ABSTRACT

INTRODUCTION: Our goal was to demonstrate the effects of occupational exposure to antineoplastic drugs on oxidative stress parameters and DNA damage in health professionals who manipulate and administer antineoplastic drugs in a University Hospital in Southern Brazil. METHODS: The case-control study with a longitudinal design, involved 64 individuals, 29 of them pharmacists, pharmacy technicians and nurses who were occupationally exposed to antineoplastic drugs and 35 professionals who were not exposed. Gene mutations were determined by micronucleus from salivary fluid; DNA damage by comet assay and oxidative stress parameters in whole blood were also evaluated. RESULTS: All workers exposed to antineoplastic drugs used personal protective equipment (PPE). It was demonstrated that the total nonprotein thiol and thiobarbituric acid reactive substances levels showed interaction between group and time, with higher levels one week after handling/administration of antineoplastic drugs in the exposed group (GEE, p ≤ 0.0001 and p = 0,013, respectively). Additionally, there was a group effect on the activities of the catalase and glutathione peroxidase antioxidant enzymes (GEE, p = 0.027 and p ≤ 0.0001, respectively), and workers occupationally exposed to antineoplastic drugs had higher enzyme activities compared to those not exposed. No genotoxic damage was demonstrated through the evaluated parameters. CONCLUSIONS: Despite the correct use of PPE, professionals occupationally exposed to antineoplastic drugs were more susceptible to oxidative stress than those not exposed. The evaluation of the studied parameters is especially important for the definition of conducts and practices in the area, always in search of guaranteeing the establishment of a rational policy to protect workers' health.

ABSTRACT

Organochalcogens are extensively produced and employed by industry and agriculture, and the risk of occupational and environmental toxicity to them has been poorly understood. Here, we investigated the acute effect of a new organochalcogen 3-methyl-1-phenyl-2-(phenylseleno)oct-2-en-1-one on biochemical and hematological parameters in male Wistar rats. The animals were treated with a single intraperitoneal injection of the organochalcogen at doses of 125, 250 or 500 µg·kg(-1). After 60 min, the animals were sacrificed by decapitation, and the trunk blood was collected for determination of glucose, triglycerides, cholesterol, alanine aminotransferase (ALT), aspartate aminotransferase, lactate dehydrogenase, urea, creatinine, C-reactive protein, red blood cells, hematocrit, hemoglobin and white blood cells (WBC). Our results showed a reduction in cholesterol levels in all treated groups, an increase in ALT activity at doses of 250 and 500 µg·kg(-1), a decrease of hemoglobin and an increase in WBC in animals that received 250 and 500 µg·kg(-1) of the organoselenium. In addition, we observed an increase in neutrophil counts at 125 µg·kg(-1) dose and a decrease at 500 µg·kg(-1) dose. We also verified an increase in lymphocyte counts at the dose of 500 µg·kg(-1). Thus, the present study shows that the acute treatment with this new organochalcogen causes biochemical changes and hematological disorders in male rats.

ABSTRACT

The objective of this study was to verify the effect of the organochalcogen 3-butyl-1-phenyl-2-(phenyltelluro)oct-en-1-one on some parameters of oxidative stress in human serum. Serum of volunteers were incubated for 30 min in the presence or absence of 1, 10, or 30 microM of 3-butyl-1-phenyl-2-(phenyltelluro)oct-en-1-one and oxidative stress was measured. First, we tested the influence of the compound on 1,1-diphenyl-2-picrylhydrazyl (DPPH(*)) radical-scavenging and verified that the organotellurium did not have any antioxidant properties. The organochalcogen was capable to enhance TBARS but the compound was not able to alter carbonyl assay. Furthermore, the organochalcogen provoked a reduction of protein thiol groups measured by the sulfhydryl assay. Moreover, the organotellurium enhanced the activity of catalase and superoxide dismutase, inhibited the activity of glutathione peroxidase and did not modify the glutathione S-transferase activity. Furthermore, nitric oxide production and hydroxyl radical activity were not affected by the compound. Our findings showed that this organochalcogen induces oxidative stress in human serum, indicating that this compound is potentially toxic to human beings.

