ABSTRACT

RESUMEN INTRODUCCIÓN: La COVID-19 se acompaña de eventos tromboembólicos venosos y arteriales en pacientes con historia de hipertensión y aterosclerosis. Se reporta el caso de una paciente joven y sin factores de riesgo cardiovascular que adquirió la infección por el SARS-COV2 y luego presentó un accidente cerebrovascular isquémico. CASO: Paciente de 44 años hospitalizada por la covid-19 en tratamiento protocolario con anticoagulación, esteroide y oxígeno suplementario. Al día 7 del inicio de los síntomas respiratorios presentó infarto cerebral por trombosis de arteria carótida derecha, arteria cerebral media derecha e izquierda y edema cerebral hemisférico derecho, tras lo cual fue diagnosticada con muerte encefálica. El estudio vascular no documentó una etiología diferente a la covid-19. DISCUSIÓN: Se plantea que la infección condiciona un estado inflamatorio sistémico, endotelialitis, ulceración de placas ateroscleróticas preexistentes e hipercoagulabilidad. Lo anterior facilita la aparición de trombosis vascular. Es frecuente encontrar elevación de reactantes de fase aguda y de los productos del metabolismo de la fibrina. El accidente cerebrovascular (ACV) isquémico se ha descrito en el 1 al 5 % de los pacientes con la covid-19. En pacientes con factores de riesgo cardiovascular, durante las primeras 2 semanas desde el inicio de los síntomas respiratorios, el compromiso isquémico suele afectar a múltiples territorios vasculares. CONCLUSIÓN: La covid-19 por sí sola es un factor de riesgo para trombosis arterial secundaria a la inflamación sistémica y estados de hipercoagulabilidad, por lo que debe ser tenida en cuenta en el estudio de ACV isquémico en pacientes jóvenes.


ABSTRACT INTRODUCTION: It is known that COVID-19 is associated with venous and arterial thromboembolic events in patients with history of hypertension and atherosclerosis. Herein, it is presented a case of a young woman without known cardiovascular risk, who was infected by SARS-COV 2 and then an ischemic stroke. CASE: 44 years-old woman with moderate COVID-19 in protocolary treatment with anticoagulants, steroid and oxygen. In the seventh day since symptoms onset, she presented acute ischemic stroke due to acute thrombosis of right carotid artery, and right and left medial cerebral arteries, then rapidly developed malignant cerebral edema and brain death. Vascular study failed to report and etiology other than COVID-19. DISCUSSION: It is suggested that the infection produces a systemic inflammatory state, endothelialitis, ulceration of preexisting atherosclerotic plaques, and hypercoagulability that facilitate thrombosis. Ischemic stroke is described in 1 to 5% of COVID-19, it is common to find an elevation of acute phase reactants and products of fibrin metabolism. Multiple vascular territories are usually compromised in patients with known cardiovascular risk factors and 12 days after the onset of symptoms. CONCLUSION: COVID-19 by itself is a risk factor for arterial thrombosis secondary to systemic infection and hypercoagulability states and it should be taken into account in the study of ischemic stroke in young patients.

ABSTRACT

The prevalence of insulin resistance and risk factors for chronic diseases is not known in Colombia. The purpose of the study was of determine the association of insulin resistance and risk of chronic diseases in young, apparently healthy adults. By convenience, 97 subjects, mean age of 24 years were studied. Family and personal history, anthropometrics, lipid profile, and a short insulin tolerance test was done to each subject to identify prevalence of insulin resistance and its association with risk factors. Plasma lipids and anthropometrics were within acceptable range. Insulin sensitivity was categorized as high, border high, border low and low. Out of 97 subjects 47 had altered sensitivity. Most women (68%) were classified as border low, and most men (60%) as low. There was a difference between these two categories in waist circumference and weight. Border low and low cases had not less than 3 risk factors but the total number of factors was not different within the 4 groups. More than 50% of cases had family history of chronic diseases, sedentary life and low C-HDL. There was a negative association between insulin sensitivity and fasting Glycaemia and positive with fasting triglycerides, BMI, Waist hip ratio and weight. Results suggest a surprisingly high prevalence of risk factors in a young group associated with altered insulin sensitivity. It may be possible to identify early indicators of risk and develop appropriate strategies for prevention.

ABSTRACT

El cáncer de pene tiene una incidencia de 1.11 por 100.000 habitantes en Colombia, representado en el 95% por carcinoma de células escamosas el cual representa una alta morbilidad y mortalidad, La Sociedad Colombiana de Urología realizo la adaptación de la guía de cáncer de pene para el año 2021 con revisión de la literatura, esta guía permite realizar una evaluación y tratamiento del cáncer de pene, enmuchas ocasiones con el objetivo de preservación de órgano utilizando la ecografía como determinante para evaluar el compromiso de la lesión a las estructuras del pene, los estudios de extensión dependerán de hallazgos al examen físico como ganglios o compromiso local de la enfermedad, de acuerdo al tipo de lesión y su estadificación puede recibir terapia con agentes tópicos, radioterapia, cirugía láser, cirugía preservadora de órgano o penectomía total; La linfadenectomía inguinal permitirá mejorar la supervivencia en tumores de riesgo intermedio y alto (>pT1G2), posteriormente la quimioterapia adyuvante esta en el grupo de pacientes en el cual la intención sea curativa, aunque en terapia paliativa de segunda línea se encuentran los platinos y texanos como alternativa pero con pobre respuesta (<30%); los esquemas de seguimiento se realiza de acuerdo al estadio y el compromiso ganglionar. Al final se presentan los resultados de actividad sexual después de cirugía preservadora de órgano; Esta guía abarca la literatura actualizada del cáncer de pene, el cual es útil para el manejo por parte de los profesionales de salud del país.

At the end, the results of sexual activity after organ-sparing surgery are presented; This guide covers the updated literature on penile cancer, which is useful for the management of health professionals in the country.

ABSTRACT

Las quemaduras extensas pueden generar gran morbilidad y altas tasas de mortalidad determinadas tanto por las lesiones mismas como por la calidad de la reanimación inicial. Si esta es adecuada, puede prevenir la evolución hacia una mayor profundidad de la lesión primaria y, por ende, mejorar el pronóstico del paciente. El objetivo de esta revisión fue recopilar información que permita optimizar la atención del paciente quemado, con miras a obtener una disminución de la mortalidad y de las secuelas y complicaciones derivadas de una inadecuada reanimación. Se hizo una búsqueda en Internet, utilizando la base de datos Pubmed. Se usaron las palabras clave “Resuscitation” y “Burn”, y se limitó la búsqueda a los artículos publicados después del año 2000. Se incluyeron además algunos artículos considerados de importancia para la comprensión del tema, publicados con anterioridad a ese año.


Extensive burns can cause significant morbidity and high mortality, determined by both the injuries themselves and the quality of the initial resuscitation measures applied to the patient. If adequate, such measures can prevent the worsening of the burn and, consequently, they can improve the prognosis for the patient. The objective of this review was to compileinformation that may help resuscitation teams to improve the care of burned patients, looking for lower mortality and less frequent sequels and complications. The Pubmed database was reviewed searching for studies made on the subject from 2000 to 2007, using the key words “Resuscitation” and “Burn”. Some articles published before 2000 were also included because they wereconsidered relevant for understanding this subject.
