RESUMEN

Paediatric biliary tract and gallbladder diseases include a variety of entities with a wide range of clinical presentations. Cholestasis represents an impaired secretion of bilirubin by hepatocytes, manifesting with high blood levels of conjugated bilirubin and jaundice. Various causes may be involved, which can be recognised analysing blood tests and hepatobiliary imaging, while sometimes liver biopsy or surgery may be necessary. High-resolution real-time ultrasonography is an important tool for differentiation of obstructive and non-obstructive causes of jaundice in infants and children. In this paper, we briefly review the normal anatomy and the ultrasound aspects of main pathologies affecting gallbladder and biliary tree in neonatal and paediatric age.

RESUMEN

Ultrasound scan is a painless and radiation-free imaging modality and, therefore, it is widely considered the first-choice diagnostic tool in the setting of hepatopathies in paediatric patients. This article focuses on the normal ultrasound anatomy of the liver in neonatal and paediatric age and reviews the ultrasound appearance of the most common diffuse and focal liver affections.

RESUMEN

The pancreas is easily investigated in children thanks to the relative lack of fat tissue and the large left hepatic lobe with an optimal acoustic window. The use of high frequency, even linear transducers, usually results in detailed images of all pancreatic areas. A wide spectrum of pancreatic pathologic conditions can be identified and monitored at ultrasound although they are relatively uncommon during childhood compared to the adult. In this paper we briefly review the anatomy, technique, and sonographic aspects of normal and pathological pediatric pancreas.

RESUMEN

A very rare localization of infantile hemangioma (IH) is mesentery. It may manifest with intra-luminal or intra-peritoneal bleeding. Here there is the case of a 45-day-old infant, presenting with anemia and poor growth, subsequently referred to an uncommon hemangioma of the mesentery. As well as for most of IH, conservative treatment with only propranolol resulted in the disappearance of the lesion. Here we analyze ultrasound and magnetic resonance (MR) features of this uncommon lesion, trying to underline main characteristics at imaging in order to monitor the response to therapy.

RESUMEN

Percutaneous renal biopsy (PRB) is an important procedure in the diagnostic and prognostic evaluation of renal disorders. Despite the relative simplicity, it can be a possible cause of iatrogenic renovascular injury. We describe two cases of iatrogenic pseudoaneurysm, a major post-biopsy complication, in two pediatric patients with persistent micro- and macro-hematuria, promptly diagnosed using color Doppler ultrasound, confirmed with renal arteriography and treated with embolization.

PURPOSE: Medullary thyroid cancer (MTC) is a neuroendocrine tumour of the thyroid C cells. Pasireotide, a multi-receptor targeted somatostatin analogue, and everolimus, an inhibitor of mTOR, showed antitumour properties in neuroendocrine tumours. Aim of this study was to evaluate pasireotide alone and in combination with everolimus in patients with MTC. METHODS: Patients with progressive metastatic or persistent postoperative MTC received pasireotide LAR 60 mg/m for at least 6 months. Patients exhibiting progressive disease received everolimus 10 mg/d as combination therapy. Primary endpoint was progression free survival (PFS). Secondary endpoints included, overall survival, objective response rates, change in circulating markers, safety. Study registration no. NCT01625520. RESULTS: Nineteen consecutive patients were enrolled. Median follow-up was 31 months. Median PFS with pasireotide was 36 months (95% CI: 19.5-52.5). Nine patients (47%) had tumour progression: seven of them started everolimus in combination with pasireotide, achieving a median PFS of 9.0 months (95% CI: 0-21.83). Five of them (71%) had further tumour progression, one objective response (14.3%), one stopped treatment because of pulmonary embolism. Pasireotide alone and with everolimus was safe and required withdrawal only in one case. Diarrhoea and hyperglycaemia were the most frequent adverse events with pasireotide (grade 3 in 5.3% each). Hyperglycaemia was the most frequent grade 3 toxicity with the combination therapy (28.6%). CONCLUSIONS: Pasireotide therapy shows antiproliferative effects in persistent postoperative MTC suggesting further investigation on larger series of patients. In progressive MTC lesions, the combination pasireotide plus everolimus may be of benefit. Both schemes were safe and well tolerated.

