RESUMEN

OBJECTIVE: To summarize the clinical evidence on the relationship between cancer and non-syndromic oral cleft (NSOC). METHODS: The review was reported according to the Preferred Reporting Items for Systematic Reviews and Meta-Analyses checklist, and a literature search was conducted in six databases and gray literature. Studies published in any language mentioning cancer in patients with NSOC and their relatives and NSOC in patients with cancer and their relatives were included. Risk of bias was assessed using the Joanna Briggs Institute appraisal tool. The certainty of the evidence was evaluated using the GRADE (Grading of Recommendation, Assessment, Development, and Evaluation) assessment. After a 2-step selection process, 33 studies were included: 17 case-control studies, 13 cross-sectional studies, and 3 case reports. RESULTS: The study evaluated 206,096 patients from 20 countries. Of these, 0.35% of patients with cancer (95% CI: 0.0%-1.1%; I2  = 86%), 3.0% of relatives of patients with cancer (95% CI: 1.19%-5.46%; I2  = 55%), and 0.26% of controls (95% CI: 0.0%-0.83%; I2  = 87%) had NSOC. Among the studies that examined the prevalence of cancer, 2.4% (95% CI: 0.0%-19.3%; I2  = 99%) of patients with NSOC, 15.4% of relatives of patients with NSOC (95% CI: 2.0%-37.6%; I2  = 99%), and 5.3% of controls (95% CI: 0.0%-22.8%; I2  = 99%) had cancer. Although no relationship was observed between the risk of cancer in patients with NSOC and the risk of NSOC in patients with cancer, there was an association for an increased risk of cancer in relatives of patients with NSOC (OR: 9.96, 95% CI: 1.55-63.99; p = 0.01) and a significant association for the NSOC risk in relatives of patients with leukemia (OR: 9.31; 95% CI: 1.13-76.67; p = 0.03). CONCLUSION: Our findings demonstrate an increased risk of cancer in relatives of patients with NSOC and that relatives of patients with leukemia were more frequently affected by NSOC. Together, these findings can help guide cancer screening in patients with NSOC and their relatives and shed light on the risk of NSOC in families with a history of cancer.

RESUMEN

OBJECTIVES: The aim of this study was to evaluate the clinical performance of silorane-based composite restorations applied after different surface treatments. MATERIALS AND METHODS: This controlled and randomized clinical trial included 26 patients with class I restoration indications. The teeth were randomly assigned to the following treatments: control 1/G1, silorane specific self-etching adhesive (P90 self-etch primer and bond) + silorane-based composite resin (Filtek™ P90 low shrink posterior restorative); G2, 37% phosphoric acid + silorane specific self-etching adhesive + silorane-based composite resin; G3, blasting with aluminum oxide + silorane specific self-etching adhesive + silorane-based composite resin; and control 2/G4, self-etching adhesive (Adper™ SE Plus self-etch adhesive) + dimethacrylate-based composite resin (Filtek™ P60 posterior restorative). The clinical performance was evaluated at baseline and after 1 year. RESULTS: A total of 141 restorations were made and evaluated. For all clinical criteria evaluated, no significant difference was found between the surface treatments at baseline and after 1 year (p > 0.05). After 1 year, only the group with 37% phosphoric acid + silorane specific self-etching adhesive (G2) showed a significant reduction in marginal adaptation (p < 0.05). CONCLUSION: In general, all surface treatments showed an adequate clinical performance for silorane-based composite resin in class I restorations. However, a reduction in the marginal adaptation after 1 year was found when additional phosphoric acid etching was used prior to silorane specific self-etching adhesive. CLINICAL RELEVANCE: The use of phosphoric acid etching prior to specific self-etching adhesive can adversely affect the marginal adaptation of silorane-based restorations.