Elevated filling pressures are associated with heart failure deterioration, but mechanisms underlying this association remain poorly understood. We sought to investigate whether or not elevated filling pressures are associated with increased collagen turnover, evaluated by procollagen type III aminoterminal peptide (PIIINP) levels, in stable systolic heart failure. Eighty patients with heart failure with severe systolic dysfunction (ejection fraction 26 +/- 7%) were included. Patients underwent simultaneous echocardiogram with evaluation of haemodynamic parameters and blood sampling for PIIINP measurement. Mean PIIINP level was 6.11 +/- 2.62 microg l(-1). PIIINP was positively associated with estimated right atrial pressure (RAP) (r = 0.36; p = 0.001). Mean PIIINP values were 5.04 +/- 2.42 microg l(-1) in patients with estimated RAP < or = 5 mmHg, and 7.59 +/- 2.54 microg l(-1) in those with RAP > 15 mmHg (p < 0.01). In conclusion, elevated right-side filling pressures are associated with evidence of active extracellular matrix turnover, as indicated by elevated PIIINP levels, in stable systolic heart failure. Activation of extracellular matrix turnover may be implicated in the accelerated progression of heart failure syndromes seen in patients with persistent congestion.

BACKGROUND: Partial hepatectomy is a surgical intervention of the liver that can trigger its regenerative process, where the residual lobes deflagrate a compensatory hyperplasia, causing its restoration almost to the original volume. Nevertheless, depending on the extent of liver damage its regeneration might be impaired. The low-power laser has been studied with beneficial results. AIM: To investigate the possible functional and mutagenic damage arising from the use of low-power laser used in liver regeneration after partial hepatectomy. METHODS: Fifteen male adult Wistar rats were hepatectomizated in 70% and laser irradiated or not with dose of 70 J/cm2, 650 nm, 100 mW, directly on the remaining liver, during the perioperative period. These animals were divided into four groups: G1 (control, 7 days); G2 (laser, 7 days); G3 (control, 14 days); G4 (laser, 14 days). Were analyzed the liver weight; number of hepatocytes; deposition of collagen fibers; liver function tests: serum alanine aminotransferase, aspartate aminotransferase, alkaline phosphatase, gamma glutamyl transferase, bilirubin and micronucleus test in peripheral blood erythrocyte. RESULTS: The liver weight was greater in G3 and G4 (p=0.001 and p=0.002) compared to other groups. The deposition of collagen fibers in G1 was statistically higher than the other groups (p=0.01). In tests of liver function and micronucleus test was not found significant differences between the studied groups. CONCLUSION: Low-power laser stimulation did not cause loss of liver function or mutagenic damage.

RESUMO Este trabalho visa a caracterizar cuidadores de idosos da região metropolitana de Porto Alegre quanto a: capacitação para a profissão, seus sentimentos frente ao idoso, sobrecarga de trabalho, estado cognitivo e condição física. Entre janeiro e fevereiro de 2015, 20 participantes responderam a um questionário sobre idade, gênero, escolaridade, estado civil, breve investigação de estado de saúde e sentimentos frente ao cuidado e à Escala de Zarit Burden Interview, Escala de Depressão Geriátrica Abreviada, Mini Exame do Estado Mental, Escala de Avaliação do Equilíbrio e da Marcha de Tinneti. Verificou-se que as participantes nunca fizeram capacitação e apresentam perdas de saúde.


ABSTRACT This study aims to characterize the caregivers of the elderly in metropolitan area of Porto Alegre regarding: the qualification for the profession, their feelings towards the elderly, the work overload, cognitive status and physical condition. Between January and February 2015, 20 participants answered a questionnaire about age, gender, education, marital status, brief health status investigation and feelings about care and the Zarit Burden Interview Scale, Geriatric Depression Scale, Mini-Mental State Examination, Tinneti's Balance and Gait Assessment Scale. It was verified that the participants have never done training and that they present losses in their health.

