RESUMEN

The purpose of this paper is to discuss the importance of good quality audio for video production. Well-recorded audio is an essential component of any video project and this paper will explain and demystify the recording process. The equipment used to capture audio will be described and best practices will be outlined. Following the advice in this paper will help videographers to achieve the highest quality crystal clear recordings.

This study compared the dimensions of the peri-implant soft tissues around immediately and early loaded one-piece implants as well as conventionally loaded one-piece implants. Comparisons of the peri-implant soft tissue dimensions were made among four different loading periods. Forty-eight titanium sandblasted/acid-etched implants were placed in four foxhounds. The implants were placed at four time periods in groups of three. The first group (A) had implants placed 3 months before the placement of restorations. Further groups of three implants each were placed at 21 days (group B), 10 days (group C), and 2 days (group D) before restoration. Three months after abutment connection, all dogs were sacrificed. Histometric analysis of the undecalcified histologic sections included dimensional measurements of the sulcus depth plus junctional epithelium, the connective tissue contact area, and recession measured from the interface to the gingival margin. No statistically significant differences were observed among the four groups. The data suggest that the dimensions of the peri-implant soft tissues around immediately and early loaded one-piece implants are similar to those around conventionally loaded one-piece implants and comparable to the dimensions of the biologic width around natural teeth.

OBJECTIVE: Studying adolescent-onset brief depressive spells, we saw a chance to adapt a cross-cohort approach for successive cross-sectionally gathered epidemiological estimates on birth cohorts. METHODS: The United States (US) study population estimates are for noninstitutionalized community-dwelling 17- to 18-year-olds, sampled, recruited, and assessed using audio computer assisted self-interviews for the National Surveys on Drug Use and Health each year, 2008-2016. Estimated age and year-specific lifetime history cumulative incidence proportions are presented, with 95% confidence intervals, as well as age-specific meta-analysis summary estimates. RESULTS: Occurrence of a brief depressive spell (BDS) among the 2008 U.S. 17-year-olds showed a surprising difference with the 2009 statistically independent sample of that same "birth cohort" assessed at age 18 years. "Negative growth" was seen instead of an expected increase in those affected. Independent replication estimates of this BDS negative growth difference were seen in multiple successive birth cohorts. CONCLUSIONS: This well-replicated difference in BDS estimates offers a fascinating puzzle to be solved with focused inquiry. The puzzle's solution is important because National Surveys on Drug Use and Health does not assess Major Depression unless a BDS lifetime history is ascertained.

PURPOSE: The purpose of this investigation was to determine whether early and immediate loading of dental implants resulted in adverse consequences as determined clinically, radiographically, and histologically. MATERIALS AND METHODS: In a canine model, 48 sand-blasted, large-grit, acid-etched (SLA) surfaced implants were placed at 4 different times before definitive restoration and loading. These times were 3 months (group A), 21 days (group B), 10 days (group C), and 2 days (immediately) (group D) before loading. Each implant was restored at the same time with a single gold screw-retained crown. Immediately after restoration all crowns were placed in function. Standardized periapical radiographs were made 1, 2, and 3 months after restoration. At the end of the study, block sections were obtained for histologic examination. Changes in crestal bone height on the mesial and distal aspects of each implant and the change in bone density of the coronal 3 mm of crestal bone were recorded. Primary, secondary, and total bone-to-implant contact; bone marrow-to-implant contact; and connective tissue-to-implant contact were evaluated histologically. RESULTS: All implants were osseointegrated at the end of the study; no clinical failures of integration were noted. The changes in crestal bone heights for groups A, B, C, and D (means +/- SE) were 0.02 +/- 0.07 mm, 0.30 +/- 0.08 mm, 0.15 +/- 0.08 mm, and 0.35 +/- 0.18 mm, respectively. Total bone-to-implant contact for the 4 groups was 69.1%, 71.3%, 74.6%, and 75.2%, respectively (P > .57). DISCUSSION: Under the conditions of this study no statistically significant differences were noted between the 4 different loading protocols for any of the parameters recorded. This finding is consistent with other recent studies and case reports. CONCLUSION: The findings of this study indicate that early and immediate loading of single-unit SLA surfaced implants was possible in this model. (More than 50 references.)

