INTRODUCTION: Displacement of provisional fixed prostheses may result in undesirable and embarrassing outcomes in dental treatments, especially in endodontic treatment. Development of certain counter measures has been necessary to avoid such discomforts. AIM: The aim of this report was to propose a pre-endodontic post and core technique to achieve smooth progress of the treatment. MATERIALS AND METHODS: The patient was a 59-year-old male diagnosed with an infraocclusion caused by wear of his teeth. He received full mouth provisional fixed restorations for a complete oral rehabilitation. Displacement and fracture of the restorations frequently occurred during the observation period for the function of the restorations. Therefore, the pre-endodontic post and core technique was applied to the abutment teeth before their endodontic treatments were started. The technique consisted of three steps as follows: Step 1: Caries removal and dowel preparation were performed for the abutment teeth having apical periodontitis. Composite cores were indirectly fabricated, which had access holes for endodontic treatment. Step 2: The cores were bonded to the teeth. In endodontic treatment, rubber dam appliances were easily placed owing to the core, and proper tooth isolation was accomplished. Step 3: Fiberposts were bonded to the dowel holes through the access holes after the root canal filling. During endodontic treatment, displacement and/or fracture of the provisional restorations did not occur. CONCLUSION: The pre-endodontic post and core technique was effective in obtaining improved retention of provisional restoration, appropriate isolation for endodontic treatment, and sufficient retention of the post and core. CLINICAL SIGNIFICANCE: The pre-endodontic post and core technique is useful for avoiding the discomforts in dental treatments, namely, a smooth transition from endodontic to prosthodontic treatment can be achieved.

PURPOSE: The objective of this study was to assess the influence of the addition of Na2CO3 on the shelf life and bond strength of 4-META self-etch primer. MATERIALS AND METHODS: Two 4-META self-etch primers were experimentally formulated with and without the addition of Na2CO3 (primer A and primer B, respectively). The primers were stored at 37°C for several periods of time after formulation. Clearfil SE Bond Primer (Kuraray Noritake Dental) was used as a control. A composite was bonded to bovine dentin using a combination of the primer and a bonding agent (Clearfil SE Bond), and the shear bond strengths were measured. Changes in the pH of the experimental primers were monitored and macroscopic observations were made as a function of the storage periods. RESULTS: With 0-day storage, the bond strength of primer A was equivalent to that of primer B and significantly lower than that of Clearfil SE Primer. The bond strengths of primer A were stable during 90-day storage; by contrast, the bond strengths of primer B significantly decreased during 7-day storage. primer A exhibited stable pH values during 180-day storage; primer B exhibited significantly lower pH than primer A with 0-day storage and discolored gelation during 7-day storage. CONCLUSION: By adding Na2CO3, the shelf life of the 4-META self-etch primer was obviously improved without significant deterioration in dentin bond strength. Adjusting the pH to around 4.5 was effective for obtaining sufficient bond strength and hydrolytic stability of the primer.

The degree of conversion (DC) of camphorquinone/amine-based adhesives is affected by acidic functional monomers as a result of inactivation of the amine co-initiator through an acid-base reaction. During bonding, functional monomers of self-etch adhesives chemically interact with hydroxyapatite (HAp). Here, we tested in how far the latter interaction of functional monomers with HAp counteracts the expected reduction in DC of camphorquinone/amine-based adhesives. The DC of three experimental adhesive formulations, containing either of the two functional monomers [10-methacryloyloxydecyl dihydrogen phosphate (10-MDP) or 4-methacryloxyethyl trimellitic acid anhydride (4-META)] or no functional monomer (no-FM; control), was measured with and without HAp powder added to the adhesive formulations. Both the variables 'functional monomer' and 'HAp' were found to be significant, with the functional monomer reducing the DC and HAp counteracting this effect. It is concluded that the functional monomers 10-MDP and 4-META interfere with the polymerization efficiency of adhesives. This interference is less prominent in the presence of HAp, which would clinically correspond to when these two functional monomers of the adhesive simultaneously interact with HAp in tooth tissue.

