ABSTRACT

BACKGROUND: Knee osteoarthritis is associated with higher kinetic friction in the knee joint, hence increased acoustic emissions during motion. Decreases in compressive load and improvements in movement quality might reduce this friction and, thus, sound amplitude. We investigated if an exercise treatment acutely affects knee joint sounds during different activities of daily life. METHODS: Eighteen participants with knee osteoarthritis (aged 51.8 ± 7.3 years; 14 females) were included in this randomized crossover trial. A neuromuscular exercise intervention and a placebo laser needle acupuncture treatment were performed. Before and after both interventions, knee joint sounds were measured during three different activities of daily living (standing up/sitting down, walking, descending stairs) by means of vibroarthrography. The mean amplitude (dB) and the median power frequency (MPF, Hz) were assessed at the medial tibial plateau and the patella. Differences in knee acoustic emissions between placebo and exercise interventions were calculated by analyses of covariance. RESULTS: Controlled for participant's age, knee demanding activity level and osteoarthritis stage, the conditions significantly differed in their impact on the MPF (mean(± SD) pre-post-differences standing up: placebo: 9.55(± 29.15) Hz/ exercise: 13.01(± 56.06) Hz, F = 4.9, p < 0.05) and the amplitude (standing up: placebo:0.75(± 1.43) dB/ exercise: 0.51(± 4.68) dB, F = 5.0, p < 0.05; sitting down: placebo: 0.07(± 1.21) dB/ exercise: -0.16(± .36) dB, F = 4.7, p < 0.05) at the tibia. There were no differences in the MPF and amplitude during walking and descending stairs (p > 0.05). At the patella, we found significant differences in the MPF during walking (placebo 0.08(± 1.42) Hz/ exercise: 15.76(± 64.25) Hz, F = 4.8, p < .05) and in the amplitude during descending stairs (placebo: 0.02 (± 2.72) dB/ exercise: -0.73(± 2.84) dB, F = 4.9, p < 0.05). There were no differences in standing up/ sitting down for both parameters, nor in descending stairs for the MPF and walking for the amplitude (p > 0.05). CONCLUSION: The MPF pre-post differences of the exercise intervention were higher compared to the MPF pre-post differences of the placebo treatment. The amplitude pre-post differences were lower in the exercise intervention. In particular, the sound amplitude might be an indicator for therapy effects in persons with knee osteoarthritis. ABSTRACT

A glenohumeral internal rotation deficit (GIRD) of the shoulder, is associated with an increased risk of shoulder injuries in tennis athletes. The aim of the present study was to reveal the impact of 1) age, sex, specific training data (i.e. training volume, years of tennis practice, years of competitive play) and 2) upper extremity injuries on GIRD in youth competitive tennis athletes.A cross-sectional retrospective study design was adopted. Youth tennis players (n = 27, 12.6 ± 1.80 yrs., 18 male) belonging to an elite tennis squad were included. After documenting the independent variables (anthropometric data, tennis specific data and history of injury), the players were tested for internal (IR) and external (ER) shoulder rotation range of motion (RoM, [°]). From these raw values, the GIRD parameters ER/IR ratio and side differences and TRoM side differences were calculated. Pearson's correlation analyses were performed to find potential associations of the independent variables with the GIRD outcomes.A significant positive linear correlation between the years of tennis training and IR side asymmetry occurred (p < .05). A significant negative linear relation between the years of tennis training and the ratio of ER to IR range of motion (RoM) in the dominant side (p < .05) was found. The analysis of covariance showed a significant influence of the history of injuries on IR RoM (p < .05).Injury and training history but not age or training volume may impact on glenohumeral internal rotation deficit in youth tennis athletes. We showed that GIRD in the dominant side in youth tennis players is progressive with increasing years of tennis practice and independent of years of practice associated with the history of injuries. Early detection of decreased glenohumeral RoM (specifically IR), as well as injury prevention training programs, may be useful to reduce GIRD and its negative consequences.