OBJETIVOS: Analisar o perfil e a prevalência de doenças em idosos admitidos em duas estações do ano distintas, em um serviço público de fisioterapia de um município de médio porte localizado em zona de clima subtropical de altitude. MÉTODOS: Um estudo transversal analisou prontuários de pacientes encaminhados nos meses que compreenderam o verão e o inverno de 2011. Foram incluídos os prontuários de idosos (acima de 60 anos de idade) de ambos os sexos e excluídos os prontuários sem informações sobre o diagnóstico médico atual. Foram coletadas as variáveis idade e sexo e os diagnósticos atuais e pregressos. Para análise comparativa quanto ao perfil dos idosos admitidos nos dois períodos do ano, utilizou-se o teste Qui-quadrado. RESULTADOS: Foram analisados dados de 71 idosos. A média de idade foi de 69,2±8,1 anos e 48 (70,4%) eram do sexo feminino. Os diagnósticos mais frequentes no momento do encaminhamento foram de doenças traumato-ortopédicas (60,6%). O maior número de admissões ocorreu durante o inverno (62,0%). Os diagnósticos no momento do encaminhamento foram, respectivamente no verão e no inverno: traumatoortopédicos 17 (63,0%) e 26 (59,1%); reumatológicos 3 (11,1%) e 10 (22,7%); neurológicos 5 (18,5%) e 4 (9,1%); pós-operatório 2 (7,4%) e 4 (9,1%) (p=0,64). CONCLUSÕES: No inverno ocorreu maior demanda de encaminhamentos de idosos ao serviço de fisioterapia em comparação ao verão, com predomínio de mulheres em ambas as estações do ano. As causas de encaminhamento mais prevalentes foram relacionadas à traumatoortopedia, mas não houve diferença significativa na frequência dos diagnósticos conforme a estação do ano. O conhecimento sobre o perfil dos idosos admitidos em serviços de fisioterapia é importante para o planejamento de ações voltadas a essa faixa etária, com a realização de campanhas de informação, prevenção e promoção da saúde.


AIMS: To analyze the profile and the prevalence of diseases in elderly patients admitted to a physical therapy center in a medium-sized Brazilian city located in a subtropical zone in two different seasons of the year. METHODS: A cross-sectional study assessed the medical records of patients treated during summer and winter months in 2011. Medical records of male and female elderly patients (aged over 60 years) were included, and those without information on present medical diagnosis were excluded. Age and sex and both current and previous diagnostic records were analyzed. The chi-square test was used for the comparative analysis of the profile of the elderly patients for both seasons. RESULTS: Seventy-one medical records were analyzed. The mean age was 69.2±8.1 years and 48 (70.4%) patients were female. There was a higher prevalence of trauma-orthopedic diseases (60.6%). The greatest number of admissions was in winter (62.0%). At the time of referral, there were 17 cases (63.0%) of trauma-orthopaedic diseases in summer and 26 (59.1%) in winter; 3 cases (11.1%) of rheumatologic diseases in summer and 10 (22.7%) in winter; 5 cases (18.5%) of neurologic disorders in summer and 4 (9.1%) in winter; and 2 cases (7.4%) of postoperative complications in summer and 4 (9.1%) in winter (p=0.64). CONCLUSIONS: There was a greater demand for referrals of elderly patients to the physical therapy center in winter than in summer, with a predominance of female patients in both seasons. The most prevalent causes of referral were related to trauma-orthopaedic diseases, but there was no significant difference in the frequency of diagnoses according to season. Knowledge about the profile of the elderly admitted to physical therapy centers is important for targeting actions at this age group, conducting information, prevention, and health promotion campaigns.

RESUMO Racional: A hepatectomia parcial é intervenção cirúrgica que pode desencadear processo regenerativo, onde os lobos residuais deflagram resposta de hiperplasia compensatória, ocasionando restauração próxima ao seu volume original. Contudo, dependendo da extensão das lesões hepáticas a regeneração pode ser prejudicada. O laser de baixa potência tem sido pesquisado com resultados benéficos no processo de regeneração hepática. Objetivo: Investigar os possíveis danos funcionais e mutagênicos decorrentes da utilização do laser de baixa potência utilizado na regeneração hepática após hepatectomia parcial. Métodos: Quinze ratos adultos Wistar foram hepatectomizados a 70%, irradiados ou não com laser, dose de 70 J/cm2, 650 nm,100 mW, de forma direta sobre o fígado remanescente, durante o período transoperatório. Os animais foram distribuídos em quatro grupos: G1 (controle, 7 dias); G2 (laser, 7 dias); G3 (controle 14 dias); G4 (laser,14 dias). Foram analisados o peso do fígado; número de hepatócitos; deposição de fibras colágenas; teste de função hepática: alanina aminotransferase, aspartato aminotransferase, fosfatase alcalina, gama glutamiltransferase, bilirrubinas e teste de micronúcleo em eritrócitos. Resultados: O peso do fígado apresentou-se aumentado nos grupos G3 e G4 (p=0,001 e p=0,002) comparados aos demais grupos. A deposição das fibras colágenas no G1 foi estatisticamente maior em relação aos demais grupos (p=0,01). Nos testes de função hepática e teste de micronúcleo não foram encontradas diferenças significativas entre os grupos. Conclusão: O laser de baixa potência não ocasionou perda de função hepática ou dano mutagênico.

