ABSTRACT

Objective: Give pharmacotherapeutic following and study the variability and suitability of medical prescriptions given to patients during their hospitalization, through optimal and individualized pharmaceutical attention, that allows the best treatment stablished for each patient. Method: Data was captured prospectively, descriptively, and longitudinally with the purpose of analyzing the treatment's suitability in patients with heart disease, through their pharmacotherapeutic following. Results: The pharmacotherapeutic evaluation of a population of 1,228 subjects proved that elderly male patients with multiple comorbidities and in polypharmacy are more susceptible of presenting severe drug interactions (65%) and prescription errors (14.4%). Conclusions: It is essential to have a specialized pharmacist within a multi-disciplinary medical team who manages the validation of each prescription's suitability, making pharmaceutical interventions which allows the early detection of drug interactions and prescription errors, minimizing the probability of presenting real side effects and ensuring the effectiveness and security of the medical treatment.


Objetivo: Dar seguimiento farmacoterapéutico (SFT) y estudiar la variabilidad e idoneidad de la prescripción farmacológica de los pacientes durante su estancia hospitalaria, por medio de una atención farmacéutica individualizada que permita obtener mejores resultados en el tratamiento farmacológico. Método: Los datos fueron capturados de manera prospectiva, descriptiva y longitudinal para analizar la idoneidad del tratamiento, a pacientes cardiópatas, mediante el SFT. Resultados: La evaluación del SFT de población de 1,228 pacientes demostró que los pacientes cuentan con múltiples comorbilidades, polifarmacia, predominio del sexo masculino y de edad avanzada, por lo que son más propensos a presentar interacciones farmacológicas (IF) graves (65%) y errores en la medicación (14.4%). Conclusiones: Es indispensable la integración de un farmacéutico facultado en el equipo multidisciplinario de salud, que lleve a cabo la validación de la idoneidad en la prescripción médica, con una intervención farmacéutica que permita identificar oportunamente IF y errores en la medicación, disminuyendo así la probabilidad de presentar efectos reales ante la presencia de estas, asegurando la efectividad, seguridad y eficacia de los medicamentos.

ABSTRACT

Although deficiencies in the retromer sorting pathway have been linked to late-onset Alzheimer's disease, whether these deficiencies underlie the disease remains unknown. Here we characterized two genetically modified animal models to test separate but related questions about the effects that retromer deficiency has on the brain. First, testing for cognitive defects, we investigated retromer-deficient mice and found that they develop hippocampal-dependent memory and synaptic dysfunction, which was associated with elevations in endogenous Abeta peptide. Second, testing for neurodegeneration and amyloid deposits, we investigated retromer-deficient flies expressing human wild-type amyloid precursor protein (APP) and human beta-site APP-cleaving enzyme (BACE) and found that they develop neuronal loss and human Abeta aggregates. By recapitulating features of the disease, these animal models suggest that retromer deficiency observed in late-onset Alzheimer's disease can contribute to disease pathogenesis.

BACKGROUND: Clinical and functional imaging evidence suggests that cerebellar dysfunction occurs in essential tremor (ET). In recent postmortem studies, we documented increased numbers of torpedoes (Purkinje cell axonal swellings) in ET patients without Lewy bodies. Purkinje cell loss, however, has never been rigorously assessed. OBJECTIVE: To quantitatively assess the number of Purkinje cells in brains of ET patients and similarly aged controls. METHODS: Postmortem cerebellar tissue was available in 14 ET cases (6 with Lewy bodies and 8 without Lewy bodies) and 11 controls. Calbindin immunohistochemistry was performed on paraffin sections of the cerebellum. Images were digitally recorded and blinded measurements of the number of Purkinje cells per millimeter of cell layer (linear density) were made. RESULTS: Purkinje cell linear density was inversely correlated with age (r= - 0.53, P= .006) and number of torpedoes (r= - 0.42, P= .04). Purkinje cell linear density differed by diagnosis (mean [SD], controls, 3.46 [1.27] cells/mm; ET cases with Lewy bodies, 3.33 [1.06] cells/mm; and ET cases without Lewy bodies, 2.14 [0.82] cells/mm; P= .04), with the most significant difference between ET cases without Lewy bodies and controls, where the reduction was 38.2% (P= .04). In an adjusted linear regression analysis that compared ET cases without Lewy bodies with controls, decreased linear density (outcome variable) was associated with ET (beta= .56, P= .03). CONCLUSIONS: We demonstrated a reduction in Purkinje cell number in the brains of patients with ET who do not have Lewy bodies. These data further support the view that the cerebellum is anatomically, as well as functionally, abnormal in these ET cases.

