BACKGROUND: Macular telangiectasia (MacTel), also known as idiopathic juxtafoveolar telangiectasis (IJFTs), involves telangiectatic changes in the macular capillary network. The most common variant, MacTel type 2, has distinct clinical features and management strategies. METHODS: This study offers a comprehensive review of MacTel and focuses on a series of three patients diagnosed with MacTel type 2 in our clinic. A meticulous ophthalmological evaluation, augmented by high-resolution imaging techniques like optical coherence tomography (OCT), OCT angiography (OCT-A), fundus autofluorescence (FAF), fluorescein angiography (FA), and adaptive optics (AOs) imaging, was conducted. RESULTS: The findings revealed normal anterior segment features and a grayish discoloration in the temporal perifoveal area on fundus examination. OCT exhibited hyporeflective cavities in the inner and outer neurosensory retina, along with other changes, while OCT-A identified retinal telangiectatic vessels in the deep capillary plexus. FAF demonstrated increased foveal autofluorescence, while FA initially detected telangiectatic capillaries followed by diffuse perilesional leakage in the later phase. Adaptive optics images showed the cone mosaic pattern. Notably, one patient developed a macular hole as a complication, which was successfully managed surgically. CONCLUSION: This study underscores the challenges in diagnosing and managing MacTel, emphasizing the importance of a multidisciplinary approach and regular follow-ups for optimal outcomes.

BACKGROUND/OBJECTIVES: Age-related macular degeneration (AMD) is one of the main causes of blindness and visual impairment worldwide. Intravitreal complement inhibitors are an emergent approach in the treatment of AMD, which have had encouraging results. This systematic review analyzes the outcomes and safety of complement inhibitor therapies for GA in AMD cases. METHODS: A comprehensive search on the PubMed and Web of Science databases returned 18 studies involving various complement inhibitor agents, with a total of 4272 patients and a mean follow-up of 68.2 ± 20.4 weeks. RESULTS: Most treated patients were white (96.8%) and female (55.8%), with a mean age of 78.3 ± 7.8 years and a mean GA area of 8.0 ± 3.9 mm2. There were no differences in visual function change between treated and control participants. The mean GA area change was 2.4 ± 0.7 mm2 in treated participants vs. 2.7 ± 0.8 mm2 in control groups (p < 0.001). The ocular and systemic side effects were similar to those of intravitreal anti-VEGF. A less-understood effect was that of the onset of choroidal neovascularization (CNV) in 1.1-13% of patients; this effect was found to be more frequent in patients with neovascular AMD in the fellow eye or nonexudative CNV in the study eye at baseline. CONCLUSIONS: Complement inhibitors may represent a useful therapy for GA in AMD, but a personalized approach to patient selection is necessary to optimize the outcomes.

Ophthalmic non-motor impairments are common in Parkinson's disease patients, from the onset of the neurodegenerative disease and even prior to the development of motor symptoms. This is a very crucial component of the potential for early detection of this disease, even in its earliest stages. Since the ophthalmological disease is extensive and impacts all extraocular and intraocular components of the optical analyzer, a competent assessment of it would be beneficial for the patients. Because the retina is an extension of the nervous system and has the same embryonic genesis as the central nervous system, it is helpful to investigate the retinal changes in Parkinson's disease in order to hypothesize insights that may also be applicable to the brain. As a consequence, the detection of these symptoms and signs may improve the medical evaluation of PD and predict the illness' prognosis. Another valuable aspect of this pathology is the fact that the ophthalmological damage contributes significantly to the decrease in the quality of life of patients with Parkinson's disease. We provide an overview of the most significant ophthalmologic impairments associated with Parkinson's disease. These results certainly constitute a large number of the prevalent visual impairments experienced by PD patients.