OBJECTIVE: This study assessed the development of stress from polymerization of light-cured dental resin composites for 24 hours after irradiation. MATERIALS AND METHODS: Initial cracks were made near the edge of a cylindrical hole in glass, and crack lengths were measured. Revolution Formula 2, Kalore and Venus Diamond were polymerized in the hole. Crack lengths were measured at several intervals after irradiation up to 24 hours. Stresses at the composite-glass interface were calculated using the crack lengths. Elastic moduli of the composites were measured at the same time intervals. RESULTS: Interfacial stress and elastic modulus were significantly related to material and time. Stress continued to increase up to 12 hours after irradiation. Significantly lower stresses were measured in Kalore and Venus Diamond than Revolution Formula 2 throughout 24 hours. Stress at 24 hours was two times greater than the stress at 30 minutes. The increase in elastic modulus from 30 minutes to 24 hours ranged from 8 to 24%, which was less than the increase in stress over the same time period. CONCLUSIONS: Interfacial stress and elastic modulus of the composites demonstrated the average increases of 155% and 14%, respectively, from 30 minutes to 24 hours after irradiation. CLINICAL SIGNIFICANCE: Since the elastic modulus is a measure of stiffness and resistance to load, clinicians might consider advising their patients to avoid heavy occlusal function until the elastic modulus has reached a plateau, which would be 60 minutes for Venus Diamond, 12 hours for Kalore, and 24 hours for Revolution Formula 2.

This study investigated residual stresses in glass crowns cemented with resin cements. Glass caps were cemented to cylindrical cores using a conventional resin composite cement, a self-adhesive resin cement, or a methyl methacrylate (MMA)-based cement in dual-cure or self-cure mode. The cemented caps were stored in 37°C water for 28 days, and stresses on the cap surface were repeatedly measured. The water sorption, water solubility, and elastic modulus of the cements were also measured. Polymerization of the cements initially generated compressive stresses on the surfaces. Dual-curing or a greater modulus yielded greater initial stress. The stresses gradually decreased over time and lingered on the surfaces at 28 days with all the cements. Greater sorption tended to lead to greater stress reduction; however, the MMA-based cement exhibited less stress reduction despite exhibiting the greatest sorption. The use of a resin composite cement or dual-curing is recommended to reinforce crown restorations.

Chimpanzees (Pan troglodytes) with teeth severely damaged by dental caries and/or periodontal disease are often managed with medication and/or tooth extraction. A common endodontic treatment for severely decayed teeth in a 26-year-old female chimpanzee is reported. The left maxillary central incisor tooth had lost its crown, probably due to trauma that was not recent, and it had a fistula most likely due to chronic apical periodontitis. The diagnosis was confirmed radiographically before treatment. To treat the infected root canal, endodontic treatment used in humans was adapted for a chimpanzee. After the treatment, the tooth was sealed using an adhesive resin composite. At 11-years post-treatment, there were no signs of recurrence of the lesion or of failure of the tooth seal. The results of this case report suggest that common endodontic treatments used in humans are also effective in chimpanzees.

PURPOSE: To compare contraction stresses in direct and indirect composite restorations using crack analysis. MATERIALS AND METHODS: Glass disks with a central cylindrical hole were prepared. Initial cracks were made in the glass using a Vickers indenter at various distances from the edge of the hole. The indentation crack lengths were measured parallel to the tangent of the edge of the hole. Silanized holes were directly or indirectly filled with a resin composite. The composite was either self-curing and light curing or only self-curing. Various thicknesses of composite cement were applied to the indirect restorations. The crack lengths were re-measured 15 and 30 min after they were filled. The contraction stresses were calculated from the crack lengths and fracture toughness of the glass. Elastic moduli of light-cured and self-cured composite samples were measured using a nanoindentation method 15 and 30 min after curing. RESULTS: The stress could not be calculated for the light-cured direct composite and light-cured indirect composite restorations with thicker cement, due to glass failure caused by the contraction stress. Glass failure did not occur in the indirect composite restoration with thinner cement or in the self-cured direct composite restoration. The stress in the glass was lower for greater distances or shorter times. Factors of indentation-hole distance and restorative procedure significantly affected the stress. The measuring time and the type of curing had significant influence on the elastic modulus. CONCLUSION: Light-cured indirect composite restorations with a cement thickness < 200 µm generated less contraction stress than did light-cured direct composite restorations. The lowest contraction stress was developed in the self-cured direct composite restoration.

