Objective: To analyze the dynamics of the spatial and temporal distribution of visceral leishmaniasis in Brazil from 2007 to 2020. Method: This ecological study focused on the spatial pattern of visceral leishmaniasis cases in Brazil from 2007 to 2020 using data from the Information System for Notifiable Diseases (SINAN). The variables analyzed were the incidence rate of visceral leishmaniasis and the visceral leishmaniasis composite indicator (VLCI), from which triennial thematic maps were constructed. The global Moran index was calculated to assess the existence of spatial autocorrelation, and the local Moran index was used to identify areas with higher and lower risk of visceral leishmaniasis in Brazil. Results: During the study period, there were 48 705 cases of visceral leishmaniasis, with an incidence rate of 25.53 cases per 100 000 population. There was spatial autocorrelation in all triennial blocks, with municipalities in the North and Northeast regions having an incidence equal to or greater than 50 cases per 100 000 population. Regarding the VLCI there was an increase in the number of municipalities classified as low risk for transmission, and a growing presence of state capitals with a classification of very high risk. Conclusion: The incidence of visceral leishmaniasis varied over the trienniums. The border region between the states of Tocantins, Maranhão, and Pará, along with the state of Ceará, stood out in the spatial distribution of the disease incidence and risk stratification by VLCI. These areas should be a priority for surveillance and control efforts for the disease.


Objetivo: Analizar la dinámica de la distribución espaciotemporal de la leishmaniasis visceral en Brasil en el período 2007-2020. Métodos: En este estudio ecológico del patrón de distribución espacial de los casos de leishmaniasis visceral ocurridos en Brasil en el período 2007-2020 se utilizaron datos del sistema de información de enfermedades de declaración obligatoria (SINAN). Las variables analizadas fueron el coeficiente de incidencia de leishmaniasis visceral y el índice compuesto de leishmaniasis visceral, a partir de los cuales se construyeron mapas temáticos trienales. Se calcularon el índice general de Moran para verificar la existencia de autocorrelación espacial, y el índice local de Moran para delimitar las zonas de mayor y menor riesgo de leishmaniasis visceral en Brasil. Resultados: En el período estudiado, hubo 48 705 casos de leishmaniasis visceral, con una incidencia de 25,53 casos por 100 000 habitantes. En todos los bloques trienales hubo una autocorrelación espacial, en la cual varios municipios de las regiones norte y nordeste registraron una incidencia de 50 casos por 100 000 habitantes o una tasa superior. En relación con el índice compuesto de leishmaniasis visceral, se observó un aumento del número de municipios considerados de riesgo bajo de transmisión y una tendencia ascendente del número de ciudades capitales con clasificación de riesgo muy alto. Conclusión: La incidencia de leishmaniasis visceral varió a lo largo de los períodos trienales. La región fronteriza entre Tocantins, Maranhão y Pará, junto con el estado de Ceará, se destacó en términos de la distribución espacial de la incidencia de la enfermedad y la estratificación del riesgo según el índice compuesto de leishmaniasis visceral. Es preciso dar prioridad a estos lugares en lo referente a las medidas y los servicios de vigilancia y control de esta enfermedad.

Objective: To describe the spatial distribution of tuberculosis-diabetes comorbidity and identify the social determinants of the double burden of disease in the period from 2012 to 2018 in Brazil. Method: In the present ecological study, municipalities were the unit of analysis. All cases of tuberculosis reported from 2012 to 2018 to the National Notifiable Disease Information System SINAN were included. Socioeconomic variables covering employment, income, and development, and the primary care coverage indicator were analyzed as determinants. The global Moran's I statistic was used to verify spatial autocorrelation in the comorbidity rate. The local Moran statistic was used to delimit tuberculosis-diabetes clusters: high/high cluster (municipalities with high rates of tuberculosis-diabetes comorbidity with neighboring municipalities also presenting high comorbidity rates) and low/low cluster (municipalities with tuberculosis-diabetes comorbidity below the mean, surrounded by municipalities with low comorbidity rates). Results: A high proportion of tuberculosis-diabetes was detected in most Brazilian regions. Spatial autocorrelation was observed for tuberculosis-diabetes comorbidity, as well as a high-high comorbidity cluster encompassing 88 municipalities located mostly in the Northeast, Southeast, and South, with mean tuberculosis-diabetes prevalence of 28.04%. The variables "percent population living in households with more than two people per bedroom," "percent unemployment in the population above 18 years of age" and "per capita income" were associated with the presence of comorbidity. Conclusion: The results showed a non-random distribution of tuberculosis-diabetes comorbidity, with high-risk areas and associated explanatory variables. The findings underscore the need to operationalize cooperation between tuberculosis and diabetes programs, with the aim of controlling both the individual diseases and the tuberculosis-diabetes syndemic.


