BACKGROUND: Pain management plays an essential role in postoperative recovery after lung surgeries. The Erector Spinae Plane Block (ESPB) is a widely used regional anaesthesia technique; however, few clinical trials have compared this block to active control in thoracic surgeries. This study evaluated the non-inferiority of the analgesia provided by ESPB when compared to paravertebral block (PVB) in lung surgeries. METHODS: Randomised, active-controlled, blinded for patients and assessors, non-inferiority trial. Patients who underwent unilateral lung surgeries were divided into two groups according to the regional anaesthesia technique-continuous ESPB or PVB at the T5 level. The primary outcome was to assess pain using a numerical rating scale (NRS) with a test of the interaction of three measures over 24 h postoperatively. An NRS score ≥ 7 was considered analgesia failure, and the prespecified non-inferiority margin was 10%. RESULTS: In the interim analysis that terminated this study, 120 participants were enrolled. ESPB patients reported higher mean NRS general values over 24 h, 4.6 ± 3.2 in the ESPB group versus 3.9 ± 2.9 in the PVB group, with a difference of -0.67 (-15.2%) and 95%CI: -1.29 to -0.05 (p = .02), demonstrating not non-inferiority. In addition, the ESPB group presented higher NRS failure of analgesia over 24 h (p < .01) and required more postoperative opioids (p = .01 over 24 h). There was no difference in patient satisfaction between groups. CONCLUSION: This trial demonstrated that a continuous erector spinae plane block was not non-inferior to a continuous paravertebral block for analgesia after lung surgery but resulted in higher levels of postoperative pain and opioid consumption.

Cardio/cerebrovascular diseases (CVD) have become one of the major health issue in our societies. But recent studies show that the present pathology tests to detect CVD are ineffectual as they do not consider different stages of platelet activation or the molecular dynamics involved in platelet interactions and are incapable to consider inter-individual variability. Here we propose a stochastic platelet deposition model and an inferential scheme to estimate the biologically meaningful model parameters using approximate Bayesian computation with a summary statistic that maximally discriminates between different types of patients. Inferred parameters from data collected on healthy volunteers and different patient types help us to identify specific biological parameters and hence biological reasoning behind the dysfunction for each type of patients. This work opens up an unprecedented opportunity of personalized pathology test for CVD detection and medical treatment.

The U.S. wine and grape industry loses $3B annually due to viral diseases including grapevine leafroll-associated virus complex 3 (GLRaV-3). Current detection methods are labor-intensive and expensive. GLRaV-3 has a latent period in which the vines are infected but do not display visible symptoms, making it an ideal model to evaluate the scalability of imaging spectroscopy-based disease detection. The NASA Airborne Visible and Infrared Imaging Spectrometer Next Generation was deployed to detect GLRaV-3 in Cabernet Sauvignon grapevines in Lodi, CA in September 2020. Foliage was removed from the vines as part of mechanical harvest soon after image acquisition. In September of both 2020 and 2021, industry collaborators scouted 317 hectares on a vine-by-vine basis for visible viral symptoms and collected a subset for molecular confirmation testing. Symptomatic grapevines identified in 2021 were assumed to have been latently infected at the time of image acquisition. Random forest models were trained on a spectroscopic signal of noninfected and GLRaV-3 infected grapevines balanced with synthetic minority oversampling of noninfected and GLRaV-3 infected grapevines. The models were able to differentiate between noninfected and GLRaV-3 infected vines both pre- and postsymptomatically at 1 to 5 m resolution. The best-performing models had 87% accuracy distinguishing between noninfected and asymptomatic vines, and 85% accuracy distinguishing between noninfected and asymptomatic + symptomatic vines. The importance of nonvisible wavelengths suggests that this capacity is driven by disease-induced changes to plant physiology. The results lay a foundation for using the forthcoming hyperspectral satellite Surface Biology and Geology for regional disease monitoring in grapevine and other crop species. Oilseed cakes (OC) are natural sources of lignocellulosic biomass, produced every year in large amounts. In addition to their main applications as animal feed, plant or soil fertilizer, and compost, they present enormous potential for being used in biotechnological processes for the obtainment and extraction of valuable bioactive compounds. This work evaluated the effect of solid-state fermentation on the bioactive properties of extracts obtained from the bioprocessing of OC and evaluated the effect of solvents on the recovery of compounds with higher bioactive potential. A general decrease of EC50 values was observed for fermented extracts obtained using a mixture of water/methanol (1:1) as extraction solvent. A decrease in the minimum inhibitory concentration was observed for fermented water extracts compared to non-fermented. Additionally, growth inhibition of Listeria monocytogenes was observed when using aqueous methanolic fermented extracts. These extracts also exhibited a higher percentage of growth reduction against phytopathogenic fungi, and some extracts exhibited increased protection against genotoxic agents such as camptothecin and bisphenol A. It was demonstrated that bioprocessing of OC through SSF is an effective approach to obtaining valuable compounds with bioactive properties for use in the food, pharmaceutical or cosmetic industries.

