higher among adults who had lower per capita income, were dissatisfied with the dental services provided, did not use dental floss, had unsatisfactory physical control of the quality of life, and self-perceived their oral health as fair/poor/very poor. The likelihood of not having access to information about dental problems among those dissatisfied with the dental services used was 3.28 times higher than for those satisfied with the dental services used. Thus, decreased access to information was related to unfavorable conditions among adults. Health services should ensure appropriate information to their users in order to increase health literacy levels and improve satisfaction and equity.


Objetivou-se investigar os fatores associados à falta de acesso à informação em saúde bucal entre adultos. Trata-se de um estudo transversal, conduzido entre 831 adultos (35-44 anos). A variável dependente foi o acesso a informações sobre como evitar problemas bucais e as independentes foram reunidas em subgrupos conforme modelo teórico de Alfabetização em saúde. Conduziu-se Regressão Logística Binária com correção pelo efeito de desenho. Observou-se que 37,5% não tiveram acesso a informações sobre problemas bucais. A falta de acesso foi maior entre os adultos que possuíam renda per capita menor, estavam insatisfeitos com os serviços odontológicos utilizados, não utilizavam fio dental, apresentavam o domínio físico da qualidade de vida insatisfatório e que autopercebiam a saúde bucal como regular/ruim/péssima. A chance de não ter recebido informações sobre com evitar problemas bucais entre os insatisfeitos com os serviços odontológicos utilizados foi 3,28 vezes aquela observada entre os satisfeitos com os serviços odontológicos utilizados. Assim, o menor acesso esteve relacionado a condições desfavoráveis entre adultos. Os serviços de saúde devem garantir informações adequadas a seus usuários, a fim de aumentar os níveis de Alfabetização em saúde, maior satisfação e equidade.

This study aimed to analyze the validity of self-perceived dental caries and associated factors in a sample of 795 adults (35-44 years). The dependent variable was self-perceived dental caries, and the independent variables were combined in blocks. Three logistic models were performed: (1) all adults; (2) adults with a formal diagnosis of caries; and (3) adults without such caries. Self-perceived dental caries showed 77.7% sensitivity, 58% specificity, 65% accuracy, 52% positive predictive value, and 81% negative predictive value. In Model 1, self-perceived dental caries was associated with time of use of dental services, access to information, flossing, formal diagnosis of caries, self-perceived need for treatment, toothache, and dissatisfaction with oral health and general health. In Model 2, self-perceived dental caries was associated with time of use of dental services, self-perceived need for treatment, and dissatisfaction with oral health and general health. In Model 3, self-perceived dental caries was associated with time of use of dental services, access to information, flossing, self-perceived need for treatment, and dissatisfaction with oral health. Self-perceived dental caries showed limited utility as a diagnostic method.

This study aimed to identify factors associated with dissatisfaction with dental services among adults. It analyzed 830 adult participants of an epidemiological survey of oral health. The dependent variable was dissatisfaction with the dental service, and the independent ones were selected according to the theoretical model set forth by Andersen and Davidson (1997). Estimates were corrected by the sample design effect, and Binary Logistic Regression was carried out. It was found that about 11% of adults were dissatisfied with the dental service. In the final model, dissatisfaction with dental services was lower among older adults (OR = 0.559) and among smokers (OR = 0.332). On the other hand, it was higher among adults who self-perceived their chewing as negative (OR = 2,804), who self-perceived some discomfort in the mouth and head and neck region (OR = 2.065), and among those who did not have access to information on how to avoid oral problems (OR = 3.020). Therefore, the services need to access the perceptions and expectations expressed by users, and provide information in appropriate quantity and quality, in the context of "health literacy" in order to achieve greater satisfaction among its users.

