RESUMEN

INTRODUCTION: Thrombosis of the aortic arch is a rare and often underdiagnosed source of peripherical arterial embolic events. EXEGESIS: We report a case with a non-typical initial clinical presentation of polyarteritis nodosa. A mobile thrombus in the aortic arch was secondarily suspected when disseminated arterial embolism appeared. Transthoracic echocardiography failed twice to diagnosticate the source of embolism. The diagnosis was only performed with transesophageal echocardiography and confirmed by computed tomography and magnetic resonance imaging of the thoracic aorta. The thrombus completely disappeared after six months of oral anticoagulant therapy. CONCLUSION: Although rare, this diagnosis mustn't be disregarded in an etiologic view of recurrent and disseminated peripherical ischemic events (even clinically silent ones) under penalty of detrimental functional consequences due to a delayed diagnosis.

RESUMEN

Two cases of neonatal spontaneous rupture of the esophagus are described, one of which is associated with a duodenal stenosis. Seventeen cases have been reported. They present clinically with non-specific signs of respiratory insufficiency and radiologically by a right-sided hydropneumothorax. Esophageal opacification shows a leak above the diaphragm. A sudden increase in esophageal pressure at birth is considered to be a major etiologic factor leading to rupture. The presence of a duodenal obstruction support this hypothesis.

RESUMEN

PURPOSE: The purpose of this study was to describe the MR appearance of the acetabular labrum in asymptomatic hips on high resolution MRI. METHOD: Fifty-two hips in 46 asymptomatic volunteers with an age range of 15-85 years were evaluated with coronal and axial T1-weighted and T2-weighted sequences. The shape, margin, size, and signal intensity of the acetabular labrum were analyzed. RESULTS: The labrum was absent in its anterosuperior aspect in five hips (10%). Intralabral regions of intermediate of high signal intensity were detected in 30 hips (58%) imaged with T1-weighted and proton density-weighted sequences. Intralabral linear hyperintense foci reaching the articular surface and consistent with a labral tear were detected in four labra on T2-weighted images. Intralabral microcysts were seen in three labra. CONCLUSION: The MR appearance of the hip labrum is varied in asymptomatic volunteers. Intralabral increased signal intensity and absent anterosuperior labra are especially frequent and may represent asymptomatic lesions or normal variations.
