Coxsackievirus A16 (CV-A16) is a significant etiologic agent of hand, foot, and mouth disease (HFMD) and herpangina (HA), with the capacity to progress to severe complications, including encephalitis, aseptic meningitis, acute flaccid paralysis, myocarditis, and other critical conditions. Beijing's epidemiological surveillance system, established in 2008, encompasses 29 hospitals and 16 district disease control centers. From 2019 to 2021, the circulation of CV-A16 was characterized by the co-circulation of B1a and B1b clades. Multiple cases of HFMD linked to clade B1c has not been reported in Beijing until 2022. This study enrolled 400 HFMD and 493 HA cases. Employing real-time RT-PCR, 368 enterovirus-positive cases were identified, with 180 selected for sequencing. CV-A16 was detected in 18.89% (34/180) of the cases, second only to CV-A6, identified in 63.33% (114/180). Full-length VP1 gene sequences were successfully amplified and sequenced in 22 cases, revealing the presence of clades B1a, B1b, and B1c in 14, 3, and 5 cases, respectively. A cluster of five B1c clade cases occurred between June 29 and July 17, 2022, within a 7-km diameter region in Shunyi District. Phylogenetic analysis of five complete VP1 gene sequences and two full-genome sequences revealed close clustering with the 2018 Indian strain (GenBank accession: MH780757.1) within the B1c India branch, with NCBI BLAST results showing over 98% similarity. Comparative sequence analysis identified three unique amino acid variations (P3S, V25A, and I235V). The 2022 Shunyi District HFMD cases represent the first instances of spatiotemporally correlated CV-A16 B1c clade infections in Beijing, underscoring the necessity for heightened surveillance of B1c clade CV-A16 in HFMD and HA in this region.

Coxsackievirus A16 (CV-A16) is a significant pathogen responsible for causing hand foot and mouth disease (HFMD) and herpangina (HA). This study aimed to investigate the recent evolution and spread of CV-A16 by monitoring HFMD and HA cases in 29 hospitals across 16 districts in Beijing from 2019 to 2021. The first five cases of HFMD and the first five cases of HA each month in each hospital were included in the study. Real-time reverse transcription polymerase chain reaction was used to identify CV-A16, CV-A6, and EV-A71. From each district, two to four CV-A16 positive samples with a relatively long sampling time interval every month were selected for sequencing. A total of 3344 HFMD cases and 2704 HA cases were enrolled in this study, with 76.0% (2541/3344) of HFMD and 45.4% (1227/2704) of HA cases confirmed to be infected by enterovirus. Among the EV-positive samples, CV-A16 virus was detected in 33.61% (854/2541) of HFMD cases and 13.4% (165/1227) of HA cases, with the predominant cluster being B1a. Both B1a and B1b had a co-circulation of local and imported strains, with different origin time (1993 vs. 1995), different global distribution (14 countries vs. 10 countries), and different transmission centers but mainly distributed in the southern and eastern regions of Beijing. Strengthening surveillance of HFMD in southern and eastern regions will improve the prevention and control efficiency of enterovirus infections.

BACKGROUND: Enterovirus 71(EV71)-associated hand, foot and mouth disease (HFMD) decreased dramatically in Beijing from 2009 to 2019. This study was to investigate the epidemiological characteristics, evolutionary dynamics, geographic diffusion pathway, and other features of EV71 in Beijing, China. METHODS: We conducted a retrospective study of EV71-associated HFMD and its causative agent in Beijing, China, from 2009 to 2019. Phylogenetic and phylogeographic methods based on the EV71 genome were used to determine the evolution features, origin, and spatiotemporal dynamics. Positive selection sites in the VP1 gene were identified and exhibited in the tertiary structure. Bayesian birth-death skyline model was used to estimate the effective reproductive number (Re). RESULTS: EV71-associated HFMD decreased greatly in Beijing. From 2009 to 2019, EV71 strains prevalent in Beijing shared high homology in each gene segment and evolved with a rate of 4.99*10- 3 substitutions per site per year. The genetic diversity of EV71 first increased and peaked in 2012 and then decreased with fluctuations. The time to the most recent common ancestor (TMRCA) of EV71 in Beijing was estimated around 2003 when the EV71 strains were transmitted to Beijing from east China. Beijing played a crucial role in seeding EV71 to central China as well. Two residues (E145Q/G, A293S) under positive selection were detected from both the VP1 dataset and the P1 dataset. They were embedded within the loop of the VP1 capsid and were exposed externally. Mean Re estimate of EV71 in Beijing was about 1.007. CONCLUSION: In recent years, EV71 was not the primary causative agent of HFMD in Beijing. The low Re estimate of EV71 in Beijing implied that strategies for preventing and controlling HFMD were performed effectively. Beijing and east China played a crucial role in disseminating EV71 to other regions in China.