The scope of this study is to establish the profile of elderly users of dental services provided by the Brazilian Unified Health System(SUS) and associated factors from the standpoint of equity. It involves an analytical cross-sectional study with hierarchical modeling conducted on the basis of a complex probabilistic sample of groups of the elderly (65-74 years of age) living in a densely populated Brazilian city. Independent variables were included relating to: socio-demographic characteristics, access to information on health, behaviors/health-care system and health outcomes. Descriptive, bivariate and multiple hierarchical analysis was performed. Of the 480 elderly persons included, 138 (31.2%) used dental services from the SUS. Use of these services was greater as per capita income and level of schooling decreased. It was lower among those who had not conducted exams of their own mouths (oral self-examinations) and higher among those individuals who used dental services for non-routine procedures. In addition, people whose relationship had been affected by oral health issues and a negative perception of their appearance used the SUS more frequently. The conclusion drawn is that the use of dental services of the SUS was most prevalent among the elderly living in precarious conditions.

This study aimed to investigate factors associated with the physical health components (PHC) and mental health components (MHC) of health-related quality of life in adults. It is a population-based study, with household cluster sampling. The dependent variables were the PHC and MHC scores in the 12-Item Short-Form Health Survey (SF-12); the independent variables were social-demographic characteristics relating to health and behaviors. A multiple regression was made by the General Linear Model. Of the 841 interviewees, 31% had PHC adversely affected, and 37.2% had MHC adversely affected; 57% had adverse score in at least one domain. The average scores were 49.9 for the physical health component and 47.1 for the MHC. Higher scores on the PHC were associated with: being male (ß = 1.94), having a car in the family (ß = 0.89), having recently used dental services (ß = 1.86), not having a chronic disease (ß = 4.60), not using any medication (ß = 2.09), not being a smoker (ß = 2.04) and practicing physical activities (ß = 1.73). Higher scores on the MHC were associated with not using medication (ß = 1.91) and not being a smoker (ß = 1.26). There is a need for further studies and policies aimed at maintaining and/or recovery of the physical and mental wellbeing of adults without specific diseases.

The aim of this study was to examine the prevalence of oral cancer self-examinationamong the elderly and confirm whether prevalence was higher among users of the dental services provided by Brazil's Unified Health System (SUS, acronym in Portuguese). A transversal study of elderly people aged between 65 and 74 years living in a large-sized Brazilian municipality was conducted using simple random sampling. Logistic regression was conducted and results were corrected for sample design and unequal weighting using the SPSS(r) software. The study assessed 740 individuals. A total of 492 met the inclusion criteria, of which 101 (22.4%) reported having performed an oral cancer self-examination. Prevalence was higher among users of the dental services provided by the SUS, higher-income individuals, people with higher levels of education, individuals that used a removable dental prosthesis, and people who had not experienced discomfort attributed to oral condition, and lower among people who sought regular and periodic dental treatment and individuals who did not have a drinking habit. This type of self-care should be encouraged by public health policies which respond to the needs of the elderly, with emphasis on users of private and philanthropic services, and other services outside the public health network.

Educative actions are an important component of health promotion in Brazil's primary healthcare program, the Family Health Strategy (FHS). The efficacy of these actions is evidenced by compliance with healthy behaviors and in the reduction of rates of mortality and morbidity. The objective of this study was to identify whether access to information regarding the prevention of oral cancer is greater among elders whose residences are registered with the FHS. SPSS® was utilized to obtain estimates that were corrected for sample design, considering the magnitude of the associations between access to such information with personal determinants, the use and cost of healthcare, health-related behaviors and health outcomes. 58.9% of the 492 participating elders reported having access to such information. We verified that there was a greater chance for access among residents of houses registered by the FHS; those with greater per capita income (2.01/1.183.43); non-smokers (2.00/1.16-3.46); those that realized oral self-examination (6.35/3.46-11.64); and those that did not perceive discomfort in the mouth, head or neck (2.06/1.02-4.17). Access was greater among residents of homes registered by the FHS. Personal determinants of health, health-related behaviors and health outcomes are influenced or influence access to information regarding the prevention and management of oral diseases.