Objetivo: Describir la distribución espacial de la comorbilidad tuberculosis-diabetes y determinar los determinantes sociales de la doble carga entre el 2012 y el 2018 en Brasil. Métodos: En este estudio ecológico se utilizaron los municipios como unidad de análisis. Se incluyeron todos los casos de tuberculosis notificados al Sistema de Información sobre Enfermedades de Notificación Obligatoria desde el 2012 hasta el 2018. Como determinantes, se analizaron variables socioeconómicas de empleo, ingresos y desarrollo y el indicador de cobertura de la atención básica. Se calculó el índice global de Moran para verificar la existencia de autocorrelación espacial de la comorbilidad. Se utilizó el índice local de Moran para delimitar los conglomerados de tuberculosis-diabetes: alto/alto (municipios con una proporción elevada de tuberculosis-diabetes, cuyos vecinos también presentaban una proporción elevada) y bajo/bajo (municipios con una proporción inferior a la media, rodeados por municipios con una proporción baja). Resultados: Se observó un aumento de la proporción de casos de tuberculosis-diabetes en la mayoría de las regiones brasileñas. Se verificó la existencia de una autocorrelación espacial de la comorbilidad tuberculosis-diabetes y se identificó un conglomerado alto-alto de tuberculosis-diabetes en 88 municipios, pertenecientes principalmente a las regiones Nordeste, Sudeste y Sur, con una prevalencia media de comorbilidad del 28,04%. Las variables "porcentaje de la población residente en viviendas con una densidad de ocupación superior a dos personas por habitación", "porcentaje de desempleo de las personas mayores de 18 años" e "ingresos per cápita" guardaron relación con la aparición de comorbilidad. Conclusión: Los resultados mostraron una distribución no aleatoria de la comorbilidad tuberculosis-diabetes, con zonas de alto riesgo y variables explicativas de su ocurrencia. Estos resultados refuerzan la necesidad de operacionalizar la colaboración entre los programas contra la tuberculosis y la diabetes, con miras a controlar tanto cada enfermedad en forma aislada como la sindemia.

INTRODUCTION AND OBJECTIVES: Research has shown that hepatitis B (HBV) and Delta virus (HDV) are a worldwide public health problem. This study aims to estimate the prevalence rates of HBV and HDV infection in five municipalities of Maranhão, Northeastern Brazil. MATERIALS AND METHODS: A total sample between 3856 and 4000 individuals. Questionnaires were used to register sociodemographic characteristics and factors associated with transmission. Patients were tested for hepatitis B virus surface antigen (HBsAg), anti-hepatitis B core antigen (anti-HBc), and antibodies against hepatitis Delta virus (anti-HDV). Factors associated with HBV were detected by means of multivariate Poisson regression. RESULTS: Overall, 3983 subjects were included. Ninety-two of the participants were HBsAg-positive (2.30%, 95% CI 1.80-2.80), and anti-HBc was detected in 1535 (38.50%, 95% CI 37-40). The factors associated with the presence of anti-HBc were: (1) Municipality (P<0.001); Age (P<0.001); School education (P<0.001); Illicit drug use (P=0.001); non-HBV vaccine (P=0.041). Among the HBsAg carriers, eight were anti-HDV-positive (8.69%, 95% CI 2.90-14.40). The most frequent HBV genotype was D4. The only HDV genotype was HDV-8. CONCLUSION: HBV exhibited intermediate endemicity in the studied region. Traditional factors were associated with exposure to the virus. The presence of the HDV was confirmed. The most frequent HBV and HDV genotypes were unlike the ones currently described in Brazil.

RESUMEN

OBJECTIVE: To describe the construction aboutthe (lack of) knowledge and stigma of leprosy by Community Health Workers participating in the Culture Circle. METHOD: Qualitative, action-research type study supported by the Paulo Freire Culture Circle framework, carried out with 21 Community Health Workers. Data collected in November 2021, in the municipality of São Luís, Maranhão. The following categories were evidenced: knowledge about leprosy, signs and symptoms, stigma. RESULTS: The participants had knowledge about the disease, but they verbalized people's disinformation about leprosy, disbelief in relation to the cure, and situations of prejudice and stigma that are still present today. FINAL CONSIDERATIONS: The culture circle enabled the intertwining of scientific and empirical knowledge in the construction of a critical and reflective knowledge committed to welcoming and comprehensive care for people and families affected by leprosy.