Bone defects still pose various challenges in osteology. As one treatment method for bone defects, tissue engineering requires biomaterials with good biocompatibility and stem cells with good differentiation. This study aimed to fabricate a 3D-printed polylactic acid and hydroxyapatite (PLA/HA) composite scaffold with urine-derived stem cells (USCs) to study its therapeutic effect in a rat model of skull defects. USCs were isolated and extracted from the urine of healthy adult males and inoculated onto PLA/HA and PLA scaffolds fabricated by 3D printing technology. A total of 36 skull defect models in eighteen Sprague-Dawley rats were randomly divided into a control group (no treatment of the defects), PLA group (treated with PLA scaffolds with USCs), and PLA/HA group (treated with PLA/HA scaffolds with USCs). The therapeutic efficacy was evaluated by real-time PCR, microcomputed tomography (micro-CT), and immunohistochemistry at 4, 8, and 12 weeks. We found that the PLA/HA scaffold loaded with USCs effectively promoted new bone regeneration in the defect area. CT images showed that in the PLA/HA group, the defect area was almost entirely covered by newly formed bone (coverage of 96.7 ± 1.6%), and the coverage was greater than that in the PLA group (coverage of 74.6 ± 1.9%) at 12 weeks. Histology and immunohistochemical staining showed the highest new bone formation on the PLA/HA scaffolds containing USCs in the defect site at 12 weeks. These findings demonstrate the broad application prospects of PLA/HA scaffolds with USCs in bone tissue engineering. Graphical abstract.

We investigated the co-assembly of amphiphilic diblock copolymers in solutions containing drugs and functional nanoparticles using the dissipative particle dynamics (DPD) method. By controlling the size and the concentration of the functional nanoparticles, the length of the hydrophobic blocks, and the interaction parameters between the hydrophobic block/solvent and the functional nanoparticles, we obtained the desired aggregates to load drugs. The aggregates loaded with drugs can be disk-like micelles, sphere-like micelles and vesicles with functional nanoparticles on the surface. When the solvent environment changes, the drugs loaded in the functional vesicles can release into the solvent. The release content is critically dependent on the repulsive interaction between the drugs and the solvent. The dynamic curve of drug release is obtained. The result is in agreement with the experiments about drug release. Our studies showed that we can precisely control the formation of functional vesicles to load and release drugs. Loading drugs in the process of self-assembly and controlling the release have broad potential in the field of clinical medicine and adding functional nanoparticles can be of great help in drug delivery and medical diagnosis.

Three novel Zn-based coordination polymers (CPs), [Zn(MIPA)]n (1), {[Zn(MIPA)(4,4'-bipy)0.5(H2O)]·1.5H2O}n (2), and {[Zn(MIPA)(bpe)]·H2O}n (3) (MIPA = 4-methoxyisophthalic acid, 4,4'-bipy = 4,4'-bipyridine, bpe = (E)-1,2-di(pyridine-4-yl)ethane), were constructed by ligand 4-methoxyisophthalic acid under solvothermal conditions. Compound 1 features a beaded 2D-layer architecture, while compound 2 presents a 2-fold interpenetrating structure with a uninodal three-connected hcb topology. Compound 3 has a 3-fold interpenetrated four-connected dmp topology. Photoluminescence investigations of compound 2 were explored in detail, by which ions were detected, and it was observed to have the highest quenching efficiency toward Al3+ and S2- ions. The possible fluorescence quenching mechanisms of 2 toward Al3+ and S2- ions were also explored. To the best of our knowledge, this is the first potential dual-responsive luminescent probe based on a Zn(II) coordination polymer for detecting Al3+ and S2- ions via a luminescence quenching effect in ethanol.