Introduction: Medicine faces nowadays the trend of increasing life expectancy of human population, with the resulting increase of degenerative age related diseases prevalence, combined with the risks of less tempered sun radiations environment exposure. Under these circumstances, our work pointed out on evaluating the effect of some xanthophyll pigments dietary supplements, actually widely recommended, for prevention of retinal degenerative damages and for slowing down the progression of such age related changes if they have already occurred. These dietary supplements are already well known for their total antioxidant activity, proven by photochemiluminescence method using Total Antioxidant Capacity in Lipid soluble-substances procedure. Materials and methods: The study recruited a number of 120 subjects equally divided on genders. The lot included a first group of 60 patients with comparable ages (all of them over 50 years and divided in 2 segments of age: 50-60 and over 60) and suffering from comparable retinal age-related degenerative abnormalities (mild/medium severity age-related macular degeneration according to Wisconsin Age-Related Maculopathy Grading System), and a second group, considered control, including a similar number of healthy, normal retina subjects belonging to same age and gender categories. There were evaluated at baseline the eye medical status and the retinal risk by specific methods: complete eye check-up, Amsler grid, specific standardized questionnaires focused on visual function and its impact on the quality of current life. Both groups, patients and control, received similar dosages of xanthophyll pigments dietary supplements including lutein and zeaxanthin during 18 months after baseline; at the end of this supplementation period a new evaluation was conducted. In the second part of the research all subjects involved received a new dietary supplement in which the same xanthophylls were enriched with C and E vitamins and oligo-elements Zinc and Copper. At the end of three years duration supplementation, the subjects were reevaluated and the paper presents the conclusions on the matter, pointing on the impact of xanthophyll supplements on visual health. Results: Correlation tests were applied to the complete set of data. Correlation tests have values between -1 and +1. The value -1 represents the negative correlation (reverse proportionality) meanwhile the value +1 represents the positive correlation (direct proportionality). The charts show the curves that are fitting experimental data. The dependence is linear in nature, and the value R2, as it approaches more the value 1, represents a better match with the experimental data (the data are in a percentage of approximately 99% on these straight lines of type y = ax + b). In the charts, there were noted the average values of the scores for healthy control patients with "Control", and the average values of the scores for the patients with existing age related degenerative retinal pathology at baseline with "Patients". Discussion: The retinal function and the impact of visual condition on health were both evaluated at baseline, 18 months and 36 months after baseline, by visual acuity, ophthalmoscopy fundus examination, Amsler test and by asking the subjects to answer the visual function questionnaires: EQ-5D, NEI-VFQ-25, as measures of health status quality and of the influence on welfare. The study revealed that under supplementation both control healthy subjects and patients with known degenerative retinal pathology included in the 50-60 years of age group evolved almost the same way, leading to the conclusion that administered xanthophyll pigments-based supplements, simple or enriched, managed to slow down the progression of abnormal degenerative vision loss to a rate comparable to physiological aging-related vision loss. It was also observed that intake of xanthophyll pigments dietary supplements preserved the general health condition and maintained relatively constant vision on the entire 36th months follow-up research duration in patients presented with existing age related degenerative retinal pathology at baseline. For healthy subjects, evaluation showed an improvement in results after dietary supplementation, with maintenance of constant vision and a significantly increase of general condition, in a positive sense. For subjects over the age of 60 dietary supplements intake was even more effective compared to younger group in providing better control of degenerative processes.

Infections represent a possible risk factor for parkinsonism and Parkinson's disease (PD) based on information from epidemiology and fundamental science. The risk is unclear for the majority of agents. Moreover, the latency between infection and PD seems to be very varied and often lengthy. In this review, the evidence supporting the potential involvement of infectious microorganisms in the development of Parkinson's disease is examined. Consequently, it is crucial to determine the cause and give additional treatment accordingly. Infection is an intriguing suggestion regarding the cause of Parkinson's disease. These findings demonstrate that persistent infection with viral and bacterial microorganisms might be a cause of Parkinson's disease. As an initiating factor, infection may generate a spectrum of gut microbiota dysbiosis, engagement of glial tissues, neuroinflammation, and alpha-synuclein accumulation, all of which may trigger and worsen the onset in Parkinson's disease also contribute to its progression. Still uncertain is the primary etiology of PD with infection. The possible pathophysiology of PD infection remains a matter of debate. Furthermore, additional study is required to determine if PD patients develop the disease due to infectious microorganisms or solely since they are more sensitive to infectious causes.

The aim of the present study was to accurately measure the diameter of the ophthalmic artery (OA) and investigate whether bilateral variations in diameter can be recorded in relation to patient age and sex. A retrospective study including 80 computed tomography angiographic (CTA) examinations and a total of 160 arteries was conducted to demonstrate the morphometric aspects of the OAs analyzed bilaterally by CTA examinations, while considering the references of the internal carotid artery (ICA) caliber. Precise measurements performed on the OA and the ICA below and above the ophthalmic emergence revealed an OA diameter of 1.38±0.24 mm and a narrowing of the ICA between the origin of the OA of 1.5±0.25 mm. Variations in the OA and the ICA calibers were studied in subjects with normal cervical vasculature on CTA. After a thorough statistical study, variations in OA and ICA caliber on each side were identified, between both sex and age-related groups, revealing morphometric parameters of the OA in relation to the ICA.

