PURPOSE: Postoperative education and symptom tracking are essential following cystectomy to reduce readmission rates and information overload. To address these issues, an internet-based tool was developed to provide education, alerts, and symptom tracking. We aimed to evaluate the tool's feasibility, acceptability, and impact on complication and readmission rates. MATERIALS AND METHODS: Thirty-three eligible patients over 18 years old scheduled for cystectomy were enrolled. Patients were asked to use the mobile health (mHealth) tool daily for the first 2 weeks, then less frequently up to 90 days after discharge. Descriptive statistics were used to summarize study variables. Feasibility was defined as at least 50% of patients using the tool once a week, and acceptability as patient satisfaction of > 75%. RESULTS: Use of the mHealth tool was feasible, with 90% of patients using it 1 week after discharge, but engagement declined over time to 50%, with technological difficulties being the main reason for nonengagement. Patient and provider acceptability was high, with satisfaction > 90%. Within 90 days, 36% experienced complications after discharge and 30% were readmitted. Engagement with the mHealth application varied but was not statistically associated with readmission (P = .21). CONCLUSIONS: The study showed that the electronic mobile health intervention for patients undergoing cystectomy was feasible, acceptable, and provided valuable educational content and symptom management. Future larger studies are needed to determine the tool's effectiveness in improving patient outcomes and its potential implementation into routine clinical care.

INTRODUCTION AND HYPOTHESIS: The aim of this study was to describe patient experiences with an enhanced recovery protocol (ERP) after minimally invasive sacrocolpopexy (SCP). METHODS: We conducted 14 semi-structured telephone interviews with women who had undergone SCP and were discharged the day of the surgery (POD#0, n = 7) or spent 1 night at the hospital (POD#1, n = 7). Interviews occurred between 2 and 6 weeks after surgery. We explored the following topics: patient preparedness, preoperative education, physical recovery, emotional recovery, and overall perception of the ERP. Interviewing continued until thematic saturation was achieved. All interviews were recorded, transcribed, and analyzed to identify relevant themes. RESULTS: Both POD#0 and POD#1 discharge patients reported largely positive experiences regarding their preparation for surgery, at-home recovery, and access to follow-up care. Yet for some patients, the accelerated pace of the ERP felt rushed and was perceived as an absence of care rather than as an advance in treatment. Patients that elected to stay the night lived farther from ready access to care, had less robust systems of postoperative support, and worried more about the management of their pre-existing conditions. CONCLUSIONS: Our research found that despite having consistently positive impressions of the ERP, patients shared common anxieties surrounding their surgeries including worries about access to care and the prospect of going home with a urinary catheter. Furthermore, we found that the therapeutic value of protocol recommendations such as early discharge must be made explicit to patients and often individualized to avoid being interpreted as sub-standard care.

In bladder urothelial carcinoma, ERBB2 mutations have been associated with favorable response to platinum-based neoadjuvant chemotherapy. However, this association has not been reported in upper tract urothelial carcinoma (UTUC). We describe an excellent response to cisplatin-based chemotherapy in metastatic UTUC with an ERBB2 mutation. Our patient is a 54-year-old female with metastatic UTUC who received systemic cisplatin and gemcitabine. Postchemotherapy imaging demonstrated decreased size of pyelocaliceal mass and decreased retroperitoneal adenopathy compared to initial imaging. Surgical pathology from consolidative resection showed 3 mm residual renal tumor and no viable lymph node disease. Genomic testing demonstrated an ERBB2 gain of function mutation.

PURPOSE: Hydronephrosis is a common antenatal diagnosis and is present in approximately 1-5% of pregnancies. The urinary tract dilation (UTD) classification system was introduced in 2014 and stratifies post-natal hydronephrosis risk into three groups: low-risk (P1), intermediate-risk (P2), and high-risk (P3). Recommendations for P3 hydronephrosis have been established, whereas those for P1 and P2 UTD are often left to the discretion of providers with P1 considered low-grade and less concerning significant pathology. Given the obscure nature of P2 hydronephrosis, we sought to determine the natural history and outcomes of pediatric patients with P2 hydronephrosis within a single institution. MATERIALS AND METHODS: Children <18 years old diagnosed with hydronephrosis between January 2015 and December 2018 were identified by ICD-9 and ICD-10 codes. Patients with P1 hydronephrosis, P3 hydronephrosis, known vesicoureteral reflux, complex anomalies (ex. Posterior urethral valve), neurological impairments, neurogenic bladder secondary to spinal abnormalities, and <6 months of follow-up were excluded. The development of urinary tract infection (UTI; ≥100 000 CFU/mL of bacterial growth, UA > 10 WBCs/hpf with fever >38C), need for surgical intervention (impaired renal function, worsening hydronephrosis, and/or delayed drainage on diuretic renography), and stability of hydronephrosis were collected retrospectively. RESULTS: Eighty-seven patients [105 renal units (RU)] were included. Twenty-six patients (30%) were female and 61 (70%) were male. Of the male patients, 30 (49%) of them were circumcised. The median age at initial evaluation was 1 month, and the median duration of follow-up was 13 months. Thirty-four (32%) RU had complete resolution, 24 (23%) improved to P1 hydronephrosis, 33 (31%) remained stable, and 14 (13%) progressed to P3 hydronephrosis. The median duration to resolution and improvement was 8.5 months and 5 months, respectively. Eleven (11%) RU required surgical intervention, 10 of which underwent pyeloplasty, with a median duration to intervention of 9 months. Fifty-five patients (63%) received antibiotic prophylaxis (amoxicillin or sulfamethoxazole-trimethoprim) for a median duration of 5 months. Nine patients (10%) developed a UTI, 3 of which were taking antibiotic prophylaxis at the time of infection. CONCLUSIONS: Intermediate-risk hydronephrosis diagnosed in the pediatric population will either improve, resolve, or remain stable during 1-year follow-up in 87% of RU. Only 11% of RU required surgical intervention, and 19% of patients developed a UTI in the absence of antibiotic prophylaxis. These findings will assist with counseling parents concerning the importance of follow-up imaging and monitoring for UTI. However, the low risk of surgical intervention is encouraging and should be discussed with the children's caretakers.

