RESUMEN

AIMS: To evaluate interobserver reliability of the Orthopaedic Trauma Association's open fracture classification system (OTA-OFC). PATIENTS AND METHODS: Patients of any age with a first presentation of an open long bone fracture were included. Standard radiographs, wound photographs, and a short clinical description were given to eight orthopaedic surgeons, who independently evaluated the injury using both the Gustilo and Anderson (GA) and OTA-OFC classifications. The responses were compared for variability using Cohen's kappa. RESULTS: The overall interobserver agreement was ĸ = 0.44 for the GA classification and ĸ = 0.49 for OTA-OFC, which reflects moderate agreement (0.41 to 0.60) for both classifications. The agreement in the five categories of OTA-OFC was: for skin, ĸ = 0.55 (moderate); for muscle, ĸ = 0.44 (moderate); for arterial injury, ĸ = 0.74 (substantial); for contamination, ĸ = 0.35 (fair); and for bone loss, ĸ = 0.41 (moderate). CONCLUSION: Although the OTA-OFC, with similar interobserver agreement to GA, offers a more detailed description of open fractures, further development may be needed to make it a reliable and robust tool. RESUMEN

The timing of fracture fixation in polytrauma patients has been debated for a long time. The decision between DCO (damage control orthopaedics) and ETC (early total care) is a difficult dilemma. Overzealous ETC in haemodynamically compromised patients with significant chest and head injuries can be detrimental. It has been shown, however, that early fracture fixation has a trend towards better outcome in patients with less severe injuries. Delaying all orthopaedic surgery in critically injured patients can be a safe alternative, but has several disadvantages like longer ICU stay and septic complications. The literature shows equivocal evidence for both settings. This article will summarize the historical background and controversies regarding patient assessment and decision making during the treatment of polytrauma patients. It will also give guidance for choosing DCO versus ETC in the clinical setting.

Although many studies have measured the functional outcome after surgical treatment of osteomyelitis, there have been few published attempts to evaluate the long-term quality of life. We therefore undertook this study to assess the quality of life in a large patient population after operative treatment for this condition. All patients who underwent operative treatment for osteomyelitis from 1993 until 1997 at our institution were included in the study. The patients were assessed with a questionnaire which contained the SF-36 (German version) and questions about the activity and history of the illness. The result was compared to the data set from a standard population. Of the 502 patients, 345 (69%) returned questionnaires for evaluation. The infection was inactive in 301 (88%). Compared to a standard population, the investigated patients showed a significant reduction in their overall psychological well being and physical functional capacities. Surgical treatment was able to inactivate the infection in 88% of the patients. Because of persistent deficits, the psychological well being and physical functional capacities are reduced compared to a standard population.