BACKGROUND: The neurobiology of bipolar disorder is not completely understood. Cytokines have received increasing attention as potential mediators of the interaction with immune, neuroendocrine system and specific pathways involved in mood, energy, and activity control. Previous reports have suggested the association of mania and bipolar depression with a proinflammatory state. However, they did not compare cytokine levels in all phases of bipolar disorder. METHODS: Sixty-one bipolar patients were recruited for assessment of serum cytokine levels. Of these, 14 were in euthymic state, 23 and 24 were in manic and depressive episodes, respectively. A healthy comparison group included 25 healthy volunteers. Cytokines involved in Th1/Th2 balance, such as TNF-alpha, IL-2, IL-4, IL-6, IL-10, IFN-gamma, were examined by flow cytometry. RESULTS: During mania, proinflammatory cytokines, IL-2, IL-4 and IL-6, were increased in comparison with healthy subjects. Patients in depressive episode showed only increased IL-6 levels. There were no significant differences in cytokine levels between patients in remission and healthy subjects, except for IL-4. Mood symptoms showed a positive correlation with IL-6 and IL-2. DISCUSSION: These findings suggest that mania, and to a less extent, depression are associated with a proinflammatory state. These changes seem to be related to mood state, as changes in cytokine profile were more pronounced during acute episodes than in euthymia. This study provides further support to investigate the immune system as a target for future treatment development.

ABSTRACT

O objetivo do presente estudo foi traçar o perfil dos usuários de substâncias psicoativas atendidos em ambulatório de dependência química. Trata-se de estudo observacional transversal realizado com usuários de substâncias psicoativas atendidos no Ambulatório de Transtorno Aditivo com Ênfase em Dependência Química da Cruz Vermelha Brasileira, filial do Estado do Rio Grande do Sul, abrangendo o período de 2005 a 2010. A coleta de dados ocorreu através da análise dos prontuários de pacientes (n=1.469), com levantamento de dados demográficos, socioeconômicos e de uso de drogas. Na análise estatística, foi utilizada frequência, média, desvio padrão e teste do ?2, com um nível de significância de p<0,05. O perfil predominante dos usuários de substâncias psicoativas foi de jovens, na faixa etária de 21 a 30 anos, sexo masculino, solteiros, cor branca, com baixo nível de escolaridade, baixa condição socioeconômica e sem vínculos empregatícios. Encontrou-se maior consumo de tabaco (86,0%) entre as substâncias lícitas, e de crack (83,3%) entre as ilícitas. O uso de tabaco, maconha e inalantes demonstrou-se significativamente associado aos jovens. Os resultados contribuem para a elaboração de programas de intervenção, que abordem uma articulação multidisciplinar, numa ofensiva contra a problemática do uso indevido de drogas.