Artificial tears or lubricants is a developing category in pharmaceutical research, due to the permanent increasing incidence of dry eye syndrome caused by the extensive use of personal computers and other gadget screens, associated with global warming and pollution. Considering the role of inflammation in dry eye syndrome, characterized by the production of free radicals, it is imperative to determine which elements are more significant in forming an artificial tear more effectively and more comfortably for the eye state and for the quality of life. Thus, the aim of the present study was to examine the evolution of the total antioxidant capacity of some frequently commercialized artificial tears via the photochemiluminescence method, using an antioxidant capacity of lipid soluble substances procedure, prior and subsequent to the exposure of these therapy agents to some physical agents. This is a preliminary research aiming to evaluate the impact of various environmental factors on these ophthalmic products, to be continued by evaluating whether the effectiveness of these products, in terms of objective examination and patient preference and adherence criteria, is impacted by the conditions of use and storage. Thus, the total antioxidant capacity of the evaluated artificial tear samples after UVC irradiation at 254 nm wavelength was studied, in order to investigate whether their status suffered any change in terms of antioxidant potential. In addition to the findings obtained in the study, some recommendations were also made.

Having a classical role in the regulation of calcium homeostasis in skeletal system, vitamin D has also been recognized as being involved in the activity of the immune system, as well as in the pathology of the visual analyzer. Thus, regarding the function of vitamin D in the eye, this is supported by the identification of vitamin D receptors (VDR) in several structures of the eyeball, such as corneal epithelial and endothelial cells, ciliary body and retinal cells. One of the ophthalmological pathologies in which vitamin D plays an important role is diabetic retinopathy, both through its effects on the immune system (reduction of the pro-inflammatory cytokines - IL-1, IL-6, IL-012, TNF alpha, and stimulation of anti-inflammatory cytokines IL-10), as well as by reducing the level of vascular endothelial growth factor (VEGF) and thus inhibiting retinal neovascularization. Vitamin D demonstrates a protective role on the development and progression of diabetic retinopathy by reducing blood sugar, hypertension and atherosclerosis, but randomized studies are still needed to establish the direct causal relationship between the development of diabetic retinopathy and vitamin D levels.

Cat-scratch disease is an illness caused by Bartonella henselae that occurs as a result of contact with an infected kitten or dog, such as a bite or scratch. It is more prevalent in children and young adults, as well as immunocompromised individuals. There are limited publications examining the features of CSD in patients. As such, the purpose of this research was to assess the clinical neuro-ophthalmological consequences of CSD reported in the literature. Among the ophthalmologic disorders caused by cat-scratch disease in humans, Parinaud oculoglandular syndrome, uveitis, vitritis, retinitis, retinochoroiditis and optic neuritis are the most prevalent. The neurological disorders caused by cat-scratch disease in humans include encephalopathy, transverse myelitis, radiculitis, and cerebellar ataxia. The current review addresses the neuro-ophthalmological clinical manifestations of cat-scratch disease, as described in papers published over the last four decades (1980-2022). All the data gathered were obtained from PubMed, Medline and Google Scholar. The current descriptive review summarizes the most-often-encountered clinical symptomatology in instances of cat-scratch disease with neurological and ocular invasion. Thus, the purpose of this review is to increase knowledge of cat-scratch disease's neuro-ophthalmological manifestations.

Out of the multiple vascular complications of diabetes, retinopathy is the easiest to diagnose and monitor as the examination of the eye fundus is an easy investigation to perform, does not require expensive medical equipment and can be repeated without any risk to the patient. The appearance of the retinal vessels, the optic nerve and the retina can provide useful information on the coronary and cerebral circulation, plasma lipid levels, renal function, and the quality of the arteries of the lower limbs. It is known that visual acuity changes variably depending on the macular alteration and may decrease when edema is installed in the macular region or is altered by the appearance of hemorrhages or the presence of foveolar neovascular tissues resulting from traction retinal detachment. In the absence of proper treatment, diabetes leads to blindness. The lesions that appear are not specific to diabetes, but by combining them they create a clinical picture characteristic of this disease.

The recent years has revealed an intense interest in the study of nicotinamide adenine dinucleotide (NAD+), particularly in regards to its intermediates, such as nicotinamide and nicotinic acid known as niacin, and also nicotinamide riboside. Besides its participation as a coenzyme in the redox transformations of nutrients during catabolism, NAD+ is also involved in DNA repair and epigenetic modification of gene expression and also plays an essential role in calcium homeostasis. Clinical and experimental data emphasize the age-dependent decline in NAD+ levels and its relation with the onset and progression of various age-related diseases. Maintaining optimal levels of NAD+ has aroused a therapeutic interest in such pathological conditions; NAD+ being currently regarded as an important target to extend health and lifespan. Based on a systematic exploration of the experimental data and literature surrounding the topic, this paper reviews some of the recent research studies related to the roles of the pyridine nucleotide family focusing on biosynthesis, NAD+ deficiency-associated diseases, pathobiochemistry related to retinal degeneration and potential therapeutic effects on human vision as well.