OBJECTIVE: The objective of this study was to evaluate whether an enhanced recovery after surgery (ERAS) protocol was associated with a higher rate of same-day discharge after robot-assisted or laparoscopic sacrocolpopexy and to describe the safety and feasibility of same-day discharge after these procedures. METHODS: A historical control, retrospective cohort study of women undergoing minimally invasive sacrocolpopexy comparing rates of same-day discharge before and after implementation of an ERAS protocol was conducted. Secondary outcomes were obtained by comparing women discharged the same day with those discharged postoperative day ≥1, including postoperative complications and unplanned postoperative patient encounters within 30 days of surgery. Logistic regression was performed to control for potential confounders. RESULTS: Of the 166 women identified (83 before ERAS implementation; 83 after ERAS implementation), 43 underwent same-day discharge versus 123 admitted overnight. The rate of same-day discharge increased 28 percentage points after ERAS implementation (12% vs 40%, P < 0.01). Compared with women admitted overnight, same-day discharge women had shorter procedures (154 vs 173 minutes, P = 0.01), spent longer time in the postanesthesia care unit (130 vs 106 minutes, P = 0.01), and were more likely to be discharged with a Foley catheter (58% vs 28%, P < 0.01). After multivariable logistic regression analysis, ERAS was associated with increased odds of same-day discharge (odds ratio, 4.91; 95% confidence interval, 2.17-11.09). There were no differences in unplanned postoperative patient contacts or postoperative complications within 30 days between same-day discharge and overnight admission groups. CONCLUSIONS: Implementation of an ERAS protocol for minimally invasive sacrocolpopexy was associated with a 3-fold increase in same-day discharge.

OBJECTIVES: To evaluate the effect of vaginal intraoperative infiltration of liposomal bupivacaine on vaginal pain among patients undergoing posterior colporrhaphy and perineorrhaphy. METHODS: This was a randomized, placebo-controlled trial offered to women undergoing posterior colporrhaphy and perineorrhaphy with concomitant pelvic reconstructive procedures. Liposomal bupivacaine or normal saline placebo (30 mL) was injected into the posterior vaginal compartment and perineal body in 2- to 3-mL increments, using a systematic technique. All participants received 10-mL 0.25% bupivacaine with epinephrine prior to incision. Perioperative care was standardized. The primary outcome was vaginal pain as measured by a visual analog scale. RESULTS: One hundred twenty-six women were screened, and 72 were included. Demographic characteristics were similar. Median visual analog scale was not different at any time point (P = 0.81). There were no differences in secondary outcomes, including narcotic use (37.5 vs 37.5 mg morphine equivalents, P = 0.51; placebo vs liposomal bupivacaine), time to first opioid (68 vs 89.5 minutes, P = 0.56), antiemetic doses (3 vs 2, P = 0.07), hospital length of stay (24 vs 21.9 hours, P = 0.98), length of stay in postanesthesia care unit (93 vs 100 minutes, P = 0.32), proportion of patients who had a bowel movement within the first 3 postoperative days (65.7 vs 59.5% P = 0.36), or successful voiding trials (45.7 vs 59.5%, P = 0.24). There were no differences in patient satisfaction or postoperative adverse events. CONCLUSIONS: In this study of pelvic reconstructive surgeries with posterior colporrhaphy and perineorrhaphy, there were no differences in pain scores or any secondary outcomes between liposomal bupivacaine and placebo injected into the posterior vaginal compartment.

Duplex collecting system of the kidney is a common congenital anomaly of the urinary tract and is less reported in the adult population. Rarely, this anomaly can result in ureterovesical junction compression. Herein, we present a case of ureterovesical junction compression occurring in an adult patient with a duplex collecting system and describe the surgical management.

Anastomosing hemangiomas are rare variants of vascular tumors found in adrenal, hepatic, and gastrointestinal tissue. Frequently, renal anastomosing hemangiomas are misdiagnosed on computed tomography (CT) as kidney cancers, resulting in unnecessary workups and detrimental treatments. We present a rare case of bilateral renal and adrenal anastomosing hemangioma found incidentally on renal biopsy. Patient is a 39 year-old African American male on hemodialysis with a history of end-stage renal disease secondary to lupus who presented with acute pericarditis and worsening renal insufficiency.