RESUMEN

Renal infarction is a rare cause of referral to the emergency department, with very low estimated incidence (0.004%-0.007%). Usually, it manifests in patients aged 60-70 with risk factors for thromboembolism, mostly related to heart disease, atrial fibrillation in particular. We report a case of idiopathic segmental renal infarction in a 38-year-old patient, presenting with acute abdominal pain with no previous known history or risk factors for thromboembolic diseases. Because of its aspecific clinical presentation, this condition can mimic more frequent pathologies including pyelonephritis, nephrolithiasis, or as in our case appendicitis. Here we highlight the extremely ambiguous presentation of renal infarct and the importance for clinicians to be aware of this condition, particularly in patients without clear risk factors, as it usually has a good prognosis after appropriate anticoagulant therapy.

Bilateral adrenal hemorrhage is a rare condition, which is burdened by potentially life-threatening consequences related to the development of acute adrenal insufficiency. Despite treatment with stress-dose glucocorticoids, a mortality rate of 15% has been reported, which varies according to the severity of underlying predisposing illness and could be much more higher if the adrenal insufficiency is not promptly recognized. An early diagnosis is crucial, though, because of nonspecific clinical and laboratory findings, adrenal hemorrhage is rarely suspected. Therefore, imaging has a pivotal role for the diagnosis of this uncommon condition but, despite adrenal hematomas characteristically appear round or oval with peripheral fat stranding, their initial presentation could be ambiguous. The authors describe a case of postoperative bilateral adrenal hemorrhage initially presenting at computed tomography scan as thickening of both glands surrounded by fat stranding, which led to close monitoring of adrenal function before unequivocal hemorrhage developed.

Tumoral calcinosis is a rare and benign hereditary tumor-like periarticular calcium deposit. It is painless and it is found commonly around large joints such as hip, shoulder and elbow. The condition predominately affects young black African patients with an equal gender ratio. In this report, a case of primary idiopathic localized tumoral calcinosis in a 22-year-old African man and its distinctive radiographic "chicken wire" pattern was described.

Cystic dystrophy of the duodenal wall in heterotopic pancreas, recently described as paraduodenal pancreatitis, is a rare condition characterized by multiple cysts or pseudocysts located in the submucosa or muscularis propria of a thickened duodenal wall. They result from multiple episodes of obstruction of the small ducts in aberrant pancreatic islets. Cystic dystrophy of the duodenal wall in heterotopic pancreas usually affects alcoholic males, but here we present the case of a female without a history of alcohol abuse who was referred to our emergency department with abdominal pain and vomiting. She was treated by a pancreas-preserving surgical approach, thanks to a proper pre-operative differential diagnosis. Even though differentiating this benign condition from pancreatic cancer is a challenge, some characteristic findings on multidetector CT scan and MRI/MR cholangiopancreatography, such as a thickened duodenal wall containing cysts and sheet-like tissues in the pancreaticoduodenal groove, could lead to the correct diagnosis.

The incidental detection of a tubulovillous adenoma at a contrast-enhanced computed tomography (CECT) with nondedicated protocol, performed in emergency conditions, is an uncommon finding. We report a case of a woman presenting with a subocclusive episode. A CECT scan was performed, and a pedunculated polyp could be appreciated at 3D-reconstruction images. A particular depiction of pedunculus of the polypoid lesion, resemble a clapper-bell, could help to define the vegetating lesion at the volume-rendering reconstruction images. This case emphasizes the fundamental role of postprocessing in the clinical practice to improve the diagnostic accuracy of abdominal CT scan. In addition, a potential new radiologic sign, the "clapper-bell sign", is proposed, as literature about the appearance of a polyp at CECT, performed without a dedicated protocol for colonoscopy, is poor.

