RESUMEN

Objective To observe the clinical efficacy of FANG's scalp acupuncture combined with timing auricular point pressing therapy in the treatment of insomnia patients with maintenance hemodialysis(MHD).Methods A total of 70 patients with insomnia on MHD were randomly divided into observation group and control group,with 35 patients in each group.Both groups were given conventional treatment,the control group was given oral use of Estazolam Tablets on the basis of conventional treatment,and the observation group was given FANG's scalp acupuncture combined with timing auricular point pressing therapy.Both groups were treated for a total of 4 weeks of treatment.After 1 month of treatment,the clinical efficacy of the two groups was evaluated,and the changes in the Pittsburgh Sleep Quality Index(PSQI)score and the Kidney Disease Quality of Life Short Form(KDQOL-SF)score,as well as the scores of the Hamilton Depression Scale(HAMD)and the Hamilton Anxiety Scale(HAMA),were observed in the patients of the two groups before and after treatment.The changes in hemoglobin(Hb),serum creatinine(Scr),and blood urea nitrogen(BUN)levels were compared before and after treatment between the two groups,and the safety of the two groups was evaluated.Results(1)After treatment,the PSQI and KDQOL-SF scores of patients in the two groups were significantly improved(P<0.05),and the observation group was significantly superior to the control group in improving PSQI and KDQOL-SF scores,and the difference was statistically significant(P<0.05).(2)After treatment,the HAMD and HAMA scores of patients in the two groups were significantly improved(P<0.05),and the observation group was significantly superior to the control group in improving HAMD and HAMA scores,and the differences were statistically significant(P<0.05).(3)After treatment,the Hb,Scr,BUN levels of patients in the two groups were significantly improved(P<0.05),and the observation group was significantly superior to the control group in improving Hb,Scr,BUN levels,and the differences were all statistically significant(P<0.05).(4)The total effective rate was 77.14%(27/35)in the observation group and 62.86%(22/35)in the control group.The efficacy of the observation group was superior to that of the control group,and the difference was statistically significant(P<0.05).(5)Comparison of the incidence of adverse reactions in the two groups of patients,the difference was not statistically significant(P>0.05).Conclusion FANG's scalp acupuncture combined with timing auricular point pressing therapy in the treatment of insomnia patients with MHD can effectively improve the sleep quality of patients and alleviate anxiety and depression,so as to improve the quality of life of patients,with remarkable efficacy.

Objective To investigate the expressions of Smac/Diablo and caspase-9 and their corelations in the cytoplasm of cerebral cortex of neonatal rats with hypoxic-ischemic brain damage (HIBD), and the possible interference effects of Shenfu injection on them. Methods Postnatal 7-d SD rats were randomly divided into Shenfu treatment group (group SF), normal saline group (group C) and sham-operated group (group S). Rat models of HIBD were established according to Rice's method.Based on the observing times after the success of model making, each group (n=48) was equally divided into 6 subgroups (3, 6, 12 and 24 h, and 3 and 7 d observing groups). Rats in the group S were not performed ligation of the right common carotid artery and not underwent hypoxia but only performed the separation of right common carotid artery. Shenfu injection was administered by intraperitioneal injection right after H/I insult and then once daily at a dosage of 10 mL/kg for 3 d into the rats of group SF. Saline was also administered into the rats of group C by the same methods and at the same dosage. Cytoplasm proteins were extracted from the cerebral cortex tissue, and then, Western blotting was employed to detect the protein levels of Smac/Diablo and activated caspase-9 in cytoplasm. Results Western blotting showed that the expression levels of Smac/Diablo and activated caspase-9 in cytoplasm of group S were very low; the expression levels of Smac/Diablo and activated caspase-9 in cytoplasm of group SF and group C were increased markedly since the 3rd h of the success of model making as compared with those in group S (P<0.05): these in the 2 groups reached their peak levels 24 h after H/I insult, gradually decreased 3 d after H/I insult, and almost returned to the original levels 7 d after H/I insult. The expressions of Smac/Diablo in cytoplasm of group SF 12, 24 h and 3 d after H/I insult was significantly lower than that of group C (P<0.05); the expression of activated caspase-9 in cytoplasm of group SF 12,24 h and 3 d after H/I insult was significantly decreased as compared with that of group C (P<0.05).Conclusion Smac/Diablo cytoplasm translocation and increased expression level of caspase-9 appear in the cytoplasm of cerebral cortex cells after H/I injury; Shenfu injection can inhibit the expressions of Smac/Diablo and caspase-9; Smac/Diablo and caspase-9 involve in the pathophysiological mechanisms of neuronal damage after H/I.

Objective To study the effect of shenfu injection on the neuron apoptosis in hippocampal CA1 region of newborn with hypoxic-ischemic brain damage(HIBD).Methods The experiment included 2 parts.One was to measure the apoptosis rate by the flow cytometry,and the other was to investigate the expression of Bcl-2 and Bax of neurons in left hippocampal CA1 region.Models of postnatal 7-day Sprague-Dawley(SD)rats with HIBD were established,and were equally divided into 4 groups:sham operation(group S),control group(group C),shenfu injection pretreatment(group P),and shenfu injection treatment(group SF).The neuron apoptosis rate in hippocampal CA1 region in every group was measured at 2 h before and 2,12,24 h,3,7,14 and 28 d after hypoxic ischemic(HI) insult.The expressions of Bcl-2 and Bax were performed by immunohisochemistry.Results The apoptosis of neuron in hippocampal CA1 region in group P and group SF after HI insult was significantly less than that of group C(P