A geologic map is both a visual depiction of the lithologies and structures occurring at the Earth's surface and a representation of a conceptual model for the geologic history in a region. The work needed to capture such multifaced information in an accurate geologic map is time consuming. Remote sensing can complement traditional primary field observations, geochemistry, chronometry, and subsurface geophysical data in providing useful information to assist with the geologic mapping process. Two novel sources of remote sensing data are particularly relevant for geologic mapping applications: decameter-resolution imaging spectroscopy (spectroscopic imaging) and meter-resolution multispectral shortwave infrared (SWIR) imaging. Decameter spectroscopic imagery can capture important mineral absorptions but is frequently unable to spatially resolve important geologic features. Meter-resolution multispectral SWIR images are better able to resolve fine spatial features but offer reduced spectral information. Such disparate but complementary datasets can be challenging to integrate into the geologic mapping process. Here, we conduct a comparative analysis of spatial and spectral scaling for two such datasets: one Airborne Visible/Infrared Imaging Spectrometer-Classic (AVIRIS-classic) flightline, and one WorldView-3 (WV3) scene, for a geologically complex landscape in Anza-Borrego Desert State Park, California. To do so, we use a two-stage framework that synthesizes recent advances in the spectral mixture residual and joint characterization. The mixture residual uses the wavelength-explicit misfit of a linear spectral mixture model to capture low variance spectral signals. Joint characterization utilizes nonlinear dimensionality reduction (manifold learning) to visualize spectral feature space topology and identify clusters of statistically similar spectra. For this study area, the spectral mixture residual clearly reveals greater spectral dimensionality in AVIRIS than WorldView (99% of variance in 39 versus 5 residual dimensions). Additionally, joint characterization shows more complex spectral feature space topology for AVIRIS than WorldView, revealing information useful to the geologic mapping process in the form of mineralogical variability both within and among mapped geologic units. These results illustrate the potential of recent and planned imaging spectroscopy missions to complement high-resolution multispectral imagery-along with field and lab observations-in planning, collecting, and interpreting the results from geologic field work.

OBJECTIVE: Psychotherapy studies have revealed that therapist characteristics are responsible for 5% to 9% of outcome variance. The therapist-facilitative interpersonal skills (FIS) have been shown to predict both alliance and outcomes, indicating that higher FIS therapists are more effective than lower FIS therapists. The current study focused on the development and validation of the FIS-client version (FIS-C) instrument, aimed at collecting the clients' perspectives on relevant therapist characteristics. METHOD: The clinical outcomes in routine evaluation-outcome measures, the session rating scale, and the FIS questionnaire-client version were filled out by psychotherapy clients. Exploratory, confirmatory factor, and test-retest analysis were conducted. RESULTS: Results indicate robust psychometric characteristics, in terms of validity (factorial, convergent, discriminant, and nomological), reliability, and sensitivity. CONCLUSION: The validation of the FIS-C represents an important contribution to clinical research and practice, namely to the field of client feedback and therapist expertise.