It is not well established whether cytokine production differs in response to different clinical forms of visceral leishmaniasis (VL). In this work, we performed a cross-sectional study to investigate the plasma levels of cytokines [interferon (IFN)-γ, tumour necrosis factor (TNF)-α, interleukin (IL)-2, IL-4, IL-10 and IL-12] involved in the pathogenesis of VL in 80 subjects from VL endemic areas, including subjects with active VL, subjects with asymptomatic infection, subjects with cured VL and uninfected controls. The patients were recruited by sampling from a referral hospital and by random selection from a population-based cohort study. The results showed significant differences in the plasma concentration of all cytokines between the groups (p < 0.05). Patients with the active disease had higher plasma levels of IL-10, IL-4, INF-γ and TNF-α relative to the other groups and they produced more IL-12 than asymptomatic and cured subjects. Only the IL-2 concentration was higher in the asymptomatic and cured subjects relative to the patients with active disease (p < 0.05). Our results suggest that these cytokines can be used as markers in epidemiological studies conducted in endemic areas to distinguish between different clinical forms of VL. However, their usefulness should be confirmed in investigations conducted in other endemic areas.

Epidemiological studies have demonstrated that the variability of the clinical response to infection caused by Mycobacterium leprae is associated with host genetic factors. The present study investigated the frequency of human leukocyte antigen (HLA) class II (DRB1) alleles in patients with leprosy from São Luís, Maranhão, Brazil. A case-control study was performed in 85 individuals with leprosy and 85 healthy subjects. All samples were analysed via polymerase chain reaction-sequence specific oligonucleotide probes. The HLA-DRB1*16 allele showed a higher frequency in the group with leprosy [(9.41% vs. 4.12%) odds ratio (OR) = 2.41 95% confidence interval (CI) (0.96-6.08) p = 0.05], whereas the HLA-DRB1*11 allele was less frequent in the group with leprosy [(6.47% vs. 11.76%) OR = 0.51 95% CI (0.23-1.12) p = 0.09]. The frequency of HLA-DRB1* alleles between the control group and leprosy patient subgroups presenting different forms of the disease showed that the HLA-DRB1*16 (16.13% vs. 8.24%, OR = 4.10, CI = 1.27-13.27, p = 0.010) and HLA-DRB1*14 (5% vs. 3.53%, OR = 4.63, CI = 1.00-21.08, p = 0.032) alleles were significantly more frequent in patients with different clinical subtypes of leprosy. The sample size was a limitation in this study. Nevertheless, the results demonstrated the existence of a genetic susceptibility associated with the clinical forms of leprosy. The low frequency of the HLA-DRB1*11 allele should be further studied to investigate the possible protective effect of this allele.

Ecological study that analyzes the trend and the spatiotemporal distribution of new cases of tuberculosis (TB) that abandoned treatment in Brazil, notified in the Notifiable Diseases Information System, in the period from 2012 to 2018. For the study of the trend used the Prais-Winsten generalized linear regression model was used and the Moran Global and Local indices for spatial analysis. The mean and median proportion of TB treatment dropout in Brazil was 10.4%. The highest averages of the indicator were concentrated in the Southeast (10.78 ± 1.38), South (10.70 ± 2.94) and North (10.35 ± 1.13), and; in the states of Rondônia (14.35 ± 2.34), Rio Grande do Sul (13.60 ± 4.23) and Rio de Janeiro (12.64 ± 1.73), only Acre and Piauí showed this indicator below 5%. In Brazil, there was a tendency towards stability in the proportion of abandonment of TB treatment, a decrease in the North, Northeast and South regions and only in the Federal District there was growth. There was a heterogeneous and non-random distribution, with five capitals comprising the High-Risk cluster. We conclude that the proportion of TB treatment abandonment in Brazil is above what is acceptable and that the identification of high-risk areas can contribute to the elaboration and strengthening of more specific control actions.


Estudo ecológico que analisa a tendência e a distribuição espaço-temporal dos casos novos de tuberculose (TB) que abandonaram o tratamento no Brasil, notificados no Sistema de Informação de Agravos de Notificação no período de 2012 a 2018. Para o estudo da tendência, utilizou-se o modelo de regressão linear generalizada de Prais-Winsten, e para a análise espacial os índices global e local de Moran. A média e a mediana da proporção de abandono do tratamento da TB no Brasil foram de 10,4%. As maiores médias do indicador se concentraram nas regiões Sudeste (10,78 ± 1,38), Sul (10,70 ± 2,94) e Norte (10,35 ± 1,13), e nos estados de Rondônia (14,35 ± 2,34), Rio Grande do Sul (13,60 ± 4,23) e Rio de Janeiro (12,64 ± 1,73). Apenas Acre e Piauí apresentaram esse indicador abaixo de 5%. No Brasil, houve tendência de estabilidade na proporção do abandono do tratamento da TB, decréscimo nas regiões Norte, Nordeste e Sul, e somente no Distrito Federal houve crescimento. Verificou-se distribuição heterogênea e não aleatória, com cinco capitais compondo o cluster de alto-risco. Concluímos que a proporção de abandono do tratamento da TB no Brasil encontra-se acima do aceitável e que a identificação de áreas de alto risco pode contribuir para a elaboração e fortalecimento de ações de controle mais específicas.