BACKGROUND: Hand-foot-mouth disease (HFMD) is a common infectious disease in China and occurs mostly in infants and children. Beijing is a densely populated megacity, in which HFMD has been increasing in the last decade. The aim of this study was to quantify spatio-temporal characteristics of HFMD and the relationship between meteorological factors and HFMD incidence in Beijing, China. METHODS: Daily counts of HFMD cases from January 2010 to December 2012 were obtained from the Beijing Center for Disease Prevention and Control (CDC). Seasonal trend decomposition with Loess smoothing was used to explore seasonal patterns and temporal trends of HFMD. Bayesian spatiotemporal Poisson regression models were used to quantify spatiotemporal patterns of HFMD incidence and associations with meteorological factors. RESULTS: There were 114,777 HFMD cases reported to Beijing CDC from 1 January 2010 to 31 December 2012 and the raw incidence was 568.6 per 100,000 people. May to July was the peak period of HFMD incidence each year. Low-incidence townships were clustered in central, northeast and southwest regions of Beijing. Mean temperature, relative humidity, wind velocity and sunshine hours were all positively associated with HFMD. The effect of wind velocity was significant with a RR of 3.30 (95%CI: 2.37, 4.60) per meter per second increase, as was sunshine hours with a RR of 1.20 (95%CI: 1.02, 1.40) per 1 hour increase. CONCLUSIONS: The distribution of HFMD in Beijing was spatiotemporally heterogeneous, and was associated with meteorological factors. Meteorological monitoring could be incorporated into prediction and surveillance of HFMD in Beijing.

BACKGROUND: Coxsackievirus A6 (CVA6) is one of the major agents to cause hand, foot and mouth disease (HFMD) outbreaks globally. The objective of this study is to investigate the epidemiologic and clinical manifestations of CVA6 outbreak, and thus guide the diagnosis and treatment of the disease, as well as disease prevention. METHODS: An HFMD outbreak in a kindergarten was reported to Shijingshan District Center for Disease Control and Prevention (SCDC) on November 2, 2015 in Beijing, China. Epidemiological investigation was conducted. We performed a nine-week follow-up study to collect and analyze the clinical manifestations of HFMD cases. RESULTS: The outbreak yield 56 (15.7%) clinical diagnosed HFMD cases out of 357 registered children in the kindergarten with the mean age of 3.5 years old. This outbreak lasted for three days and ceased after initiating infectious disease controlling procedures, including periodical suspension of the kindergarten activities, environmental disinfection, and family health education. Fifty-one cases were followed for nine weeks. The positive rate of clinical manifestations of rash, fever, desquamation, pigmentation and onychomadesis were 100.0%, 84.3%, 68.6%, 17.6% and 43.1%, respectively. Children developed desquamation within the first 4 weeks after disease onset and developed onychomadesis between the 3th and 8th week after disease onset. Children with desquamation had 9.3 (95%CI: 1.836-47.437) times higher odds of developing onychomadesis compared to those without this manifestation. Ten out of 14 collected samples were CVA6 positive, and five positive samples shared a high degree of similarity in the VP1 nucleotide and amino acid sequences (99.9-100.0% and 100%). CONCLUSION: This HFMD outbreak was caused by CVA6, featured with delayed symptoms. Emerging CVA6-associated HFMD and its delayed symptoms should be paid more attention to reduce outbreaks and provide more information to doctors and parents.