BACKGROUND: Vegetable oils are yearly produced in large amounts generating solid by-products, the oilseed cake (OC). OCs are lignocellulosic materials that have been used for animal feed with some limitations due to high fibre content from the plant cell walls. Biotechnological processes can help to overcome these limitations and contribute to up-grading such by-products, enhancing their nutritional value as feed ingredients. RESULTS: All fungal species were able to decrease neutral detergent fibre and acid detergent fibre in all by-products. Additionally, relevant enzymes were produced by the three fungi studied resulting in an improved antioxidant capacity of all fermented OCs. Aspergillus niger led to the highest activity of cellulase (109 U g-1 ), xylanase (692 U g-1 ) and protease (157 U g-1 ) per dry OC matter and to the recovery of an extract rich in antioxidants, with the highest scavenging potential of free radicals and superoxide anion, iron chelation ability and reducing power. Rhyzopus oryzae produced the highest activity of ß-glucosidase (503 U g-1 ) and led to the highest liberation of total phenolic content (TPC). Principal components analysis showed that extracts with high antioxidant potential were obtained in solid-state fermentation (SSF) with high enzymatic activity. A positive correlation was established between the action of ß-glucosidase and TPC. CONCLUSION: Within the same bioprocess it was possible to improve the nutritional value of OCs and to obtain relevant bioactive compounds such as lignocellulosic enzymes and phenolic compounds with antioxidant potential, resulting in a significant improvement of already valuable by-products with commercial interest for animal feed. © 2021 Society of Chemical Industry.

This study aimed to evaluate intake, animal performance and carcass traits with the inclusion of corn dried distiller grains (DDGs) replacing cornmeal (Zea mays L.) in non-forage-based diets for finishing feedlot Nellore cattle. Sixty-four uncastrated Nellore cattle weighing 267.22 ± 20.4 kg and 18 ± 3 months old were distributed in 16 pens. Experimental diets were based on cornmeal, cottonseed cake, a mineral mixture and increased levels of DDG replacing cornmeal (0, 10, 20 and 30% on a dry matter basis). The study lasted 135 days, and the intake of nutrients and body weight gain were monitored. At the end of the study, the animals were slaughtered to measure their carcass traits. During the first 34 days of the study, the DDG levels linearly increased dry matter intake and intake of all nutrients. In other phases of the feedlot, the DDG levels did not have any effect on DMI or ADG but caused a quadratic effect on hot carcass weight. Thus, DDG can replace cornmeal in non-forage diets fed to feedlot Nellore cattle without any negative effect on intake or animal performance.

Fear is a conscious state caused by exposure to real or imagined threats that trigger stress responses that affect the body and brain, particularly limbic structures. A sub-group of patients with mesial temporal lobe epilepsy related to hippocampus sclerosis (MTLE-HS) have seizures with fear, which is called ictal fear (IF), due to epileptic activity within the brain defensive survival circuit structures. Synaptic transmission efficacy can be bi-directionally modified through potentiation (long-term potentiation (LTP)) or depression (long-term depression (LTD)) as well as the phosphorylation state of Ser831 and Ser845 sites at the GluA1 subunit of the glutamate AMPA receptors, which has been characterized as a critical event for this synaptic plasticity. In this study, GluA1 levels and the phosphorylation at Ser845 and Ser831 in the amygdala (AMY), anterior hippocampus (aHIP) and middle gyrus of temporal neocortex (CX) were determined with western blots and compared between MTLE-HS patients who were showing (n = 06) or not showing (n = 25) IF. Patients with IF had an 11% decrease of AMY levels of the GluA1 subunit (p = 0.05) and a 21.5% decrease of aHIP levels of P-GluA1-Ser845 (p = 0.009) compared to patients not showing IF. The observed associations were not related to imbalances in the distribution of other concomitant types of aura, demographic, clinical or neurosurgical variables. The lower levels of P-GluA1-Ser845 in the aHIP of patients with IF were not related to changes in the levels of the serine/threonine-protein phosphatase PP1-alpha catalytic subunit or protein kinase A activation. Taken together, the GluA1 subunit levels in AMY and P-GluA1-Ser845 levels in the aHIP show an overall accuracy of 89.3% (specificity 95.5% and sensitivity 66.7%) to predict the presence of IF. AMY levels of the GluA1 subunit and aHIP levels of P-GluA1-Ser845 were not associated with the psychiatric diagnosis and symptoms of patients. Taken together with previous findings in MTLE-HS patients with IF who were evaluated by stereotactic implanted depth electrodes, we speculate our findings are consistent with the hypothesis that AMY is not a centre of fear but together with other sub-cortical and cortical structures integrates the defensive circuit that detect and respond to threats. This is the first report to address neuroplasticity features in human limbic structures connected to the defensive survival circuits, which has implications for the comprehension of highly prevalent psychiatric disorders and symptoms.

