RESUMEN

Right atrial varices are rare. They were described for the first time by anatomo-pathologists at the end of the 19th century and beginning of the 20th century. They are situated in the lower part of the inter-atrial septum and rarely exceed 2 cm in diameter. Descriptions have been from post-mortem studies which have led to epidemiological analyses and have given rise to nosological controversies. The authors report a case characterised by the exceptional volume of the varices. This could have enabled the diagnosis to be suspected at transoesophageal echocardiography before surgery. Thoracic CT scan and MRI completed the iconography. In the literature, two other cases of cardiac varices diagnosed at echocardiography have been published: they were small tumours on the lower part of the interatrial septum and the diagnosis before surgery was that of a myxoma. These formations seem to correspond to chance findings and do not appear to give rise to symptoms.

RESUMEN

Phentolamine (Regitine) at the dose of 0.3 mg/mn behaves as an arterial and, above all venous, vasodilatator agent, resulting in a marked and early lowering of the pulmonary pressures in acute oedema of the lung and in cardiac asthma. It was used alone in 47 attacks of acute severe left ventricular failure with very favourable results in 43 cases, as proved by the rapid improvement of the haemodynamic status and of the aicd-base balance. Under strict observation, tolerance has been excellent. This therapeutic method seems of great interest in the cases of acute pulmonary oedema with a maintained blood pressure level, and in the forms with severe arterial hypertension which might tolerate larger doses.

RESUMEN

Two days after an elective Caesarean section, a 30-year-old woman suffered a pulmonary oedema. The diagnosis of cardiomyopathy was obtained with echocardiography, right heart catheterization and a myocardial biopsy. Peripartum cardiomyopathy is defined as a heart failure occurring in the last trimester of pregnancy or up to the third postpartum month and the lack of another etiology. The cause of myocarditis resulting in peripartum cardiomyopathy remains yet unknown. An immunosuppressive therapy can be associated with the treatment of the congestive heart failure.

In this paper, the authors present a case of phyllodes tumor whose original interest lies in its association with an in situ lobular carcinoma. The authors present the clinical, radiological, and anatomopathological characteristics of these cases. Finally, they discuss the therapeutic management in light of data from the literature.

RESUMEN

A case of treatment of an advanced ovarian adenocarcinoma (Stage IIIc of the FIGO) is reported. The patient underwent a maximal tumor reduction followed with polychemotherapy. After this treatment, a "second look" laparotomy shows an incomplete remission. An intensive chemotherapy with abdomini-pelvic radiotherapy and protection of medullary toxicity by a bone marrow autograft is performed, with a satisfactory result, without relapse, 16 months after this treatment (or 25 months after the first laparotomy). A review of the literature concerning the treatment to be undertaken following the "second look" operation, is briefly exposed. Reasons which led the authors to perform an autograft and mostly the conditions under which this treatment may be offered, are then presented.