ABSTRACT

Knee acoustic emissions provide information about joint health and loading in motion. As the reproducibility of knee acoustic emissions by vibroarthrography is yet unknown, we evaluated the intrasession and interday reliability of knee joint sounds. In 19 volunteers (25.6 ± 2.0 years, 11 female), knee joint sounds were recorded by two acoustic sensors (16,000 Hz; medial tibial plateau, patella). All participants performed four sets standing up/sitting down (five repetitions each). For measuring intrasession reliability, we used a washout phase of 30 min between the first three sets, and for interday reliability we used a washout phase of one week between sets 3 and 4. The mean amplitude (dB) and median power frequency (Hz, MPF) were analyzed for each set. Intraclass correlation coefficients (ICCs (2,1)), standard errors of measurement (SEMs), and coefficients of variability (CVs) were calculated. The intrasession ICCs ranged from 0.85 to 0.95 (tibia) and from 0.73 to 0.87 (patella). The corresponding SEMs for the amplitude were ≤1.44 dB (tibia) and ≤2.38 dB (patella); for the MPF, SEMs were ≤13.78 Hz (tibia) and ≤14.47 Hz (patella). The intrasession CVs were ≤0.06 (tibia) and ≤0.07 (patella) (p < 0.05). The interday ICCs ranged from 0.24 to 0.33 (tibia) and from 0 to 0.82 (patella) for both the MPF and amplitude. The interday SEMs were ≤4.39 dB (tibia) and ≤6.85 dB (patella) for the amplitude and ≤35.39 Hz (tibia) and ≤15.64 Hz (patella) for the MPF. The CVs were ≤0.14 (tibia) and ≤0.08 (patella). Knee joint sounds were highly repeatable within a single session but yielded inconsistent results for the interday reliability.

ABSTRACT

PURPOSE: A small share of the world's population meets current physical activity guidelines, which recommend regular engagement in endurance, strength, and neuromotor exercise. As lack of time represents a major cause of inactivity, multidimensional training methods with short durations may provide a promising alternative to classical, volume-oriented approaches focusing on one biomotor ability. This trial examined the effects of a high-intensity functional circuit training (HIFCT) on motor performance and exercise motivation in untrained adults. METHODS: Thirty-three inactive participants were randomly allocated to two groups exercising for six weeks. The intervention group (HIFCT, n = 20) 3×/week performed functional whole-body exercises in a circuit. Each 15-minute workout included repetitive 20s all-out bouts with 10s breaks. In the comparison group (moderate aerobic exercise, MAE, n = 13), the participants walked 3×/week for 50 minutes at moderate intensity. Measured motor outcomes were cycling endurance capacity (respiratory threshold, maximum workload), maximum strength (leg and chest press), postural control (force plate), and jump capacity (counter-movement jump, single leg hop for distance); exercise motivation was assessed using the self-concordance index. RESULTS: In comparison with MAE, HIFCT enhanced maximum leg strength (between-group difference of relative pre- to post-changes of 5.0%), shoulder strength (7.6%), and endurance workload (5.0%; P < 0.05), while increasing motivation to exercise (+5.5 points, P < 0.05). No between-group differences occurred for postural control and jump capacity (P > 0.05). CONCLUSION: Despite considerably shorter training duration, HIFCT enhances motor function and motivation to exercise more effectively than MAE. Further research should investigate the long-term adherence to the program and its effectiveness in other settings.

ABSTRACT

CONTEXT: Recent research has underpinned the occurrence of nonlocal exercise effects. For instance, self-myofascial release (SMR) of the plantar fascia increases hamstring extensibility. A possible mechanism consists in a mechanical force transmission across myofascial chains. However, as the degree of structural continuity in these chains decreases with age, the magnitude of the above described remote effects might also be reduced throughout a lifespan. OBJECTIVE: To examine the age dependency of nonlocal exercise effects following plantar fascia SMR. DESIGN: Regression experimental study. SETTING: General population. PARTICIPANTS: 168 healthy participants (Mage = 45 [21] y, 85 males). INTERVENTION: One 120 s bout of plantar foot SMR, performed in standing position. MAIN OUTCOMES MEASURE: Hamstring extensibility was assessed using sit and reach testing. Relative pre-post differences were classified as no improvement, clinically nonrelevant improvement, or clinically relevant improvement according to previously published data. The age dependency of the effects was calculated by means of multinomial stepwise logistic regression. If the latter revealed other factors than age to affect treatment effectivity, their effect was eliminated using partial correlation. RESULTS: SMR increased hamstring extensibility by 10.1% (pre: 24.9 [9.7] cm, post: 27.4 [9.3] cm, P < .001). About 99 participants (58.9%) attained a clinically relevant change. Multinomial logistic regression demonstrated no influence of sex, amount of physical activity, height, body mass index, and time of day, but a significant impact of baseline flexibility and age (Nagelkerke's r2 = .32, P < .001). Post hoc partial correlation analysis demonstrated that age, corrected for baseline flexibility, had a small to moderate association with treatment effectivity (r = .29, P < .001). CONCLUSIONS: Plantar foot SMR increases hamstring extensibility, which is explained by age to a small degree. Additional research is warranted to delineate the substrate of remote exercise effects. Besides mechanical force transmission, cortical adaptations might also represent the driving factor.