Interictal dysphoric disorder (IDD) is a poorly understood psychiatric disorder of epilepsy patients. Interictal dysphoric disorder is characterized by depressive, somatoform, and affective symptoms observed in up to 5.9% of drug-resistant mesial temporal lobe epilepsy with hippocampal sclerosis (MTLE-HS). This study aimed to evaluate the association between ictal fear (IF) and the psychiatric symptoms and diagnosis in MTLE-HS patients. We included 116 (54.3% male) consecutive adult patients (36 ±â€¯11 years) with MTLE-HS. Anxiety and depression symptoms were evaluated by the Hospital Anxiety and Depression Scale (HADS) and the psychiatric diagnosis were according to Fourth Edition of the Diagnosis and Statistical Manual of Mental Disorders (DSM-IV). The independent association between the occurrence of IF aura and the psychiatric diagnosis was determined by binary regression. When compared to those with other auras or without aura, patients reporting IF have higher HADS anxiety, but not HADS depression, scores. Ictal fear was independently associated with the diagnosis of interictal dysphoric disorder (OR, IC 95% = 7.6, 1.3-43.2, p = 0.02), but not with the diagnosis of anxiety (OR, CI 95% = 0.72, 0.08-6.0, p = 0.73), depression (OR, CI 95% = 0.94, 0.19-4.8, p = 0.94) or psychotic disorders (p = 0.99). Only patients with drug-resistant MTLE-HS were included and the small number of cases with DD diagnosis in the sample. In MTLE-HS patients, the occurrence of IF is associated with higher levels of anxiety symptoms and IDD. The results provide insights about fear-related neural network connections with anxiety symptoms and the IDD in MTLE-HS.

BACKGROUND: To examine the association of handgrip strength (HGS) and bone mineral density (BMD) of Brazilian children and adolescents. METHODS: The sample included 243 children and adolescents aged from 4 to 15 years (9.3 ± 2.2 years), 171 males and 72 females. The following measurements were performed: weight, height, trunk length, and years to the peak height velocity (PHV). The percentage lean soft tissue (PLST), percentage fat mass (PFM), and BMD were obtained using Dual Energy X-ray Absorptiometry (DXA) and HGS using a dynamometer. RESULTS: In girls, HGS was positively associated with higher BMD, even after the adjustments, by arms [ß = 0.006; p < 0.001], legs [ß = 0.014; p < 0.001], pelvis [ß = 0.019; p < 0.001], trunk [ß = 0.013; p < 0.001], spine [ß = 0.013; p = 0.008], and total body [ß = 0.009; p < 0.001]. The same occurred in the boys, even after the adjustments a positive relationship was observed, whereas higher HGS was related to greater BMD in arms [ß = 0.006; p < 0.001], legs [ß = 0.017; p < 0.001], pelvis [ß = 0.014; p < 0.001], trunk [ß = 0.009; p < 0.001], spine [ß = 0.008; p < 0.001], and total body [ß = 0.007; p < 0.001]. CONCLUSION: HGS was positively associated to BMD in boys and girls, regardless of age, PHV, PLST, and PFM.

RESUMEN

Stroke treatment has dramatically improved in recent decades. However, although new treatments have reduced its mortality and the severity of its physical and cognitive sequelae, many people still have incapacitating disabilities following a stroke. Depression is the most common psychiatric disorder following stroke; it is important to recognise and treat as it limits motor and cognitive rehabilitation. Antidepressant medication is an effective treatment and can improve adherence to clinically recommended physical and cognitive tasks, thereby enhancing functional remodelling of neuronal pathways and improving rehabilitation outcomes.