Resumo Objetivou-se identificar fatores associados à falta de acesso aos serviços odontológicos. Estudo transversal, amostra complexa probabilística por conglomerados em dois estágios de 857 participantes, com mais de 18 anos em um município de grande porte populacional. Foram feitas análises múltiplas através da regressão logística e multivariada em árvores de decisão. Considerou-se como variável dependente o acesso aos serviços odontológicos. Identificou-se que 10,3% não obtiveram acesso. Nas análises múltipla e multivariada constataram-se associação com a idade, na regressão logística constatou-se maior chance de falta de acesso a cada ano de idade incrementado, entre aqueles com menor renda per capita e entre os que classificaram a aparência dos dentes e gengivas como "regular/ruim/péssima". A falta de acesso aos serviços odontológicos é maior entre os mais vulneráveis socialmente. Há necessidade de incremento na alocação de recursos públicos que promovam educação em saúde e gere conhecimento sobre como acessar os serviços quando necessitar, tendo em foco os cuidados odontológicos como um direito humano e que possibilitem que não ocorra a falta de acesso à medida que os usuários envelhecem ou entre os com baixa renda e também os insatisfeitos com a aparência bucal.


Abstract The scope of this study was to identify factors associated with lack of access to dental services. It involved a cross-sectional study and a probabilistic complex sample by conglomerates in two stages with 857 participants over 18 years of age in a large city. Multiple analyses by means of logistic and multivariate regression in decision trees were made. The lack of access to dental services was considered a dependent variable. It was identified that 10.3% did not have access. In the multiple and multivariate analyses an association with age was verified and in the logistic regression a greater possibility of lack of access was found for each year of increased age, among those with the lowest per capita income and those who ranked appearance of teeth and gums as "fair/poor/very poor." The lack of access to dental services was greater among the most socially vulnerable. There is a pressing need to increase the allocation of public resources to promote health education and provide knowledge about how to access services when they are needed, focusing on dental care as a human right and ensuring that lack of access does not occur as users get older or among those with low income and also those dissatisfied with their oral appearance.

This study sought to establish the association between tooth loss due to dental caries and variables related to access to information on oral health in adults aged 35-44 years (n = 780) who participated in a large epidemiological study. The dependent variable was total tooth loss due to caries. The independent variables were grouped into blocks: demographic and socio-economic characteristics, use of dental service, access to oral health information (about how to prevent oral problems; oral hygiene; and diet), and behaviors. The analysis was conducted using hierarchical multiple linear regression. The mean number of lost teeth due to caries was 7.03. High numbers of lost teeth were higher among females and older adults; low levels of education; adults who rarely/never received oral hygiene information; those who brushed their teeth once a day or less; and those adults who did not use dental floss (p < 0.05). Adults who rarely/never received oral hygiene information from dental services lost 2.15 more teeth due to caries than those who always/often received such information (p = 0,000). Thus, it is suggested that access to information should be encouraged to contribute to greater equity in oral health.

Resumo Introdução Adultos usam serviços odontológicos públicos ou privados. Objetivo Identificar fatores associados ao uso dos serviços odontológicos provenientes do SUS entre adultos brasileiros, considerando aspectos contextuais e individuais. Método Foram utilizados dados contextuais de 177 municípios do DATA/SUS, do IBGE, do Atlas Brasil e do Projeto SB Brasil/2010, bem como dados individuais de uma amostra pobabilistica do SB Brasil/2010 coletados por dentistas calibrados. O SPSS 19.0® foi utilizado nas análises descritivas e bivariadas, e o STATA 14.0®, na análise multinível. Resultados A prevalência do uso de serviços no SUS foi de 37,9%. O uso de serviços no SUS foi maior nos municípios com pior acesso à coleta de lixo, em indivíduos com menor escolaridade e menor renda familiar, em negros/pardos/amarelos/indígenas, naqueles com dentes cariados e nos que avaliaram a consulta odontológica como regular/ruim/péssima. Já o uso foi menor nos municípios com menor cobertura de equipes de saúde bucal, entre os mais velhos e em indivíduos do sexo masculino. O maior uso entre os mais vulneráveis sugere equidade. Conclusão Constatou-se efeitos contextuais e individuais sobre o comportamento dos adultos quanto ao tipo de serviço odontológico utilizado que devem ser considerados nas políticas de saúde bucal.