The purpose of this study was to examine the efficacy of the Youth-Nominated Support Team-Version II (YST-II) for suicidal adolescents, an intervention based on social support and health behavior models, which was designed to supplement standard treatments. Psychiatrically hospitalized and suicidal adolescents, 13-17 years of age, were randomly assigned to treatment-as-usual (TAU) + YST-II (n = 223) or TAU only (n = 225). YST-II provided tailored psychoeducation to youth-nominated adults in addition to weekly check-ins for 3 months following hospitalization. In turn, these adults had regular supportive contact with adolescents. Adolescents assigned to TAU + YST-II had an average of 3.43 (SD = 0.83) nominated adults. Measures included the Suicidal Ideation Questionnaire-Junior (SIQ-JR; W. M. Reynolds, 1988), Children's Depression Rating Scale-Revised (E. O. Poznanski & H. B. Mokros, 1996), Beck Hopelessness Scale (A. T. Beck & R. A. Steer, 1993), and Child and Adolescent Functional Assessment Scale (CAFAS; K. Hodges, 1996). YST-II had very limited positive effects, which were moderated by history of multiple suicide attempts, and no negative effects. It resulted in more rapid decreases in suicidal ideation (SIQ-JR) for multiple suicide attempters during the initial 6 weeks after hospitalization (small-to-moderate effect size). For nonmultiple attempters, it was associated with greater declines in functional impairment (CAFAS) at 3 and 12 months (small effect sizes). YST-II had no effects on suicide attempts and no enduring effects on SIQ-JR scores.

BACKGROUND: Each year over 600 million prescription items are dispensed in community pharmacies in England and Wales. Despite this, there is little published evidence relating to dispensing errors and near misses occurring in this setting. This study sought to determine their incidence, nature and causes. METHODS: Prospective study over a 4-week period in 35 community pharmacies (9 independent pharmacies and 26 chain pharmacies) in the UK. Pharmacists recorded details of all incidents that occurred during the dispensing process, including information about: the stage at which the error was detected; who found the error; who made the error; type of error; reported cause of error and circumstances associated with the error. RESULTS: 125,395 prescribed items were dispensed during the study period and 330 incidents were recorded relating to 310 prescriptions. 280 (84.8%) incidents were classified as a near miss (rate per 10,000 items dispensed=22.33, 95%CI 19.79-25.10), while the remaining 50 (15.2%) were classified as dispensing errors (rate per 10,000 items dispensed=3.99, 95%CI 2.96-5.26). Selection errors were the most common types of incidents (199, 60.3%), followed by labeling (109, 33.0%) and bagging errors (22, 6.6%). Most of the incidents were caused either by misreading the prescription (90, 24.5%), similar drug names (62, 16.8%), selecting the previous drug or dose from the patient's medication record on the pharmacy computer (42, 11.4%) or similar packaging (28, 7.6%). CONCLUSIONS: This study has demonstrated that a wide range of medication errors occur in community pharmacies. On average, for every 10,000 items dispensed, there are around 22 near misses and four dispensing errors. Given the current plans for reporting adverse events in the NHS, greater insight into the likely incidence and nature of dispensing errors will be helpful in designing effective risk management strategies in primary care.

RESUMEN

A 17-year-old adolescent female presented to a psychiatric emergency room with excitement, confusion, and psychotic symptoms. After brief exposure to haloperidol and olanzapine, she developed fever, rigidity, waxy flexibility, autonomic instability, and elevated creatinine phosphokinase enzyme. Approximately 6 weeks after the onset of the illness, multiple laboratory tests, and evaluation at three different hospitals, the condition was effectively treated with electroconvulsive therapy (ECT). This case is a lesson in delayed recognition and the delayed use of ECT for the malignant catatonia/neuroleptic malignant syndrome.

The effects of altering the type of n-3 polyunsaturated fatty acid (PUFA) in the mouse diet on the ability of monocytes and neutrophils to perform phagocytosis were investigated. Male weanling mice were fed for 7 d on one of nine diets which contained 178 g lipid/kg and which differed in the type of n-3 PUFA and in the position of these in dietary triacylglycerol (TAG). The control diet contained 4.4 g alpha-linolenic acid/100 g total fatty acids. In the other diets, eicosapentaenoic acid (EPA) or docosahexaenoic acid (DHA) replaced a proportion (50 or 100 %) of the alpha-linolenic acid, and were in the sn-2 or the sn-1(3) position of dietary TAG. There were significant increases in the content of n-3 PUFA in spleen-cell phospholipids when EPA or DHA was fed. These increases were largely independent of the position of EPA or DHA in dietary TAG except when EPA was fed at the highest level, when the incorporation was greater when it was fed in the sn-2 than in the sn-1(3) position. There was no significant effect of dietary DHA on monocyte or neutrophil phagocytic activity. Dietary EPA dose-dependently decreased the number of monocytes and neutrophils performing phagocytosis. However, when EPA was fed in the sn-2 position, the ability of active monocytes or neutrophils to engulf bacteria was increased in a dose-dependent fashion. This did not occur when EPA was fed in the sn-1(3) position. Thus, there appears to be an influence of the position of EPA, but not of DHA, in dietary TAG on its incorporation into cell phospholipids and on the activity of phagocytic cells.