The present study examined color changes in the tooth-colored restorative materials, zirconia (3Y-TZP), resin composite, and porcelain. The colors (CIELab) of these materials were measured using a spectrophotometer. Specimens were immersed in black tea or curry for 1 and 7 days, after which colors were re-assessed. Color differences (∆E*ab) before and after immersion were calculated. Specimens after the 7-day immersion were ultrasonically cleaned, and colors were measured again to assess the color recovery rate. The surface free energy, roughness, and water sorption/solubility of each material were also evaluated. Specimens were observed under a scanning electron microscope. The ∆E*ab of 3Y-TZP was the smallest with both immersions. Resin composite had the smallest recovery rate. The surface free energy and roughness of 3Y-TZP were smaller than those of porcelain. 3Y-TZP and porcelain showed almost no sorption during the 7-day period. The present results revealed that 3Y-TZP exhibited the strongest resistance to discoloration.

BACKGROUND: Polymerization stress is a major problem in dental resin composite restorations. Two indentation fracture methods can be applied to evaluate the stress, however, they often calculate different values. OBJECTIVE: To compare polymerization stresses of dental composites determined by the two methods. METHODS: Glass disks with a central hole were used. Two indentation fracture methods (Methods 1 and 2) were employed to determine the polymerization stresses of low-shrinkage and bulk-fill composites. Method 1: Cracks were made in the glass surface at 300 µm from the hole. The hole was filled with the composite. Polymerization stresses at 30 min after filling were calculated from the lengths of crack extension. Method 2: The hole was filled with the composite. Cracks were introduced in the glass at 1,000 µm from the hole at 30 min after the polymerization and the stresses were calculated from the crack lengths. Stresses at composite-glass bonded interface were calculated from the stress values obtained by the two methods. RESULTS: The bulk-fill composite generated the smallest interfacial stress, and Method 1 revealed lower values than Method 2. CONCLUSIONS: The composites yielded relatively small stresses. Method 1 calculated smaller stress values, possibly affected by the lower threshold stress intensity factor.

OBJECTIVE: This study was designed to evaluate the cariogenic potential of Lactobacillus gasseri YIT 12321. DESIGN: L. gasseri YIT 12321 was isolated from the human oral cavity as a probiotic candidate strain. Three types of experimental biofilm formations on bovine enamel specimens were assessed using Streptococcus sobrinus ATCC 33478, L. gasseri YIT 12321, and a co-culture of S. sobrinus and L. gasseri YIT 12321 in vitro. L. gasseri YIT 12321 was analyzed for its ability to utilize seven carbohydrates. L. gasseri YIT 12321 was cultured in tryptic soy broth without dextrose and containing a test carbohydrate at 37 °C for 16 h. RESULTS: The decrease in pH in the region under the biofilm produced by L. gasseri YIT 12321 was unusually slow, and the pH was maintained above 5.5 for 16 h. The amount of biofilm and the reduction in enamel hardness were minimal in the L. gasseri YIT 12321 group among the three experimental groups. The amount of co-culture-derived biofilms was less than that of S. sobrinus biofilms. When S. sobrinus was grown using sucrose, the pH decreased to 3.98. In contrast, L. gasseri YIT 12321 cultures maintained the pH above the critical pH for dentin demineralization. CONCLUSIONS: L. gasseri YIT 12321 is suggested to have a low cariogenic potential. The application of this strain as a probiotic may contribute to oral safety.

We developed a dental fiberscope that can be navigated. As a result we are able to better grasp the device position relative to the teeth, aiming at the lesion more precisely. However, the device position and the precise target setting were difficult to consistently ascertain. The aim of this study is to navigate the position of tip of the dental fiberscope fiber in the root canal with our navigation system. A 3D tooth model was made from the raw dental CT data. In addition, the optical position of the measurement device, OPTOTRAK system was used for registration of the 3D model and actual teeth position and to chase the scope movement. We developed exclusive software to unify information. We were subsequently able to precisely indicate the relation of the position between the device and the teeth on the 3D model in the monitor. This allowed us to aim at the lesion more precisely, as the revised endoscopic image matched the 3D model. The application of this endoscopic navigation system could increase the success rate for root canal treatments with recalcitrant lesion.