Leishmania infection in humans, dogs and sandflies was examined in the endemic visceral leishmaniasis (VL) municipality of Raposa, state of Maranhão, Brazil. In this study, we examined Leishmania chagasi infection in the blood serum of both humans and Canis familiaris and the natural Leishmania sp. infection rate in the sandfly vector, Lutzomyia longipalpis. Enzyme-linked immunosorbent assay, indirect immunofluorescence reaction and polymerase chain reaction were performed to detect Leishmania infections in humans, dogs and sandflies, respectively. Overall, 186 out of 986 studied human beings were infected with L. chagasi parasites, representing an infection prevalence of 18.9%. An even higher infection rate was detected in dogs, where 66 (47.8%) out of 138 were infected. Among all Lu. longipalpis captured (n = 1,881), only 26.7% were females. The Leishmania infection frequency for the vector Lu. longipalpis was 1.56%. Remarkably, all infected sandflies were found in the peridomiciliary area. Furthermore, a high incidence of asymptomatic forms of VL in the human and canine populations was observed. The results of this study suggest autochthonous transmission of L. chagasi in this endemic area for visceral leishmaniasis because infection by Leishmania sp. was identified in all important elements of the transmission chain.

OBJECTIVE: Analyze the spatio-temporal distribution of AIDS cases in Maranhão. METHODS: Ecological study of AIDS cases in the Notifiable Diseases Information System, 2011-2018. Gross and adjusted incidences were calculated using the Baysean method; then, the Moran Global and Local Indices to observe the existence of spatial autocorrelation of the cases and for the delimitation of high and low risk clusters. RESULTS: 6,349 cases were reported, which were distributed heterogeneously. There was an advance of cases to new areas and persistence in old areas, such as in the capital São Luís and its surroundings. The dissemination did not occur at random, with positive spatial autocorrelation, with evidence of the formation of clusters in the municipalities of São Luís, São José de Ribamar and Paço do Lumiar. CONCLUSION: High-risk areas have been identified and should be considered a priority for investment in health, management, and organization of health services.

INTRODUCTION: Accidents involving biological material are the main cause of occupational exposure among health care professionals. OBJECTIVES: To analyze trends in the number of accidents involving exposure to biological material among health care workers in Brazil, in the period of 2010 to 2016. METHODS: This was an ecological study based on secondary data on occupational accidents involving biological material reported to the Information System for Notifiable Diseases (Sistema Nacional de Agravos de Notificação). The data were analyzed using descriptive methods, followed by a calculation of incidence rates per 1,000 workers per year. Lastly, trend analysis was performed using Prais-Winsten regression. RESULTS: A total of 243,621 accidents involving exposure to biological materials were reported among health professionals in the study period. The highest incidence rate (16.74 accidents per 1,000 workers per year) was observed in 2014. Regional analyses showed that Midwestern, South and Southeast Brazil had higher incidence rates than other regions of the country. At the state level, the highest rates were observed in Roraima, Rio Grande do Norte, Alagoas, Goiás, Minas Gerais, São Paulo, Paraná, and Santa Catarina. National incidence rates of occupational accidents with exposure to biological material in health care workers had a stable trend over the study period. CONCLUSIONS: In Brazil, health care workers are disproportionately affected by occupational accidents with exposure to biological material. The present findings, together with other indicators, cast doubt on the stability of these figures over time.

INTRODUCTION: Personal protective equipment creates a protective barrier for mucous membranes, airways, and skin in situations with possible exposure to biological material. OBJECTIVES: To analyze temporal trends in the use of personal protective equipment at the time of accidental exposure to biological materials in Brazilian health care workers. METHODS: This was an ecological time series study of the use of personal protective equipment in health professionals who experienced accidental exposure to biological materials. Data were analyzed by region and federal unit, as well as in the country as a whole. Temporal trends were investigated using Prais-Winsten regression models with calendar year as the independent variable. RESULTS: The use of personal protective equipment increased by 4.62% at a national level. Increases were also observed in all regions of the country. Temporal trends within federal units, however, showed significant heterogeneity. CONCLUSIONS: Though the use of personal protective equipment increased in the country as a whole, the rate of change varied widely between federal units. These findings highlight the need for changes in public policy and the implementation of permanent education programs for health care professionals, especially in regions with lower or stable rates of personal protective equipment use.

