RESUMEN

Large lipid core (extended into arterial lumen) and high density of macrophages (associated with 18F-fluorodeoxyglucose "18F-FDG" uptake) in atherosclerotic plaque were shown to be an overt feature of plaque rupture. Nineteen participants were imaged with computed tomography (CT) and positron emission tomography (PET) with 18F-FDG in a dynamic mode. The mean lumen density in Hounsfield unit (HU) was measured per region of interest (ROI) on CT images and classified as non-calcified and calcified classifications. Calcified group was divided into partially calcified and calcified groups. Metabolic rate of glucose (MRG) was computed per ROI on PET dynamic images using modified 2-tissue compartmental model that is independent of partial volume effect. Data is clustered using Automatic Hierarchical K-means algorithm (AKH) with silhouette-coefficient. Arterial segments of 1180 ROIs for Aorta and iliac arteries were classified as non-calcified and calcified segments and clustered using AHK with respect to the mean of intravascular attenuation (in HU). There was a statistical difference in MRG corresponded to low intravascular attenuation cluster compared to higher intravascular attenuation clusters (P<0.05), but not within higher clusters (P>0.05), for both non-calcified and calcified classes. In partially calcified segments, same pattern was observed as the low intravascular attenuation cluster was accompanied with significant metabolic activity but not for calcified segments. Low intravascular attenuation is associated with high MRG measured on 18F-FDG PET images, which may reflect the instability of atherosclerotic plaque. Partially calcified plaque is metabolically active compared to calcified plaque.

INTRODUCTION: Awareness of screening procedures and illness warning signals is critical for expanding and implementing screening programs in society, which would improve the odds of early identification of breast cancer. OBJECTIVES: This study aimed to evaluate the knowledge, awareness, attitudes, and practices related to breast cancer risk factors, signs, symptoms and methods of screening among female faculty and students at Hail University in the Kingdom of Saudi Arabia. METHODS: A cross-sectional study was conducted from January 2021 through February 2021 in the Hail region of Saudi Arabia. A closed-ended questionnaire, which consisted of 37 questions, was distributed online (using a Google Forms link) in both English and Arabic languages. Data was collected from 425 female subjects who participated in the study. RESULTS: The study showed an overall knowledge level of 46.36% regarding breast cancer. Participants had average knowledge about risk factors, signs, and symptoms, whereas their awareness and practice of breast self-examination and screening methods were weak. CONCLUSION: The current study concluded that public awareness of breast cancer remains relatively low, and Saudi Arabia still needs several public awareness initiatives using mass media, such as television, the Internet, and radio, as well as social media. Special awareness programs should also be held in places where a large number of women can easily be reached, such as colleges, universities, and hospitals.

Arterial inflammation is an indicator of atheromatous plaque vulnerability to detach and to obstruct blood vessels in the heart or in the brain thus causing heart attack or stroke. To date, it is difficult to predict the plaque vulnerability. This study was aimed to assess the behavior of 18F-sodium fluoride (18F-NaF) and 18F-fluorodeoxyglucose (18F-FDG) uptake in the aorta and iliac arteries as a function of plaque density on CT images. We report metabolically active artery plaques associated to inflammation in the absence of calcification. 18 elderly volunteers were recruited and imaged with computed tomography (CT) and positron emission tomography (PET) with 18F-NaF and 18F-FDG. A total of 1338 arterial segments were analyzed, 766 were non-calcified and 572 had calcifications. For both 18F-NaF and 18F-FDG, the mean SUV values were found statistically significantly different between non-calcified and calcified artery segments. Clustering CT non-calcified segments, excluding blood, resulted in two clusters C1 and C2 with a mean density of 30.63 ± 5.06 HU in C1 and 43.06 ± 4.71 HU in C2 (P < 0.05), and their respective SUV were found statistically different in 18F-NaF and 18F-FDG. The 18F-NaF images showed plaques not detected on CT images, where the 18F-FDG SUV values were high in comparison to artery walls without plaques. The density on CT images alone corresponding to these plaques could be further investigated to see whether it can be an indicator of the active plaques.

Glucose metabolism in atherosclerotic arteries has been shown to be an indicator of inflammation, which might be a precursor of plaque rupture. In this prospective study, we assessed the correlation between artery calcification and glucose metabolism by means of 18F-FDG PET/CT imaging in elderly subjects. Nineteen elderly subjects, with age ranging from 65 to 85 years, underwent CT and dynamic 18F-FDG-PET imaging. The artery calcification was determined with a threshold of 130 Hounsfield units. Intensity of calcification and ratio of calcification area to total artery area were classified in four sequential classes from CT images. The CT artery images were also classified as having single or multi-spot calcifications. Their respective glucose metabolism was assessed with fractional uptake rate (FUR). Factor analysis was used in this study to separate blood images from tissue to extract the blood time activity curves for FUR calculations. The artery images in PET data were corrected for partial volume effect. The total arterial segments analyzed were 1332, with 1085 without calcification (81%), 247 (19%) with calcification, and 94 segments were having multi-spot of calcifications. There was a statistically significant difference in FUR values between non-calcified to calcified segments and between subjects under medication to non-medication when comparing the subjects based on calcification area. No statistically significant differences of FUR were found between single spot as a function of intensity, while in the multi-spots, there was a statistically significant difference for all artery segments. Metabolism activity varies for non-calcified to calcified segments. Based on the metabolic activity represented by FUR, calcifications in multi-spots have different effects than in single spots.

Alanine dosimeters in the form of pure alanine powder in PMMA plastic tubes were investigated for dosimetry in a clinical application. Electron paramagnetic resonance (EPR) spectroscopy was used to measure absorbed radiation doses by detection of signals from radicals generated in irradiated alanine. The measurements were performed for low-dose ranges typical for single-fraction doses often used in external photon beam radiotherapy. First, the dosimeters were irradiated in a solid water phantom to establish calibration curves in the dose range from 0.3 to 3 Gy for 6 and 18 MV X-ray beams from a clinical linear accelerator. Next, the dosimeters were placed at various locations in an anthropomorphic pelvic phantom to measure the dose delivery of a conventional four-field box technique treatment plan to the pelvis. Finally, the doses measured with alanine dosimeters were compared against the doses calculated with a commercial treatment planning system (TPS). The results showed that the alanine dosimeters have a highly sensitive dose response with good linearity and no energy dependence in the dose range and photon beams used in this work. Also, a fairly good agreement was found between the in-phantom dose measurements with alanine dosimeters and the TPS dose calculations. The mean value of the ratios of measured to calculated dose values was found to be near unity. The measured points in the in-field region passed dose-difference acceptance criterion of 3% and those in the penumbral region passed distance-to-agreement acceptance criterion of 3 mm. These findings suggest that the pure alanine powder in PMMA tube dosimeter is a suitable option for dosimetry of radiotherapy photon beams.
