Functional structure of a high resolution thyroid nodule unit. Economic impact and results of its implementation in an Endocrinology department.

BACKGROUND AND OBJECTIVE: The high incidence of nodular thyroid pathology has led to growing concern about the economic impact that this pathology represents on the healthcare system. There are conclusive data about the cost-effectiveness of high-resolution units for nodular thyroid pathology; however, their implementation is not homogeneous in the Endocrinology and Nutrition services of our country. The objective of the present study was to evaluate the economic impact of the implementation of the high-resolution thyroid nodule unit (HRTNU) in our center. PATIENTS AND METHODS: The present work is a prospective, observational and descriptive study carried out in 1314 patients (82% women, mean age 58 years ds = 11) evaluated at the HRTNU during the period of August 2022-August 2023. Demographic data (age and sex) were analyzed, referral center and consultation type, number of total consultations and neck ultrasound performed, number of fine needle aspiration (FNAB) performed, and cytology results were analyzed. RESULTS: In the period from August 2022 to August 2023, a total of 1314 patients were evaluated (neck ultrasound and clinical consultation) and a total of 133 FNAB were performed, of which only 2.26% were non-diagnostic. Compared to the percentage of unsatisfactory FNAB from the previous year August 2021-July 2022 of 25%, a mean saving of 9931.43 euros was estimated. 84.47% of the patients evaluated for the first time by the HRTNU were discharged, estimating a mean saving of 133,200 euros. CONCLUSIONS: The implementation of a HRTNU at the Endocrinology and Nutrition departments, coordinated with primary and specialized care, is a cost-effectiveness alternative, as it reduces the number of medical consultations and is accompanied by a higher rate of diagnostic FNAB.

[Routine performance of thyroid ultrasound and fine-needle aspiration biopsy in the setting of a high-resolution endocrinology practice]. / Incorporación de la ecografía y la punción de tiroides a la actividad de endocrinología en una consulta de alta resolución.

INTRODUCTION: Fine-needle aspiration biopsy (FNAB) is considered the reference diagnostic procedure for thyroid nodules. MATERIALS AND METHODS: Routine performance of thyroid ultrasound and ultrasound-guided FNAB by endocrinologists allows a more efficient approach in the setting of a high-resolution practice, thus reducing costs and the time elapsed until diagnosis. RESULTS: We present our initial results of this procedure 2 years after its introduction, with a total of 286 biopsies performed. After a progressive learning curve over time and according to the endocrinologists' previous experience, 72.72% samples were considered satisfactory for diagnosis. Greater difficulty was observed in obtaining optimal cytological specimens in smaller nodules. In conclusion, we have successfully incorporated thyroid ultrasound and ultrasound-guided FNAB into routine endocrine practice. CONCLUSION: Routine performance of thyroid ultrasound in endocrine practice will considerably aid the management of nodular thyroid disease.

Consensus statement for use and technical requirements of thyroid ultrasound in endocrinology units. / Criterios sobre la utilización y requerimientos técnicos de la ecografía tiroidea en los servicios de endocrinología y nutrición.

Thyroid nodule detection has increased with widespread use of ultrasound, which is currently the main tool for detection, monitoring, diagnosis and, in some instances, treatment of thyroid nodules. Knowledge of ultrasound and adequate instruction on its use require a position statement by the scientific societies concerned. The working groups on thyroid cancer and ultrasound techniques of the Spanish Society of Endocrinology and Nutrition have promoted this document, based on a thorough analysis of the current literature, the results of multicenter studies and expert consensus, in order to set the requirements for the best use of ultrasound in clinical practice. The objectives include the adequate framework for use of thyroid ultrasound, the technical and legal requirements, the clinical situations in which it is recommended, the levels of knowledge and learning processes, the associated responsibility, and the establishment of a standardized reporting of results and integration into hospital information systems and endocrinology units.

Adequate thyroid nodule assessment requires several diagnostic tests and multiple medical appointments, which results in a substantial delay in diagnosis. Implementation of a high-resolution thyroid nodule clinic largely avoids these drawbacks by condensing in a single appointment all tests required for adequate evaluation of thyroid nodule. This paper reviews the diagnostic and functional structure of a high-resolution thyroid nodule clinic.

La ecografía se ha convertido en un instrumento imprescindible en la asistencia a los pacientes con enfermedades tiroideas. La detección de los nódulos tiroideos se ha incrementado con el uso generalizado de la misma, siendo la herramienta principal para su detección, orientación diagnóstica, seguimiento y, en ocasiones, también terapéutica. Los Grupos de Trabajo de Cáncer de Tiroides y de Técnicas ecográficas de la Sociedad Española de Endocrinología y Nutrición han promovido este documento en el que se resumen los requisitos necesarios para la mejor práctica clínica posible con esta técnica. Los objetivos del trabajo incluyen encuadrar su utilización dentro de la especialidad, los requisitos técnicos y legales necesarios, las situaciones clínicas de su aplicación, los niveles de conocimiento y aprendizaje, la responsabilidad asociada, la comunicación estandarizada de resultados e integración en los sistemas de información hospitalarios, posicionando la técnica ecográfica dentro de la cartera de servicios en las actuales unidades de Endocrinología y Nutrición (AU)

La aparición de un nódulo tiroideo se ha convertido en un hecho cotidiano en la práctica clínica diaria. Habitualmente, la adecuada evaluación del nódulo tiroideo requiere la realización de diversas pruebas diagnósticas y múltiples citas médicas, con la consiguiente demora diagnóstica. La instauración de una consulta de alta resolución de nódulo tiroideo evita en gran medida estos inconvenientes, condensando en una única cita todas las pruebas necesarias para la correcta evaluación del nódulo tiroideo. Incorporación de la ecografía y la punción de tiroides a la actividad de endocrinología en una consulta de alta resolución / Routine performance of thyroid ultrasound and fine-needle aspiration biopsy in the setting of a high-resolution endocrinology practice

Introducción La PAAF es el procedimiento de referencia para el diagnóstico del nódulo tiroideo. Material y métodos La incorporación de la ecografía cervical y la PAAF guiada por ecografía a la actividad del médico endocrinólogo permite abordar esta patología en el ámbito de una consulta de alta resolución, con la consiguiente reducción en el coste del proceso y en la demora del diagnóstico. Resultados Presentamos los resultados iniciales de esta consulta; tras los dos primeros años y con un total de 286 punciones, nos situamos en un 72,72% de citologías aptas para diagnóstico, siguiendo una curva de aprendizaje acorde con el tiempo transcurrido y con la experiencia previa. Observamos una mayor dificultad en la obtención de citologías óptimas para diagnóstico en los nódulos de menor tamaño. En resumen, hemos incorporado la ecografía y la PAAF guiada por ecografía de manera satisfactoria a la consulta endocrinológica. Conclusión El uso de manera rutinaria de la ecografía en la consulta de endocrinología permite agilizar el manejo de la patología nodular tiroidea (AU)