ABSTRACT

Background: The transition from school to university is often accompanied by a change in students' lifestyles. So far little is known whether convenience behavior is an essential factor affecting students' health and social interaction. In a heterogeneous population regard to sociodemographic and anthropometric characteristics the Convenience Behavior Questionnaire (CBQ) showed a better relationship between convenience-related behavior with overweight and obesity than established questionnaires. Here we assessed convenience behavior in a large well characterized cohort of university students and its association with health-related (mainly sedentary behavior and physical activity), study-related and sociodemographic factors with the Convenience Behavior Questionnaire (CBQ). Methods: A total of 4,351 students participated in an online survey, of which 3,983 (23.6 ± 5.3 years old, 71.3% females) answered the questions concerning convenience behavior. A low value in the CBQ indicates more convenience behavior [Convenience Behavior Index (CBI) range: 3-15]. Differences with regard to sociodemographic (age, gender, body mass index), study-related (semester, degree, field of study) and health-related (physical activity, sedentary behavior) variables were examined with Mann-Whitney-U test or Kruskal-Wallis test and post-hoc tests (Bonferroni). Results: The CBI of men and women differed significantly (z = -6.847, p < 0.001, r = 0.11). First-year students and students beyond their first year showed significant differences (z = -2.355, p ≤ 0.05, r = 0.04). Differences were also found in the field of study (Chi2 (6) = 147.830, p < 0.001) and the targeted degree (Chi2 (7) = 79.985, p < 0.001). Furthermore, differences were found in the body mass index (Chi2 (5) = 70.678, p < 0.001), physical activity (Chi2 (2) = 279.040, p < 0.001) and sedentary behavior (z = -4.660, p < 0.001, r = 0.07). Conclusion: The results showed risk groups of convenience behavior among students [men, first-year, students enrolled in "Science, Technology, Engineering and Mathematics (STEM)," bachelor]. Our results confirm for the first time in a very homogeneous population a gender difference and an association between CBI and health-related factors. Further studies are needed to analyze the health behavior of students in more detail, especially their convenience behavior.

ABSTRACT

This study investigated the effects of a daily plyometric hopping intervention on running economy (RE) in amateur runners. In a randomized, controlled trial, thirty-four amateur runners (29 ± 7 years, 27 males) were allocated to a control or a hopping exercise group. During the six-week study, the exercise group performed 5 min of double-legged hopping exercise daily. To progressively increase loading, the number of hopping bouts (10 s each) was steadily increased while break duration between sets was decreased. Pre- and post-intervention, RE, peak oxygen uptake (VO2peak), and respiratory exchange ratio (RER) were measured during 4-min stages at three running speeds (10, 12, and 14 km/h). ANCOVAs with baseline values and potential cofounders as cofactors were performed to identify differences between groups. ANCOVA revealed an effect of hopping on RE at 12 km/h (df = 1; F = 4.35; p < 0.05; η2 = 0.072) and 14 km/h (df = 1; F = 6.72; p < 0.05; η2 = 0.098), but not at 10 km/h (p > 0.05). Exercise did not affect VO2peak (p > 0.05), but increased RER at 12 km/h (df = 1; F = 4.26; p < 0.05; η2 = 0.059) and 14 km/h (df = 1; F = 36.73; p < 0.001; η2 = 0.520). No difference in RER was observed at 10 km/h (p > 0.05). Daily hopping exercise is effective in improving RE at high running speeds in amateurs and thus can be considered a feasible complementary training program.Clinical trial registration German Register of Clinical Trials (DRKS00017373).