Abstract Background Adults who demand for public or private dental services. Objective Factors associated with the use of dental services from SUS were identified among Brazilian adults, considering context and individual factors. Method It was used contextual data from 177 municipalities of DATA/SUS, IBGE, Atlas Brazil and SB Brazil / 2010 Project; and individual data from SB Brazil/2010. SPSS 19.0® was used in descriptive and bivariate analyzes and STATA 14.0® in multilevel analysis. Results The prevalence of use of services in SUS was 37.9%. The use of services in SUS was higher in municipalities with poor access to waste collection and lower in those with lower coverage of oral health teams. The use was lower among older men. Among those with lower schooling, lower family income, among blacks/brown/yellows and Indigenous, those with decayed teeth, and those who evaluated the dental appointment as regular/bad or bad. The greater use among the most vulnerable suggests equity. Conclusion Contextual and individual effects on the behavior of adults regarding the type of dental service used, which should be considered in oral health policies were verified in this study.

Resumo Objetivou-se avaliar a satisfação quanto aos serviços de assistência odontológica e identificar associação entre a insatisfação e as variáveis contextuais/individuais. Estudo transversal multinível de dados secundários de uma amostra representativa de 8.943 adultos de 177 municípios. Encontravam-se insatisfeitos 14,9% dos adultos. Registrou-se maior chance de insatisfação com os serviços odontológicos entre adultos que residiam em municípios com maior desigualdade social (OR:1,53;IC95%:1,31-1,81) e com menor proporção de dentistas por habitante (OR:1,17;IC95%: 1,00-1,37); amarelos/negros/pardos/indígenas (OR:1,12; IC95%: 0,99-1,27); menor escolaridade (OR:1,14; IC95%: 0,98-1,33); consulta por motivo de problemas bucais (OR:1,23; IC95%: 1,04-1,44); insatisfeitos com os dentes e boca (OR:2,60;IC95%:2,53-3,02) e com impacto das desordens bucais no desempenho diário (OR:1,48;IC95%:1,30-1,69). A implementação ou adequação de políticas públicas com o intuito de melhorar a satisfação com os serviços odontológicos deve priorizar os municípios com maior desigualdade social e com menos dentistas e usuários socialmente desfavorecidos, que autopercebem problemas bucais, insatisfeitos com sua saúde bucal e com impactos decorrentes dos problemas bucais.


Abstract The scope of this paper was to evaluate the satisfaction regarding dental care services and to identify the association between dissatisfaction and contextual/individual variables. It involved a cross-sectional study of a representative sample of 8,943 adults from 177 municipalities, in which 14.9% of adults were dissatisfied. In the multiple analysis there was a greater chance of dissatisfaction with dental services among adults residing in cities with greater social inequality (OR: 1.53, 95% CI: 1.31-1.81) and with a lower proportion of dentists per inhabitant (OR: 1.17; 95% CI: 1.00-1.37); yellow/black/brown/indigenous (OR: 1.12; 95% CI: 0.99-1.27); lower schooling (OR: 1.14; 95% CI: 0.98-1.33); consultation due to oral problems (OR: 1.23; 95% CI: 1.04-1.44); (OR: 2.60; 95% CI: 2.53-3.02) and impact of oral disorders on daily performance (OR: 1.48; 95% CI: 1.30-1.69). The implementation or adequacy of public policies with the aim of improving satisfaction with dental services should prioritize those municipalities with greater social inequality and with fewer dentists and socially disadvantaged users, who self-perceive oral problems, are dissatisfied with their oral health and suffer impacts resulting from oral problems.

