RESUMEN

AIM: To assess the efficacy and safety of mandibular block performed by two different techniques. MATERIAL AND METHODS: The study involved 92 patients divided in two groups according to mandibular block technique: method of P.M. Egorov (45 patients) and G. Gow-Gates modified by S.A. Rabinovich and O.N. Moskovets in 1999 (47 patients). RESULTS: The number of local complications was reduced to 1/9% when modified Gow-Gates mandibular block technique was used with anesthesia efficacy of 98%. CONCLUSION: Modified Gow-Gates mandibular block technique shows better efficacy and is associated with lower complications rate for oral surgical procedures and inflammatory oral diseases treatment in mandible.

RESUMEN

Needle breakage may be a damaging experience for both medical practitioners and patients. Medical literature provides numerous cases of dental practitioners being confronted with this problem. We have studied the clinical case of the patient L. at the Moscow State University of Medicine and Dentistry involving needle breakage during conductive anesthesia. We propose our medical approach to solving this issue alongside with a desirable course of action, providing additional recommendations and suggesting intraosseous anesthesia as a safer type of pain management.

RESUMEN

The aim of the study was to improve dental treatment quality in patients with arterial hypertension. The study included 256 patients with arterial hypertension (153 females, 103 males) aged 35 to 60 years. Anesthesia efficacy was assessed by visual analog scale after treatment. Oral adverse effects caused by regular intake of antihypertensive drugs and interrelation with articaine and mepivacaine were described. The study proved high efficacy of 4% articaine 1:200 000. The efficacy of 3% mepivacaine with no vasoconstriction agents was 86.4±2.3% but may be enhanced to 93.3±2.3% by preventive non-steroid anti-inflammatory agents taken 25-30 minutes before treatment in maximal daily dosage.

RESUMEN

The study involved 153 patients (92 females, 61 males aged 30-75 years) with arterial pressure from 120/80 to 180/100 mm Hg. The role of dentist in primary diagnosis of hypertension was identified. The study highlights the need for functional status assessment before dental treatment and the importance of safe and effective local anesthesia for the patients with arterial hypertension. The paper presents rationale for the choice of local anesthetic agent for these patients.

RESUMEN

The paper presents basic principles of local anesthesia selection in patients with concomitant somatic diseases. These principles are history taking; analysis of drugs interaction with local anesthetic and sedation agents; determination of the functional status of the patient; patient anxiety correction; dental care with monitoring of hemodynamics parameters. It was found that adhering to this algorithm promotes prevention of urgent conditions in patients in outpatient dentistry.

RESUMEN

Directed delivery and distribution of anaesthetics (novocaine, lidocaine, trimecaine) immobilized on finely dispersed iron powders under the influence of an external magnetic field in body tissues of test animals have been studied. The data of emission spectral analysis relating to concentration of iron in soft and bone tissues of animals influenced by constant and alternating magnetic field on front and reverse sides over a period from 0 to 180 min are given.

RESUMEN

The effect of external magnetic field (direct and alternating) on the targeted delivery and distribution in body tissues of experimental animals of anesthetics (novocain, lidocaine, and trimecaine) immobilized on highly dispersed ferromagnetics with biocompatible coating was studied by the method of emission spectral analysis. The results are compared with the results obtained earlier on highly dispersed iron powders with dextran coating. The parameters for the powders with dextran coating are considerably lower than those for the magnetic carriers with the polyacrylamide coating.

RESUMEN

The major constituents of acinar cell auto-oxidative system in acute postoperative parotitis were determined in experiments on 32 dogs. Two types of cellular reactions were identified: free-radical processes with increased levels of lipid peroxidation products and insufficiency of enzymatic and nonenzymatic glutathion antioxidants and reaction of defense metabolism inhibition. A conclusion is made about the necessity of altering the current methods for prevention and therapy of postoperative parotitis.