The aim was to investigate the attitude towards and perceived stress from emergency remote teaching (ERT) among teachers during the COVID-19 pandemic. A Germany-wide online survey was conducted among teachers from all school types in March 2021. Data from 31,089 teachers entered analysis. ANOVAs or Welch's t-tests with post-hoc analyses were performed to determine sociodemographic and work-related group differences in teachers' attitude towards and perceived stress from ERT. The mean attitude towards ERT was 3.47 (± .84) out of 5 and the mean perceived stress was 5.03 (± .62) out of 6. Regarding the attitude towards ERT, we revealed significant differences for gender, age groups, number of children, occupational group, school management membership, and employment status (p < .05). Regarding perceived stress, significant differences were obtained for gender, age groups, and employment status (p < .05). A more positive attitude towards ERT seems to be associated with lower stress levels. Being female, a higher age, a higher number of children living in the own household as well as working full-time might hinder an effective implementation of remote teaching in school settings in Germany. Policy-makers and schools should think of strategies to improve the attitude towards and decrease perceived stress from remote teaching. This could include subgroup-specific training on the use of digital media, adapted to the work environment.

ABSTRACT

Background: The SARS-CoV-2 pandemic exacerbated existing health-related challenges in schools and created new ones. Under pandemic conditions, health risks increased, and with them the requirements for occupational safety and health (OSH) measures. The aim of the study was (a) to examine the status quo of OSH measures in German schools, (b) to analyze whether the implementation of OSH measures was associated with preferable outcomes and (c) to identify predictors for the implementation of OSH measures. Methods: A nationwide cross-sectional online survey was conducted among teachers at all school types in Germany in March 2021. Data on the implementation of OSH measures (risk assessments, infection protection instructions and instructions on occupational safety), associated health-related parameters (e.g., somatic symptoms, PHQ-15) and predictor variables (e.g., gender, age or federal states) were assessed using descriptive statistics, analysis of variance and multiple linear regression analysis. Results: Less than 10% of surveyed teachers (N = 31,089) reported that their schools met legal requirements for occupational safety and health measures. Beneficial associations became apparent where more measures were implemented, e.g., significantly better somatic and mental health. Predictors for the implementation of OSH measures were found, especially on a systemic level (e.g., federal states schools were located in). Conclusions: Our study can serve as a basis for future studies. It provides a status quo regarding the implementation of, associations with and predictors for OSH measures in German schools. Our results are best understood as evidence-based arguments to encourage political decision makers to improve the implementation of OSH measures in German schools and thereby foster teachers' health.

Introduction: During the SARS-CoV-2 pandemic teaching was changed several times to distance learning. To consider the associated stresses and challenges for teachers a nationwide cross-sectional study was performed in March 2021 in which N = 31,089 teachers from Germany participated. Methods: A multiple linear regression model with stepwise inclusion of thematically sorted variables (sociodemographic, SARS-CoV-2- and work-related variables) was used to identify relevant predictors of job satisfaction. Results: The analysis revealed that work-related variables were significant predictors of job satisfaction. In the third regression model, when all variables are included the adjusted R2 was 0.364. Overall, the results showed that, e.g., work predictability (b = 0.097), influence at work (b = 0.118), and meaning of work (b = 0.212) increased job satisfaction. In contrast, increased emotional exhaustion (b = -0.016), feelings of unfair treatment (b = -0.048), and work family conflicts (b = -0.154) deteriorated job satisfaction. Discussion: The results indicate that future research should focus especially the work-related topics in more detail and that job satisfaction is a useful concept for analyzing working conditions from a public health point of view.

Background: Sedentary behavior (SB) is highly prevalent among university students and has increased during COVID-19 pandemic. As SB is associated with negative health outcomes, appropriate prevention measures in the university setting are needed. Objective: This pilot study aimed at investigating the effects of videos using different message strategies to interrupt SB in the collective of university students during online lectures. Methods: During online lectures, university students (N = 96) were shown one of three videos on the interruption of SB. The videos differed in their message strategies with regard to evidence type (statistical vs. narrative) and vividness (static vs. animated images). Demographics, health variables (SB intentions, SB attitudes) and selected media reception variables (identification, homophily, counterarguing) were examined as possible influence factors on the interruption of SB evoked by watching the video. Results: Approximately half of the students interrupted sedentary behavior during watching the videos and students of the older age group (cut-off: median = 22 years) interrupted SB significantly more often (p = 0.046). The interruption of SB was predicted by SB intentions (p < 0.05). Identification with characters significantly predicted the intentions to reduce SB (p < 0.001), with a large effect of the overall regression model (R2corr = 0.47). Conclusion: Considering the increased digitalization in general and restrictions due to COVID-19 pandemic, videos seem to be a useful tool to interrupt SB among university students during online lectures. Narrative formats could facilitate the intention to reduce SB, which in turn could have a positive impact on the interruption of SB. However, further research on effective communication and message strategies is needed.