AIM: To investigate the association between depression, the normative conditions of oral health and serotonin-transporter-linked promoter region (5-HTTLPR) polymorphisms in a community-dwelling elderly sample. METHODS: Elderly adults living in a community (n=200) were submitted to an oral clinical examination. Depression was assessed using the 15-item Geriatric Depression Scale. The 5-HTTLPR was analyzed by polymerase chain reaction-restriction fragment length polymorphism. Associations between the biomarkers and clinical covariates were assessed by bivariate and multivariate statistical tests. Statistical significance was set at P<0.05. RESULTS: The prevalence of dental caries, periodontal disease, edentulism and oral mucosal lesions was 51.2%, 20.8%, 58.0% and 21.5%, respectively. The prevalence of depression was 21.5%. A higher frequency of the S allele was recorded in older individuals with depression (P=0.017). The present findings showed no significant association between depression and the normative conditions of oral health. The observed allelic polymorphisms were 44% S and 56% L, respectively. Genotypic variants were 33.5% LL, 45.0% SL and 21.5% SS. Individuals carrying the SS genotype showed a significantly high prevalence of periodontal disease (prevalence ratio=3.077, CI 95%=1.008-9.393, P=0.048). CONCLUSION: The present findings suggest that depressive community-dwelling older individuals did not show any association with alterations of oral health normative conditions. However, the SS genotypic polymorphic variant of 5-HTTLPR seems to contribute to the occurrence of periodontal disease in the elderly.

Resumo Objetivou-se investigar os fatores associados à falta de acesso à informação em saúde bucal entre adultos. Trata-se de um estudo transversal, conduzido entre 831 adultos (35-44 anos). A variável dependente foi o acesso a informações sobre como evitar problemas bucais e as independentes foram reunidas em subgrupos conforme modelo teórico de Alfabetização em saúde. Conduziu-se Regressão Logística Binária com correção pelo efeito de desenho. Observou-se que 37,5% não tiveram acesso a informações sobre problemas bucais. A falta de acesso foi maior entre os adultos que possuíam renda per capita menor, estavam insatisfeitos com os serviços odontológicos utilizados, não utilizavam fio dental, apresentavam o domínio físico da qualidade de vida insatisfatório e que autopercebiam a saúde bucal como regular/ruim/péssima. A chance de não ter recebido informações sobre com evitar problemas bucais entre os insatisfeitos com os serviços odontológicos utilizados foi 3,28 vezes aquela observada entre os satisfeitos com os serviços odontológicos utilizados. Assim, o menor acesso esteve relacionado a condições desfavoráveis entre adultos. Os serviços de saúde devem garantir informações adequadas a seus usuários, a fim de aumentar os níveis de Alfabetização em saúde, maior satisfação e equidade.


Abstract This study sought to investigate factors associated with the lack of access to information on oral health among adults. It is a cross-sectional study, carried out among 831 adults (35-44 years of age). The dependent variable was access to information on how to avoid oral problems, and the independent variables were gathered into subgroups according to the theoretical model for literacy in health. Binary logistic regression was carried out, and results were corrected by the design effect. It was observed that 37.5% had no access to information about dental problems. The lack of access was higher among adults who had lower per capita income, were dissatisfied with the dental services provided, did not use dental floss, had unsatisfactory physical control of the quality of life, and self-perceived their oral health as fair/poor/very poor. The likelihood of not having access to information about dental problems among those dissatisfied with the dental services used was 3.28 times higher than for those satisfied with the dental services used. Thus, decreased access to information was related to unfavorable conditions among adults. Health services should ensure appropriate information to their users in order to increase health literacy levels and improve satisfaction and equity.