The objective of this study was to compare the changes in the residual stresses present on the surface of leucite-reinforced dental ceramic restorations luted with a self-adhesive and a conventional resin composite cement during aging in water. Ring specimens made of a leucite-reinforced ceramics were luted to ceramic cylinders using a self-adhesive (Panavia SA Luting Plus) or a conventional resin composite cement (Panavia V5) in dual-cure or self-cure mode. Residual stresses on the ring surface were measured using indentation fracture method at 1 h, 1, 3, 7, 14 and 28 days of the 37 °C water immersion. Water sorption, water solubility and elastic modulus of the cements were also measured. Compressive stress was generated on the surface of the ceramic rings by the polymerization of the resin composite cements, and the stresses appeared to decrease over time by water sorption of the cements. The dual-cured conventional resin composite cement remained compressive stresses on the ceramic surface, while only the self-cured self-adhesive cement, which demonstrated the greatest water sorption, generated tensile stresses during the four weeks of aging in water. The elastic moduli of cements did not significantly change through the immersion, suggesting that the stresses were less affected by the modulus. To prevent the generation of tensile stresses on the leucite-reinforced ceramic restoration, self-adhesive cements exhibiting small water sorption should be clinically selected.

The purpose of this multicenter clinical study was to compare the mid-term clinical effectiveness of direct resin composite restorations using one-step or two-step self-etch adhesives (1-SEAs or 2-SEAs). In total, 352 restorations of class I-V cavities and non-carious cervical lesions in vital teeth (1-SEAs; 52 cases, 2-SEAs; 300 cases) were placed at nine university hospitals and evaluated according to the modified USPHS criteria at baseline, and after 1, 2, and 3 years. The recall rates were 86.6% (1 year), 80.1% (2 years), and 62.2% (3 years). Two restorations failed due to fracture during the follow-up, and there was no significant difference in survival rates between 1-SEAs (97.6%) and 2-SEAs (99.4%). However, 2-SEAs exhibited significantly lower occurrences of discoloration, marginal discoloration, fracture, and plaque retention. Moreover, the subjects reported a significantly lower postoperative hypersensitivity and higher overall satisfaction at all evaluation periods if 2-SEAs were used.

OBJECTIVES: Root-caries, which frequently occurs in elderly people, is more difficult to treat than caries in a tooth crown, especially in filling restorations. To overcome this difficulty, it is essential to find a strategy for sufficiently sterilizing the infected dentin; however, techniques for sterilizing carious pathogens inside the biofilm, called dental plaque, have not yet been established. Recently, dental applications of plasma sterilization technology have attracted attention. The mechanism of plasma sterilization became clear, and revealed that peroxynitric acid (PNA) is an effective sterilization substance. Highly concentrated PNA solutions can be chemically synthesized in large quantities without using plasma technology. We thought that the application of PNA solution could be a novel treatment for root caries, and examined the microbicidal effect and safety of PNA. METHODS: A sterilization experiment was performed using an extracted tooth model infected with Streptococcus mutans. Subsequently, a biofilm of S. mutans and Candida albicans was formed on a plate or a dentin slice, and sterilization experiments were performed in comparison with chlorhexidine. Furthermore, a toxicity test of PNA was performed using an epithelial tissue model. RESULTS: In the infection model, sterilization was achieved with a 22 mM PNA solution in only 10 s. In the biofilm model, a 22 mM PNA solution showed a higher microbicidal effect than 2.0% chlorhexidine. In the toxicity test, 2.0% chlorhexidine was toxic, but a 220 mM PNA solution showed no toxicity. CONCLUSIONS: PNA is an unprecedented disinfectant that has high microbicidal activity on biofilm and is safe for tissues.

The objective of this study was to assess the influence of filling techniques on residual polymerization stresses in resin composite restorations of the tooth. Flat planes were ground in buccal enamel surfaces of extracted human premolars, followed by preparing Class II cavities. Indentation cracks were introduced in the planes and crack lengths were measured mesio-distally (x-direction) and cervico-incisally (y-direction). Cavities were filled with a resin composite and an adhesive using three methods; one with bulk filling and two with differing incremental filling techniques. The x- and y-tensile stresses were calculated from crack lengths measured repeatedly over 360 min after filling. Elastic modulus and polymerization shrinkage of the composite were also measured. Filling technique and time after fillings were statistically significant only for the y-stress. The incremental techniques generated smaller stresses than the bulk filling. The stresses developed for 60 min after filling, while the modulus and the shrinkage stopped developing within 10 min and 2 min after irradiation, respectively. The incremental technique, in which the proximal portion of the cavity was filled first, was effective in decreasing the residual tensile stress generated by the polymerization of resin composite.