Planned hyperspectral satellite missions and the decreased revisit time of multispectral imaging offer the potential for data fusion to leverage both the spectral resolution of hyperspectral sensors and the temporal resolution of multispectral constellations. Hyperspectral imagery can also be used to better understand fundamental properties of multispectral data. In this analysis, we use five flight lines from the Airborne Visible/Infrared Imaging Spectrometer (AVIRIS) archive with coincident Landsat 8 acquisitions over a spectrally diverse region of California to address the following questions: (1) How much of the spectral dimensionality of hyperspectral data is captured in multispectral data?; (2) Is the characteristic pyramidal structure of the multispectral feature space also present in the low order dimensions of the hyperspectral feature space at comparable spatial scales?; (3) How much variability in rock and soil substrate endmembers (EMs) present in hyperspectral data is captured by multispectral sensors? We find nearly identical partitions of variance, low-order feature space topologies, and EM spectra for hyperspectral and multispectral image composites. The resulting feature spaces and EMs are also very similar to those from previous global multispectral analyses, implying that the fundamental structure of the global feature space is present in our relatively small spatial subset of California. Finally, we find that the multispectral dataset well represents the substrate EM variability present in the study area - despite its inability to resolve narrow band absorptions. We observe a tentative but consistent physical relationship between the gradation of substrate reflectance in the feature space and the gradation of sand versus clay content in the soil classification system.

BACKGROUND: Mediterranean agro-food industries (such as wineries, breweries and olive mills) dispose of great amounts of waste. This generates environmental problems, and the waste has a low nutritional value for use as animal feed. In this sense, solid-state fermentation (SSF) can increase the nutritional value of these wastes and simultaneously produce lignocellulolytic enzymes. RESULTS: All fermented wastes were enriched in protein by the three fungi studied. Aspergillus ibericus was the fungus with the biggest increase of protein, which ranged from 1.4 times to 6.2 times with respect to unfermented wastes. Likewise, A. ibericus achieved the maximum cellulase and xylanase activities. The relationships among substrates composition, fungi used and SSF performance were evaluated by principal components analysis. The high content of cellulose and hemicellulose favoured lignocellulolytic enzymes production, and the phenolics content was negatively correlated with enzymes production and with the increase of protein by SSF. Furthermore, the scanning electron microscopy analysis showed the growth of fungi over solid wastes, the formation of conidiophores and the changes in their structures. CONCLUSION: The nutritional value of Mediterranean wastes was improved and other value-added products such as lignocellulolytic enzymes were produced in the same process, which could facilitate the efficient reuse of these wastes. © 2018 Society of Chemical Industry.

Religiosity/spirituality can affect health and quality of life in myriad ways. Religion has been present since the first moments of our evolutionary history, whether it is understood as a byproduct or as an adaptation of our cognitive evolution. We investigated how religion influences medicinal plant-based local medical systems (LMSs) and focuses on how individual variation in the degree of religiosity/spirituality affects the structure of LMSs. The knowledge of people about their medical systems was obtained through the free-listing technique, and level of religiosity/spirituality was calculated using the Brazilian version of the Brief Multidimensional Measure of Religiousness/Spirituality. We employed a Generalized Linear Model to obtain the best model. Religiosity/spirituality is predictive of structural and functional aspects of medicinal plant-based LMSs. Our model encourages a discussion of the role of religion in the health of an individual as well as in the structure of an individual's support system. Religiosity/spirituality (and the dimensions of Commitment and Religious and Spiritual History, in particular) act to protect structural and functional elements of LMSs. By providing protection, the LMS benefits from greater resilience, at both the individual and population levels. We suggest that the socialization process resulting from the religious phenomenon has contributed to the complexity and maintenance of LMSs by means of the interaction of individuals as they engage in their religious observances, thus facilitating cultural transmission.