The connection between central obesity and the development and metastasis of various visceral tumors is largely accepted and one of the main causes seems to be the local synthesis of proangiogenic molecules. Progranulin (PRG), recently identified as an adipokine, is a novel pleiotropic growth factor acting on the proliferation and development of fast-growing epithelial cells, cancer cells, and also a proangiogenic factor whose expression is induced in activated endothelial cells. One of the molecules that seems to trigger the angiogenic activity of PRG is vascular endothelial growth factor (VEGF). Two groups of human subjects were considered and adipose tissue was processed for an immunohistochemical and morphometric study after surgery for abdominal tumoral or non-tumoral pathology. The presence of PRG in adipose pads of the omentum was analyzed and its association with VEGF, CD34 and collagen IV in tumoral and non-tumoral visceral pathology was examined. The results showed that PRG but not VEGF expression was upregulated in adipose tissue in tumoral visceral pathology. In conclusion, the involvement of the proangiogenic activity of PRG and VEGF in adipose tissue under tumor conditions may be dependent on the visceral tumor type.

The climate changes, which occurred during the last decades, put all living species in front of new challenges. Human biology is no exception to it, all tissues have to face new effects, with unpredictable consequences. Many cancers, mainly affecting the skin, but also many of the eye various structures diseases, have ultraviolet radiation as recognized causative agent. The aim of our work is to highlight the changes that can occur after exposure to ultraviolet radiations of soft tissues, including eye structures, by reviewing data from the scientific literature regarding the matter. Responsible for severe diseases, including cancer, ultraviolet negative effects on various soft tissues can be limited by better comprehension. Their knowledge can contribute to improving public health, by finding new preventive methods, which might represent the foundation of effective public health programs.

Ligneous conjunctivitis represents a very rare form of chronic membranous conjunctivitis, with unknown etiology; less than 200 cases have been reported in the literature, most of them in infants and children. After 40 years, this condition appears in exceptional circumstances. We present, in this study, two patients (of 55 and 64 years old) with very severe forms of ligneous conjunctivitis, certified by histopathological examination, one of them being previously diagnosed with Lyell's syndrome. The histological examination in ligneous conjunctivitis is important for diagnostic but also to elucidate the etiopathological context. A discontinued hyperplastic conjunctiva with numerous nuclear abnormalities and marked intra- and inter-cellular edema was characteristic for both cases. The epithelium layer presents large area of extension into the connective tissue, in the form of cysts and gland-like structures, or creating deep epithelial lacunas, with goblet cells. The connective subepithelial tissue presents diffuse inflammatory infiltration, with round cells, especially near vessels. Inside the conjunctival tissue, big hyaline areas with rare cellularity are detected. The amorphous deposits containing fibrillar material are also present. At the level of pseudo-membranes, we recorded a massive exudation of fibrin with an inflammatory cellular infiltration, and large areas containing an amorphous eosinophilic hyaline material. The association of ligneous conjunctivitis with Lyell's syndrome in one of our patients suggests its immuno-allergic etiology.

Keratoconjunctivitis sicca represents a progressive deterioration of ocular surface produced by a deficient secretion of lachrymal film (quantitative disorder) or excessive tear evaporation (qualitative disorder). The cytological analysis of conjunctival impression in 42 patients with dry eye syndrome established a strong correlation between the clinical grade of severity of disease and the grade of squamous metaplasia, including goblet cell loss. The cellular anomalies were represented by modifications of keratinization, epithelial cells' anisocytosis, anisochromia, the nuclear condensation and the cytoplasmic vacuolization. Pyknotic nuclei and anucleated cells were only seen in the most severe dry eye. The modifications in epithelial cells and conjunctival goblet cells reveal cellular sufferance, with an evident parallelism between these anomalies and clinico-functional signs in dry eye. Conjunctival impression provides an easy and quick identification of the lachrymal film alterations with high specificity and sensitivity, giving valuable information about the qualitative disorder.

Graves disease associates thyroid and non-thyroid symptoms and signs with autoimmune pathogeny, including the ophthalmopathy. The treatment of Graves ophthalmopathy consists of medical immunosuppressive therapy, retrobulbar injections and general treatment. Recently, Somatostatin injections have proved their efficiency.

Vision is based on the sensitivity of the eye to visible rays of the solar spectrum, which allows the recording and transfer of visual information by photoelectric reaction. Any electromagnetic radiation, if sufficiently intense, may cause damages in living tissues. In a changing environment, the aim of this paper is to point out the impact of light radiation on ocular cells, with its phototoxicity potential on eye tissues. In fact, faced with light and oxygen, the eye behaves like an ephemeral aggregate of unstable molecules, like a temporary crystallization threatened with entropia.