RESUMEN

PURPOSE: To investigate the clinical performance of a silorane-based composite resin used for repairing dimethacrylate- based composite restorations. MATERIALS AND METHODS: One operator repaired defective dimethacrylate-based resin restorations that were randomly assigned to one of two treatment groups: control (n=50), repaired with Adper SE Plus and Filtek P60 Posterior Restorative (3M ESPE); or test (n=50), repaired with P90 System Adhesive Self-Etch Primer and Bond and Filtek P90 Low Shrink Posterior Restorative (3M ESPE). After 1 week, restorations were finished and polished. Two calibrated examiners (weighted Kappa≥0.78) evaluated the repaired restorations, blindly and independently, at baseline, after 6 months, 1 and 2 years. The parameters examined were marginal adaptation, anatomic form, surface roughness, marginal discoloration, postoperative sensitivity, and secondary caries. The restorations were classified as Alfa, Bravo, or Charlie, according to modified US Public Health Service criteria. Variation in the levels of clinical parameters over time was evaluated by Friedman's ANOVA (α=0.05). The Mann-Whitney test assessed the differences between the materials for all clinical criteria at baseline, 6-month, 1- and 2-year recalls (α=0.05). The Wilcoxon test compared each composite resin for all clinical criteria at the same recalls (α=0.05). RESULTS: After two years, 79 repaired restorations were re-examined. No statistically significant differences were found between the materials at baseline or at the 2-year recall (p>0.05). Comparing baseline and 2-year recall, there was a statistically significant difference for marginal discoloration (p=0.029) in silorane-based composite restorations. CONCLUSION: After two years, the clinical performance of the silorane-based composite was similar to that of the dimethacrylate-based composite when used to make repairs.

RESUMEN

OBJECTIVE: This study was aimed to identify the knowledge and attitudes of dentists regarding ageing and the elderly. BACKGROUND: As the population ages, there is a greater demand for differentiated care from health professionals. However, the treatment needs of the elderly population are not sufficiently addressed, particularly in developing countries. MATERIALS AND METHODS: An official list of all dentists registered in the city of Belo Horizonte, Brazil, was obtained from the Regional Dentistry Council. A questionnaire was delivered to 276 randomly selected dentists; the survey contained questions designed to characterise the sample and identify dentists' conduct towards elderly patients. A Likert scale was used to identify knowledge regarding care for the elderly and attitudes towards ageing. RESULTS: Women demonstrated more positive attitudes towards the elderly. Graduates who worked in the public sector demonstrated a greater knowledge of geriatric dentistry (p < 0.05). Attending specific continuing education courses had no influence over the dentists' willingness to treat the elderly population. Significant associations were not found between attitudes and knowledge regarding the elderly. CONCLUSION: Dentists who worked in the public sector demonstrated a greater knowledge regarding the elderly. This knowledge did not influence attitudes towards ageing. Women demonstrated a more positive attitude regarding ageing and dental care.

RESUMEN

The scope of this article is to identify the prevalence of the loss of quality of sleep and associated factors among menopausal women. It is a quantitative, cross-sectional and analytical study, the variables of which were investigated by applying a structured/pre-tested questionnaire and the Pittsburgh Sleep Quality Index with 819 menopausal women cared for under the Family Health Strategy. Simple Poisson regression was used to screen the variables (p < 0.25). For multiple analysis, Poisson regression was used based on a hierarchical model, at a significance level of 5%. Loss of quality of sleep was identified in 67% of the sample. Variables such as advanced age (PR = 1.09; CI = 1.03-1.16), moderate and severe menopausal symptoms (PR = 1.18; CI = 1.10-1.27), moderate to severe anxiety (PR = 1.17; CI = 1.10-1.25), moderate to severe depression (PR = 1.08; CI = 1.01-1.15) and arthritis/arthrosis/rheumatism (PR = 1. 07; CI = 1.01 - 1.14) revealed statistically significant associations with loss of quality of sleep. The loss of quality of sleep was highly prevalent in the population studied. Factors associated with loss of quality of sleep were advanced age, moderate to severe menopausal symptoms, moderate to severe anxiety and depression, and the presence of arthritis/arthrosis/rheumatism.

