ABSTRACT

Values were determined by means of a ligand binding assay using 17-FE (17-fluoresceinated estrone) in 53 breast cancer patients. 49.1% of the tumors were positive and 50.9 negative. There was no correlation between the positivity and tumor size and localisation. The proportion of positivity increased with advancing age. In the group of postmenopausal women, the incidence of positivity was higher than in the premenopausal group, although the difference was not statistically significant. Histopathologically, tumors with high and intermediate grades of differentiation were often more significatively positive when compared to poorly differentiated tumors.

ABSTRACT

Fourteen cases with a malignant dysembroma of the ovary were treated by resection, postoperative radiotherapy and/or chemotherapy. Eight of the 14 patients survived 2 years and 2 out of 7 are still alive 5 years later. The prognosis depends mainly on the initial stage of the disease. Radiotherapy but most of all chemotherapy may improve the survival rate even in the advanced forms of the disease. The proposed therapy is initial extensive surgery followed by chemotherapy in all cases. Also radiotherapy in those cases in which the resection was considered incomplete.

ABSTRACT

Our study groups 7 patients undergoing chest wall resection and 12 patients submitted to en bloc lung and chest wall resection. In the first group, 48 +/- 15 years old, there were 3 females (Darier-Ferrand fibrosis, chondroma, cutaneous gland epithelioma) and 5 males (3 sarcomas, 1 metastasis and 1 osteofibroma). An average of 2 ribs were resected with muscles, pleura and in 3 cases the skin, amounting to between 300 and 1,000 g. Reconstruction was performed : 1 degrees by musculocutaneous flap in 4 cases with 1 fistula; 2 degrees Marlex Mesh in 2 cases with 1 infection; 3 degrees fascia lata in the largest resection that necessitated tracheostomy and prolonged ventilation. There was no death. In the second group, 61 +/- 14 years old, all males, there were 9 bronchial carcinomas and 3 sarcomas. An average 2.5 ribs were resected en bloc with 6 lungs and 6 lobes. The defect was treated : 1. by Marlex Mesh in 3 cases without complications, 2 had tracheostomy and 2 days ventilation; 2. by muscle flap in 9 cases with 3 tracheostomies and ventilation up to 1 month : there were 3 empyemas and 2 atelectasiae. One patient died after a second operation for bronchial stump necrosis. On the whole there is no need for Marlex Mesh.

ABSTRACT

One hundred and thirty-one cases of patients operated for a parotidal tumor were reviewed. We report on 85 cases that had no previous treatment, including 68 benign and 17 malignant tumors. The surgical treatment applied is analysed according to the benign or malignant nature of the tumor and to the results. For benign mixed tumors (43 cases) there were 29 total resections, 12 superficial lobectomies and 2 simple enucleations (intraparotidal location unforseen). For tumors of Warthin (19 cases) the treatment was a superficial lobectomy in all cases. For malignant tumors (17 cases) 5 superficial lobectomies and 12 total resections of the gland were performed. In the latter, the facial nerve was severed in 6 patients and left intact in the other 6. The minimal operation realizing the resection of a parotidal tumor, is the superficial lobectomy realized in once, the deep lobe only being resected if invaded. Preservation of the facial nerve should always be tried, sacrificing it only in cases of infiltrating malignant tumor.

ABSTRACT

A retrospective analysis of 95 treated nonseminomatous germinal testicular tumors is made. The primary treatment consisted of orchidectomy with high ligation of the cord. A histological study disclosed 53 embryonal cell carcinomas (56%), 21 chorioepithelionas (22%), 7 teratocarcinomas (7%) and 14 complex composite tumors which were predominantly dysembryomatous (15%). Forty-three patients were classified as stage I, 13 as stage II and 39 as stage III. Retroperitoneal pelvic and peri-aortic lymphadenectomy performed in patients with a testicular dysembryoma classified as stage I, had a survival of 70% 5 years postoperatively versus only 46 when treated solely with radiotherapy. Stage III cases with choriocarcinoma did not survive 3 years. These results are compared with those reported in the literature. The presently applied therapeutic scheme is described.

ABSTRACT

This is a report on the pathology of the liver in two patients under continuous and prolonged oral contraceptives (11 and 8 years): the first had peliose, the other had focal nodular hyperplasia. Present etiopathogenetic theories are reminded: anabolic steroids in peliose and contraceptives in focal nodular hyperplasia. These lesions should be searched systematically in such patients at any laparotomy.

ABSTRACT

The deltopectoral flap was described by Bakamjian in 1965 for pharyngo-oesophageal reconstruction. Since then, the indications of this excellent flap have been extended and the versatility of the flap has allowed extensive skin and mucous membrane resections in cancerology. The authors have used 38 deltopectoral flaps in 36 patients with difficult reconstructive problems. The results have been good in 31 cases. There were 7 failures.

ABSTRACT

Between 1 April 1980 and 30 Septembre 1983, 412 cases of nasopharyngeal carcinoma were seen at the Bergonié Cancer Centre in Casablanca (the sole centre in the country), representing 4.76% of all malignant tumours diagnosed in that period. This percentage is low, since the majority of these tumours are not diagnosed outside the big towns but indicates that Morocco is an area of moderate incidence for this cancer. We have studied the epidemiological aspects of our series; there is a predominance of males (sex ratio, 2.25: M:F); the average age is 43 years with a large proportion of young people (24.6% are less than 30 years old). A high incidence is observed in the north of the country, where, however, more medical facilities are available. Advanced tumours are common: 15% cannot be treated; 32.5% are in stage T4, and 70% have metastatic cervical nodes. Pathologically, 90% are undifferentiated carcinomas of the nasopharyngeal type. The results of the immunological study do not exclude a viral (Epstein-Barr virus) contribution. Prospective epidemiological studies are needed to elucidate the exact profile of this cancer in our country.

ABSTRACT

The authors present a retrospective study of 383 cases of cervical cancer seen at the Bergonié Centre in Casablanca in 1971 and 1972. An epidemiological analysis of this cancer is given, as well as the results of a clinical and therapeutic study. The long-term results for this series are very satisfactory, since survival at five years for carcinomas, which are the most frequent, was 30%, despite the variety of therapeutic regimens used at the time of treatment.