BACKGROUND: Beriberi is the clinical manifestation of thiamine deficiency. It is multicausal and typically associated with poverty and food insecurity among vulnerable populations, such as indigenous people. OBJECTIVE: The objective of this study was to carry out a spatial analysis of reported cases of beriberi among indigenous people in Brazil. METHODS: Cross-sectional study using time series data on suspected cases of beriberi reported to the Ministry of Health via the FormSUS between July 2013 and September 2018. Indigenous villages were georeferenced, and Kernel density estimation was used to identify patterns of the spatial distribution of beriberi cases. RESULTS: A total of 414 cases of beriberi were reported in the country of which 210 (50.7%) were indigenous people. All the cases in indigenous people occurred in states located in the Legal Amazon (Maranhão, Roraima, and Tocantins). Kernel density estimation showed high-density areas in Tocantins and Roraima. CONCLUSIONS: This is the first nationwide study of reported cases of beriberi. The findings can be used to guide actions that contribute to the monitoring and prevention of beriberi among indigenous people.

This paper aims to analyze the trend of tuberculosis (TB) in Brazilian indigenous people from 2011 to 2017. This ecological study was carried out with all new TB cases in indigenous people reported in the Notifiable Diseases Information System during the 2011-2017 period. Thematic maps were prepared to monitor the spatial-temporal evolution of TB in the indigenous population each year, and the Prais-Winsten generalized linear regression model was used to analyze the trend. A total of 6,520 TB cases were reported in indigenous people during the study period. The overall incidence of TB in Brazilian indigenous people for the period was 109/100,000 inhabitants. In the analysis by region of the country, the highest incidence occurred in the Midwest, North, and Southeast regions. In the UF analysis, the highest incidence was found in Mato Grosso, São Paulo, Rondônia, Mato Grosso do Sul, Acre, Maranhão, Pará, and Rio de Janeiro. The trend of the disease in the indigenous was stable both in the country and in most of its regions and UFs. TB disproportionately affects Brazilian indigenous people, and this study can contribute to the elaboration and strengthening of more specific control actions by identifying priority regions and UFs.


O objetivo deste artigo é analisar a tendência da tuberculose (TB) em indígenas no Brasil no período de 2011-2017. Trata-se de um estudo ecológico realizado com todos os casos novos de TB em indígenas notificados no Sistema de Informação de Agravos de Notificação, durante o período 2011 a 2017. Foram confeccionados mapas temáticos para acompanhamento da evolução espaço-temporal da TB na população indígena em cada ano. Para o estudo da tendência utilizou-se o modelo de regressão linear generalizada de Prais-Winsten. No período em estudo, foram notificados 6.520 casos de TB em indígenas. A incidência geral de TB em indígenas no Brasil foi de 109/100.000 habitantes. Na análise por regiões do país, as maiores incidências ocorreram nas regiões Centro-Oeste, Norte e Sudeste. Na análise por UF, as maiores incidências foram encontradas no Mato Grosso, São Paulo, Rondônia, Mato Grosso do Sul, Acre, Maranhão, Pará e Rio de Janeiro. A tendência da doença nos indígenas foi estável tanto no país quanto na maioria das suas regiões e UF. A TB afeta desproporcionalmente os indígenas brasileiros e o presente estudo ao identificar regiões e UF prioritárias pode contribuir para a elaboração e fortalecimento de ações de controle mais específicas.

The goal of this study was to investigate the prevalence and factors associated with workplace accidents among hospital cleaning professionals. This is a cross-sectional, analytical study of a sample of 199 cleaning workers at six hospitals in São Luís, Maranhão, Brazil. We first ran a univariate analysis, and later the Poisson regression analyzes with robust variance were used, with hierarchized modeling of the data in order to estimate the prevalence ratios (PR) between the independent variables and the outcome (workplace accidents). The prevalence of workplace accidents is 13.57%, 81.48% of them with sharps. At the end of the multivariate analysis the following were found to be associated with workplace accidents: age between 18 and 30, not having a high-school degree, on the job for 2 to 5 years, inadequate segregation of health service waste (HSW), training only on admission, failure to consider the health hazards of hospital waste and failure to use personal protective equipment (PPE). Our findings reinforce the importance of continued health education, stressing periodic training and the use of PPE.