Background: The present study aimed to (1) assess and compare sedentary time (ST) of university students before and during the COVID-19 pandemic, (2) examine risk groups with regard to ST and the "extent of change" in ST (from before to during the pandemic) in association with sociodemographic (gender, age), study-related (degree aspired to, field of study, semester), and pre-pandemic physical health-related [pre-pandemic physical activity (PA) and ST levels, pre-pandemic BMI class] variables, and (3) investigate whether the change in ST was predicted by these variables. Methods: Two online surveys were conducted among students at the University of Mainz, Germany-the first in 2019 (before the pandemic) and the second in 2020 (during the pandemic). Participants of both surveys were included in a longitudinal sample. With the longitudinal sample's data, paired t-tests, single factor, and mixed analyses of variances were used to examine group differences in ST and the "extent of change" in ST. A linear regression analysis was computed to investigate the influence of the abovementioned sociodemographic, study-related, and pre-pandemic physical health-related variables on the change in ST. Results: Of the N = 4,351 (pre-pandemic) and N = 3,066 (in-pandemic) participants of the online surveys, N = 443 entered the longitudinal sample. ST increased by 1.4 h/day to critical levels (≥8 h/day) in all subgroups analyzed-even among students who were highly physically active before the pandemic. Students with a low pre-pandemic ST had the largest increase in ST. Pre-pandemic PA level negatively predicted the change in ST. Conclusion: Even during a global pandemic lockdown, individuals who were previously more physically active and had less ST showed more health-promoting behavior in terms of ST. Therefore, it can be stated that efforts to promote PA and reduce ST are always valuable. Since ST increased and was worryingly high in all subgroups analyzed, all university students should be targeted by multidimensional approaches to tackle ST and promote their health.

BACKGROUND: Schools underwent massive changes during the SARS-CoV-2 pandemic worldwide. Besides existing occupational health challenges, teachers had to deal with biological and psychological burdens that had the potential to impact their psychological well-being. The aim of the present study was to (i) assess the current state of psychological burdens in German teachers and (ii) identify highly burdened subgroups to derive and address interventions. METHODS: A nationwide cross-sectional online survey was conducted among teachers at all school types in Germany in March 2021. Data on psychological strains were assessed using established (e.g., PHQ-4) and new-pandemic-specific-(e.g., COVID-19-associated anxieties) instruments. ANOVAs and Tukey's post hoc tests were used to identify highly burdened subgroups (e.g., gender, age, and number of risk factors for severe courses of COVID-19) of teachers. RESULTS: Psychological burdens in German teachers (N = 31.089) exceeded the level of the general population, for example, regarding symptoms of depression (PHQ-2, M = 1.93 vs. 1.24) or generalized anxiety (GAD-2, M = 1.72 vs. 1.03). Subgroup analysis revealed that psychological burdens were unevenly distributed among different groups of teachers; for example, younger teachers (18-30 years) showed more depression symptoms compared with their older colleagues (56-67 years) (PHQ-2, M = 2.01 vs. 1.78). CONCLUSIONS: The online survey was conducted during the "third wave" of SARS-CoV-2 in Germany, which might have influenced risk perception and psychological strains. Future studies at different times, ideally longitudinal monitoring of the mental health of teachers, are recommended. Based on our results, evidence-based subgroup-specific interventions should be implemented to sustain teachers' mental health; for example, younger teachers or teachers with risk factors for a severe course of COVID-19 should receive special attention and support. Teachers from special needs schools whose mental health is, on average, good could also be a starting point for identifying the health promotion structural elements of this school type (e.g., fewer students per teacher). However, beyond the specific pandemic-related psychological burdens, the classic occupational health challenges of physical, biological, and chemical stress and their resulting strains should not be disregarded.