BACKGROUND: The incidence of hand, foot, and mouth disease (HFMD) is extremely high, and has constituted a huge disease burden throughout Beijing in recent years. This study aimed to determine the spatiotemporal distribution and epidemic characteristics of HFMD. METHODS: Descriptive statistics was used to analyze the data and estimate the epidemic peaks in 2009-2014. Space-time scanning detected spatiotemporal clusters and identified high-risk locations. Global and local Moran's I statistics were used to measure the spatial autocorrelation. Geocoding was performed in ArcGIS, based on the present address codes of the patients and the centroids of the towns. Maps were created in ArcGIS to show the geographic spread of HFMD. RESULTS: In total, 220,451probable cases of HFMD were reported in Beijing between January 2009 and December 2014: 12,749 (5.78 %) were laboratory confirmed, and 35 (0.02 %) were fatal. The median age of reported cases was 3.12 years (interquartile range 1.96-4.39). Coxsackievirus A16 (CV-A16), enterovirus 71 (EV-A71), and other enteroviruses accounted for 39.31, 35.36, and 25.33 % of the 12,749 confirmed cases, respectively. Many more severe cases were caused by EV-A71 (χ (2) = 186.41, df = 1, P < 0.001) and other enteroviruses (χ (2) = 156.44, df = 1, P < 0.001) than by CV-A16. A large single distinct peak occurred between May and July each year. Spatiotemporal clusters of HFMD were identified in Beijing during 2009-2014. The most likely clusters were detected and tended to move from the southwest (Fengtai and Daxing) southeastwards to Daxing and Tongzhou in 2009-2014. The incidence of HFMD was not randomly distributed, but showed global and local spatial autocorrelations. CONCLUSIONS: There were obvious spatiotemporal clusters of HFMD in Beijing in 2009-2014. High-incidence areas mainly occurred at the junctions of urban and rural zones. More attention should be paid to the epidemiological and spatiotemporal characteristics of HFMD to establish new strategies for its control. Health issues should be especially promoted in kindergartens and at urban-rural junctions.

BACKGROUND: Over the last decade, increases in the number of outbreaks of hand, foot and mouth disease (HFMD) have shifted the disease into the public health spotlight in China. Children under the age of five years are particularly susceptible, with fatalities recorded. However, estimating the burden of HFMD has been difficult to conduct to date. METHODS: In 2012, a cross-sectional survey of healthcare-seeking behaviour for HFMD was undertaken, using computer assisted telephone interviewing (CATI) technology. Sample of telephone numbers was obtained from the Beijing Immunization Information System. Respondents were parents or guardians of children under the age of five. Multiplier model was used to estimate the number of HFMD case, following the telephone survey of healthcare-seeking behavior. The number of laboratory-confirmed cases was also estimated based on the monthly positive rate of each subtype of virus causing HFMD. The age-specific case fatality rate (CFR) was calculated based on the ratio of reported deaths to the estimated number of cases. RESULTS: For children under five, the consultation rate of parent-defined cases was estimated at 77.8% ((95% CI = [75.2, 80.4]). Parents or legal guardians of children aged between two and four years were more likely to seek healthcare than those of children aged less than two years. For children under the age of five, we estimated that there were 40,165 (95% CI = [38,471, 41,974]) HFMD cases, with an incidence rate of 5.6%, and 22,166 (95% CI = [21,150, 23,295]) laboratory-confirmed cases in Beijing during 2012. The overall CFR was estimated at 10 deaths per 100,000 cases, while for children aged less than two years it was 15.6 deaths per 100,000 cases. CONCLUSIONS: Given the public health impact of HFMD in China, control measures need to be prioritized for children < 2 years, due to the higher CFR in this age group. Sentinel surveillance approaches could be used to monitor trends and the impact of control measures.

In this paper, a novel TiO(2) nanoparticle thin film coated optical fiber Fabry-Perot (F-P) sensor had been developed for refractive index (RI) sensing by monitoring the shifts of the fringe contrast in the reflectance spectra. Using in situ liquid phase deposition approach, the TiO(2) nanoparticle thin film could be formed on the fiber surface in a controlled fashion. The optical properties of as-prepared F-P sensors were investigated both theoretically and experimentally. The results indicated that the RI sensitivity of F-P sensors could be effectively improved after the deposition of nanoparticle thin-films. It was about 69.38 dB/RIU, which was 2.6 times higher than that of uncoated one. The linear RI measurement range was also extended from 1.333~1.457 to 1.333~1.8423. More importantly, its optical properties exhibited the unique temperature-independent performance. Therefore, owing to these special optical properties, the TiO(2) nanoparticle thin film coated F-P sensors have great potentials in medical diagnostics, food quality testing, environmental monitoring, biohazard detection and homeland security, even at elevated temperature.

