ABSTRACT

Two cases of intraspinal mycetoma caused by Madurella mycetomatis and Streptomyces somaliensis presenting with paraplegia are reported. In these cases, there was neither skin or bone involvement by the disease. The route of entry of the organisms is not known; however, hematogenous blood vessel invasion by S. somaliensis was identified in the second case.

ABSTRACT

Symmers' periportal fibrosis of the liver is the major cause of morbidity and mortality in Schistosoma mansoni infection. The diagnosis is best established definitively by a wedge biopsy of the liver. The ability of abdominal ultrasonography to diagnose this condition was prospectively compared with two independent pathological examinations of wedge biopsies of the liver. Both pathologists and the ultrasonographer were unaware of the clinical diagnosis and each other's findings. Twenty-eight of 41 patients had Symmers' fibrosis by pathological examination and all were diagnosed correctly by ultrasonography prior to surgery. Symmers' fibrosis was not diagnosed by ultrasound in any of 10 patients without Symmers' fibrosis on biopsy. In 3 patients the diagnosis of Symmers' fibrosis was uncertain because the pathologists disagreed as to its presence. These results confirm the findings of previous studies and establish that ultrasonography is at least as sensitive as wedge biopsy in diagnosing Symmers' fibrosis.

Two patients with histologically proven penile ossification are reported from Africa, for the first time, in this communication. The aetiological causes in these patients are unclear and the pathogenesis of the condition is discussed.

ABSTRACT

Peroral small intestinal biopsies were obtained from 17 patients with Schistosoma mansoni infection and 13 control subjects. There was no difference between the dissecting microscopic appearances of patients and controls. The histological sections of patients showed significant shortening and broadening of villi compared to controls. There was no significant difference in the cellularity of the Lamina propria between patients and controls except that patients had significantly more eosinophils. The biopsies from the control subjects showed appearances of tropical enteropathy.

ABSTRACT

Data from 1,916 cases of oral neoplasms occurring in the Sudan in a 16-year period, from January 1970 to December 1985, were retrieved and analysed. The study revealed a relatively high frequency of oral neoplasms in comparison with neighbouring countries. In this study, squamous-cell carcinoma was the most common oral malignancy (66.5%), followed by tumours of the salivary gland (14.7%), neoplasms of nonodontogenic and non-epithelial origin (9.6%) and odontogenic neoplasms (8.6%). Men had a higher frequency than women. The older age group of both genders showed a relatively high frequency of squamous-cell carcinoma. Among northern Sudanese there was a high rate of squamous-cell carcinoma, while southern Sudanese showed a high rate of odontogenic and salivary-gland neoplasms. The use of toombak has been stated to play a major role in the aetiology of oral squamous-cell carcinoma in the Sudan, and is suspected to be associated with neoplasms of the salivary glands.

ABSTRACT

The blood supply to the mycetoma lesion and its vasculature were studied in patients with various types of mycetoma using histological, ultrastructural, angiographic and sonographic techniques. The mycetoma lesion proved to be well vascularized. However, certain vascular abnormalities were demonstrated. In histological sections, the small arteries and arterioles showed medial muscular hypertrophy in 83%, intimal fibrosis in 33%, arteritis in 7% and endarteritis obliterans with narrowed lumen in 7% of the patients. No vascular occlusion, ischaemic changes or arteriovenous shunts were observed. These changes were confirmed ultrastructurally. Angiography of the lesion showed a brisk pathological circulation which was more evident in eumycetoma. The vascular Doppler study showed normal blood flow pattern in the affected limb. Regional intra-arterial chemotherapy for mycetoma is suggested as a possible treatment modality.