O objetivo deste estudo foi investigar a prevalência e os fatores associados aos acidentes de trabalho entre profissionais da limpeza hospitalar. Foram entrevistados 199 trabalhadores da limpeza de seis hospitais em São Luís, Maranhão, Brasil. Primeiramente, realizou-se análise univariada e, posteriormente, utilizaram-se as análises de regressão de Poisson com variância robusta, com modelagem hierarquizada dos dados a fim de estimar razões de prevalências (RP) entre as variáveis independentes e o desfecho (acidentes de trabalho). Houve prevalência de 13,57% de acidentes de trabalho, destes, 81,48% ocorridos com materiais perfurocortantes. Ao final da análise multivariada, identificaram-se fatores associados á ocorrência de acidentes de trabalho: idade de 18 a 30 anos, ter ensino médio incompleto, tempo de serviço de 2 a 5 anos, segregação inadequada dos resíduos de serviços de saúde (RSS), ter capacitação somente na admissão, não considerar os riscos dos resíduos à saúde e o não uso de equipamento de proteção individual (EPI). Os fatores avaliados apresentaram-se associados significativamente aos acidentes de trabalho e ressaltam a importância da educação permanente em saúde, com ênfase em capacitações periódicas e uso de EPI.

Introdução: A tuberculose é um importante problema de saúde pública e atinge de maneira dessemelhante os espaços geográficos. Objetivo: analisar a distribuição espacial dos óbitos por tuberculose no Maranhão (Brasil) e sua associação com indicadores socioeconômicos. Método: Estudo ecológico dos óbitos por tuberculose no Maranhão, no período de 2010 a 2015, com unidade de análise, os municípios. Utilizou-se o índice de Moran global, correlograma e espalhamento do índice de Moran e para avaliar a associação espacial de óbitos por tuberculose com os indicadores socioeconômicos foram ajustados diferentes modelos espaciais condicionais autoregressivos. Resultados: No período, foram notificados 949 óbitos por tuberculose. Identificou-se uma forte dependência espacial na ocorrência dos óbitos. Por meio do modelo autoregressivo ajustado, constatou-se que aproximadamente 12% dos municípios possuem mais que 75% de chance de ocorrer um óbito por tuberculose e que o norte e a parte central do Estado são as regiões que concentram as maiores probabilidades de óbito. Conclusão: Houve forte dependência espacial na ocorrência de óbitos por TB, sendo esta afetada pelas taxas das áreas vizinhas. O índice de desenvolvimento humano municipal esteve negativamente associado com a taxa de óbitos por tuberculose e a renda média per capita apresentou associação positiva.


Introducción: La tuberculosis es un importante problema de salud pública y afecta a diferentes áreas geográficas. Objetivo: Analizar la distribución espacial de las muertes por tuberculosis en Maranhão (Brasil) y su asociación con indicadores socioeconómicos. Método: Estudio ecológico de las muertes por tuberculosis en Maranhão con municipios como unidad de análisis, entre 2010 y 2015. Se utilizaron el índice global de Moran, el correlograma y la dispersión del índice de Moran. Además, para evaluar la asociación espacial de las muertes por tuberculosis con los indicadores socioeconómicos, se ajustaron diferentes modelos espaciales autorregresivos condicionales. Resultados: Durante el período, se reportaron 949 muertes por tuberculosis. Se identificó una fuerte dependencia espacial en la ocurrencia de muertes. Utilizando el modelo espaciales autorregresivos condicionales ajustado, se encontró que, aproximadamente, el 12 % de los municipios tienen más del 75 % de probabilidad de muerte por tuberculosis. Además, que el norte y centro del estado son las regiones con mayor probabilidad de muerte por tuberculosis. Conclusión: Existió una fuerte dependencia espacial de la ocurrencia de muertes por tuberculosis, la cual fue afectada por las tasas en las áreas vecinas. El índice de desarrollo humano municipal se asoció negativamente con la tasa de muertes por tuberculosis y el ingreso per cápita promedio mostró una asociación positiva.


Introduction: Tuberculosis is an important public health problem that affects different geographical areas. Objective: To analyze the spatial distribution of deaths from tuberculosis in Maranhão (Brazil) and its association with socioeconomic indicators. Method: Ecological study of tuberculosis deaths in Maranhão, from 2010 to 2015, using municipalities as the unit of analysis. The global Moran index, the correlogram, and the spread of the Moran index were used, and to assess the spatial association of tuberculosis deaths with socioeconomic indicators, different conditional autoregressive spatial models (CAR) were adjusted. Results: During the chosen period, 949 deaths from tuberculosis were reported. A strong spatial dependence was identified in the occurrence of deaths. Using the adjusted conditional autoregressive spatial model, it was found that approximately 12 % of the municipalities have more than a 75 % chance of death from tuberculosis and that the north and central parts of the state have the highest probability of death from tuberculosis. Conclusion: There was a strong spatial dependence on the occurrence of deaths from tuberculosis, which was affected by the rates in neighboring areas. The municipal human development index was negatively associated with the rate of deaths from tuberculosis and the average per capita income showed a positive association.