La presente investigación intenta describir la dinámica grupal desenvuelta entre miembros del grupo artístico de hip hop Colectribu, conformado por jóvenes en situación de riesgo social, expuestos a fenómenos como violencia, pandillas, y consumo de substancias que asisten al programa Resistencia Juvenil dependiente de la Fundación La Paz. Se describe un espacio de desenvolvimiento artístico permitiendo el crecimiento de habilidades artísticas mediante talleres de lírica, producción musical y otros. Se plantea a través de esto la reformulación de la pandilla dejando atrás la intención violenta y de transgresión a las normas para conducir el trabajo de Colectribu hacia la creación, contribución a la sociedad y fortalecimiento personal. Colectribu es la unión de jóvenes provenientes de distintas partes de La Paz que se reúnen en base a los que es el hip hop, es una forma de expresar a las personas un mensaje nuevo, consciente y reflexivo". Se realiza un estudio de acción-participativa que además de ser plasmado en dicha investigación, se realiza un documental catalogando la realidad de los miembros del grupo que puede ser encontrado en https://youtu.be/fiX6eR1FV4A.(AU)


This research tries to describe the group dynamic that unravels between members of the hip hop artistic collective Colectribu, consisting of young people in social risk, exposed to phenomena such as violence, gangs and substance abuse, they attend the Resistencia Juvenil program dependent from the La Paz Foundation. It describes a space of artistic development allowing growth of artistic abilities by means of lyricism workshops, musical production and others types of workshops. At the same time the concept of gang reformulates, leaving behind the violent intent to redirect the work from Colectribu to creation, social contribution and personal growth. Colectribu is a union of youth who come from different places of La Paz who get together joined by hip hop, it's a way of expressing to people a new message, that is conscious and reflexive. An action-participative study was made, as well as a documentary who logs the reality of the members of the group that can be found at https://youtu.be/fiX6eR1FV4A.(AU)


Esta pesquisa procura descrever a dinâmica de grupo desenvolvida entre os membros do grupo de arte hip hop Colectribu, formado por jovens em situação de risco social, expostos a fenômenos como violência, gangues e uso de substâncias que participam do programa "Resistência Jovem" dependente da Fundação La Paz. É descrito um espaço de desenvolvimento artístico que permite o crescimento de habilidades artísticas por meio de oficinas de letras, produção musical e outras. Com isso, propõe-se a reformulação da quadrilha, deixando para trás a violenta intenção e violação das normas de conduzir o trabalho de "Colectribu" à criação, contribuição à sociedade e fortalecimento pessoal. ABSTRACT

PURPOSE: To explore the effect of docosahexaenoic acid (DHA) on gene expression during human fetal retinal maturation. METHODS: Human fetal retinal explants were cultured in serum-free Waymouth's medium supplemented with DHA or oleic acid (OA), using bovine serum albumin (BSA) as the vehicle. After 14 days in culture, fatty acid composition was assessed, and the abundance of 2400 cDNAs was examined with a human cDNA microarray system. RESULTS: Transcript abundance remained unchanged for 82% and 90% of genes in the explants with added DHA or OA, respectively. Decreased expression was detected in 4% and 9% of genes, in explants supplemented with DHA or OA, respectively, whereas, 14% of genes in explants exposed to DHA and only 0.4% of genes in explants treated with OA showed increased expression. Transcripts displaying changes in abundance in explants supplemented with DHA encode for proteins involved in diverse biological functions, including neurogenesis, neurotransmission, and refinement of connectivity. These gene expression changes were not observed in explants supplemented with OA. CONCLUSIONS: The effect of DHA deficiency on retinal function during human development can be partly explained by modifications in retinal gene expression by direct or indirect mechanisms.

ABSTRACT

Introduction: Erosive tooth wear (ETW) is defined as a mechanical chemical process that results in a cumulative loss of hard tissue without the involvement of bacteria. This process may occur in permanent and deciduous teeth and may also reach the dentin tissue. Patients who report chronic diseases, such as respiratory allergy and bronchial asthma, or recurrent acute diseases, such as tonsillitis, allergic rhinitis, sinusitis, and otitis, commonly use drugs for prolonged periods; thus, more attention should be given to the dental aspects since the drugs may cause undesirable effects. Objective: This paper aims to report and discuss a clinical case of a nine-year-old, male, Caucasian, asthmatic patient who continually uses anti-asthmatic medications and has developed ETW. Case report: According to the patient's needs, oral hygiene instructions (use of fluoride stannous dentifrice, dental floss, and topical fluoride applications), daily use of mouthwash solution containing 0.05% sodium fluoride, and dietary guidance were recommended. Conclusion: An early and accurate diagnosis of ETW lesions and recognition of specific etiological factors allow the professional to elaborate an individualized prevention and control program for ETW progression.


Introdução: O desgaste dentário erosivo é definido como um processo químico mecânico que resulta em uma perda cumulativa de tecido duro, sem o envolvimento de bactérias. Esse processo pode ocorrer em dentes permanentes e decíduos, podendo atingir o tecido dentinário. Pacientes que relatam doenças crônicas, como alergia respiratória e asma brônquica ou doenças agudas recorrentes como amigdalite, rinite alérgica, sinusite e otite, comumente usam medicamentos por períodos prolongados, portanto, mais atenção deve ser dada ao aspecto odontológico, uma vez que os medicamentos podem causar efeitos indesejáveis. Objetivo : Este trabalho tem como objetivo relatar e discutir um caso clínico de um paciente de nove anos, asmático, caucasiano, sexo masculino, que faz uso contínuo de medicamentos anti-asmáticos e desenvolveu o desgate erosivo dentário. Relato do caso: De acordo com as necessidades do paciente foi realizado instruções de higiene oral (uso de dentifrício contendo fluoreto estanhoso, fio dental e aplicações tópicas de flúor). Uso diário de enxaguatório bucal contendo 0,05% de fluoreto de sódio também foi recomendado e orientações dietéticas foram realizadas. Conclusão: O diagnóstico precoce e preciso das lesões de desgaste erosivo e o reconhecimento dos fatores etiológicos específicos permite o profissional elaborar um programa individualizado de prevenção e controle da progressão do desgaste erosivo dentário.
