RESUMEN

A case of polyadenopathic syndrome is presented which involved practically all the ganglionar territories and hepatosplenomegaly in a 15 year old male without a relevant previous history, non immunedepressed, without drug addiction or having received blood er blood derivatives. The clinical picture caused by a cytomegalovirus infection has persisted for 24 months with no modification of adenopathies. The inclusion of CMV infections in the differential diagnosis of generalized adenopathic syndromes is proposed.

RESUMEN

Acute myelopathy includes a group of diseases with an important associated morbidity, thus early diagnosis and treatment is important. The most frequent etiology is extramedullary compression. Magnetic resonance (MR) is the most suitable procedure in this type of disease since it also offers information on extramedullar lesions and makes it possible to assess spine involvement. We present the case of a 57 year-old man who was admitted because of back pain for several weeks and systemic infection due to S. aureus. Later, he suffered a sudden neurological deficit with spine involvement but without compressive images on the MRI. We discuss the differential diagnosis among intramedullary abscess, epidural abscess, acute transverse myelitis and spondylodiscitis associated to acute myelopathy. We carry out a brief review of the medical literature on diagnosis criteria of those entities.

RESUMEN

A clinical case of Mediterranean Boutonneuse Fever (MBF) with circulating cryoglobulins during the acute phase, with no clinical symptoms is described and considered an epiphenomenon of the infection. The possible relationship between the infection and the cryoglobulinemia are discussed. The patient is also added to the increasing list of patients reported of Boutonneuse Fever in the Mediterranean area during the observation period.