Este estudo ecológico do padrão espacial dos casos de leishmaniose visceral ocorridos no Brasil de 2007 a 2020 utilizou dados do Sistema de Informação de Agravos e Notificação (SINAN). As variáveis analisadas foram o coeficiente de incidência da leishmaniose visceral e o índice composto da leishmaniose visceral (ICLV), a partir das quais foram construídos mapas temáticos trienais. Foi calculado o índice de Moran global, para verificar a existência de autocorrelação espacial, e o índice de Moran local, para delimitar áreas de maior e menor risco para adoecimento por leishmaniose visceral no Brasil. Resultados. No período estudado, ocorreram 48 705 casos de leishmaniose visceral, com incidência de 25,53 casos/100 000 habitantes. Em todos os blocos trienais, houve autocorrelação espacial, havendo muni- cípios das regiões Norte e Nordeste com incidência igual ou superior a 50 casos/100 000 habitantes. Com relação ao ICLV, houve aumento do número de municípios considerados de baixo risco de transmissão e presença crescente do número de capitais com classificação de risco muito intenso. Conclusão. A incidência de leishmaniose visceral variou ao longo dos triênios. A região de fronteira entre Tocantins, Maranhão e Pará, juntamente com o estado do Ceará, destacou-se na distribuição espacial da incidência da doença e na estratificação de risco pelo ICLV. ABSTRACT Objective: To describe the construction aboutthe (lack of) knowledge and stigma of leprosy by Community Health Workers participating in the Culture Circle. Method: Qualitative, action-research type study supported by the Paulo Freire Culture Circle framework, carried out with 21 Community Health Workers. Data collected in November 2021, in the municipality of São Luís, Maranhão. The following categories were evidenced: knowledge about leprosy, signs and symptoms, stigma. Results: The participants had knowledge about the disease, but they verbalized people's disinformation about leprosy, disbelief in relation to the cure, and situations of prejudice and stigma that are still present today. Final considerations: The culture circle enabled the intertwining of scientific and empirical knowledge in the construction of a critical and reflective knowledge committed to welcoming and comprehensive care for people and families affected by leprosy.


RESUMEN Objetivo: Describir la construcción sobre (des)conocimiento y estigma de la lepra por parte de los Agentes Comunitarios de Salud participantes del Círculo de Cultura. Método: Estudio cualitativo, de tipo investigación-acción, apoyado en el marco del Círculo de Cultura Paulo Freire, realizado con 21 Agentes Comunitarios de Salud. Datos recolectados en noviembre de 2021, en el municipio de São Luís, Maranhão. Se evidenciaron las categorías conocimientos sobre la lepra; signos y síntomas; estigma. Resultados: Los participantes tenían conocimiento sobre la enfermedad, pero verbalizaban la desinformación de las personas sobre la lepra, la incredulidad con relación a la cura y situaciones de prejuicio y estigma que aún hoy están presentes. Consideraciones finales: El círculo de cultura posibilitó entrelazar saberes científicos y empíricos en la construcción de un saber crítico y reflexivo comprometido con la acogida y atención integral de las personas y familias afectadas por la lepra.


RESUMO Objetivo: Descrever a construção sobre o (des)conhecimento e estigma da hanseníase pelos Agentes Comunitários de Saúde participantes do Círculo de Cultura. Método: Estudo qualitativo de tipo pesquisa-ação apoiado pelo referencial do Círculo de Cultura de Paulo Freire, realizado com 21 Agentes Comunitários de Saúde. Dados coletados em novembro de 2021, no município de São Luís, Maranhão. Foram evidenciadas as categorias conhecimento sobre a hanseníase; sinais e sintomas; estigma. Resultados: Os participantes possuíam conhecimento sobre a doença, mas verbalizaram a existência de desinformação das pessoas sobre a hanseníase, descrenças em relação à cura, além de situações de preconceito e estigma ainda presentes atualmente. Considerações finais: O círculo de cultura possibilitou o entrelace dos saberes científicos e empíricos na construção de um conhecimento crítico e reflexivo comprometido com o acolhimento e atenção integral às pessoas e famílias afetadas pela hanseníase.

