RESUMEN

Nasal-type natural killer/T-cell lymphoma (NKL) is a rare but distinct malignancy that often involves the mid-facial region and the gastrointestinal tract. This study is the first genome-wide allelotyping analysis on this rare lymphoma. We applied 382 microsatellite markers covering loci which spanned 22 autosomes to screen for allelic imbalances (AI) in six intestinal NKL. The most common chromosomal regions of allelic imbalances were found in 11p, 9q and 13q. Novel spots of allelic losses spots found at 2p21, 2q37.22, 18p11.21 and 18q12.1. In spite of presence of a few recurrent loci of imbalances, the allelotyping results show that NKL is heterogeneous.

RESUMEN

OBJECTIVE: Video-assisted thoracic surgery (VATS) is an established treatment for recurrent or complicated primary spontaneous pneumothorax (PSP). However, a proportion of patients still complains of chronic pain or discomfort after VATS pleurodesis. We aimed to investigate if paresthesia is a distinct component of the post-operative discomfort in patients receiving VATS for PSP. METHODS: Telephone interviews were conducted with 52 patients who had received VATS pleurodesis for PSP in our institute during a defined 24 month period. A standardized questionnaire was used to identify paresthetic discomforts which the patients themselves could distinguish from their wound pain. Responses were obtained from 51 patients (42 male, 9 female) with a mean age of 24.1 years (range 14-63 years), giving a response rate of 98.0%. RESULTS: With a median observation time of 19 months (range 2-24 months), 27 patients (52.9%) reported experiencing paresthesia as a post-operative complication distinct from their wound pain. The most commonly described characteristics of the paresthesia were 'pins and needles' (37.0%), 'numbness' (25.9%) or a sensation of abnormal 'swelling' in the chest wall (11.1%). Although only two of the affected patients (7.4%) described the paresthesia as 'severe', consequent functional disturbances in daily life were noted by seven patients (25.9%), and 11 patients (40.7%) actively sought medical or alternative, holistic therapies to relieve the paresthesia. Eight (21.0%) of the 38 patients followed-up for over 12 months after surgery still experienced the paresthesia. CONCLUSIONS: Although it should not detract from the proven advantages of VATS, paresthesia in the chest wall represents a distinct but previously overlooked post-VATS complication. It is a potential source of significant post-operative morbidity, and may run a chronic course in some patients. Further study is warranted to elucidate its mechanisms and optimum management.

BACKGROUND: Homosexuality is now considered an alternative lifestyle occurring with some regularity as a variant of human sexuality, rather than a pathological disorder. It is important to understand the attitudes of medical professionals toward homosexuality, as their attitudes may influence their approach to gay men and lesbians as patients. It is also important to understand the current attitudes of medical students who will be the future generation of health care providers. PURPOSE: To study Chinese medical students' attitudes toward and personal experiences with homosexuality. METHODS: A cross-sectional study was conducted at a university medical school. Medical students were asked 16 questions according to an anonymous survey questionnaire. RESULTS: Of the 780 medical students (42% male, 49% female, 10% sex anonymous) who received the questionnaire, 627 (80%) returned it. Of these students, 72% were age 18 to 22 years; 38% reported that they had homosexual friends. Only 16 students described themselves as homosexuals and 20 as bisexuals. A total of 249 students (40%) reported that they had a religion; 542 (86%) reported that they would accept their friends to be homosexuals. Two thirds thought that homosexual and heterosexual partners should have equal legal rights in terms of marriage, whereas slightly less than half (46%) thought they should have equal legal rights in adopting children. With regard to learning or working abilities, 93% thought homosexuals are not any different from heterosexuals. Two thirds thought homosexuals are not different from heterosexuals in terms of sexual behaviour such as promiscuity or loyalty. One fourth thought homosexuality is a psychological disorder that requires therapy. These findings did not differ significantly with seniority, but students with a religion held different attitudes than those without, and were more likely to believe that homosexuality is a psychological disorder, odds ratio (OR) = 2.33, 95% confidence interval (CI) = 1.59 .43, p < .001. Female students were more likely to have homosexual friends, OR = 1.72, 95% CI = 1.20 .48, p = .003, and more accepting attitudes, OR = 1.78, 95% CI = 1.24 .57, p = .002. CONCLUSIONS: The attitudes and experiences do not vary significantly between the 5 classes of medical students, but religious beliefs and gender might influence these attitudes. These findings can help medical teachers to be more sensitive in terms of educating their students.