RESUMEN

AIMS: Evaluate the seasonal influence in nonsyndromic cleft lip and/or palate (NSCL/P) in Brazilian patients. METHODS: A case-control study, with 361 unrelated patients with NSCL/P and 481 healthy individuals, was done on a reference service for craniofacial deformities in Minas Gerais State, Brazil. Information was collected from clinical records considering gender, month of birth, as well as with the seasons. RESULTS: Nonparametric tests did not show a seasonal variation in month of birth and in seasons of year of NSCL/P compared to a control group (p = 0.902 and p = 0.679, respectively). A difference in births between the groups was identified only in January, however, was not significant. Moreover, among the control group there were more births in the months of February and August, and for the cleft group, more in July and August. The males were more affected by cleft lip with or without palate (CLP) and the females by isolated cleft palate (CP) manifestation. The ratio of CL:CLP:CP indicated that CLP was predominant when compared with CL and CP, CLP was more frequent in male patients, and CP predominance was seen in females. CONCLUSION: This study did not show seasonal differences in births on NSCL/P in a Brazilian group, emphasizing that environmental factors may be related to oral clefts. These results provide a basis for further epidemiological studies of orofacial clefts in Brazil.

RESUMEN

Aim: Although bulk fill composites have been widely used as restorative material, there is no consensus regarding the best clinical protocol in terms of composite technique and adhesive system. Therefore, this clinical trial evaluated the clinical performance of bulk fill composites for class I restorations under different protocols. Methods: A randomized clinical trial including 155 class I restorations was conducted using different adhesive systems: conventional technique (phosphoric acid + conventional three-step adhesive system) (Group 1, 2 and 3); or self-etching adhesive system (Groups 4, 5 and 6). Control groups 1 and 4 were restored with conventional composite; groups 2 and 5 with low viscosity bulk fill and conventional composite as occlusal coverage; groups 3 and 6 with high viscosity bulk fill. The FDI criteria was used for clinical evaluation at baseline and after 6 months. Results: All groups showed good clinical performance. At baseline, the adhesive system did not affect postoperative hypersensitivity. After 6 months, group 5 showed a significant reduction in color and translucency; group 6 a reduction in terms of anatomical form and for postoperative sensitivity and an improvement in patient satisfaction (p<0.05). Considering the same restorative technique, the use of the self-etching adhesive system showed a significant decrease in color and translucency (p<0.05). Conclusion: All groups showed favorable clinical performance, and promising results were found for the conventional adhesive system and high viscosity bulk fill protocol

RESUMEN

BACKGROUND: Total replacement is the most common technique for defective amalgam restorations, and it represents a major part of restorative dental treatment. Repair is an alternative option for amalgam restorations with localized defects. AIMS: This study compared microleakage of amalgam restorations repaired by bonded amalgam or composite resin. MATERIALS AND METHODS: Thirty extracted human pre-molars were prepared and restored with class I amalgam. A simulated defect was prepared that included the cavosurface margin on restorations, and the pre-molars were assigned to two treatment groups (n=15): In group 1, premolars were treated by composite resin (34% Tooth Conditioner Gel + Adper Single Bond 2 + Z100) and in group 2, premolars were repaired by bonded amalgam (34% Tooth Conditioner Gel + Prime and Bond 2.1 + Permite C). The teeth were immersed in a 50% silver nitrate solution, thermocycled, sectioned longitudinally and then observed by three examiners using a stereomicroscope. Microleakage was evaluated using a 0-4 scale for dye penetration, and data was analyzed by Kruskal Wallis and Dunn tests. RESULTS: Neither of the two methods eliminated microleakage completely. Composite resin was significantly the most effective for repair/tooth interface sealing (score 0 = 80.0%; P=0.0317). For the repair/restoration interface, composite resin was also statistically more effective as a sealant (score 0=66%; P=0.0005) when compared to the bonded amalgam technique (score 0=13%; P=0.0005). CONCLUSIONS: The use of adhesive systems significantly affected the ability to seal the repair/ tooth interface. However, at the level of the repair/restoration interface, the bonded amalgam technique may increase microleakage.