BACKGROUND: Ceramic crown that fires the layering porcelain on zirconia corping is starting to be used due to aesthetic requirements. However, many ceramics are used, but wear characteristics are often unknown. OBJECTIVE: This study was performed to investigate the wear of human enamel by porcelains, and to determine the influence of porcelain hardness, surface roughness and crystal of porcelain on the enamel wear. METHODS: Enamel specimens were prepared from extracted human molar teeth. Porcelain specimens were prepared from Deguceram Gold, Vita Omega 900, and Cercon Ceram Kiss. Enamel wear volume was calculated and the porcelain wear depth, surface roughness and Vickers hardness was measured. Moreover, the observation of a crystal by scanning electron microscope and the crystal by X-ray diffraction were identified. RESULTS: Omega 900 showed the smallest enamel wear volume and porcelain wear depth. The Vickers hardnesses of Omega 900 and Cercon Ceram Kiss were almost identical. Leucite was detected from Deguceram Gold and Omega 900, and Silica and Silicon Nitride were detected from Cercon Ceram Kiss. CONCLUSIONS: The enamel wear is influenced not by the hardness of the porcelain but by the surface roughness, the size and shape of the crystal.

Protamine is an antimicrobial peptide extracted from fish. In this study, we loaded protamine onto dicalcium phosphate anhydride (DCPA), a dental material. Protamine was loaded by stirring DCPA into a protamine solution. To explore the antimicrobial activity of the materials, we cultivated Streptococcus mutans on fabricated discs for 24 h. When S. mutans was cultivated on the discs under no sucrose conditions, the loaded protamine was not released, and the ratio of dead bacteria increased on the surface of P (125) DCPA (half of the saturated level of protamine (125 ppm protamine) was loaded). Aside from P (500) DCPA (saturated level of protamine was loaded), some protamine was released, and the number of planktonic bacteria in the supernatant decreased. Using medium containing 1% sucrose, the release of protamine was promoted from P (125) DCPA due to lowered pH. However, lowering of the pH decreased the antimicrobial activity of protamine. On the other hand, P (500) DCPA released protamine before the pH was lowered, and biofilm formation was inhibited. The loaded protamine expressed antimicrobial activity, both on the surface of the materials and in the surrounding environment. The interaction of loaded protamine with calcium phosphates could promote the application of protamine in the dental field.

The objective of this study was to investigate changes in polymerization stress and elastic modulus for light-cured bulk-fill resin composites following irradiation. Crack analysis was applied to obtain the stresses for 24 h after irradiation, which were calculated from the lengths of cracks in a glass mold bonded to the composites. The elastic modulus was repeatedly measured by nano-indentation tests performed over 24 h. The lengths of the cracks, the interfacial stress, and the elastic modulus were significantly affected by the material and time after irradiation (p<0.01). The stress and elastic modulus continued to increase for more than 6 and 1 h, respectively (p<0.05). Strong relationships (r>0.85) were revealed between the stress and elastic modulus. The bulk-fill resin composites generated smaller stresses than a flowable resin composite reported to generate relatively low stress. Post-irradiation polymerization during the first 1 h is a major determinant for the magnitude of stress.

The objective of this case report is to introduce a simple technique for simultaneously taking a closed-mouth impression and functionally generated path (FGP) for a full coverage crown restoration. A monolithic zirconia crown was the restoration of choice. An alginate impression of the abutment tooth was taken to fabricate a custom-made closed-mouth impression tray covering the abutment tooth and the adjacent teeth. The tray had an FGP table and an abutment tray in cameo and intaglio surfaces, respectively. The impression was taken with silicone impression material after adjusting the abutment tray and inscribing the FGP using self-curing acrylic resins. Plaster casts were made from the impression, and a zirconia crown was fabricated. The crown was cemented to the abutment tooth with minimal adjustments. This simple technique resulted in a well-fitting crown that accounted for mandibular movements. Using the custom closed-mouth impression tray incorporating an FGP table simultaneously aids in fabricating an accurately fitting restoration that incorporates harmonious mandibular movements using a single impression capture.

The objective of this study was to analyze the distribution of tensile stresses in indirect restorations prepared in several composite cavity bases. Elastic moduli of 20 materials were measured by nanoindentation technique for finite element analysis. Axisymmetric models of posterior onlays were constructed using combinations of two onlay materials and three cavity base materials. Thickness of resin cement was 50 um. A vertical load of 95.5 N was applied on the cusp tip. Maximum stress of 18.1 MPa was found in the model consisting of a ceramic onlay and a flowable resin composite base. It was also found that tensile stress increased as the area of the base having a lower elastic modulus became wider. Base materials having higher elastic moduli were determined to be suitable as cavity base materials for posterior restorations.