El objetivo de este estudio fue determinar el perfil de los usuarios de sustancias psicoactivas atendidos en consultas externas dependencia de sustancias químicas. Este es un estudio observacional transversal realizado con los usuarios de sustancias psicoactivas tratadas en el Ambulatorio de trastorno aditivo con énfasis en dependencia de sustancias químicas de Cruz Roja Brasileña, una filial del Estado de Río Grande do Sul, que abarca el período de 2005 a 2010. La recogida de datos se ha producido a través del análisis de los registros médicos de los pacientes (n=1.469), estudio de datos demográficos y socioeconómicos y el uso de drogas. En el análisis estadístico, se utilizan frecuencias, media, desviación estándar, y con el test de chi-cuadrado, con un nivel de significación de p<0,05. El perfil predominante de los usuarios de sustancias psicoactivas era joven, en el rango de edad de 21 a 30 años de edad, de sexo masculino, soltero, blanco, con bajo nivel de educación, el estado socioeconómico bajo y sin las relaciones de empleo. Encontramos que un mayor consumo de tabaco (86,0%) entre lo lícito y crack (83,3%) entre los ilícitos. El uso del tabaco, la marihuana y los inhalantes ha demostrado ser significativamente asociada con los jóvenes. Los resultados obtenidos contribuyen al desarrollo de los programas de intervención multidisciplinar que se dirigen a un link, una embestida contra el problema del uso indebido de drogas.


The objective of the present study was to outline the profile of psychoactive substance users treated at a clinic for chemical dependence. This is a transversal observational study developed with users of psychoactive substances treated at the Clinic for Addictive Disorder with Emphasis on Chemical Dependence of the Brazilian Red Cross, State of Rio Grande do Sul branch, from 2005 to 2010. Data collection was developed through the analysis of the patients? records (n=1,469), based on surveys on demographic, socio-economic and drug use data. Frequency, average, standard deviation and ?2 test were used in the statistical analysis, with a significance level of p<0.05. The predominant profile of psychoactive substance users was: young, in the age range between 21 and 30 years, male gender, single, Caucasian, low school and socioeconomic levels, as well as unemployed. Among the licit drugs, the main one was tobacco (86.0%); and for illicit drugs, the main one was crack (83.3%). The use of tobacco, marijuana and inhaled drugs has shown to be significantly related toyounger people. The results contribute for the development of intervention programs approaching a multidisciplinary articulation, as an offensive against the problem of drugs misuse.

ABSTRACT

The Bipolar Mood Disorder is characterized by the alternation of depressive crises with episodes of mania or euphoria, having these patients 15 to 35 times more chances of suicide, as compared with people without this disorder. The pharmacotherapy is fundamental for this disease, aiming to decrease the frequency of episodes and disease severity. In these patients, the polypharmacy has recently increased and one of the main difficulties is the adherence to treatment. The objective of this study was to contribute for the improvement of bipolar patients health conditions, developing their respective pharmacotherapeutic follow-up. Twenty eight adult bipolar patients who were participants of a specialized clinic within a tertiary hospital in Porto Alegre have been randomly selected, and the Dader Method of pharmacotherapeutic follow-up has been applied. The more common clinical comorbidities were: hypertension (50 percent), obesity (46.43 percent), and hypothyroidism (36.29 percent). The bipolar patients are more susceptible to clinical comorbidities, and many of them could be due to pharmacotherapy. Only 1.43 percent of patients presented Drug Related Problems, being all of them resolved along the study. It was also observed that 32.14 percent of evaluated patients presented low adherence to treatment, and between these patients, 55.56 percent passed to have good adherence after pharmacotherapeutic follow-up. The pharmacotherapeutic follow-up is fundamental for the improvement of patient's health. New studies, with higher number of patients and longer duration, are necessary to evaluate the percentage of patients that could be beneficiary of Pharmaceutical Care.


O Transtorno do Humor Bipolar é caracterizando pela alternância de crises depressivas com episódios de mania ou euforia, tendo estes pacientes 15-35 vezes mais chances de suicídio em comparação com pessoas sem este transtorno. A farmacoterapia é fundamental, visando diminuir a freqüência dos episódios e a gravidade da doença. Nestes pacientes, a polifarmácia tem aumentado ultimamente e uma das maiores dificuldades é a adesão ao tratamento. O objetivo do estudo foi contribuir para a melhoria das condições de saúde de pacientes bipolares, realizando o acompanhamento farmacoterapêutico dos mesmos. Foram selecionados aleatoriamente 28 pacientes bipolares adultos, participantes de um ambulatório especializado de um hospital terciário em Porto Alegre e aplicado o Método Dáder de acompanhamento farmacoterapêutico. As co-morbidades clínicas mais comuns foram: hipertensão (50 por cento), obesidade (46,43 por cento) e hipotiroidismo (39,29 por cento). Os pacientes bipolares são mais suscetíveis a co-morbidades clínicas e muitas destas podem ser devidas a farmacoterapia. Apenas 1,43 por cento dos pacientes apresentavam Problemas Relacionados a Medicamentos, sendo todos solucionados no decorrer do estudo. Também se observou que 32,14 por cento dos pacientes avaliados apresentavam baixa adesão ao tratamento e entre estes, 55,56 por cento passaram a ter boa adesão após o acompanhamento farmacoterapêutico. É fundamental para a melhoria da saúde do paciente o acompanhamento farmacoterapêutico. Novos estudos, com maior número de pacientes e maior duração, são necessários para avaliar o percentual de pacientes que poderão ser beneficiados pela Atenção Farmacêutica.

