Abordagem ortodôntica de anquilose resultante de traumatismo dentário relato de caso / Orthodontic approach of ankylosis resulting from dental trauma case report

Resumo As lesões dentárias traumáticas ocorrem com grande frequência na Odontologia, sendo dever do cirurgião-dentista saber diagnosticá-las e as tratar com precisão e rapidez. Uma das lesões mais raras e de difícil tratamento é a luxação intrusiva, na qual o elemento traumatizado é levado em direção axial. Para tratamento, na maioria dos casos, é recomendado aguardar a reerupção passiva do mesmo e, em caso de insucesso, realizar o tracionamento ortodôntico ou cirúrgico. Uma das possíveis consequências da luxação intrusiva é a anquilose dentária, caracterizada pela fusão da raiz ao osso, gerando a imobilidade do elemento traumatizado. Não há como parar ou reverter o processo de anquilose. Portanto é necessário realizar alguma interceptação, existindo diversos tipos de tratamentos para dentes anquilosados. Os planos de tratamento devem ser planejados e discutidos em conjunto com o paciente, explicando suas vantagens e desvantagens até obter o prognóstico mais favorável, incluindo as necessidades e anseios do mesmo. O artigo em questão traz o relato de um caso clínico de um incisivo central superior permanente que sofreu luxação intrusiva e em que após a tentativa de tracionamento ortodôntico realizada, observou-se a anquilose dentária. Para tratamento de escolha foi realizada a extração do elemento anquilosado e reposicionamento dos elementos adjacentes, seguidos de reanatomização dentária. Serão discutidos quais os benefícios e malefícios desse tipo de tratamento.(AU)
Abstract Traumatic dental injuries occur frequently in Dentistry, and it is the dental surgeon's duty to know how to diagnose and treat them accurately and quickly. One of the rarest and most difficult to treat lesions is the intrusive luxation, in which the traumatized element is taken to the axial position. For treatment, in most cases it is recommended to wait for the passive reerection of the same, and, in case of failure, to perform the orthodontic or surgical traction. One of the possible consequences of the intrusive luxation is dental ankylosis, characterized by fusion of the root to the bone, generating immobility of the traumatized element. There is no way to stop or reverse the ankylosis process. Therefore, it is necessary to perform some interception, there are several types of treatments for ankylosed teeth. Treatment plans should be planned and discussed together with the patient, explaining their advantages and disadvantages until the most favorable prognosis is achieved, including the patient's needs and desire. The article reports a clinical case of a permanent maxillary central incisor that suffered intrusive luxation and that after orthodontic traction performed with failure, resulted in dental ankylosis. For treatment of choice the extraction of the ankylosed element and repositioning of the adjacent elements was carried out, followed by dental reanatomization. The pros and cons of this type of treatment will be discussed. (AU)
