RESUMEN

UNLABELLED: Summary OBJECTIVE: Uterine serous carcinoma (USC) is an aggressive, histological subtype of endometrial cancer with a poor prognosis. This study evaluates the additional effect of staging surgery above total abdominal hysterectomy and bilateral salpingo-oophorectomy (TAH+BSO) on the use of adjuvant therapy and subsequent survival outcomes in clinical early-stage USC patients. MATERIALS AND METHODS: This retrospective cohort study includes 75 women treated for clinical early-stage USC. RESULTS: In 33 (44%) clinical early-stage patients surgical staging was performed and 15 patients (45%) proved to have lymphatic or abdominal metastasis. Use of adjuvant therapy was similar in patients, both staged with no metastasis (n = 18) and patients who underwent TAH and BSO only (n = 42, p = 0.17). No significant survival difference was found between surgically staged and TAH+BSO patients. CONCLUSIONS: Surgical staging proved to be important to determine stage of disease and hence prognosis. Surgical staging did not lead to selective avoidance of adjuvant therapy in patients with no metastasis.

RESUMEN

Our aim is to compare olfactory and gustatory function and food preferences of patients with Mild Cognitive Impairment (MCI) and Alzheimer's disease (AD) with controls. We included 22 patients with MCI, 30 patients with AD and 40 controls and assessed olfactory threshold, odor discrimination and odor identification (Sniffin' Sticks), gustatory functioning (Taste Strips), and food preferences (Macronutrient and Taste Preference Ranking Task). Linear regression analyses were used to study associations of five cognitive domains or AD biomarkers with olfactory functioning. Groups did not differ in olfactory threshold, gustatory function and food preferences. Patients with MCI and AD scored lower on odor discrimination and identification than controls. Poorer memory, but no other cognitive domain, was associated with poorer odor discrimination and odor identification, but not with odor threshold. No associations with AD biomarkers were found. In conclusion, patients with MCI and AD have poorer odor discrimination and identification ability than controls, but similar detection thresholds. This is likely a consequence of poorer memory rather than directly caused by AD pathology.

RESUMEN

The air quality network in the Netherlands is based on a regular grid with an interstation distance of 30 to 40 km. In or close to source areas, a higher station density is realized. Atmospheric transport models form an intrinsic part of the air quality monitoring system. The models are constructed in such a way that only routinely available meteorological input data are needed. The models are applied for interpretation and generalization of the measurements in terms of contributions of source categories. In scenario studies, the potential effect of abatement strategies is explored. The output of the models consists of concentration and deposition fields of NOx, NO2, SO2, sulfate, and nitrate on the scale of the Netherlands or on the scale of northwestern Europe. In the present version the models are not directly suitable to estimate exposures to acidic aerosols; however, in combination with limited aerosol measurements, the model predictions can be used to provide information on the spatial and temporal distribution of acidic aerosols as needed for exposure assessment.

RESUMEN

Within the framework of the UN-ECE Convention on Long-Range Transboundary Air Pollution (CLRTAP) critical levels of ozone for forest trees and crops have been determined. In this paper procedures are presented for constructing maps of The Netherlands showing measured ozone concentrations for comparison with the critical levels. The critical ozone levels for forest trees and crops are exceeded widely and frequently in The Netherlands. The frequency and size of exceedances depend on meteorological conditions: during a hot, sunny summer like 1989 or 1990 exceedances of the critical levels by a factor of two or three were observed in the whole of The Netherlands; in a cold, rainy summer like 1991 or 1993 only minor exceedances were observed in some parts (mainly in the south) of the country.

RESUMEN

The averaged proton-proton torsion angles for the three side-chain rotamers in amino acids are calculated from a statistical analysis of X-ray structures of side-chain fragments. In combination with a generalized Karplus equation the limiting coupling constants for the rotamers are calculated. The presently deduced sets are compared with published sets of limiting coupling constants.

RESUMEN

A 500, 400 and 300 MHz proton NMR study of the reaction product of cis-Pt(NH3)2Cl2 or cis-[Pt(NH3)2 (H2O)2] (NO3)2 with the deoxydinucleotide d(GpG): cis-[Pt(NH3)2 d(GpG)] was carried out. Complete assignment of the proton resonances by decoupling experiments and computer simulation of the high field part of the spectrum yield proton-proton and proton-phosphorus coupling constants of high precision. Analysis of these coupling constants reveal a 100% N (C3'-endo) conformation for the deoxyribose ring at the 5'-terminal part of the chelated d(GpG) moiety. In contrast, the 3'-terminal -pG part of the molecule displays the normal behaviour for deoxyriboses: the sugar ring prefers to adopt an S (C2'-endo) conformation (about 70%). Extrapolating from this model compound, it is suggested that Pt chelation by a -dGpdG- sequence of DNA would require a S to N conformational change of one deoxyribose moiety as the main conformational alteration and lead to a kink in one strand of the double-helical structure of DNA.