BACKGROUND: Hand, foot and mouth disease (HFMD) is a common illness in young children. It also can be seen in adults occasionally. Enterovirus 71 (EV71), a pathogen that causes not only HFMD but also neurological complications and even death, has caused many HFMD outbreaks in China. However, till now the data about the duration of EV71 shedding is very limited. RESULTS: A total of 136 throat swabs and fecal samples were collected from 27 children and 3 adults, which includs 7 close contacts, 9 mild cases and 14 severe cases,. The participants were divided into three groups namely, severe case group, mild case group and close contact group. All the samples were assayed with real-time polymerase chain reaction (PCR). Kruskal-Wallis Test was employed to compare the difference in duration of viral RNA shedding among three groups. The results showed that significant difference in duration of EV71 shedding was found among three groups (P < 0.01). The longest duration of EV71 shedding in fecal samples is 54 days and 30 days in throat swabs. CONCLUSIONS: HFMD is characterized by extended excretion of EV71. Our results suggest that the duration of EV71 shedding is correlated with the severity of the disease. EV71 shedding through feces can persist more than 54 days. Prolonged virus shedding is a potential risk factor of proliferating HFMD epidemic.

An optical fiber Fabry-Perot (F-P) interferometer end surface was modified using layer-by-layer assembly and chemical covalent cross linking method, and the refractive index (RI) response characteristics of coated optical fiber F-P sensor were experimentally studied. Poly diallyldimethylammonium chloride (PDDA) and sodium polystyrene sulfonate (PSS) were chosen as nano-film materials. With the numbers of layers increasing, the reflection spectral contrast of optical fiber F-P sensor presents from high to low, then to high regularity. And the reflection spectral contrast has good temperature stability. The reflection spectra of the optical F-P sensor coated with 20 bilayers for a series of concentration of sucrose and inorganic solution were measured. Experimental results show that the inflection point extends from 1.457 to 1.462 3, and the reflection spectral contrast sensitivity to low RI material and high RI material is 24.53 and 3.60 dB x RI(-1), respectively, with good linearity. What is already known about this topic?: Current research regarding hand, foot, and mouth disease (HFMD) has primarily concentrated on the economic impacts, drawing from retrospective or sentinel hospital-based data. This approach often overlooks cases that were either not consulted or were misdiagnosed. What is added by this report?: This research systematically examined the iceberg phenomenon of HFMD and its economic implications in Beijing. Our findings indicate that each confirmed case represents 9.1 actual infections, imposing financial burdens of 25.58 United States dollars (USD) per unconsulted individual, 265.75 USD per misdiagnosed individual, 366.50 USD per individual with mild cases, and 2355.89 USD per individual with severe cases. The annual economic losses attributed to HFMD in the area range from 7.03 million USD to 13.31 million USD. What are the implications for public health practice?: This study offers insight into the actual prevalence of HFMD in Beijing, as well as conducting an economic burden analysis on a per-case, per-category basis. This could facilitate a cost-effectiveness analysis of prevention and control strategies for HFMD.

Large-river deltaic estuaries and adjacent continental shelves have experienced multiple phases of transgressions and regressions to form interlayered aquifer-aquitard systems and are expected to host vast paleo-terrestrial groundwater hundreds of kilometres offshore. Here, we used offshore hydrogeology, marine geophysical reflections, porewater geochemistry, and paleo-hydrogeological models, and identified a previously unknown offshore freshened groundwater body with a static volume up to 575.6 ± 44.9 km3 in the Pearl River Estuary and adjacent continental shelf, with the freshwater extending as far as 55 km offshore. An integrated analysis of stable isotopic compositions and water quality indices reveals the meteoric origins of such freshened groundwater and its significance as potential potable water or raw water source for desalination. Hotspots of offshore freshened groundwater in large-river deltaic estuaries and adjacent continental shelves, likely a global phenomenon, have a great potential for exploitable water resources in highly urbanized coastal areas suffering from freshwater shortage.

OBJECTIVE: To compare the differences of epidemiological and clinical characteristics in children with hand-foot-mouth disease (HFMD) caused by Coxsackievirus A16 (CA16) and Enterovirus 71 (EV71). METHODS: The samples of vesicle fluid and throat swabs of 108 children with HFMD were collected and detected for enterovirus by RT-PCR. The clinical data of children with EV71 and CA16 infection were retrospectively reviewed and compared. RESULTS: The total positive rate of enterovirus was 97.2% (105/108). Of the 105 cases, 56 cases were positive for EV71 (51.9%), 39 cases were positive for CA16 (36.1%), 2 cases were positive for other enterovirus (1.9%), and 8 cases were co-infected by EV71 and CA16 (7.4%). There were no significant differences in age and sex between EV71 and CV16 infected cases. The univariate analysis showed that the incidences of herpes of mouth, erythra of knees, and nose running in children infected by CA16 were higher than in those infected by EV71. The multivariate logistic regression analysis showed that the HFMD children who had erythra of knees had higher probability of CA16 infection. CONCLUSIONS: EV71 should be considered as the pathogen in children with HFMD who have no herpes of mouth, erythra of knees, and nose running.