RESUMEN

ABSTRACT The current study aimed to analyze through a literature review evidence of association between ocular changes and non-syndromic cleft lip and/or palate (NSCL/P). A literature review was carried out in accordance with the Cochrane Collaboration Group protocol. PubMed, Scopus, Academic Google and ISI Web of Science databases were systematically searched. A total of 16 studies were accessed, and three made up the final sample. All three studied ocular abnormalities in patients with NSCL/P.The articles found ocular abnormalities in 6.21%, 17.54% and 1.03% of patients respectively. The presence of ocular abnormalities in patients with NSCL/P was significant in this systematic review, but the articles all agreed that future studies should explore the possibility of a greater occurrence of ocular changes in individuals with NSCL/P.

PubMed, Scopus, Google Acadêmico e ISI-Web of Science. A partir de16 estudos acessados, 3 compuseram a amostra final.

Foram conduzidas análises descritivas, univariadas e múltiplas (OR/IC95%). .

Introduction: Access to oral health education is considered an important predictor for assessing the conditions and behaviors on oral health. Most 12-year children are in the school environment, being the school a propitious educational and social environment for both knowledge as behavioral changes are implemented. Objective: It is proposed to identify the prevalence of access to oral hygiene guidelines among schoolchildren of public schools and the factors associated with such access. Methodology: This is an analytical cross-sectional study conducted among 12-years schoolchildren of a Brazilian city with a large population size. The tests were performed by 24 trained and calibrated dentists with the aid of 24 annotators. Data collection occurred in 36 randomly selected public schools among the 89 schools of the city. Descriptive, univariate and multiple analyzes were conducted (OR/IC95%). Result: Of the 2,755 schoolchildren, 2,510 were included. Of these, 2,173 (86.6%) received oral hygiene guidelines. Access to guidelines for oral care was higher among male students and among those using private dental services or covenant. It was lower among those who used the service for dental treatment for over one year and among those who did not receive dental care at school, who reported a frequency of oral hygiene equal to once/day. Conclusion: Most of the students had access to guidelines for oral hygiene, with this access associated with variables related to personal determinants, health services and behaviors. .

The alterations provoked by pathologies or iatrogenesis favor the invasion of the periodontium, communicating pulp cavity with periradiculares tissues. Currently, for the treatments of those root perforations, the sealer material more suitable is the mineral trioxide aggregate, that was reported for the first time in 1993 and introduced commercially in 1998. Mineral trioxide aggregate is presented under the form of a powder white or gray that, once incorporate to the distilled water, has an easy manipulation resulting in a colloidal gel whose final time setting is about 3 hours. That aggregate reaches appropriate physical and chemical properties as well as biocompatibility with the periradiculares tissues. In the current review, the use of mineral trioxide aggregate was approached as sealer material of root perforations in both in vitro and in vivo studies. It was evidenced that mineral trioxide aggregate presents good sealing ability, superior the other materials of larger longevity in the dentistry. However, most of the clinical investigations involve isolated case reports, with few longitudinal evidences of the reached results.

RESUMEN

Objective: This study investigated the clinical performance of a silorane-based composite resin when used for repairing conventional composite restorations. Methods: Defective dimethacrylate-based composite resin restorations were randomly assigned to one of two treatment groups: Control group - Adper SE Plus + Filtek P60 (3M/ESPE, St. Paul, EUA) and Test Group - P90 Adhesive System + Filtek P90 (3M/ESPE, St. Paul, EUA). All repaired restorations were evaluated at baseline, and at six-month recall. The parameters examined were marginal adaptation, anatomic form, surface roughness, marginal discoloration, post-operative sensitivity and secondary caries. The restorations were classified according to modified USPHS criteria. Mann-Whitney and Wilcoxon tests were used to compare the groups. Results: Of the 100 restorations repaired in this study, 93 were reexamined at baseline and 91 at 6-month recall. Drop-out was about 9%. No statistically significant differences were found between the materials for all clinical criteria, at baseline and at 6-month recall (p > 0.05). No statistically significant differences were registered (p > 0.05) for each material when compared for all clinical criteria, at baseline and at 6-month recall. The hypothesis tested in this randomized controlled clinical trial was accepted. Conclusion: After the six-month evaluations, silorane-based composite exhibit a similar performance compared to dimethacrylate-based composite when used as repair material.