Pancreatic vipoma is an extremely rare tumor accounting for less than 2% of endocrine pancreatic neoplasms with a reported incidence of 0.1-0.6 per million. While cross-sectional imaging findings are usually not specific, exact localization of the tumor by means of either computed tomography (CT) or magnetic resonance (MR) is pivotal for surgical planning. However, cross-sectional imaging findings are usually not specific and further characterization of the tumor may only be achieved by somatostatin-receptor scintigraphy (SRS). We report the case of a 70 years old female with a two years history of watery diarrhoea who was found to have a solid, inhomogeneously enhancing lesion at the level of the pancreatic tail at Gadolinium-enhanced MR (Somatom Trio 3T, Siemens, Germany). The tumor had been prospectively overlooked at a contrast-enhanced multi-detector CT (Aquilion 64, Toshiba, Japan) performed after i.v. bolus injection of only 100 cc of iodinated non ionic contrast media because of a chronic renal failure (3.4 mg/mL) but it was subsequently confirmed by SRS. The patient first underwent a successful symptomatic treatment with somatostatin analogues and was then submitted to a distal pancreasectomy with splenectomy to remove a capsulated whitish tumor which turned out to be a well-differentiated vipoma at histological and immuno-histochemical analysis.

O presente trabalho tem como objetivo refletir sobre como o Apadrinhamento Afetivo pode proporcionar vivências emocionais construtivas e significativas para os envolvidos. Trata-se de um projeto que visa proporcionar a crianças e adolescentes abrigados - que não têm possibilidade de reintegração com a família de origem e que dificilmente serão adotados - o estabelecimento de vínculos afetivos e familiares fora das instituições de acolhimento. O Setor de Adoção do CEAPIA vem participando de parte deste projeto há três anos, com uma comissão de outros dois membros: o Instituto Amigo de Lucas e o Ministério Público do Estado do Rio Grande do Sul. Para atingir o propósito deste artigo, foi realizada uma revisão da literatura psicanalítica nos seguintes temas: desenvolvimento emocional, apego, vínculos afetivos e situações de vulnerabilidade. Foram consultados também artigos científicos e pesquisas relacionadas ao Apadrinhamento Afetivo


The objective of the present study is to reflect on how Foster Parenting can provide constructive and meaningful emotional experiences for those involved. The Foster Parenting Project aims to provide sheltered children and adolescents - who have no possibility of reintegration with their origin family and will hardly be adopted - the establishment of affective and family bonds outside the host institutions. The CEAPIA Adoption Sector has been participating in this project for three years, in a Commission with two others members: the Instituto Amigo de Lucas and the Ministério Público do Estado do Rio Grande do Sul. In order to achieve the purpose of this article, it was made a review of the psychoanalytic literature on the following themes: emotional development, attachment, affective bonds and situations of vulnerability. Scientific articles and research related to Foster Parenting were also consulted

Estudou-se famílias de baixa renda procedentes do bairro Partenon, na cidade de Porto Alegre. A amostra de famílias foi selecionada pelo Conselho Tutelar da regiäo, levando em conta tipos de infraçäo perpetrada por algum integrante da família: pais abusivos, fuga de casa, uso de drogas, estrupo e absenteísmo escolar de adolescentes. As famílias freqüentaram um grupo que se reuniu em nove encontros quinzenais, em horário noturno, durante os meses de agosto a dezembro de 1997. Nas oficinas usaram-se técnicas de pintura, música, fotografias e dramatizaçäo. Os pesquisadores reforçaram os aspectos sadios dessas famílias; colocaram limites nos comportamentos agressivos e ajudaram as pessoas a melhorar a auto-estima. Foram acompanhadas 11 famílias (33 pessoas). Destas, três participaram apenas do primeiro encontro, cinco famílias mostraram melhoras no final do processo e três famílias näo puderam ser ajudadas. Acredita-se que grupos como esses podem ser úteis no manejo da violência doméstica.