ABSTRACT

A natação contribui para a saúde física, mental e social das pessoas que apresentam alguma deficiência. Sabendo que o aspecto nutricional é relevante para o desempenho esportivo e qualidade de vida desses indivíduos, o objetivo deste estudo foi avaliar o estado nutricional de pessoas com deficiências que praticam natação. Quatro indivíduos do sexo masculino e quatro do feminino, com deficiências físicas ou visuais, idade entre 19 e 36 anos, que nadavam de duas a três vezes por semana, participaram do protocolo de avaliação. Foram realizadas avaliação antropométrica, avaliação da ingestão alimentar através de recordatório de três dias e avaliação bioquímica para análise do perfil lipídico (triglicerídeos, colesterol total e frações). O IMC médio encontrado foi de 24,15 ± 2,33kg/m² e 27,26 ± 4,91kg/m² para homens e mulheres, respectivamente. O consumo alimentar médio encontrado, para ambos os sexos, foi de 3161,33 + 480,08kcal, tendo composição média de 58,13 ± 5,04 por cento de carboidratos, 19,62 + 1,54 por cento de proteínas e 22,45 ± 5,64 por cento de lipídios do valor energético total (VET). As mulheres apresentaram consumo adequado de proteínas; entretanto os homens consumiam além das suas necessidades (2,14 + 0,34g/kg/dia). Para os demais nutrientes, o consumo foi adequado em ambos os sexos. As análises bioquímicas demonstraram, em geral, perfil lipídico adequado, com tendência a baixos valores de HDL. Conclui-se que os indivíduos do sexo masculino apresentaram composição corporal e perfil lipídico adequados, mesmo com consumo excedente de calorias e proteínas. Já no sexo feminino, verificou-se risco nutricional, pois as participantes apresentavam sobrepeso. Evidencia-se que esse grupo necessita de orientação nutricional individualizada para corrigir as inadequações alimentares observadas e, assim, melhorar seu desempenho físico e qualidade de vida.


Swimming contributes to physical, mental and social health of people who have some kind of disability. Bearing in mind that the nutritional aspect is relevant to sports performance as well as to life quality of these individuals, this study has aimed at evaluating the nutritional status of disabled individuals who practice swimming. Four male and four female individuals aged between 19 and 36 years who have either physical or visual impairment, as well as swim two or three weekly times, participated in the assessment protocol. Anthropometric analysis and food intake evaluation were used through three-day-recording and biochemical evaluation for the analysis of the lipid profile (triglycerides, total and fraction cholesterol). Mean BMI found was 24.15 ± 2.33 kg/m2 and 27.26 ± 4.91kg/m2 for men and women, respectively. The average food intake observed for both sexes was 3161.33 ± 480.08 kcal, having mean composition of 58.13 ± 5.04 percent of carbohydrates, 19.62 ± 1.54 percent of protein and 22.45 ± 5.64 percent of lipids from the total energetic value (TEV). Women presented adequate intake of protein, while men ingested it beyond their needs (2.14 ± 0.34 g/kg/day). Concerning the other nutrients, intake was adequate for both sexes. The biochemical analyses showed adequate lipid profile in general, with tendency for low HDL levels. It was concluded that the male individuals had satisfactory body composition and lipid profile, despite excessive intake of calories and proteins, while the female individuals presented nutritional risk due to overweight. It becomes evident the later group needs individualized nutritional orientation so as to correct their inadequate eating habits and hence improve their physical performance and life quality.