BACKGROUND: Health information systems allow for the identification of problems in this field, and often represent the single source of information; therefore, accurate determination of their quality is crucial. AIMS: To analyze accessibility, opportunity and completeness as criteria for the quality of the information provided by System of Information for Notifiable Conditions (Sistema de Informação sobre Agravos de Notificação - SINAN) on Work Accidents under Exposure to Biological Materials (Acidentes de Trabalho com Exposição a Material Biológico - ATEMB) from 2010 to 2015. METHODS: Observational and descriptive study based on secondary SINAN data on ATEMB. The object of analysis was all ATEMB records from the 26 Brazilian states and Federal District from 1 January 2010 to 31 December 2015. RESULTS: Information is accessible and timely, while the data for the ongoing year are only made available the following one. The percentage of incomplete data in SINAM-ATEMB was high for the following variables: educational level, length of work in the current job, organic materials, serological status of affected individuals and source-patient, measures adopted, case progression and Work Accident Notification (Comunicação de Acidente de Trabalho - CAT); therefore, these variables cannot be included in analyses of risk factors. CONCLUSION: Despite the accessibility of the database and the relevance of its variables, SINAN-ATEMP exhibits problems in its quality that indicate an indisputable need to improve the completeness of information.


CONTEXTO: Os sistemas de informação em saúde possibilitam conhecer problemas dessa área e, muitas vezes, representam a única fonte de informação, tornando-se fundamental conhecer sua qualidade. OBJETIVO: Analisar a acessibilidade, a oportunidade e a completude como critérios da qualidade das informações do Sistema de Informação sobre Agravos de Notificação a respeito de Acidentes de Trabalho com Exposição a Material Biológico (SINAN-ATEMB), no período de 2010 a 2015. MÉTODOS: Estudo observacional, descritivo, baseado em dados secundários do SINAN sobre ATEMB. O objeto de análise do estudo foi composto por todos os registros de ATEMB ocorridos nos 26 estados e no Distrito Federal no período de 1º de janeiro de 2010 a 31 de dezembro de 2015. RESULTADOS: Foi constatado que as informações são acessíveis e oportunas, com defasagem do ano vigente em relação ao início do estudo. O SINAN-ATEMB apresentou elevado percentual de dados incompletos nas variáveis: escolaridade, tempo de trabalho na ocupação, material orgânico, status sorológico do acidentado e do paciente-fonte, condutas adotadas, evolução do caso e emissão da Comunicação de Acidente de Trabalho (CAT), levando a consideração de que não é possível o uso dessas variáveis em análises de fatores de risco. CONCLUSÃO: Conclui-se que, apesar da acessibilidade da base de dados e da relevância de suas variáveis, o SINAN-ATEMB possui problemas de qualidade que apontam para a indiscutível necessidade de melhorias na completude de suas informações.

This study investigated factors associated with cases of tuberculosis retreatment due to relapse and readmission after treatment abandonment. This is an analytical cross-sectional study type of cases reported in the Information System for Notifiable Diseases in priority municipalities in the State of Maranhão, from January 2005 to December 2010. A logistic regression model was used to identify the association. Patients aged between 40 and 59 years (OR = 1.49, p = 0.029) with a pulmonary clinical form (OR = 2.79, p = 0.016) were more likely to incur relapse. Readmissions after abandonment were more likely in males (OR = 1.53, p = 0.046), aged between 20 and 39 years (OR = 1.65, p = 0.007), with less than eight years of schooling (OR = 2.01, p = 0.037) and with alcohol dependence (OR=1.66, p = 0.037), which showed a higher probability of another abandonment (OR = 5.96, p < 0.001). These data reinforce the need for strategies aimed at this group, such as increased supervised treatment, intensified active search, post-discharge follow-up and health education action.


Este estudo investigou os fatores associados aos casos de retratamento de tuberculose por recidiva e por reingresso após abandono. Trata-se de um estudo transversal tipo analítico dos casos notificados no Sistema de Informação de Agravos de Notificação em municípios prioritários do Estado do Maranhão, de janeiro de 2005 a dezembro de 2010. Para identificar as associações foi utilizado o modelo de regressão logística. Os pacientes com idade entre 40 e 59 anos (OR = 1,49; p = 0,029) e com a forma clínica pulmonar (OR = 2,79; p = 0,016) apresentaram maior chance para recidiva. Os reingressos após abandono tiveram maior chance nos indivíduos do sexo masculino (OR = 1,53; p = 0,046), com idade entre 20 e 39 anos (OR = 1,65; p = 0,007), com menos de oito anos de estudo (OR = 2,01; p = 0,037) e dependentes de álcool (OR = 1,66; p = 0,037), os quais apresentaram maior chance de novo abandono (OR = 5,96; p < 0,001). Esses dados reforçam a necessidade de estratégias direcionadas a esse grupo, como a ampliação do tratamento supervisionado, intensificação da busca ativa, acompanhamento pós-alta e ações de educação em saúde.