ABSTRACT

Background: Physical inactivity and sedentary behavior are modifiable risk factors for an unhealthy lifestyle in university students. The aim of this study was to identify subgroups among German university students with an increased risk for unhealthy behavior. For this purpose, differences in physical activity and sedentary behavior with respect to sociodemographic and study related factors were examined. Methods: A total of 4,351 students participated in an online survey. The amount of physical activity (metabolic equivalent of task-min/week) and the sitting time (h/day) were assessed using the German short form of the International Physical Activity Questionnaire. Differences in gender and age as well as field of study, targeted degree and study semester were analyzed using a single factorial ANOVA with Tukey correction or a Welch-ANOVA with Games-Howell correction. Results: For physical activity, significant differences were found for gender (F(2, 80.46) = 17.79, p < 0.001, ηp2 = 0.009), for field of study (F(5, 1738.09) = 7.41, p < 0.001, ηp2 = 0.01), and for study semester (F(1, 948.12) = 5.53, p < 0.05, ηp2 =0.001), but not for age and targeted degree (p > 0.05). For sedentary behavior, significant differences were found for field of study (F(5, 3816) = 5.69, p < 0.001, ηp2 = 0.01) and targeted degree (F(3, 3868) = 3.94, p < 0.01, ηp2 = 0.003), but not for gender, age and study semester (p > 0.05). Conclusion: Female students, students enrolled in "natural sciences, mathematics and informatics" and first year students appear to have an increased risk of an unhealthy lifestyle. Future research should identify barriers to and incentives of physical activity as well as reasons for high amounts of SB in sub-populations of university students. Suitable prevention and intervention programs are necessary.

ABSTRACT

Vibroarthrography measures joint sounds caused by sliding of the joint surfaces over each other. and can be affected by joint health, load and type of movement. Since both warm-up and muscle fatigue lead to local changes in the knee joint (e.g., temperature increase, lubrication of the joint, muscle activation), these may impact knee joint sounds. Therefore, this study investigates the effects of warm-up and muscle fatiguing exercise on knee joint sounds during an activity of daily living. Seventeen healthy, physically active volunteers (25.7 ± 2 years, 7 males) performed a control and an intervention session with a wash-out phase of one week. The control session consisted of sitting on a chair, while the intervention session contained a warm-up (walking on a treadmill) followed by a fatiguing exercise (modified sit-to-stand) protocol. Knee sounds were recorded by vibroarthrography (at the medial tibia plateau and at the patella) at three time points in each session during a sit-to-stand movement. The primary outcome was the mean signal amplitude (MSA, dB). Differences between sessions were determined by repeated measures ANOVA with intra-individual pre-post differences for the warm-up and for the muscle fatigue effect. We found a significant difference for MSA at the medial tibia plateau (intervention: mean 1.51 dB, standard deviation 2.51 dB; control: mean -1.28 dB, SD 2.61 dB; F = 9.5; p = .007; η2 = .37) during extension (from sit to stand) after the warm-up. There was no significant difference for any parameter after the muscle fatiguing exercise (p > .05). The increase in MSA may mostly be explained by an increase in internal knee load and joint friction. However, neuromuscular changes may also have played a role. It appears that the muscle fatiguing exercise has no impact on knee joint sounds in young, active, symptom-free participants during sit to stand.

ABSTRACT

We tested if ACL-reconstructed participants show a decreased quadriceps torque, a lower muscle endurance capacity and a higher peak torque variability compared to unimpaired control participants prior to and following local muscle fatigue. Participants (n = 19, 10 women; 25 ± 5yrs.) with unilateral hamstrings autograft ACL-reconstruction and a matched unimpaired control group were recruited. Participants performed two maximal isometric voluntary force (MIVF) contractions of the knee extensors. In between, standardized local muscle fatigue was induced. ACL-reconstructed knees display a lower peak torque of the knee extensors in comparison to the contralateral limb (3.2 ± .3Nm/kg vs. 3.5 ± .3 Nm/kg). Peak torque variability and fatigue resistance were not affected by local muscle fatigue (p > .05). Participants with ACL-reconstructed knees show a persistent quadriceps muscle dysfunction. This dysfunction and lower limb side asymmetries might be risk factors for ACL re-ruptures.