BACKGROUND: Since hemodynamics plays a key role in the development and evolution of cardiovascular pathologies, physician's decision must be based on proper monitoring of relevant physiological flow quantities. METHODS: A numerical analysis of the error introduced by an intravascular Doppler guide wire on the peak velocity measurements has been carried out. The effect of probe misalignment (±10°) with respect to the vessel axis was investigated. Numerical simulations were performed on a realistic 3D geometry, reconstructed from coronary angiography images. Furthermore, instead of using Poiseuille or Womersley approximations, the unsteady pulsatile inlet boundary condition has been calculated from experimental peak-velocity measurements inside the vessel through a new approach based on an iterative Newton's algorithm. RESULTS: The results show that the presence of the guide modifies significantly both the maximum velocity and the peak position in the section plane; the difference is between 6 and 17 % of the maximum measured velocity depending on the distance from the probe tip and the instantaneous vessel flow rate. Furthermore, a misalignment of the probe may lead to a wrong estimation of the peak velocity with an error up to 10 % depending on the probe orientation angle. CONCLUSIONS: The Doppler probe does affect the maximum velocity and its position during intravascular Doppler measurements. Moreover, the Doppler-probe-wire sampling volume at 5.2 and 10 mm far from the probe tip is not sufficient to prevent its influence on the measurement. This should be taken into account in clinical practice by physicians during intravascular Doppler quantification. The new numerical approach used in this work could potentially be helpful in future numerical simulations to set plausible inlet boundary conditions.

For almost four decades, antimicrobial peptides have been studied, and new classes are being discovered. However, for therapeutic use of these molecules, issues related to the mechanism of action must be answered. In this work, the antimicrobial activity of the hairpinin MBP-1 was studied by the synthesis of two variants, one replacing cysteines and one tryptophan with alanine. Antibacterial activity was abolished in both variants. No membrane disturbance, even in concentrations higher than those required to inhibit the bacteria, was observed in SEM microscopy. The gel retardation assay showed that MBP-1 possesses a higher DNA-binding ability than variants. Finally, molecular modelling showed that the lack of cysteines resulted in structure destabilization and lack of tryptophan resulted in a less flexible peptide, with less solvent assessable surface area, both characteristics that could contribute to absence of activity. In summary, the data here reported add more information about the multiple mechanisms of action of α-hairpinins.

BACKGROUND: It is a known fact that blood flow pattern and more specifically the pulsatile time variation of shear stress on the vascular wall play a key role in atherogenesis. The paper presents the conception, the building and the control of a new in vitro test bench that mimics the pulsatile flows behavior based on in vivo measurements. METHODS: An in vitro cardiovascular simulator is alimented with in vivo constraints upstream and provided with further post-processing analysis downstream in order to mimic the pulsatile in vivo blood flow quantities. This real-time controlled system is designed to perform real pulsatile in vivo blood flow signals to study endothelial cells' behavior under near physiological environment. The system is based on an internal model controller and a proportional-integral controller that controls a linear motor with customized piston pump, two proportional-integral controllers that control the mean flow rate and temperature of the medium. This configuration enables to mimic any resulting blood flow rate patterns between 40 and 700 ml/min. In order to feed the system with reliable periodic flow quantities in vivo measurements were performed. Data from five patients (1 female, 4 males; ages 44-63) were filtered and post-processed using the Newtonian Womersley's solution. These resulting flow signals were compared with 2D axisymmetric, numerical simulation using a Carreau non-Newtonian model to validate the approximation of a Newtonian behavior. RESULTS: This in vitro test bench reproduces the measured flow rate time evolution and the complexity of in vivo hemodynamic signals within the accuracy of the relative error below 5%. CONCLUSIONS: This post-processing method is compatible with any real complex in vivo signal and demonstrates the heterogeneity of pulsatile patterns in coronary arteries among of different patients. The comparison between analytical and numerical solution demonstrate the fair quality of the Newtonian Womersley's approximation. Therefore, Womersley's solution was used to calculate input flow rate for the in vitro test bench.

RESUMEN

A label-free one-step lithographically masked deposition technique was implemented for the fabrication of gold nanoparticle (Au NP) micropatterns. These micropatterns serve as active substrates for surface-enhanced infrared absorption spectroscopy (SEIRAS) and exhibit a substantial increase in the IR signal upon adsorption of multiple proteins compared to untreated surfaces. Micro-FTIR chemical imaging was conducted to evaluate the efficacy of Au NP micropatterns as singular enhancers for SEIRAS across diverse IR-active substrates demonstrating a promising application for the detection of proteins at low concentrations within biological fluids.