A simple and rapid colorimetric assay for determination of mercury(II) ion (Hg2+) in aqueous solutions was developed based on aggregation of gold/silver alloy nanocomposites (Au/Ag NCs). Au/Ag NCs were aggregated after the addition of Hg2+ and the positively charged amino acid, lysine. The different aggregation degrees of Au/Ag NCs is indicated by variations in the absorption spectra and accompanied by a color change from orange yellow to yellowish green. Under the optimal conditions, the shift of absorbance ratio (A650/A447) was proportional to Hg2+ concentrations in the range of 0.01-10.0 µM. The limit of detection is 4.8 nM. The proposed colorimetric assay was extremely specific for Hg2+ and other environmentally relevant metal ions did not interfere with the determination.

INTRODUCTION: Enterovirus A71(EV-A71)-associated hand, foot, and mouth disease (HFMD) has been reported worldwide, and poses a particularly heavy burden on patients, families, and society in China. Three Chinese companies have licensed inactivated EV-A71 vaccines, all of which have demonstrated good efficacy for preventing EV-A71-associated disease in clinical trials. However, real-world performance of EV-A71 vaccine has not been evaluated. METHODS: We used a test-negative design case-control study to estimate vaccine effectiveness (VE) against medically attended EV-A71-associated HFMD. Subjects were children 5 years of age and under who had been in health facilities participating in the HFMD case and virologic surveillance platforms in Beijing. Enterovirus infections were laboratory confirmed, and EV-A71 vaccination status was extracted from electronic immunization records. Children testing positive for EV-A71 were cases; controls were children testing negative for EV-A71 infection. Logistic regression was used to estimate VE. We assessed sensitivity of VE estimates to control group inclusion criteria by repeating the regression analyses with two alternative control groups. RESULTS: A total of 2,184 HFMD patients aged 5 years and under were enrolled in the study; 24 were severe, and 2,160 were mild. For severe cases, two-dose VE estimate was 100% (95% CI: -68.1%, 100%). For mild cases, 1-dose and 2-dose adjusted VE estimates were 69.8% and 83.7%, respectively. Two-dose VE estimates varied by less than 4 percentage points regardless of control group definition. CONCLUSIONS: Our findings suggested the vaccines performed well in the real world for children 5 years of age and under in Beijing, China.

OBJECTIVE: Chinese herbal medicine has been gradually used to treat pediatric adenoid hypertrophy. This meta-analysis were conducted to evaluate the clinical efficacy and safety of Chinese herbal medicine in the treatment of pediatric adenoid hypertrophy. METHODS: Randomized controlled trials involving Chinese herbal medicine in the treatment of pediatric adenoid hypertrophy were identified from Cochrane Central Register of Controlled Trials, PubMed, EMBASE, Chinese National Knowledge Infrastructure, Chinese Biomedical Database, Wanfang Database and VIP Information Database. The methodological quality of trials was evaluated with Cochrane Handbook criteria, and the Cochrane Collaboration's Review Manager 5.3 software was used for Meta-analysis. RESULTS: A total of 13 valid articles involving 1038 patients were included. The meta-analysis showed that: Compared with western medicine treatment, Chinese herbal medicine significantly improved clinical efficacy (RR = 1.33, 95% CI [1.24,1.43]), and significantly decreased A/N ratio (MD = -0.04,95%CI [-0.05,-0.03]). Chinese herbal medicine also prominently improved the quality of life (MD = -4.77,95%CI [-8.35,-1.20]). Meanwhile, it dramatically improved snoring (MD = -0.46,95%CI [-0.62,-0.30]); mouth breathing (MD = -0.52,95%CI [-0.66,-0.39]); nasal obstruction (MD = -0.56,95%CI [-0.68,-0.45]). CONCLUSION: Chinese herbal medicine has good clinical efficacy and safety on pediatric adenoid hypertrophy, which need to be confirmed by high quality, multiple-centre, large sample randomized controlled trials.