BACKGROUND: Crepitus of the knee may mirror structural and functional changes in the joint during motion. Although the magnitude of these sounds increases with greater cartilage damage, it is unclear whether knee joint sounds also reflect joint loading. METHODS: Twelve healthy volunteers (mean 26 (SD 3.6) years, 7 females) participated in the randomized-balanced crossover study. Knee joint sounds were recorded (linear sampling, 5512 Hz) by means of two microphones, one placed on the medial tibial plateau and one on the patella. Two activities of daily living (standing up from/sitting down on a bench; descending stairs) and three open kinetic chain knee extension-flexion cycles (passive movement, 10% and 40% loading of the individual one repetition maximum) were performed. Each participant carried out three sets of five repetitions and three sets of 15 steps downwards (stairs), respectively. For data analysis, the mean sound amplitude and the median power frequency for each loading condition were determined. Friedman test and Bonferroni-Holm adjusted post-hoc test were performed to detect differences between conditions. FINDINGS: We obtained significant differences between joint sound amplitudes for all movements, both measured at the medial tibial plateau (Chi2 = 20.7, p < 0.001) and at the patella (Chi2 = 27.6, p < 0.001). We showed a significant difference in the median power frequency of the patella between all movements (Chi2 = 17.8, p < 0.5). INTERPRETATION: Overall, the larger the supposed knee joint loading was, the louder was the recorded knee crepitus. Consequently, vibroarthrographically assessed knee joint sounds can differ across knee joint loading conditions.

BACKGROUND: Motor function, such as strength asymmetries of the lower extremities and impaired dynamic stability, have a predictive value for the risk of injury. The present study aimed to reveal potential associations between injury and motor performance. METHODS: Two hundred and five male youth elite (association) football (soccer) players (mean: 13.5±4.5 years, 57.2±30.2 kg, 168±35 cm) were included. A test battery was conducted twice per season, over two consecutive seasons (four times). Mobility (Sit and Reach Test, SnR), dynamic stability (Single Leg Hop for Distance, SLHD), linear sprinting speed (10 m, 30 m [s]), agility (Zig-Zag test with and without dribbling a ball [s]), jump performance (countermovement jump (CMJ) and drop jump (DJ), in cm) and maximal isometric voluntary force (MIVF, in N) of the knee extensors and flexors were assessed. All injuries occurring over the two-year period, as well as training and competition exposure time, were collected and used as grouping variables for statistical difference testing. RESULTS: One hundred and twenty-five injuries in 93 players occurred (an injury incidence of 2.7/1000 hours of exposure). Age was associated with injury incidence (r=0.191; P=0.006). Neither DJ, CMJ, SnR nor agility performance were statistically different between injured and non-injured participants (P>0.05). Group differences did occur for sprint and strength (P=0.011; P=0.016), but these lapsed after the inclusion of age as a covariate. Only for SLHD symmetry was a non-significant trend evident after the correction for age (P=0.08). CONCLUSIONS: The occurrence of musculoskeletal injuries in junior football players are, probably, not related to baseline motor function. Group differences between injured and non-injured youth elite football players are mostly explained by age. Only the symmetry in SLHD could be a potential risk factor for injuries and merits further investigation.

ABSTRACT

Knee joint sounds contain information on joint health, morphology and loading. These acoustic signals may be elicited by further, as yet unknown factors. By assessing potential elicitors and their relative contributions to the acoustic signal, we investigated the validity of vibroarthrographic assessments during different movement conditions with the aim to derive recommendations for their practical usage. Cross-sectional study. Nineteen healthy participants (24.7 ± 2.8 yrs, 7 females) performed five movements: level walking, descending stairs, standing up, sitting down, and forward lunge. Knee joint sounds were recorded by two microphones (medial tibial plateau, patella). Knee joint kinematics and ground reaction forces were recorded synchronously to calculate knee joint moments (Nm/Kg). The mean amplitude (dB) and the median power frequency (Hz) were determined. A repeated measures mixed model investigated the impact of potential predictors (sagittal, frontal, transverse plane and total knee joint moments, knee angular velocity, age, sex, body mass index (BMI) and Tegner Activity Score (TAS)). Most of the amplitudes variance is explained by between-subject differences (tibia: 66.6%; patella: 75.8%), and of the median power frequencies variance by the movement condition (tibia: 97.6%; patella: 98.9%). The final model revealed several predictor variables for both sensors (tibia: sagittal plane, frontal plane, and total knee joint moments, age, and TAS; patella: sagittal plane knee moments, knee angular velocity, TAS). The standardization of the execution of the activities, a between-group matching of variables and the inclusion of co-variates are recommended to increase the validity of vibroarthrographic measurements during different movement conditions of the knee joint.
