RESUMEN

This case involved a 69-year-old female patient with peritoneal dissemination of an advanced gastric cancer. She underwent chemotherapy comprisingintravenous and intraperitoneal paclitaxel combined with S-1. After 20 courses, a staging laparoscopy was performed, and pathological analysis of the peritoneal dissemination and cytologic analysis of ascites fluid yielded negative results. A radical robot-assisted total gastrectomy was successfully performed. The pathological stage was determined to be ypT4aN2M0, ypStage ⅢB. We continued to administer the same chemotherapy regimen for 15 courses (total: 35 courses)after surgery. No recurrence has been detected during the 1-year period after surgery.

RESUMEN

A 60-year-old male was admitted to our hospital on account of a duodenal submucosal tumor on the opposite side of Vater's papilla identified on a screening gastroduodenoscopy. The tumor was diagnosed as a duodenal gastrointestinal stromal tumor(GIST)due to positive c-kit and DOG1tests. Gastroduodenography and enhanced computed tomography showed a 50mm tumor widely invadingthe descendingportion of the duodenum close to the papilla of Vater and indicated the need for extended resection includingpancreaticoduodenectomy to achieve curation. Preoperative chemotherapy was initiated with imatinib mesylate(imatinib)to preserve the pancreas. A 16 month course of imatinib shrunk the tumor up to 21mm and laparoscopic partial duodenectomy was performed as a curative resection. So far, no local tumor recurrence has been seen for 26 months after the surgery. Herein, we report this case with a review of the literature.

RESUMEN

A 76-year-old woman, complained of weight loss, was admitted in our hospital. After investigation, she was diagnosed with clinical Stage III B gastric cancer with invasion into the pancreas and transverse colon. After performing the staging laparoscopy and gastrojejunostomy for pyloric stenosis, she was treated with 2 courses of SOX therapy as neoadjuvant therapy, and subsequently underwent pancreaticoduodenectomy and right hemicolectomy combined with portal vein resection due to severe adhesion by the tumor. The pathological diagnosis was pT4b(colon), pN0(0/22), pM0, pStage III B. There were no findings of tumor invasion into the pancreas or the portal vein. She was discharged without any complications. She did not receive the adjuvant chemotherapy, and died of other illness at 10 months after surgery. For the locally advanced gastric cancer such as invasion to the pancreas, the extended resection as an R0 resection could be achieved more safely, by evaluating accurately the incurable factors and planning an effective strategy according to the patient's condition.

Islet transplantation is an effective treatment for type 1 diabetes (T1D). However, a shortage of donors and the need for immunosuppressants are major issues. The ideal solution is to develop a source of insulin-secreting cells and an immunoprotective method. No bioartificial pancreas (BAP) devices currently meet all of the functions of long-term glycemic control, islet survival, immunoprotection, discordant xenotransplantation feasibility, and biocompatibility. We developed a device in which porcine islets were encapsulated in a highly stable and permeable hydrogel and a biocompatible immunoisolation membrane. Discordant xenotransplantation of the device into diabetic mice improved glycemic control for more than 200 days. Glycemic control was also improved in new diabetic mice "relay-transplanted" with the device after its retrieval. The easily retrieved devices exhibited almost no adhesion or fibrosis and showed sustained insulin secretion even after the two xenotransplantations. This device has the potential to be a useful BAP for T1D.

Cross-linked polymers prepared by the in-situ polymerization of liquid-crystalline salts were found to work as solid-state hosts with a flexible framework. As a component of such hosts, four kinds of polymerizable amphiphilic carboxylic acids bearing alkyl chains with acryloyloxy (A), dienyl (D), and/or nonreactive (N) chain ends (monomeric carboxylic acids; M(AAA), M(ANA), M(DDD), and M(DND)) were used. The carboxylic acids were mixed with an equimolar amount of a template unit, (1R,2S)-norephedrine (guest amine; G(RS)), to form the corresponding salts. Every salt exhibited a rectangular columnar LC phase at room temperature, which was successfully polymerized by (60)Co γ-ray-induced polymerization without serious structural disordering to afford the salt of cross-linked carboxylic acid (polymeric carboxylic acid; P(AAA), P(ANA), P(DDD), and P(DND)) with G(RS) . Owing to the noncovalency of the interactions between the polymer framework P and the template G(RS), the cross-linked polymers could reversibly release and capture a meaningful amount of G(RS). In response to the desorption and adsorption of G(RS), the cross-linked polymers dramatically switched their nanoscale structural order. A systematic comparison of the polymers revealed that the choice of polymerizable groups has a significant influence on the properties of the resultant polymer frameworks as solid-state hosts. Among these polymers, P(DDD) was found to be an excellent solid-state host, in terms of guest-releasing/capturing ability, guest-recognition ability, durability to repetitive usage, and unique structural switching mode.

The salts of trialkoxybenzoic acids and 2-amino alcohols showed a columnar liquid crystalline phase; in the case of the salt of a polymerizable acid and norephedrine, the photopolymerization proceeded efficiently in the liquid crystalline state, and the resultant solid adsorbed 2-amino alcohols size, regio-, and enantio-selectively.

The cross-linking of multicomponent liquid crystals could be applied to the synthesis of nanometer-sized porous materials with a well-defined structure. In this work we demonstrate the template polymerization of columnar liquid crystals composed of the salts of a carboxylic acid and enantiopure 2-amino alcohols, and the application of one of them as a solid-state host. The salts of 3,4,5-tris(11-acryloyloxyundecyloxy)benzoic acid with (S)-2-amino-1-propanol and with (1R,2S)-norephedrine showed hexagonal and rectangular columnar liquid-crystalline structures, respectively. The successful application of gamma-ray-induced polymerization to the cross-linking of the liquid-crystalline salts, which was more advantageous than photoinduced polymerization from the standpoint of the retention of the original structural order in the gram-scale preparation of the polymers with a homogeneous columnar structure. The cross-linked polymer thus obtained from the gallic acid derivative and (1R,2S)-norephedrine was applicable as a heterogeneous host to capture amines from a guest solution through acid-amine interactions. When (1R,2S)-norephedrine was replaced with other amines through the guest-exchange reaction, a "template effect" was observed; the size and shape of the guests were determining factors for the efficiency of the guest exchange. The guest adsorption was found to proceed in an enantioselective manner when racemic 2-amino alcohols were used as guests, especially in the cases of substrates possessing a bulky substituent at the C1-position. The guest preference was again elucidated by the template effect, although the enantioselection mode was switched depending on the presence of a C2 substituent.

A cross-linked polymer, prepared by the in situ polymerization of a thermotropic columnar liquid crystal, was found to work as a host with a flexible framework, which was reminiscent of intercalation hosts, such as clays, graphites, and coordination polymers. The structural order of the cross-linked polymer was reversibly switched by changing the amount or shape of a guest incorporated in the polymer.
